@#Objective    To evaluate mid-term outcomes of pulmonary valve replacement surgery after repair of tetralogy of Fallot. Methods    A total of 73 patients with repaired tetralogy of Fallot who underwent pulmonary valve replacement surgery in our hospital from January 2010 to January 2020 were enrolled, including 42 males and 31 females. The median age was 3.9 (0.2-42.8) years at initial repair and 20.0 (2.0-50.0) years at pulmonary valve replacement. The clinical data of the patients were recorded and analyzed. Results    There was no death in postoperative 30 d. The average follow-up time was 35.6±28.5 months, and no death occurred during the follow-up. One patient underwent a second reintervention after initial pulmonary valve replacement. The 1- and 5-year survival rates were both 100.0%, the 1- and 5-year reintervention-free rates were both 100.0%, and the 1- and 5-year valve failure-free rates were 100.0% and 67.1%. There was no significant difference in valve failure-free rates between different age groups (P=0.49) and different type of valve groups (P=0.74). The right (P=0.006) and left (P=0.002) ventricular ejection fractions were significantly improved, and the QRS duration was shortened after pulmonary valve replacement (P=0.006). Conclusion    Mid-term outcomes of surgical pulmonary valve replacement were satisfactory in patients with repaired tetralogy of Fallot, while the long-term effects should be further emphasized in clinical practice.

Objective:This study aims to review two surgical techniques results of the reoperation for left atrioventricular valve(LAVV) failure in repaired atrioventricular septal defect.Methods:All consecutive patients with repaired atrioventricular septal defect(AVSD) who underwent redo-LAVV surgery from 2005 to 2019 were included. Patients with single ventricles, banding, atrial isomerism, and complex associated anomalies were excluded. Univariate analysis included repair and replacement. Data analyzed included number and year of primary AVSD and redo-LAVV operation, morphology of AVSD, mortality, and reoperation, early and long-term survival.Results:There were a total of 28 patients including 7 boys(25%) with age of 114.5 months(63.0-194.5 months). The mean body weight was 28.55 kg(15.5-55.9 kg). There were 11(39.3%) patients with complete AVSD and 17(60.7%) with partial AVSD, and 21(75%) patients with LAVV valvuloplasty(LAVVP), 7(25%) patients with LAVV replacement(LAVVR). The aortic cross clamp time was significantly longer in patients who underwent LAVVR compared to LAVVP[LAVVR 94 min(79-107)min vs. LAVVP 66 min(45-83 min), P<0.05]. The technique of cleft closure with LAVVP 18(87.5%) more than LAVVR 2(28.6%), P<0.05. Conclusion:The postoperative left atrio-ventricular valve(LAVV) regurgitation is the main reason of the reoperation. The technique of cleft closure with LVVR-Repair is favorable. At follow-up, survivors with re-LVVR-Repair have high rates with numerous operations.

Resumen La sífilis es una infección bacteriana producida por el Treponema pallidum (espiroqueta). Consta de varias etapas: primaria, secundaria y terciaria, según el tiempo de evolución desde el momento en que se adquiere la infección. Cada etapa comprende diferentes manifestaciones clínicas; los síntomas cardiovasculares forman parte de la sífilis terciaria, en cuyo caso la aortitis sifilítica es la principal forma de presentación. Se expone el caso de un paciente que consultó por déficit neurológico focal, en quien por medio de estudios de extensión se documentó neurosífilis e insuficiencia valvular aórtica severa secundaria a perforación de la válvula coronaria derecha, que requirió recambio valvular aórtico por bioprótesis. Adicionalmente, se hace una revisión de las principales manifestaciones cardiovasculares de esta enfermedad. Aunque en la era postantibiótica este tipo de manifestaciones tardías son cada vez menos frecuentes, es imperativo conocerlas.


Abstract Syphilis is a bacterial infection caused by Treponema pallidum (spirochete). It has various stages: primary, secondary and tertiary; depending on the time to progression from the moment the infection is acquired. Each stage involves various clinical manifestations; cardiovascular symptoms are part of tertiary syphilis, and syphilitic aortitis is the main form of presentation. We present the case of a patient who consulted with a focal neurological deficit, in whom extension studies reported neurosyphilis and severe aortic failure secondary to perforation of the right coronary valve, which required aortic valve replacement by bioprosthesis. We also undertake a review of the main cardiovascular manifestations of this disease. Although in the post-antibiotic era this type of late manifestation is increasingly less frequent, it is imperative that we are aware of it.

Résumé

Prosthetic valve dysfunction due to pannus formation is an infrequent but serious complication of tricuspid valve replacement. An 87-year-old woman underwent tricuspid valve re-replacement for severe prosthetic valve stenosis and regurgitation. On removal, thick fibrous pannus and chordal attachments were observed on the ventricular side of the cusp, which corresponded to the septal leaflet of the native valve. Microscopic examination revealed inflammatory cell infiltration accompanied with severe fibrosis and scarring had compromised and broken the prosthetic valve cusp under the pannus. The elastic fiber, which was detected in the base of the pannus, suggested it was a remnant of the native tricuspid valve leaflet. Prevention of native tissue attachment to the prosthetic valve cusp, which may cause severe pannus formation, appears to be extremely important for the long-term outcome and valve durability. The choice of prosthesis for the tricuspid position remains controversial. We should especially consider the height of stent posts and the continuity between the cusp and suture ring in the choice of the bioprosthetic valve for tricuspid position.

Résumé

Edwards Duromedics Valve has been suspended from the market since May 1988 due to the frequent reports of leaflet escape. There were 2 case-reports of leaflet escapes in this country. We report a case of leaflet escape in an Edwards Duromedics bileaflet valve, in the mitral position.
