Résumé

Objective: To understand the combination use and rationality of Chinese patent drugs containing Salvia Miltiorrhiza or its active ingredients and western medicines to provide reference for regulating prescriptions and improving rational drug use level. Methods: The prescriptions of outpatients in our hospital during January 2016 and December 2016 were collected by a retrospective study method, and the ones with the combination use of Chinese patent drugs containing Salvia Miltiorrhiza or its active ingredients and western medicines were analyzed. The rationality of the combination use was analyzed in aspects of physicochemical properties, phar-macological effects, pharmacodynamics and pharmacokinetics. Results: Totally 257 730 prescriptions were examined selectively, and among them, 8 894 were with the combination use of western medicines and Chinese patent medicines containing Salvia Miltiorrhiza or its active ingredients including 1 213 irrational prescriptions (13. 64% ). The irrational prescriptions showed such problems as antago-nism in pharmacologic action causing decreased efficacy (510, 42. 04% ), overlaying in pharmacological effects causing increased effi-cacy (492, 40. 56% ), precipitation because of physical and chemical reactions and chemical taboos (299, 24. 65% ) and reduced bioavailability and affected efficacy due to complex reactions (174, 14. 34% ). Conclusion: The combination use of western medicines and Chinese patent medicines containing Salvia Miltiorrhiza or its active ingredients should be careful to avoid irrational problems. When they are auditing prescriptions and monitoring medication, clinical pharmacists should strengthen intervention of the combination use of Chinese patent medicines and western medicines reasonably combining with physical and chemical properties of medicines, phar-macological effects, pharmacodynamics and other characteristics.

For the basic research on the traditional Chinese medicine(TCM), objective syndrome of traditional Chinese medicine and evaluation criteria of traditional Chinese medicine compounds are hardly to break though. While, the modern immunology points out that the body is a counterbalance state and immune imbalance is the root of sickness. The thinking mode of treating diseases in traditional Chinese medicine is also "balance", considering disease is the result of bias which present the imbalance of "Yin counters Yang", "exterior counters interior", "cold counters heat" and "weak counters strong". The Chinese herbal compound formula preparation was applied on disease therapy based on theory of Chinese medicine, which was confirmed by long period clinical application. It is composed of multi-compounds and has the characteristic of multi-targeting. Integrative medicine has spawned pan-immunomics, and the evaluation of immune function (immune balance) has become an important basis for diagnosis and treatment models of integrative medicine. In addition, balance is the core idea of whole-systemic conception of traditional Chinese medicine. Therefore, we speculate that immune balance under pan-immunomic can bridge the traditional Chinese medicine and modern integrative medicine and is the important basis for objective syndrome of traditional Chinese medicine and evaluation criteria of traditional Chinese medicine compounds. According to the bridging theory, we attempt to utilize informatics and statistical methods to construct an evaluation system for pharmacodynamics of traditional Chinese medicine based on its moderate regulation and the balanced adjustment of immunity under pan-immunomic, which further reveal the scientific essence of the whole-systemic view of traditional Chinese medicine. This research brings out a new valuable strategy and provides a theoretical basis for accelerating the transformation of traditional Chinese medicine, especially the exploitation of Chinese herbal compound formula, and constructing the new drug innovation and review system for traditional Chinese medicine. Besides as a reference for traditional Chinese medicine objective syndrome and pharmacodynamics of traditional Chinese medicine compounds, the evaluation system can screen the immunity of sub-health population also. With the continuous accumulation of clinical sample and data, the evaluation system will be more accurate and intelligent.

Objective: To investigate the breakthrough points and methods of pharmaceutical care performed by clinical pharmacists for chemotherapy-induced Ⅳ degree myelosuppression.Methods: One advanced lung adenocarcinoma patient suffering from chemotherapy-induced Ⅳ degree myolosuppression was selected as the example, clinical pharmacists provided suggestions on the chemotherapy regimen, assisted physicians in making reasonable treatment plan and implemented comprehensive pharmaceutical care for the patient.Results: Clinical pharmacists played a positive role in the clinical treatment through the comprehensive pharmaceutical care in oncology department.Conclusion: With the professional knowledge of pharmacy, clinical pharmacists can improve the level of clinical rational drug use.

Objective To compare the clinical features of drug induced hepatitis caused by traditional Chinese medicines (TCM) and western medicines. Methods A total of 181 patients hospitalized with drug in-duced hepatitis between January and December 2015 were enrolled. Among the patients ,75 cases were in TCM group,66 cases in western medicine group and 40 cases in combined group(accepted both TCM and western medi-cine treatment). Liver biopsies were performed and Roussed Uclaf Causality Assessment Method (RUCAM) scores showed that all patients were with scores higher than 3. The data including gender,age,clinical manifesta-tions,physical signs,laboratory tests and image characteristics were analyzed individually. Results There was no significant difference in age,the pattern of liver injury,and the course of liver damage among the three groups (P>0.05). Anorexia was common symptom in all the three groups without any significant difference(P>0.05). Jaundice was most frequently observed in TCM group while fever was most commonly found in western medicine group,which both had significant difference(P0.05). Compared with western medicine group ,the patients in TCM group had a higher level of platelet counts ,serum levels of total bilirubin ,total bile acids(TBA)and serum iron(P<0.05), and less proportion of eosinophils and lower level alkaline phosphatase(P< 0.05). Conclusions The age,the pattern of liver injury,the course of liver damage and aminotransferase levels in patients with drug induced hepati-tis caused by TCM and western medicines were similar;however,western medicines were more likely to cause al-lergic responses and hyperbilirubinemia occurred more frequently in patients with drug induced hepatitis caused by TCM.

[Objective]To observe the clinical effect of the treatment of hormone dependent asthma by Bushen Yiqi Decoction. [Methods]84 cases of hormone dependent asthma in the Affiliated Hospital of Nanjing University of Chinese Medicine from December 2011 to December 2015 were selected. According to the random number table, 84 cases were randomly divided into two groups:control group and observation group, 42 cases in each group. Control group was in accordance with the GINA guidelines recommended fourth level of treatment. Observation group, on the basis of the control group, treated hormone dependent asthma by western medicine in combination with Bushen Yiqi Decoction. [Results] The total effective rate of observation group was 88.1%, and the total effective rate of control group was 71.43%. Two groups of comparison, the difference was statistically significant( P<0.05); IL-5, IL-4, EOS levels were decreased after treatment in the two groups(P<0.05), but the observation group decreased more significantly, compared with the control group, the difference was statistically significant(P<0.05);During treatment and after treatment, reduction rate of using hormone and antiasthmatics in control group and observation group was respectively(85.7%VS64.3%) and(78.6%VS61.9), two groups of comparison, the difference was statistically significant( P<0.05). [Conclusion]Compared with the pure oral hormone plus inhalation in the treatment of hormone dependent asthma, application of Bushen Yiqi Decoction has obvious curative effect and can reduce the dosage of hormone and side effects. It is worthy of clinical promotion.

Clinical observation on treatment of type 2 cardiac and kidney syndrome by combination of traditional Chinese and Western medicine. The patients were divided into two groups: the simple Western medicine treatment group (control group) and the traditional Chinese medicine and Western medicine treatment group (treatment group). The patients in the two groups were treated with conventional western medicine.The treatment group was given based on Buxin Yishen decoction, a total of three courses of treatment to observe the two groups of patients before and after treatment of total efficacy, cardiac function indicators, changes in renal function indicators. The total efficacy of the treatment group and the control group were 91.80% and 72.41%, respectively. There were significant differences between the two groups (P<0.01). The cardiac function indexes and renal function indexes of the treatment group and the control group before and after treatment (P<0.01). Compared with the two groups, the left ventricular function, Hematuria natriuretic peptide, serum creatinine, urea nitrogen, cystatin-C were improved, and the treatment group (P<0.05~0.01). The results showed that the combination of traditional Chinese and Western medicine treatment can improve the clinical efficacy of type 2 heart and kidney syndrome, significantly improve heart and kidney function, better than conventional Western medicine treatment, and has good safety.

Objective]To explore the pathogenesis and rules of syndrome and treatment of different groups and stages of patients with malignant tumor,we hope to broden the clinical thinking and improve clinical therapeutic effect. [Methods]Starting from the theory of Chinese medicine,combined with the author's clinical experience,according to patients’selection treatment,we summarize the syndrome patterns, treatment principle and prognosis of different groups and different treatment stages.We divided it into two categories: pure Chinese medicine treatment and traditional Chinese and western medicine treatment. [Outcome]There were many types of malignant tumors,whose pathogenesis and treatment law can be changed under the intervention of treatment measures. Pure Chinese medicine treatment groups were divided into early, middle and late three stages.Combination of traditional Chinese and western medicine treatment was divided into Perioperative group,Tumor free survival group and Survival group with tumor.There were 3 stages in the tumor free survival groups:adjuvant chemotherapy, adjuvant radiotherapy, and no tumor rehabilitation.The survival of the tumor bearing groups was divided into five stages:neoadjuvant chemotherapy,palliative anticancer therapy,maintenance therapy of traditional Chinese medicine,malliative symptom control and the final stage. [Conclusion] Traditional Chinese medicine treatment of malignant tumor is divided into two categories, four groups, thirteen phases.Group and phased treatment based on classification is easy to master and conducive to optimizing the traditional Chinese medicine diagnosis and treatment program.

Objective:To investigate the role of clinical pharmacist in the treatment of patients with severe infection. Methods:Clinical pharmacist participated in the consultations for a patient with severe intracranial infection after craniocerebral operation in neurosurgery. According to the conditions of the patient and the results of antibiotic susceptive test,clinical pharmacist made an individualized medication for the patient,and the regimen included cefoperazone/ sulbactam,meropenem, vancomycin and Angong Niuhuang Wan. Results:The intracranial infection in the patient was improved gradually and controlled finally. Conclusion:Based on self professional knowledge,clinical pharmacist can participate in the clinical treatment of patients with severe infection,and assist doctors to develop treatment regimen in order to improve treatment effects. It will help clinical pharmacist preferably blend in clinical treatment team.

Objective: To observe the efficacy of combined Chinese and Western medicines in the treatment of senile functional constipation. Methods: One hundred patients with senile functional constipation were divided into treatment group and control group. There were 50 in each group. The patients in the treatment group were treated with Danggui Shaoyao Decoction and Cisapride Tablets, while the patients in the control group were treated with Cisapride Tablets only. The efficacy in the two groups were compared after three courses. Results: The total effective rate of the treatment group was 92% and the control group's was 78%, the former was higher than latter with the comparatively significant difference (P < 0.05). Conclusion: The results indicate that the combination of Chinese and Western medicines is highly effective in the treatment of senile functional constipation.

Objective To investigate and evaluate the improvement of the quality of life of pulmonary infection in AIDS patients treated by Chinese traditional treatment combined Western medicines treatment.Methods Sixty cases with HIV infection combined with pulmonary infection were selected as our subjects.They were randomly divided into two groups and each group was 30 cases.Patients in western medicine group were given medicine plus placebo,and in Chinese medicine group were given west medicine combined with traditional Chinese medicine.Patients hospitalized for 4 weeks.WHO QOL-HIV plus AIDS-related quality of life scale were used to collect the information about quality of life.Results There were no significant difference between two groups in terms of quality of life,life activity,health status,sleep status,ability to work,leisure opportunities scores at before treatment (P > 0.05).After treatment,the score of quality of life,life activity,health status,sleep status,ability to work,leisure opportunities in patients of Chinese group were (3.78 ± 0.69),(3.55 ± 0.67),(3.55 ± 0.67),(3.18 ± 0.72),(3.66 ± 0.70),(3.03 ± 0.73) respectively,higher than that of western medicine group ((2.90 ± 0.77),(2.75 ± 0.85),(2.75 ± 0.85),(2.85 ± 0.68),(3.35 ± 0.57),(2.95 ± 0.85) respectively,t =-2.791,-2.791,-4.28,-3.08,-4.08,-2.56,P < 0.05).There were no significant differences between the two groups in the terms of the score of satisfaction,sex ability,HIV impaction on the body,the meaning of life,the energy aspects of patients at before treatment (P > 0.05),but after treatment,these scores in Chinese medicine group were higher that in the control group ((3.51 ± 0.67) vs.(2.93 ±0.71),(3.13±0.46) vs.(2.86±0.74),(3.96±0.71) vs.(3.45 ±0.98),(3.66±0.72) vs.(3.23 ± 0.88),(3.23 ± 0.67) vs.(2.90 ± 0.65)),and the differences were statistically significant (t =-4.38,-2.34,-3.75,-2.76,-2.38,P < 0.05).There was no significant difference in the term of the mental effect of HIV within groups at before treatment,but after treatment,the score in Chinese group was lower than the Western group ((3.05 ± 0.90) vs.(3.46 ± 0.87),t =2.31,P < 0.05).Conclusion Chinese medicine combined western medicine can significant improve the quality of life of HIV patients with pulmonary infection.

[Objective]Exploring from the specific case analysis of Chinese and western medicine treatment of primary amenorrhea with the clinical curative effect of infertility. [Method]In2013 January by Professor Xu Xiaofeng, doctor diagnosed primary amenorrhea with infertility and 1 patient of typicalcase, combined traditional Chinese medicine(TCM) and western medicine artificial menstrual cycle for 3 months, 3 months after the Chinese medicine Tiaozhou method combined with low dose of HMG with HCG inovulation induction treatment. [Result]After the treatment, the patients had pregnancy 70D, blood hormone and B Ultrasound examination were normal. [Conclusion]Combined treatment of TCM and western medicine is better than pure Chinese medicine or western medicine, suggesting that combination of TCM and western medicine is effective for the treatment of primary amenorrhea with infer-tility.

OBJECTIVE:To discuss the irrational combined use of traditional Chinese with western medicines to provide basis for clinical safe medication. METHODS:Referring to the domestic pertinent literature together with our clinical experiences, some common and typical incompatibility between the traditional Chinese medicines and western medicines were analyzed, sorted and classified and summarized to derive the mechanism for the incompatibility. RESULTS & CONCLUSION:Irrational combined use between traditional Chinese medicines and western medicines can result in lower curative efficacy and higher toxic effect or even severe drug-induced diseases that may lead to serious consequences, therefore, great importance should be attached to this matter.

Aim To observe effects of compound dialysate of Chinese herbs and Western medicines on small intestinal mucosa ultrafiltrative function in dogs. Methods Twenty Beagle dogs were divided into 4 groups: NS, control (CG), lower ligustrazine dose (LLDG) and higher ligustrazine dose group (HLDG). According to the arteriovenous region of small intestine mesentery to make three isolation loops for comparative study, they were respectively jejunum loop (JL), jejunoileum loop (JIL) and ileum loop(IL), and input hypersonic sample solution through vein after blocking the renal arteries to make the man-made renal failure models such as electrolytic disorder, azotemia etc. Meanwhile, the dialysate was perfused from the inlet of these isolation loops, respectively.Finally, these secretion of small intestine loops and blood samples which arteria and venae of the small intestine mesentery every 30 minutes were collected to determine the density of K+, Na+, Cl-, UN, CR, UA and the clearance and filtration rate in each group were calculated. Objective To observe the effect of combination of Chinese and western medicines on rheumatoid arthritis(RA) at active stage. Methods Totally 150 cases of RA at active stage were randomized into two groups. The control group was given western medicines including Leflunomide, Sulfasalazine suppository and Celecoxib. The treatment group (100 cases) was given Chinese medicines Sihua Siteng Sichong Decoction (Decoction of four flowers, flour stems and four worms) in addition to those western medicines. The course for two groups was 3 months respectively. Clinical symptoms,ESR,CRP,RF and Immuno hemoglobin before and after treatment were observed. Results Of the 100 cases in the treatment group, 30 cases were remarkably effective, 43 cases improved, 20 cases effective, and 7 cases ineffective. The total effective rate was 93%. Of the 50 cases in the control group, 8 cases were remarkably effective, 20 cases improved, 12 cases effective, and 10 cases ineffective. The total effective rate was 80%.
