Advances in breast imaging: a review on where we are and where we are going

Breast radiology has undergone significant advances in recent years, and, naturally, several possibilities open up for attending physicians. Concomitantly, it increases the responsibility to keep up to date and provide the best care for each patient. Aware of the complex implications that the implementation of some of the technological advances may bring, such as increased costs, limited availability of equipment, and a potential increase in examination time, the objective of this study is to carry out a narrative review and provide a collection of advances that, in our opinion, are already gaining ground and should be consolidated in clinical practice. We will discuss new breast imaging methods that can be used both for screening and for the diagnostic investigation of breast lesions and we will summarize the most relevant aspects of each of them, addressing the technique, applicability, positive aspects, and limitations of each modality in a standardized way. Masas de alta sospecha Bi-Rads V (ACR): correlación con hallazgo benigno en el examen anatomopatológico / Breast lesions Bi-Rads V: mammographic-sonographic-pathologic correlation

Para analizar en forma retrospectiva lesiones radiológicas, masas espiculadas y distorsiones clasificadas como Bi-Rads V, cuyo resultado anatomopatológico fue benigno, se evaluaron 28 casos en 25 pacientes de masas clasificadas radiológicamente como Bi-Rads V. Todas fueron sometidas a biopsia quirúrgica. Dieciocho lesiones se manifestaron como masas espiculadas y 10 como distorsiones glandulares; el 60 por ciento se observó en la ecografía como masa hipoecoica. Los resultados anatomopatológicos fueron: 16 radial scar, 10 adenosis esclerosante y 2 mastopatía fibroquística. Las lesiones que en la mamografía se manifestaron como masas o distorsiones glandulares y que se categorizaron como Bi-Rads V, tienen PPV del 80 por ciento de ser malignas. Sin embargo, existen lesiones benignas como radial scar y adenosis esclerosante, en la misma categoría Bi-Rads V. Dada la alta incidencia de carcinoma en las mamas Bi-Rads V y la baja de lesiones benignas, se requiere de la biopsia excisional para el diagnóstico
