RÉSUMÉ

Supraventricular tachycardia is the most common symptomatic arrhythmia in young patients, frequently associated with Wolff-Parkinson-White (WPW) syndrome. We report a case of supraventricular tachycardia with Wolff-Parkinson-White (WPW) syndrome associated with partial eventration of a diaphragm in a neonate.

RÉSUMÉ

Bilateral eventration of the diaphragm associated with malrotation of gut and unilateral renal agenesis is a very rare entity. The authors treated a seven mth old male child who presented to them with respiratory distress and gastrointestinal problem. The child was anemic and was in type II respiratory failure. His X-ray chest showed bilateral eventration of diaphragm and ultrasonography abdomen was consistent with left renal agenesis which was confirmed by radionuclide scan. Bilateral plication of the diaphragm done with correction of malrotation which was diagnosed peroperatively. The association of these three anomalies are rare and to be kept in mind whenever treating a patient with bilateral eventration of the diaphragm.

RÉSUMÉ

We report eventration of right hemi-diaphragm resulting in gastric volvulus of the right sided stomach in an infant. The diagnosis of this rare association was made with contrast CT scan. Patient was initially managed with reduction of stomach, plication of right hemi diaphragm, anterior gastropexy and Ladds procedure, but required re-laparotomy after two months for recurrent volvulus.

RÉSUMÉ

The eventration of diaphragm is usually found incidentally on chest X-ray or sometimes presented as acute gastric volvulus. However, colonic volvulus on splenic flexure area complicated by diaphragmatic eventration is extremely rare. A 25 year old man complained of upper abdominal pain for three days. He had a history of brain injury during infant period, and had epilepsy and mental retardation. Plain chest X-ray showed left diaphragmatic eventration and marked dilatation of colon on splenic flexure area which had not been changed for last three years. Barium enema showed bird beak appearance on distal colon near the splenic flexure. Colonoscopic reduction failed. After decompression with rectal and nasogastric tubes, colonic volvulus was relieved. To prevent the recurrence of volvulus, we performed segmental resection of left colon including splenic flexure area and repaired the left diaphragmatic eventration. After the operation, the patient had no further recurrent episode of volvulus although ileus persisted.

RÉSUMÉ

Congenital eventration of the diaphragm is uncommon. Its association with a hiatal hernia has not been reported earlier. We report a case of such an association in a 2-month-old boy who presented with tachypnea aggravated by feeding and recurrent vomiting. Diagnostic uncertainty and a unique course of postoperative recovery complicated by gastric volvulus are described. The patient underwent surgical plication of the right diaphragm, followed by an emergency hiatal repair on the first postoperative day. Timely recognition, and repair of the hiatal hernia averted a fatal complication.

El síndrome de dificultad respiratoria del neonato es una entidad común en el niño de pretérmino, pero menos frecuente en el recién nacido que alcanzó la maduración pulmonar. Una causa excepcional de síndrome de dificultad respiratoria del neonato la constituye la eventración diafragmática, causada por una injuria del nervio frénico o por una deficiencia muscular congénita del diafragma. Se presenta el caso clínico de un recién nacido con síndrome de dificultad respiratoria del neonato que evolucionó favorablemente, en el que la imagenología evidenció una eventración diafragmática que luego permaneció asintomática sin tratamiento quirúrgico. Se destacan los elementos que permitan el diagnóstico diferencial entre hernia diafragmática, parálisis frénica, deficiencia muscular congénita del diafragma y las controversias respecto a las indicaciones de plicatura del diafragma en pacientes libres de síntomas

RÉSUMÉ

Dez pacientes portadores de vôlvulo gástrico foram estudados num período de 22 anos. O tipo mais comum de vôlvulo gástrico axial (70 por cento). Na maioria dos pacientes, o sintoma mais comumente encontrado foi a dor epigástrica. Todos os pacientes foram submetidos a gastropexia associada a correçäo da hérnia diafragmática. O índice de complicaçäo operatória foi de 10 por cento e näo houve recorrências
