Introdução:O envelhecimento da população está associado ao aumento da prevalência de demências, incluindo a doença de Alzheimer. O diagnóstico precoce é crucial para intervenções terapêuticas eficazes. Estudos recentes investigam o impacto da suplementação de ácidosgraxos ômega-3 na função cognitiva de idosos, devido à falta de tratamentos farmacológicos conhecidos para prevenir ou retardar o início da demência. Objetivo:Analisar os resultados de diversos estudos sobre a suplementação de ácidos graxos ômega-3 na função cognitiva de idosos. Metodologia:Revisão integrativa da literatura a partir das bases de dados BVS, PubMed e Scielo nos últimos dez anos (2013-2023), a partir dos descritores: (Fatty Acids, Omega-3) AND (Cognitive Aging) sem restrição quanto ao idioma e de acesso livre. Foram identificados 107 artigos, dos quais, 32 foram analisados e 14 utilizados nesta revisão. Resultados:O ômega-3, encontrado em peixes e oleaginosas, está associado à saúdecognitiva, especialmente os componentes EPA e DHA. Fatoresgenéticos, como o gene APO E4, podem influenciar sua eficácia na prevenção de doenças como Alzheimer. Estudos variam sobre seus benefícios na cognição em idosos, com resultados mistos. Conclusões:Os estudos revisados apresentam achados divergentes sobre os efeitos da suplementação de ácidos graxos ômega-3 na função cognitiva de idosos. Enquanto alguns sugerem benefícios, outros não identificam diferenças significativas (AU).


Introduction:The aging population is associated with an increased prevalence of dementia, including Alzheimer's disease. Early diagnosis is crucial for effective therapeutic interventions. Recent studies are investigating the impact of omega-3 fatty acid supplementation on the cognitive function of older adults, due to the lack of known pharmacological treatments to prevent or delay dementia onset. Objective:To analyze the results of various studies on omega-3 fatty acid supplementation in the cognitive function of older adults. Methodology:Integrative literature review from the databases BVS, PubMed, and Scielo in the last ten years (2013-2023), using the descriptors: (Fatty Acids, Omega-3) AND (Cognitive Aging) with no language restrictions and open access. A total of 107 articles were identified, of which 32 were analyzed, and 14 were used in this review. Results:Omega-3, found in fish and nuts, is associated with cognitive health, especially the EPA and DHA components. Genetic factors, such as the APO E4 gene, may influence its effectiveness in preventing diseases like Alzheimer's. Studies vary on its benefits in cognition in older adults, with mixed results. Conclusions:The reviewed studies present conflicting findings on the effects of omega-3 fatty acid supplementation on the cognitive function of older adults. While some suggest benefits, others do not identify significant differences (AU).


Introducción: El envejecimiento de la población está asociado con un aumento en la prevalencia de demencia, incluida la enfermedad de Alzheimer. El diagnóstico temprano es crucial para intervenciones terapéuticas efectivas. Estudios recientes investigan el impacto de la suplementación de ácidos grasos omega-3 en la función cognitiva de adultos mayores, debido a la falta de tratamientos farmacológicos conocidos para prevenir o retrasar el inicio de la demencia. Objetivo:Analizar los resultados de diversos estudios sobre lasuplementación de ácidos grasos omega-3 en la función cognitiva de adultos mayores. Metodología: Revisión integrativa de la literatura en las bases de datos BVS, PubMed y Scielo en los últimos diez años (2013-2023), utilizando los descriptores: (Ácidos Grasos Omega-3) Y (Envejecimiento Cognitivo) sin restricciones de idioma y de acceso abierto. Se identificaron un total de 107 artículos, de los cuales se analizaron 32 y se utilizaron 14 en esta revisión. Resultados: El omega-3, presente en pescados y frutos secos, está asociado con la salud cognitiva, especialmente los componentes EPA y DHA. Factores genéticos, como el gen APO E4, pueden influir en su eficacia para prevenir enfermedades como el Alzheimer. Los estudios varían en cuanto a sus beneficios en la cognición en adultos mayores, con resultados mixtos. Conclusiones:Los estudios revisados presentan hallazgos contradictorios sobre los efectos de la suplementación con ácidos grasos omega-3 en la función cognitiva de adultos mayores. Mientras que algunos sugieren beneficios, otros no identifican diferencias significativas (AU).

Background and Objectives@#Pruritus is a common and disabling symptom affecting as much as 50-90% of chronic kidney disease (CKD) patients undergoing dialysis. The pruritus experienced by these patients is often resistant to common anti-pruritic agents and has an overall negative impact on quality of life. With its antioxidant property and anti-inflammatory effects, omega-3 fatty acids have been used to alleviate pruritus. The objective of this study is to assess the effect of omega-3 fatty acid supplementation in reducing the severity of pruritus among dialytic CKD patients. @*Methods@#Various electronic databases were searched from inception to August 2022. Randomized controlled trials comparing the effect of omega-3 fatty acids versus placebo on the pruritus scores were included. The studies were independently assessed by three reviewers. Revman version 5.4 was used to analyze the data extracted from the studies while heterogeneity was evaluated using Chi2 and I2.@*Results@#A total of four studies with a population of 166 patients were included in the meta-analysis. The results show an overall beneficial effect of omega-3 fatty acids with a standardized mean difference of -1.40 (CI -1.74 to -1.05, Z=7.95, p value <0.00001). With a Chi2 of 2.91 (p=0.41) and I2 of 0%, there was no significant heterogeneity observed in the pooled analysis.@*Conclusion@#Overall, the results of the meta-analysis support the finding that omega-3 fatty acid supplementation may have a beneficial effect on reducing the severity of pruritus among CKD patients on dialysis.

Objetivo: Evidenciar através de uma revisão integrativa os resultados clínicos atuais do impacto do consumo de ômega 3 frente a depressão pós-parto. Método: Revisão integrativa da literatura realizada no período de Fevereiro a Julho de 2023 nas bases de dados Pubmed, LILACS, Medline e Scielo. Resultados:Foi realizada uma busca pelos descritores em saúde determinados e foram selecionadas 5 produções científicas que atenderam os critérios de inclusão. De modo geral, os trabalhos mostraram relações com a saúde do bebê e da mãe. No bebê, observou-se aumento do crescimento intrauterino, maior resposta do sistema nervoso central, melhor desenvolvimento neural, de retina, imunológico, cognitivo e físico. Já na saúde materna, observou-se aumento no processo antiinflamatório, melhor resposta imune, melhora no efeito neurotrófico do cérebro, aumento do metabolismo, melhora hormonal, menor risco cardiovascular, menores distúrbios neurológicos (incluindo a depressão) e distúrbios visuais. Conclusão:Mais estudos são necessários para elucidar os benefícios da suplementação de ômega-3 em gestantes no pós-parto


Objective: To show, through an integrative review, the current clinical results of the impact of omega 3 consumption on postpartum depression. Method:Integrative literature review carried out from February to July 2023 in the Pubmed, LILACS, Medline and Scielo databases. Results:A search was performed for specific health descriptors and 5 scientific productions that met the inclusion criteria were selected. In general, the studies showed relationships with the health of the baby and the mother. In the baby, there was an increase in intrauterine growth, greater response of the central nervous system, better neural, retinal, immunological, cognitive and physical development. In maternal health, there was an increase in the anti-inflammatory process, better immune response, improvement in the neurotrophic effect of the brain, increased metabolism, hormonal improvement, lower cardiovascular risk, lesser neurological disorders (including depression) and visual disturbances. Conclusion:More studies are needed to elucidate the benefits of omega-3 supplementation in postpartum pregnant women.


Objetivo: Mostrar, a través de una revisión integradora, los resultados clínicos actuales del impacto del consumo de omega 3 en la depresión posparto. Método:Revisión integrativa de la literatura realizada de febrero a julio de 2023 en las bases de datos Pubmed, LILACS, Medline y Scielo. Resultados:Se realizó una búsqueda de determinados descriptores de salud y se seleccionaron 5producciones científicas que cumplían con los criterios de inclusión. En general, los estudios mostraron relaciones con la salud del bebé y de la madre. En el bebé hubo un aumento del crecimiento intrauterino, mayor respuesta del sistema nervioso central,mejor desarrollo neural, retiniano, inmunológico, cognitivo y físico. En salud materna, hubo aumento del proceso antiinflamatorio, mejor respuesta inmunológica, mejora del efecto neurotrófico del cerebro, aumento del metabolismo, mejora hormonal, menor riesgo cardiovascular, menos trastornos neurológicos (incluyendo depresión) y alteraciones visuales. Conclusión:Se necesitan más estudios para dilucidar los beneficios de la suplementación con omega-3 en mujeres embarazadas posparto

Résumé

Los trastornos autoinmunes representan una familia de al menos 80 condiciones diferentes que surgen de una respuesta aberrante del sistema inmunológico resultando finalmente en la destrucción de tejidos y órganos específicos del cuerpo. Es importante destacar que durante las últimas tres décadas los estudios epidemiológicos han proporcionado evidencia de un aumento constante en la incidencia y prevalencia de trastornos autoinmunes. En los últimos años, varios estudios han demostrado que la vitamina D y los ácidos grasos poliinsaturados (AGPs) omega-3 ejercen propiedades inmunomoduladoras y antiinflamatorias sinérgicas que pueden aprovecharse positivamente para la prevención y el tratamiento de trastornos autoinmunes. En este sentido, el reciente ensayo clínico denominado VITAL (ensayo de vitamina D y omega 3); un estudio a gran escala, aleatorizado, doble ciego, controlado con placebo encontró que la suplementación conjunta de vitamina D y AGPs omega-3 (VIDOM) puede reducir la incidencia de enfermedades autoinmunes. En esta revisión de la literatura, resumimos los mecanismos moleculares detrás de las propiedades inmunomoduladoras y antiinflamatorias de la vitamina D y los AGPs omega-3, así como la posible interacción bidireccional entre el metabolismo de la vitamina D y el metabolismo de los AGPs omega-3 que justifica la co- suplementación VIDOM en trastornos autoinmunes(AU)


Autoimmune disorders represent a family of at least 80 different conditions that arise from an aberrant immune system response, which ultimately results in the destruction of specific body tissues and organs. It is important to highlight that during the last three decades epidemiological studies have provided evidence of a steady increase in the incidence and prevalence of autoimmune disorders. In recent years, several studies have shown that vitamin D and omega-3 polyunsaturated fatty acids (PUFAs) exert synergistic immunomodulatory and anti-inflammatory properties that can be positively harnessed for the prevention and treatment of autoimmune disorders. In this sense, the recent clinical trial called VITAL (Vitamin D and Omega 3 trial) - a large, randomized, double-blind, placebo- controlled study - found that co-supplementation of vitamin D and omega-3 PUFAs (VIDOM) can reduce the incidence of autoimmune diseases. In this literature review, we summarize the molecular mechanisms behind the immunomodulatory and anti-inflammatory properties of vitamin D and omega-3 PUFAs, as well as the possible bidirectional interaction between vitamin D metabolism and omega-3 PUFA metabolism that justifies VIDOM co- supplementation in autoimmune disorders(AU)

Résumé

Os distúrbios do olfato (DO) impactam de forma significativa na qualidade de vida dos indivíduos, e o conhecimento teórico a respeito do assunto deve ser de domínio dos alergologistas e imunologistas clínicos, possibilitando, assim, o seu diagnóstico e implementação de intervenções. Suas causas podem ser variadas, entre elas estão: rinite alérgica, rinossinusite crônica com ou sem pólipos, infecções de vias aéreas superiores, exposição a substâncias químicas, doenças neurológicas, drogas, traumas e o próprio envelhecimento. O olfato pode ser avaliado e mensurado através de testes com metodologias diferentes, cujo objetivo é avaliar parâmetros como a identificação de odores, limiar e discriminação olfativa. Esses testes são de fundamental importância para caracterizar objetivamente a queixa do paciente, como também avaliar o olfato antes e após determinada aplicação terapêutica. O tratamento das desordens olfativas é baseado em sua etiologia, portanto determinar a sua causa é indispensável para uma melhor eficácia no manejo. Entre as principais opções estão os corticoides tópicos, com impacto significativo nos pacientes com doença sinusal associada, treinamento olfatório e outras intervenções como ômega 3, vitamina A intranasal, e terapias que ainda requerem mais estudos.


Olfactory dysfunction significantly impacts quality of life, and allergists and clinical immunologists must be informed about it for diagnostic and interventional purposes. The causes are varied: allergic rhinitis, chronic rhinosinusitis with or without polyps, upper airway infections, exposure to chemicals, neurological diseases, drugs, trauma, and aging itself. Olfactory function can be evaluated and measured by several tests that use different methodologies to evaluate and identify odors, olfactory threshold, and olfactory discrimination. These tests are fundamental for objectively characterizing patient complaints and evaluating olfactory function before and after therapeutic interventions. Olfactory disorders are treated according to their etiology, so determining their cause is a major factor in treatment efficacy. The main options include topical corticosteroids, which have a significant impact on patients with sinus disease, olfactory training, other therapies (such as omega 3 and intranasal vitamin A), in addition to therapies that require further research.

Résumé

Fatty acids are considered metabolic intermediaries, although new facts indicate they also work as signaling molecules with different roles in the immune response. Based on that, in this study, we investigated the anti-inflammatory effects of n-3 polyunsaturated fatty acids (PUFAs) as eicosapentaenoic acid (EPA), docosahexaenoic acid (DHA), and α-linolenic acid (LNA) in ex vivo bovine endometrial explants. For this, two groups were formed: (1) LPS-challenged and (2) control, both to evaluate the accumulation of proinflammatory cytokines as interleukin 1ß (IL1B) and interleukin 6 (IL6). To develop the study, bovine female reproductive tracts from non-pregnant Angus heifers without evidence of reproductive diseases were selected. Endometrial explants were processed and treated for 24 h with EPA, DHA, and LNA in five different concentrations (0µM, 50µM, 100 µM, 200µM and 400 µM) and then, challenged with LPS for 24 h. Supernatants were collected to evaluate the concentration of IL1B and IL6 by ELISA. Explants treated with EPA from control groups reduced the concentrations of ILB (200µM) and IL6 (400 µM), and IL6 (50 µM; 100 µM) from the LPS-challenged group. DHA decreased the accumulation of IL1B and IL6 at 200 µM on explants from the LPS-challenged group, and 200 µM reduced IL6 from the control group. In contrast, explants treated with LNA only reduced the accumulation of IL1B to 400µM (from both groups). In conclusion, the EPA acid is the best anti-inflammatory option to decrease the concentration of both pro-inflammatory cytokines (IL1B and IL6) from LPS-challenged and control groups in bovine endometrial explants; while LNA evidence to be the last option to promote an anti-inflammatory response.(AU)


Os ácidos graxos são considerados intermediários metabólicos, embora novos fatos indiquem que eles também atuem como moléculas sinalizadoras com diferentes papéis na resposta imune. Dessa forma, este estudo investigou os efeitos anti-inflamatórios de ácidos graxos poliinsaturados n-3 (PUFAs) como ácido eicosapentaenóico (EPA), ácido docosahexaenóico (DHA) e ácido α-linolênico (LNA) em explantes endometriais ex vivo de bovinos. Para tal, o experimento foi divido em dois grupos: (1) Desafiado-LPS e (2) Controle, para que então pudesse avaliar o acúmulo de citocinas pró-inflamatórias como interleucina 1ß (IL1B) e interleucina 6 (IL6). Foram selecionados tratos reprodutivos de fêmeas bovinas de novilhas Angus não prenhes sem evidência de doenças reprodutivas. Explantes endometriais foram processados e tratados por 24h com EPA, DHA e LNA em cinco concentrações diferentes (0µM, 50µM, 100 µM, 200µM e 400 µM) e, em seguida, desafiados com LPS por mais 24h. Os sobrenadantes foram colhidos para avaliar a concentração de IL1B e IL6 pelo teste de ELISA. Os explantes tratados com EPA dos grupos de controle reduziram as concentrações de ILB (200 µM) e IL6 (400 µM) e no grupo desafiado com LPS houve redução das concentrações de IL6 (50 µM; 100 µM). Nos explantes do grupo desafiado com LPS, o DHA diminuiu o acúmulo de IL1B e IL6 nas concentrações de 200 µM, e no grupo controle reduziu IL6 nas concentrações de 200 µM, enquanto os explantes tratados com LNA reduziram apenas o acúmulo de IL1B a 400 µM (de ambos os grupos). Em conclusão, o ácido EPA provou ser a melhor opção anti-inflamatória para diminuir a concentração de ambas as citocinas pró-inflamatórias (IL1B e IL6) de grupos desafiados com LPS e controle em explantes endometriais bovinos; enquanto o LNA evidencia ser a opção menos viável para promover uma resposta anti-inflamatória.(AU)

Introdução - A dieta cetogênica (DC) tem sido utilizada como tratamento adjuvante da epilepsia fármaco resistente e tem se popularizado no manejo de outras doenças neurológicas e metabólicas (obesidade, diabetes e dislipidemias). Entretanto, também está associada a efeitos adversos que precisam ser melhor compreendidos. Objetivo - Avaliar o potencial efeito protetor de uma dieta cetogênica modificada (DCM), rica em ácidos graxos monoinsaturados (MUFAs) e ácidos graxos poli-insaturados (PUFAs), e suplementada com ácido docosahexaenóico (DHA), sobre a neuroinflamação e o estresse oxidativo e suas repercussões no comportamento de ratos. Materiais e Métodos - Estudo experimental com 18 ratos Wistar, alocados em 3 grupos de 6 animais. No grupo da dieta padrão (DP), os animais consumiram dieta comercial. No grupo da dieta cetogênica clássica (DCC), os animais consumiram uma dieta rica em gorduras saturadas. No grupo DCM, os animais consumiram dieta rica em MUFAs e PUFAs e enriquecida com DHA. Após 100 dias de intervenção foram realizados testes comportamentais e, após a eutanásia, foram feitos extratos lipídico e aquoso do lobo frontal e do hipocampo para análise de ácidos graxos e parâmetros oxidativos e inflamatórios. Resultados - Os animais DCC apresentaram um comportamento mais ansioso que ao animas DP, segundo os testes de Labirinto em Cruz Elevado e Medo ao Contexto. O grupo DCC teve mais incorporação de ácidos graxos saturados (AGs) no lobo frontal e no hipocampo em relação ao DCM. O grupo DCM teve maior incorporação de PUFAs, com destaque para o aumento de DHA no hipocampo. As citocinas inflamatórias IL1-ß e IL-6 tiveram menor concentração nos grupos DCM e DCC em relação ao grupo DP no lobo frontal, perfil semelhante ao observado com a IL-6 no hipocampo, e diferente da IL1-ß que foi reduzida significativamente apenas no DCM. Não houve diferença da concentração da Neuroprotectina D (NPD) e da enzima antioxidante Superóxido Dismutase (SOD) entre os grupos. A LDLox teve menor concentração nos grupos DCC e DCM em relação ao DP. A enzima catalase (CAT) foi menor no grupo DCM em relação ao grupo DP. Conclusão - Embora a modulação da composição de ácidos graxos da DC possa ser promissora, uma vez que a DCC induziu a um estado mais ansioso, não foi possível explicar essa diferença pelas vias inflamatória e antioxidante avaliadas.


Introduction - The ketogenic diet (KD) has been applied as an adjuvant treatment for drug-resistant epilepsy and has become popular in the management of other neurological and metabolic diseases (obesity, diabetes and dyslipidemia). However, it is also associated with adverse effects that need to be better understood. Objective - To evaluate the potential protective effect of a modified ketogenic diet (MKD), rich in monounsaturated fatty acids (MUFAs) and polyunsaturated fatty acids (PUFAs), and supplemented with docosahexaenoic acid (DHA), on neuroinflammation and oxidative stress and their repercussions on the behavior of rats. Materials and Methods - Experimental study with 18 Wistar rats, allocated into 3 groups of 6 animals. In the standard diet (SD) group, the animals consumed a commercial diet. In the classical ketogenic diet (CKD) group, the animals consumed a diet rich in saturated fats. In the MKD group, the animals consumed a diet rich in MUFAs and PUFAs and enriched with DHA. After 100 days of intervention, behavioral tests were carried out and, after euthanasia, lipid and aqueous extracts were made from the frontal lobe and hippocampus for analysis of fatty acids and oxidative and inflammatory parameters. Results - The CKD animals showed more anxious behavior than the DP animals, according to the Elevated Cross Maze and Context Fear tests. The CKD group had more incorporation of saturated fatty acids (SFAs) in the frontal lobe and hippocampus compared to MKD. The MKD group had greater incorporation of PUFAs, with emphasis on the increase in DHA in the hippocampus. The inflammatory cytokines IL1-ß and IL-6 had lower concentrations in the MKD and CKD groups compared to the PD group in the frontal lobe, a profile similar to that observed with IL-6 in the hippocampus, and different from IL1-ß, which was significantly reduced in the MKD. There was no difference in the concentration of Neuroprotectin D (NPD) and the antioxidant enzyme Superoxide Dismutase (SOD) between the groups. LDLox had a lower concentration in the CKD and MKD groups compared to the DP. The catalase enzyme was lower in the MKD group compared to the DP group. Conclusion - Although the modulation of the fatty acid composition of KD may be promising, since CKD induced a more anxious state, it was not possible to explain this difference through the inflammatory and antioxidant pathways evaluated.

Las enfermedades cardiovasculares son la principal causa de muerte a nivel mundial, manifestándose principalmente como enfermedad isquémica coronaria. El pronóstico y desenlace del Síndrome Coronario Agudo (SCA) depende en gran proporción de la disfunción endotelial asociado a este cuadro. Objetivo:Evaluar el efecto de los ácidos grasos omega­3, sobre la función endotelial en el Síndrome Coronario Agudo (SCA). Métodos: Se realizó estudio con 16 pacientes con síndrome coronario agudo (SCA) tratados con Omega-3 a dosis de 3 gramos/día+Terapia Convencional (O3+Tc) durante 30 días. Se les realizó a las 12 horas del ingreso y los a 30 días, perfil lipídico, proteína C reactiva ultrasensible (PCRus), Endotelina 1 (ET-1), y Péptido Natriurético cerebral (NTproBNP). Resultados: Durante 4 semanas de seguimiento, el uso de 3 gramos/día de ácidos grasos omega-3, añadido a la terapia convencional en el Síndrome Coronario Agudo, mostró una reducción significativa de la concentración plasmática de NTproBNP (p = 0,000), PCRus (p =0,000) y triglicéridos (p = 0,004). Conclusión: Tres gramos al día de ácidos grasos omega-3 contribuye a la reducción de la concentración plasmática de Triglicéridos y NTproBNP en pacientes con síndrome coronario agudo(AU)


Mainly as coronary ischemic disease. the prognosis and outcome of Acute Coronary Syndrome (ACS) depend largely on the endothelial dysfunction associated with this condition. Objective: To evaluate the effect of omega-3 fatty acids on endothelial function in Acute Coronary Syndrome (ACS). Method: A study was conducted with 16 patients with acute coronary syndrome (ACS) treated with Omega-3 at a dose of 3 grams/day+Conventional erapy (O3+Tc) for 30 days. Lipid profile, ultrasensitive C-reactive protein (usCRP), Endothelin 1 (ET-1), and Brain Natriuretic Peptide (NTproBNP) were performed 12 hours aer admission and 30 days later. Results: During 4 weeks of follow-up, the use of 3 grams/day of omega-3 fatty acids, added to conventional therapy in Acute Coronary Syndrome, showed a significant reduction in the plasma concentration of NTproBNP (p = 0.000), hsCRP (p =0.000) and Triglycerides (p = 0.004). Conclusion: three grams a day of omega-3 fatty acids contributes to the reduction of the plasmatic concentration of Triglycerides and NTproBNP in patients with acute coronary syndrome(AU)

Durante los últimos años se ha incentivado la suplementación con omega 3 durante el embarazo principalmente debido a la evidencia que mostraría beneficios en el desarrollo neuronal y visual del hijo en gestación, y a la prevención de patologías obstétricas asociadas a un aumento de la morbi-mortalidad perinatal. Los ácidos grasos poliinsaturados (PUFAs) omega 3, específicamente el ácido eicosapentaenoico (EPA) y el ácido docosahexaenoico (DHA), poseen propiedades antiinflamatorias, vasodilatadoras, además de propiedades anti-agregantes, las cuales han estimulado el uso de PUFAs en la prevención de enfermedades cardiovasculares. En esta revisión detallamos los efectos de la suplementación con omega 3 en diferentes aspectos del embarazo tales como la prevención del parto prematuro, preeclampsia, depresión post-parto y mejora del metabolismo durante la diabetes gestacional. Si bien existen diversos ensayos clínicos randomizados que estudian la suplementación con omega 3 durante la gestación, la evidencia sigue siendo no concluyente, debido a la variabilidad de las dosis y tiempo de administración. Ciertamente, un mayor número de estudios de calidad son necesarios para determinar el real impacto de la suplementación con omega 3 durante la gestación en la prevención de patologías obstétricas(AU)


During pregnancy, omega 3 supplementation has raised its popularity due to evidence that it would show not only benefits in the neural and visual development of the unborn child, but also in the prevention of obstetrical pathologies associated with of perinatal morbidity and mortality. Omega 3 polyunsaturated fatty acids (PUFAs), specifically, docosahexaenoic acid (DHA) and eicosapentaenoic acid (EPA), possess anti-inflammatory, vasodilatory and anti-aggregating properties, which have led to the use of PUFAs in the prevention of cardiovascular diseases. In this review, we detail the effects of omega 3 supplementation on different aspects of pregnancy such as prevention of preterm birth, pre-eclampsia, postpartum depression, and improved metabolism during gestational diabetes. Although there are several randomized clinical trials using omega-3 supplementation during pregnancy, the evidence remains inconclusive, due to variability in dosage and administration time. Certainly, a greater number of high-quality studies including randomized clinical trials are necessary to determine the impact of omega 3 supplementation during pregnancy in the prevention of obstetric pathologies(AU)

Introdução: A Doença Periodontal tem caráter multifatorial, já que depende de condições microbiológicas, imunogenéticas e sistêmicas do hospedeiro. Representa inflamação crônica das estruturas de suporte e proteção dental. Desencadeia uma complexa estimulação imunológica, bem como a produção de citocinas inflamatórias, que mediam a destruição óssea e de tecido conjuntivo, provocando perda dental e complicações à distância. A compreensão da etiopatogênese, permitiu os conceitos de modulação, que referem-se às modificações dos aspectos danosos da resposta inflamatória. Objetivo: O presente artigo tem como objetivo realizar uma revisão dos estudos sobre as principais terapêuticas adjuvantes na modulação da resposta imune frente à doença periodontal. Revisão de Literatura: Foi realizada uma revisão da literatura, onde foram selecionados artigos científicos em inglês, publicados entre os anos 2005 a 2020, por meio das bases de dados PubMed e ScienceDirect. No decorrer das buscas, foram utilizadas as palavraschaves "Inflamation", "Periodontal Disease", "Subantimicrobial Dose of Doxycycline", "Periodontal Disease", "Host Response Modulation". Resultados e Conclusão: A literatura é bem promissora em relação à terapia de controle complementar da doença periodontal. Dessa forma, novas pesquisas nessa área podem trazer inúmeros beneficos aos pacientes, sendo, assim, um novo caminho para o contorno da resistência bacteriana(AU)


Introduction: Periodontal disease has a multifactorial character, depending on the host's microbiological, immunogenetic and systemic conditions. It represents chronic inflammation of dental support and protection structures. It triggers a complex immune stimulation, as well as the production of inflammatory cytokines, which mediate bone and connective tissue destruction, causing tooth loss and complications at a distance. The understanding of etiopathogenesis allowed the concepts of modulation, which refers to the modifications of the harmful aspects of the inflammatory response. This article has the escape of conducting a review of studies on the main mechanisms of modulation against periodontal disease. Objective: This article aims to rev iew the studies on the main modulation mechanisms in the face of periodontal disease. Literature Review: A literature review was carried out in which scientific articles were selected in English, published between 2005 and 2020, through the PubMed and ScienceDirect databases. During the searches, the keywords "Inflammation", "Periodontal Disease", "Subantimicrobial Dose of Doxycycline", "Periodontal Disease", "Host Response Modulation". Results and Conclusion: The literature is very promising with complementary control therapy for periodontal disease. Thus, new research in this area can bring countless benefits to patients, thus being a new way to bypass bacterial resistance(AU)

Essential fatty acids are those that could not be synthesized by the body itself but crucial for health and life. Studies have shown that ω-3 fatty acids may facilitate human physiological functions. Mammals lack ω-3 desaturase gene, and the Δ15 fatty acid desaturase (Δ15 Des) from Caenorhabditis elegans can transform the ω-6 polyunsaturated fatty acids (PUFAs) into ω-3 PUFAs. Transgenic mice expressing Δ15 Des enzyme activity was constructed by using a PiggyBac transposon (PB). Homozygous transgenic mice with stable inheritance was bred in a short time, with a positive rate of 35.1% achieved. The mice were fed with 6% ω-6 PUFAs and the changes of fatty acids in mice were detected by gas chromatography (GC). The expression level of Δ15 Des in mice was detected by quantitative PCR (qPCR) and Western blotting (WB). qPCR and GC analysis revealed that the percentage of positive mice harboring the active gene was 61.53%. Compared with traditional methods, the transformation efficiency and activity of Δ15 Des were significantly improved, and homozygotes showed higher activity than that of heterozygotes. This further verified the efficient transduction efficiency of the PiggyBac transposon system.

RESUMEN: Objetivo: Determinar el efecto de omega-3 sobre la calidad/cantidad de sueño en humanos. Métodos: Se realizó la búsqueda sistemática de artículos (2010-2019), incluyendo pacientes sin limitación de edad/sexo, sometidos a cambio o suplemento de dieta con omega-3 y evaluaciones de sueño. La revisión se realizó bajo los criterios PICOT y PRISMA, la calidad de la evidencia y riego de sesgo fueron evaluados con criterios GRADE. Resultados: Seis ensayos superaron todos los filtros, dos estudios incluyeron pescado en la dieta y cuatro ingesta de cápsulas de omega-3. En cuatro estudios Omega-3 favorece el sueño presentando impacto moderadamente positivo en calidad y alto en cantidad. Los estudios fueron efectuados con variables de alta heterogeneidad, imposibilitando el análisis cuantitativo de los datos. El riesgo de sesgo fue moderado-alto. Conclusión: Omega-3, como suplemento dietético o directamente en los alimentos, interviene como coadyuvante para mejorar el sueño. No se pudo concluir sobre su recomendación de uso clínico en la gestión del sueño debido a la heterogeneidad de las metodologías para medir la intervención, las poblaciones estudiadas y cantidad de ingesta. Se necesitan estudios con mayor estandarización metodológica, para determinar dosis óptima, período de intervención y proporción de ácidos eicosapentaenoico/docosahexaenoico, necesarios para mejorar la calidad y la cantidad del sueño.


ABSTRACT: Objective: To determine the effect of omega-3 on the quality/quantity of sleep in humans. Methods: We conducted a systematic search for articles (2010-2019), including patients without age or sex limitation, undergoing omega-3 diet change or supplementation and sleep assessments. The review was conducted under the PICOT and PRISMA criteria, the quality of evidence and risk of bias were evaluated with GRADE criteria. Results: Six trials passed all filters. Two studies included dietary fish and four omega-3 capsule intake. In four studies, omega-3 favored sleep with moderately positive impact on quality and high impact on quantity. The studies were conducted with highly heterogeneous variables, making a quantitative analysis of the data impossible. The risk of bias was moderate to high. Conclusion: Omega-3 as a dietary supplement or directly in food intervenes as an adjuvant to improve sleep. We could not conclude on its recommendation for clinical use in sleep management due to the heterogeneity of the methodologies to measure the intervention, the populations studied and amount of intake. Studies with greater methodological standardization are needed, to determine optimal dose, intervention period and eicosapentaenoic/docosahexaenoic acid ratio, needed to improve sleep quality and quantity.

RESUMO O dualismo entre saúde física e mental vem sendo desfeito nos últimos anos em virtude das novas descobertas neurocientíficas. Vias biológicas que ajudam a entender a relação entre transtornos psiquiátricos e hábitos de vida têm sido estudadas. A ativação do sistema imune, uma dessas vias, e a inflamação crônica de baixo grau decorrente disso podem participar da gênese de transtornos depressivos, assim como de outras condições, podendo afetar até mesmo a produção de neurotransmissores. Em muitas dessas condições, observa-se a presença de marcadores inflamatórios tanto a nível periférico quanto central. Várias intervenções vêm sendo propostas para tratar os transtornos depressivos com componente inflamatório, e a dieta tem se mostrado um fator preventivo e terapêutico adjuvante significativo. Uma dieta balanceada e livre de alimentos processados tem se mostrado útil para reduzir a inflamação crônica e sutil de pacientes predispostos. A dieta mediterrânea, a qual já era vista como ideal por profissionais médicos de diversas áreas, tem sido estudada em psiquiatria e demonstra bons resultados. O uso de suplementos dietéticos e nutracêuticos, por sua vez, é controverso. Entretanto, o ômega-3 tem se mostrado benéfico para quadros depressivos, isso em razão, principalmente, de suas propriedades anti-inflamatórias. ABSTRACT The dualism between physical and mental health has been undone in recent years due to new neuroscientific discoveries. Biological pathways that help to understand the relationship between psychiatric disorders and lifestyle habits have been studied. Activation of the immune system, one of these pathways, and the resulting low-grade chronic inflammation may participate in the genesis of depressive disorders, as well as other conditions, and may even affect the production of neurotransmitters. In many of these conditions, the presence of inflammatory markers is observed both peripherally and centrally. Several interventions have been proposed to treat depressive disorders with an inflammatory component, and diet has been shown to be a significant preventive and therapeutic adjuvant factor. A balanced diet free of processed foods has been shown to reduce chronic and subtle inflammation in predisposed patients. The Mediterranean diet, which was already seen as ideal by medical professionals from different areas, has been studied in psychiatry and shows good results. The use of dietary supplements and nutraceuticals, in turn, is controversial. However, omega-3 has been shown to be beneficial for depressive conditions, mainly due to its anti-inflammatory properties. Abstract Background High dietary sodium intake can induce endothelial stiffness even without changes in blood pressure. Objectives To evaluate the effects of exercise training and chronic intake of sodium chloride solution on aortic morphology of male offspring of rat dams who consumed flaxseed during lactation. Methods Female rats were fed with a control diet or a flaxseed diet during lactation. At weaning, two male offspring of each rat dam were allocated into eight groups for 180 days: four groups received a control diet e four received a flaxseed diet, with /without exercise and with/without NaCl solution supply. Aorta was collected for histomorphometric analysis. The one-way analysis of variance was used and P value < 0.05 was considered statistically significant. Results The chronic use of 1% NaCl solution led to changes in aortic histoarchitecture in the control group: increase in aortic intima-media thickness (10,4%, p<0.0001) and reduced number of elastic lamellae (-8,1%, p<0.0001). Groups of offspring of mother that consumed flaxseed during lactation, the chronic use of 1% NaCl alone did not lead to an increase in the aortic intima-media thickness. Exercise training of adult offspring increased aortic intima-media thickness (13.3%, p<0.0001), with preservation of elastic components and aortic flexibility. Conclusion Chronic salt overload caused adverse effects on the aorta of rats, and maternal consumption of the flaxseed diet during lactation protected against aortic remodeling. La grasa alimentaria influye en la modulación de las funciones inmunitarias y los procesos inflamatorios; la mayor parte del impacto se atribuye a los ácidos grasos poliinsaturados de cadena larga (long-chain polyunsaturated fatty acids, LCPUFA, sus siglas en inglés). Los ácidos grasos esenciales (AGE), como el ácido linoleico (AL) y el ácido α-linolénico (AAL), que deben incorporarse por la dieta, son precursores de otros AG de gran importancia para el organismo. El AAL, perteneciente a la familia ω3, da origen a los ácidos eicosapentaenoico (EPA) y docosahexaenoico (DHA). Ellos confieren flexibilidad, fluidez y permeabilidad selectiva a las membranas lo que favorece la salud cardiovascular, reduce el riesgo de deficiencias en la visión y el desarrollo neural de bebés y niños, y de demencia en adultos mayores; algunos son precursores en la síntesis de prostaglandinas. También se observan efectos en la prevención y tratamiento de enfermedades coronarias, hipertensión, diabetes, artritis, inflamaciones, desórdenes autoinmunes y cáncer. Estos efectos pueden explicarse a través de las acciones específicas de cada uno de ellos1-3. El EPA ejerce: efecto hipotrigliceridémico a nivel de LDL y VLDL, efecto hipocolesterolémico por aumento de eflujo biliar y del transporte reverso de colesterol, y efecto antitrombótico por formación de eicosanoides de la serie 3. El DHA aumenta la fluidez de las membranas neuronales, gliales, y de conos y bastoncitos, disminuye la apoptosis neuronal, facilita el reciclaje de neurotransmisores, regula la expresión de enzimas involucradas en el metabolismo de lípidos como ligando de PPARs (peroxisome proliferator activated receptors) e inhibe la resistencia a la insulina a los tejidos musculares y adiposo3-7. Las recomendaciones de ingesta son: ácidos grasos poliinsaturados ω6: 2,5-9% de ingesta energética/diaria, y ácidos grasos poliinsaturados ω3: 0,6-2,0% de ingesta energética/diaria8


Omega 3 fatty acids are polyunsaturated fatty acids, essential since the human body does not produce them and they are obtained mainly from the diet. They confer flexibility, fluidity and selective permeability to the membranes, which favors cardiovascular health, reduces the risk of deficiencies in vision and neural development in infants and children, and dementia in older adults; some of them are precursors in the synthesis of prostaglandins. Some effects have also been seen in the prevention and treatment of coronary heart disease, hypertension, diabetes, arthritis, inflammation, autoimmune disorders, and cancer. These effects can be explained through the specific actions of each of them. Dietary fat influences the modulation of immune functions and inflammatory processes; most of the impact is attributed to long-chain polyunsaturated fatty acids (LCPUFA). The EPA exerts: hypotriglyceridemic effect at LDL and VLDL level; hypocholesterolemic effect due to increase in bile efflux and reverse cholesterol transport; antithrombotic effect due to the formation of Series 3 eicosanoids. DHA: increases the fluidity of neuronal, glial, and cone and rod membranes; decreases neuronal apoptosis; facilitates the recycling of neurotransmitters; regulates the expression of enzymes involved in lipid metabolism as a ligand for PPARs; inhibits insulin resistance to muscle and fat tissues. The intake recommendations are: 6 polyunsaturated fatty acids: 2.5-9% of energy intake/daily, and ω3 polyunsaturated fatty acids: 0.6-2.0% of energy intake/daily

Résumé

Nonalcoholic fatty liver disease (NAFLD) and hyperhomocysteinemia (HHcy) both are major health problems worldwide, whose incidence are closely related with each other. We previously reported the mechanism of HHcy-caused hepatic steatosis, but the role of n-3 polyunsaturated fatty acid (n-3 PUFA) in HHcy-induced hepatic steatosis remains unclear. In this study, 6-week-old C57BL/6 male mice were given a high methionine diet (HMD, 2% methionine diet), and plasma homocysteine levels were measured by ELISA to confirm the establishment of an HHcy model. Meantime, mice were fed HMD with or without n-3 PUFA supplement for 8 weeks to determine the role and mechanism of n-3 PUFA in hepatic steatosis induced by HHcy. Results showed that n-3 PUFA significantly improved hepatic lipid deposition induced by HHcy. qRT-PCR analysis demonstrated that n-3 PUFA inhibited the upregulation of Cd36, a key enzyme of fatty acid uptake, caused by HHcy. Further, the inhibition of hepatic Cd36 expression was associated with the inactivation of aryl hydrocarbon receptor (Ahr) induced by n-3 PUFA. Of note, mass spectrometry revealed that hepatic content of lipoxin A

Résumé

Resolution of inflammation plays an important part in maintaining homeostasis. It is an actively programmed progress involving multiple immune cells and mediators. Specialized pro-resolving mediators (SPMs) derived from Ω-3 polyunsaturated fatty acids include resolvins, protectins and maresins, and they exert abilities in the resolution of inflammation, host defense, organ protection, and tissue generation. Periodontitis is an inflammatory and destructive disease in the periodontal tissue initiated by dental plaque. Inadequate proinflammatory or proresolving responses, or the imbalance between the two, may contribute to the pathogenesis of the disease. Studies have shown that activating specialized receptors SPMs displayed multiple biological effects towards periodontitis, including resolution of inflammation, alveolar bone protection, periodontal tissue regeneration, and pathogen resistance. Thus, the relationship between SPM and periodontitis and the potentials and challenges in SPM application were reviewed.

Résumé

The evidences about the cardioprotective effects of omega-3 fatty acids (n-3 FA), especially EPA (eicosapentaenoic acid) and DHA (docosahexaenoic acid), have increased the consumption of these fatty acids. Echium plantagineum is a plant from Boragenacea family, known as potential source of non-marine omega-3 fatty acids (n-3 FA). Echium seeds presents 12-16% of stearidonic acid (SDA), that can be converted to EPA and DHA at a more elevated rate than the conversion obtained from α-linolenic acid (ALA), present in several other vegetable oils. However, echium oil is highly susceptible to oxidation because it has a high proportion of polyunsaturated fatty acids. Thus, the objective of this study was to combine three natural strategies to inhibit the oxidative damage in echium oil. In the first step, a mixture containing hydrophilic (HM: synaptic + ascorbic + citric acids) or lipophilic (LM: α-tocopherol + ascorbyl palmitate + citric acid) antioxidants was applied in the flaxseed oil, kept at 40oC/ 15 days. The oxidative markers were compared with the oil added of TBHQ (120 ppm) and EDTA (75 ppm), both artificial compounds. The results showed that LM and HM had an oxidative protection similar to the artificial antioxidants and that, HM promoted a better protection than LM. Based on this result, HM was selected as a strategy to be applied in the next step. In the second part of this study, Echium oil was obtained by two process: continuous screew pressing (PRESS) and extraction using hexane (SOLV). Both samples were added of HM combined with a high oleic sunflower oil and kept at different temperatures during storage. Two conditions were analyzed: 6 months into sealed flasks and 30 days into opened flasks. Oxidation reaction was followed by measuring the concentration of hydroperoxide, malondialdehyde, tocopherol and volatile compounds. In general, results showed that temperature reduction was enough to keep the oils stability during storage. Thus, the focus of the strategy's combination was directed toward samples after exposition to oxygen. In this context, better results were obtained by blending 20% of high oleic sunflower oil and the hydrophilic antioxidant mixture (500 ppm of synaptic acid, 250 ppm of ascorbic acid and 150 ppm of citric acid). In this condition it was observed 37-41% reduction in the hydroperoxide values and 40-75% in the malondialdehyde concentration in the samples prepared according to the optimized condition, when compared with the standard conditions by which the oil is currently extracted and processed


As evidências do efeito cardioprotetor dos ácidos graxos ômega 3 (AG n-3), principalmente do ácido eicosapentenoico (EPA) e docosahexaenoico (DHA), tem aumentado o consumo desses ácidos graxos. Echium plantagineum é uma planta da família Boragenacea, conhecida como uma fonte potencial AG n-3 de origem não marinha. As sementes de Echium apresentam 12-16% de ácido estearidônico (SDA), que pode ser convertido em EPA e DHA a uma maior taxa que a obtida através do consumo do ácido alfa linolênico (ALA), presente em diversos óleos vegetais. Porém, o óleo de echium é extremamente suscetível à oxidação, por ter um alto teor de ácidos graxos poli-insaturados. Portanto, o objetivo desse estudo foi combinar três estratégias naturais para inibir a oxidação no óleo de echium. Na primeira parte do estudo, misturas contendo antioxidantes hidrofílicos (HM: ácido sinápico + ácido ascórbico + ácido cítrico) ou lipofílicos (LM: alfa-tocoferol + palmitado de ascobila + ácido cítrico) foram aplicados no óleo de linhaça, e mantidos a 40oC por 15 dias. Os marcadores de oxidação foram comparados com óleo de linhaça no qual foram adicionados compostos artificiais: TBHQ (120 ppm) e EDTA (75 ppm). Os resultados mostraram que LM e HM apresentaram uma proteção antioxidante similar ao efeito apresentado pelos compostos artificiais, e que a mistura HM promoveu uma melhor proteção antioxidante que a mistura LM. A partir desse resultado, a mistura HM foi selecionada como estratégia a ser aplicada na etapa seguinte. Assim, na segunda parte do estudo, o óleo de echium foi obtido por dois processos de extração: prensagem mecânica continua (PRESS) e extração usando hexano (SOLV). A mistura HM e o óleo de girassol alto oleico foram selecionados como estratégias antioxidantes, além da redução de temperatura de estocagem. Duas condições foram analisadas: 6 meses em frascos fechados e 30 dias em frascos abertos. A oxidação foi quantificada através da determinação das concentrações de hidroperóxido, malonaldeído, tocoferol e compostos voláteis. No geral, os resultados mostraram que a redução de temperatura foi suficiente para manter a estabilidade do óleo durante o estoque. Portanto, objetivou-se combinar estratégias para aumentar a estabilidade oxidativa das amostras expostas ao oxigênio. Neste contexto, os melhores resultados foram obtidos quando 20% de óleo de girassol alto oleico foi combinado com a mistura hidrofílica de antioxidantes naturais (500 ppm de ácido sinápico, 250 ppm de ácido ascórbico e 150 ppm de ácido cítrico). Nessa condição, foi observada uma redução de 37-41% nos valores de hidroperóxidos e 40-75% na concentração de malonaldeído, quando comparado com a condição padrão
