Résumé
We report a case of a rare disease, Grisel syndrome, which manifests as a kind of rotational fixation of the atlas on the axis, resulting from an infectious process of the upper airways. In the present report, we discuss etiology, clinical presentation, diagnosis, treatment and outcome after intervention.

Retropharyngeal calcific tendinitis is defined as inflammation of the longus colli muscle and is caused by the deposition of calcium hydroxyapatite crystals, which usually involves the superior oblique fibers of the longus colli muscle from C1-3. Diagnosis is usually made by detecting amorphous calcification and prevertebral soft tissue swelling on radiograph, CT or MRI. In this report, we introduce a case of this disease which was misdiagnosed as a retropharyngeal tuberculous abscess, or a muscle strain of the ongus colli muscle. No calcifications were visible along the vertical fibers of the longus colli muscle. The lesion was located anterior to the C4-5 disc, in a rheumatoid arthritis patient with atlantoaxial subluxation. Calcific tendinitis of the longus colli muscle at this location in a rheumatoid arthritis patient has not been reported in the English literature.

Three rare cases of stenosis of spinal canal at the level of atlas associated with atlantoaxial dislocation are presented. An atlantoaxial lateral mass fixation with plate and screws after posterior midline bony decompression was successfully performed in these cases.

A artrite reumatóide pode lesar as articulaçöes, ligamentos e ossos da coluna cervical, podendo ocasionar: subluxaçäo atlanto-axial, impacçäo atlanto-axial e subluxaçäo subaxial. Em geral o tratamento conservador é o preferido, mas há indicaçöes para tratamento cirúrgico nas seguintes subluxaçöes: 1) déficit neurológico importante, especialmente quando progressivo; 2) dor intensa; 3) grandes instabilidades. Os autores apresentam 15 casos de artrite reumatóide acometendo a coluna cervical que foram tratados cirurgicamente, sendo cinco subluxaçöes atlanto-axiais, duas impacçöes atlanto-axiais e oito subluxaçöes subaxiais
