Résumé

Se realizó una revisión bibliográfica y actualización sobre el control de pacientes con condrosarcoma. Se analizaron aspectos importantes como clasificación, sexo, edad, localización, cuadro clínico, imagenología; con énfasis en los estudios gammagráficos de Morphey, anatomía patológica, diagnóstico diferencial, tratamiento quirúrgico y complicaciones. En el tratamiento quirúrgico se hizo referencia fundamentalmente a la utilización de diferentes técnicas basadas en su comportamiento histológico y radiológico. Por último se expusieron las complicaciones más importantes de este tumor que fueron la recurrencia y la metástasis, también se explicaron algunos factores relacionados.


A bibliographic review and an updating of the control of patients with chondrosarcoma were made. Important aspects such as classification, sex, localization, clinical picture, imaging with emphasis in Morphey's gammagraphic studies, pathological anatomy, differential diagnosis, surgical treatment and complications, were analyzed. In the surgical treatment, reference mainly was made to the utilization of the different techniques based on their histological and radiological behaviour. The most important complications of this tumor, which are recurrence and metastasis, were exposed, and some related factors were also explained.

Résumé

A 20-year-old male reported with right-sided facial swelling, epistaxis and right sided proptosis of two months duration. Computerized tomography scan of the face revealed a mass in the right maxillary sinus. The patient underwent surgery and the postoperative histopathology was suggestive of dedifferentiated chondrosarcoma. The present report discusses this rare presentation and the issues in its management.

Presentamos un caso de un condrosarcoma mesenquimal primario de hueso occipital, en una paciente de 21 años, operada en esta institución, a la que se le realizó resección casi total del tumor, recibiendo en forma completa radioterapia y quimioterapia; se produjo recidivia tumoral 30 meses después, siendo reintervenida quirúrgicamente. Revisamos la literatura de esta rara neoplasia primaria de cráneo

Résumé

Between June 1985 and March 1990, 25 patients with primary malignant bone tumors, including 15 cases of osteogenic sarcoma, two cases of periosteal osteogenic sarcoma, six cases of chondrosarcoma, and two cases of malignant fibrous histiocytoma, were treated with limb-salvage procedures. Fourteen patients underwent limb salvage operations with tumor prosthesis arthroplasty, 6 with resection-arthrodesis, 4 with wide resection, and 1 with autoclaved autograft. Pre- and/or post-operative chemotherapy with or without radiotherapy was combined with these limb salvage operations. The average follow-up period was 25.2 months (6 to 52 months) since diagnosis. The estimated 3.5-year survival rate of the total 25 patients was 39.5% based on the Kaplan-Meier survival plot. In the 15 cases of osteogenic sarcoma, the estimated Kaplan-Meier 34-month survival rate was 46.2%. Seventeen patients who were followed up for more than 1 year were grouped by functional grading: 11 (65%) were excellent; 3 (18%) good; 2 fair (12%); and 1 poor. Complications occurred in 4 patients, 1 had a local recurrence, 2 had superficial wound infections, and 1 a loosening of the femoral stem. Metastases were shown in 15 case, and the most common location was the lung.
