RESUMO Objetivo: Descrever as características clínico-epidemiológicas, técnicas cirúrgicas e resultado do tratamento das lacerações canaliculares operadas em nosso serviço. Métodos: Estudo retrospectivo, realizado de janeiro de 2012 a junho de 2020, considerando-se as lesões de canalículo lacrimal operadas em um serviço de referência. Dados demográficos, características das lesões, detalhes das cirurgias e resultado do tratamento foram obtidos de prontuários eletrônicos e analisados estatisticamente. Resultados: Foram incluídos 26 portadores de lesões canaliculares, com idade entre 2 e 71 anos, sendo 73,1% homens. A lesão acometia o canalículo superior em 53,9%; 80,8% pacientes procuraram pelo serviço nas primeiras 24 horas, e 46,2% tiveram a cirurgia realizada entre 24 e 72 horas após o traumatismo. Todos os pacientes tiveram intubação mono ou bicanalicular, e o tempo transcorrido entre a cirurgia e a retirada do silicone, variou de zero a 183 dias. Após a cirurgia, 21 pacientes (80,8%; p<0,05) não apresentaram complicações, 2 (7,7%) evoluíram com obstrução canalicular, 2 (7,7%) com granuloma e 1 (3,8%) com ectrópio de ponto lacrimal. Conclusão: As lesões de canalículo encontradas em nossa prática são mais comuns em crianças ou homens jovens, acometem mais o canalículo superior e as nossas condutas levam a sucesso no tratamento na maior parte dos casos. As grandes controvérsias no assunto persistem, como o tipo e o tempo de permanência do tubo de silicone na via lacrimal. Somente estudos com grandes amostras podem consolidar esses conceitos.


ABSTRACT Objective: To describe the clinical and epidemiological characteristics, surgical techniques, and results of the canalicular laceration treatment at our service. Methods: A retrospective study was carried out from January 2012 to June 2020, considering canalicular injuries operated at a reference center. Demographic data, lesion characteristics, surgical details, and treatment outcomes were obtained from electronic medical records and were statistically analyzed. Results: Twenty-six cases of people with canalicular lesions aged between 2 to 71 years old were included, of whom 73.1% were men. The superior canaliculus was affected in 53.9%; 80.8% of patients searched for care within the first 24 hours, and 46.2% had the surgery performed between 24-72 hours after trauma. All patients had mono or bicanalicular intubation and the time elapsed between surgery and silicone removal ranged from 0 to 183 days. After surgery, 21 patients (80.8%, p<0.05) did not present any complications, two (7.7%) evolved with canalicular obstruction, two (7.7%) with granuloma, and one (3.8 %) with lacrimal puncta ectropion. Conclusion: In our practice, canalicular injuries are more common in children or young men, affecting mainly the superior canaliculus, and treatment success using our approach can be achieved in most of the cases. However, great controversies remain on the subject, such as type of intubation and when to remove the silicone tube from the lacrimal pathway. Larger series are required to consolidate controversial concepts.

Abstract Objective: To evaluate the clinical effectiveness of amniotic membrane transplantation for ocular surface reconstruction. Methods: Prospective study including 23 eyes of 21 patients who underwent amniotic membrane transplantation at Hospital de Clínicas da Universidade Federal do Paraná (HC-UFPR) and at Cirurgia e Diagnose em Oftalmologia do Paraná (CDOP) clinic, located in Curitiba, PR, Brazil, from may 2015 to july 2019. The amniotic membrane was collected from elective and term cesarean delivery, and conserved in preservation medium and glycerol 1:1, stored at -80° Celsius. The membrane was fixed on the ocular surface with 10-0 nylon, 8-0 vicryl, biological glue or a combination of these materials. Results: The ocular surface reconstruction was successful in 22 eyes (95.6%). Failure was observed only in 1 case (bullous keratopathy) in which the condition was maintained postoperatively. Patients' age ranged from 11-82 years, with a mean age of 37.4 years. There was a higher incidence in males (66.6%). A difference was perceived in the distribution of the affected eye (which was greater in the right eye - 65.2%). As for the previous ophthalmic surgery history, 12 of the 23 eyes had a positive history (52.2%). It was observed that all patients who had preoperative visual acuity assessed showed improvement or maintenance of corrected visual acuity. In the postoperative period, complications associated with the underlying disease were observed, although not particularly related to the amniotic membrane transplantation. There were not any cases of postoperative infection. Conclusions: There was an improvement in the general state of the ocular surface in almost all of the cases in which the transplant was performed. Therefore, the amniotic membrane can be considered a good alternative for reconstructing the ocular surface, as a single or supporting treatment.


Resumo Objetivo: Avaliar a eficácia clínica do transplante de membrana amniótica na reconstrução da superfície ocular. Métodos: Estudo prospectivo incluiu 23 olhos de 21 pacientes que realizaram transplante de membrana amniótica no Hospital de Clínicas da Universidade Federal do Paraná (UFPR) e na clínica de Cirurgia e Diagnose em Oftalmologia do Paraná (CDOP), localizados em Curitiba, PR, Brasil, no período de maio de 2015 a julho de 2019. A membrana amniótica foi captada a partir de parto cesárea eletivo e a termo, conservada em meio de preservação e glicerol 1:1 e armazenada a -80° Celsius. A membrana foi fixada na superfície ocular com fio nylon 10-0 ou vicryl 8-0 e/ou cola biológica. Resultados: A idade dos pacientes variou de 11-82 anos, com média de 37,4 anos. Houve maior incidência no sexo masculino (66,6%). Ocorreu diferença na distribuição do olho acometido (maior no olho direito - 65,2%). Quanto à história de cirurgia oftalmológica prévia, 12 dos 23 olhos tinham história positiva (52,2%). Observamos que nos pacientes em que foi possível a avaliação da acuidade visual pré-operatória, todos apresentaram melhora ou manutenção da acuidade visual. No pós-operatório foi observado complicações associadas à doença de base e não propriamente ao transplante de membrana amniótica. Não foram registrados casos de infecção pós-operatória. Conclusão: Houve melhora do estado geral da superfície ocular em quase totalidade dos casos em que o transplante foi realizado. Portanto, a membrana amniótica pode ser considerada uma boa alternativa para reconstrução da superfície ocular, como tratamento único ou coadjuvante.

RESUMEN * Introducción: En el Hospital General Docente "Dr. Agostinho Neto" se desconocen las características de los pacientes atendidos por trauma ocular. Objetivo: Caracterizar el trauma ocular en pacientes ingresados en el servicio de Oftalmología del citado hospital en el período de enero de 2014 a julio de 2019. Método: Se realizó un estudio descriptivo, retrospectivo y longitudinal del total de pacientes con este diagnóstico (n=49). Se estudiaron las variables: sexo, edad, procedencia, ocupación, tipo de trauma, diagnóstico y complicaciones, y modificaciones de la agudeza visual de acuerdo al tratamiento que aplicó. Resultados: El 79,0 % de los pacientes fueron hombres, tenían entre 19 y 40 años de edad (43,0 %), procedían de áreas rurales (57,0 %) y se dedicaban a labores agrícolas (45,0 %). El 76,0 % presentó un trauma ocular abierto, la herida corneal fue el más usual (55,1 %). El 95,9 % de los pacientes recibieron tratamiento quirúrgico y reparación de la herida el 83,6 %. Antes y después del tratamiento, el 47,0 % y el 41,0 % respectivamente de los pacientes presentaron agudeza visual y visión cuenta dedos a percepción luminosa. La catarata traumática fue la complicación más frecuente (33,0 %). Conclusiones: El trauma no es un problema de salud, pero sí limita la calidad de vida de los perjudicado al afectar la agudeza visual.


ABSTRACT * Introduction: The characteristics of the patients with eye trauma treated in the General Teaching Hospital Dr. Agostinho Neto remain mostly unknown. Objective: To characterize eye trauma in the patients in the ophthalmology consultation in the mentioned institution, in the stretch of time between January 2014 to July 2019. Method: A descriptive, retrospective and longitudinal study was carried out on the total amount of patients with the diagnosis (n=49). The different variables taken into account were: gender, age, place of origin, occupation, type of trauma, diagnosis and complications, and the modifications in visual acuity according to the treatment given. Results: 79.0 % of the patients were men, with ages ranging between 19 to 40 years old (43.0 %), from rural areas (57.0 %), and involved in agricultural works (45.0 %). 76.0 % were open eye traumas, being corneal wounds the most common one (55.1%). 95.9 % of the patients were surgically treated, and 83.6 % went through wound healing processes. Before and after the treatment, the 47.0 % and the 41.0 % of the patients, respectively, presented visual acuity and good 'counting fingers' and 'visual perception' tests results. Traumatic cataracts were the most common complication (33.0 %). Conclusions: eye traumas are not a significant health issue, but impacts directly in the life quality of the affected, damaging their visual acuity.

Résumé

O xantogranuloma juvenil (XGJ) é um tumor benigno e o mais comum do grupo das doenças histiocitárias proliferativas nãoLangerhans. Lesões; 2cm são consideradas XGJ gigantes, com relatos de lesões de até 18cm. Lesões oculopalpebrais podem necessitar de tratamento cirúrgico para controle de sintomas. Esse trabalho relata o caso de um menino de 8 anos que teve as 4 pálpebras acometidas por XGJ gigantes, além do terço médio. Ele foi submetido a 3 ressecções, sendo uma bastante profunda, necessitando enxerto de pele de espessura total diretamente sobre o músculo levantador da pálpebra superior. Posteriormente, 3 procedimentos de lipoenxertia foram realizados, atingindo resultado funcional e estético adequado, sem recorrência lesional.


Juvenile xanthogranuloma (JXG) is the most common benign tumor of the group of non-Langerhans histiocytic proliferative diseases. Lesions >2 cm are considered giant JXG, with reports of lesions of up to 18 cm. Oculopalpebral lesions may require surgical treatment to control symptoms. El trauma ocular es un problema significativo en todo el mundo. El objetivo de este estudio fue determinar las características de los pacientes menores de 14 años operados de catarata traumática en la Unidad de Oftalmología Pediátrica, Estrabismo y Neurooftalmología "Dra. Ana María Illescas Putzeys", Hospital de Ojos y Oídos "Dr. Rodolfo Robles V.". Se realizó un estudio retrospectivo, descriptivo, observacional, en el que se revisaron expedientes de pacientes operados de catarata de 2010 a 2015. Se operaron 54 niños por catarata traumática, de los cuales 75% eran hombres. El 68% pertenecían al rango de edad de 6-11 años. El tipo de trauma fue cerrado en un 57,4%. Los pacientes que lograron una agudeza visual igual a 20/70 o mayor fueron el 68,2%.


Ocular trauma is a significant problem worldwide. The obj ective of this study was to determine the characteristics of patients younger than 14 years who underwent traumatic cataract surgery at the Unit of Pediatric Ophthalmology, Strabismus and Neuro-Ophthalmology "Dra. Ana María Illescas Putzeys" of Hospital de Ojos y Oídos "Dr. Rodolfo Robles V." A retrospective, descriptive, and observational study was carried out to review the medical records of patients who underwent cataract surgery between 2010 and 2015. A total of 54 children underwent traumatic cataract surgery; 75% were boys; 68% were in the 6-11-year-old age range. Blunt force trauma was observed in 57.4%. A visual acuity of 20/70 or better was achieved by 68.2% of patients.

ABSTRACT Post-traumatic aniridia combined with aphakia may be seen after globe injury. Aside from esthetic aspects, partial or total loss of the iris tissue may also be related to various degrees of glare and photophobia. Such patients suffer from severe visual impairment secondary to aphakia. Herein we describe a novel surgical technique for the management of an aphakic eye with traumatic aniridia for a patient who underwent transscleral fixation of a custom-tailored artificial iris prosthesis combined with a rigid intraocular lens (IOL). Tight suturing of the IOL haptic eyelets on the silicone iris prosthesis and fixation of such a complex to the scleral wall may provide excellent cosmetic and functional outcomes in aphakic eyes with aniridia.


RESUMO Aniridia pós-traumática combinada com afacia pode ser observada após lesões do globo ocular. Além do ponto de vista estético, a perda parcial ou total do tecido da íris também pode estar relacionada com vários graus de ofuscamento e fotofobia. Estes pacientes sofrem de deficiência visual grave secundária a afacia. Relata-se uma técnica cirúrgica inovadora para tratamento de um olho com afacia associada à aniridia traumática que foi submetido à fixação transescleral de uma prótese de íris artificial feita sob medida combinada com uma lente intraocular rígida (IOL). A sutura das alças da IOL sobre a prótese iriana de silicone, e a fixação desse complexo na parede escleral podem proporcionar excelente resultado estético e funcional em olhos afácicos com aniridia.

RESUMO Objetivo: Avaliar casos de trauma ocular relacionados à queda da própria altura em idosos e comparar a prevalência e gravidade dos atendimentos. Métodos: Foi realizado uma série de casos em 52 pacientes com 60 anos ou mais dentro do período de 36 meses, compreendido entre janeiro de 2012 a dezembro de 2014, com história de trauma ocular relacionado à queda da própria altura, onde foi avaliada a prevalência conforme o sexo, necessidade de internação e de cirurgia, sequela visual e a gravidade dos casos. Resultados: Trinta e três (63,5%) dos 52 pacientes eram do sexo feminino, sendo 30,3% desses com necessidade cirúrgica e 18,2% com sequelas visuais, outros 19 (36,5%) eram do sexo masculino, sendo 42,1% desses com necessidade cirúrgica e 26,3% com sequelas visuais. Conclusão: Foi mais prevalente o atendimento de casos de trauma ocular relacionado à queda da própria altura em mulheres, mas a gravidade dos atendimentos foi maior nos homens.


ABSTRACT Objective: Evaluate ocular trauma cases related to falling in elderly patients e compare the prevalence and severity of the cases. Methods: A series of cases was made with 52 patients aging 60 or more within the period of 36 months presenting ocular trauma related to falling, whereas the prevalence between the gender, the need for hospitalization or surgery and subsequent visual deficit were evaluated, as well as the severity of the cases. Results: Thirty-three (63.5%) of 52 patients were from the female gender, over which 30.3% had need for surgery and 18.2% developed visual deficit and 19 (36.5%) were from the male gender where 42.1% needed surgery and 26.3% developed visual deficit. Conclusions: The study has shown a higher prevalence of cases in the female gender, although the severity was higher in the male gender.

Résumé

Objetivos: determinar la frecuencia de evisceración según edad, sexo, ojo afectado, criterio quirúrgico y causa más frecuente. Métodos: se realizó un estudio descriptivo, longitudinal y retrospectivo a través de la revisión de las historias clínicas de 89 pacientes a quienes se les realizó la evisceración en el período de enero 2002 a diciembre 2011 en el Hospital Militar Central Dr. Luis Díaz Soto. Se aplicaron métodos de estadística descriptiva para el procesamiento de los datos. Resultados: predominó el grupo de edad de 31-40 años con frecuencia absoluta (20), que constituyó el 22,4 por ciento de la muestra; el sexo masculino representó el 67,4 por ciento; predominó el ojo izquierdo en los casos eviscerados con 53 por ciento y la forma electiva de tratamiento con 78,7 por ciento. La causa más frecuente fue la ptisis bulbi con 32,7 por ciento. Conclusiones: son eviscerados con mayor frecuencia los ojos izquierdos de adultos jóvenes del sexo masculino, con criterio electivo de tratamiento quirúrgico. La ptisis bulbi es la causa más frecuente de evisceración(AU)


Objectives: to determine the evisceration frequency according to age, sex, affected eye, surgical approach and more frequent cause. Methods: descriptive, longitudinal and retrospective study was done through the revision from the clinical histories of 89 patients to all of them were applied the evisceration in the period of January 2002 to December 2011 in the Central Military Hospital Dr. Luis Díaz Soto. Methods of descriptive statistic were applied for the prosecution of the data. Results: there was bigger incidence in the 31-40 year-old group representing 22,4 percent of the sample, the male sex represented 67,4 percent, the left eye prevailed with 53 percent in the cases evisceration and the elective way of treatment with 78,7 percent. The most frequent cause was the ptisis bulbi occupying 32,7 percent. Conclusions: evisceration was done frequently in left eyes of young men, with elective approach of surgical treatment. The ptisis bulbi was the most frequent cause in evisceration(AU)

Résumé

Objetivo: Ilustrar las consecuencias oculares de pacientes que han sufrido trauma ocular con Paintball. Diseño: serie de casos. Materiales y Métodos: se realizó una revisión de las historias clínicas de pacientes tratados en el servicio de oftalmología del Hospital Militar Central que habían sido examinados por presentar trauma ocular secundario a lesiones por paintball. Se realizó seguimiento de los casos por 12 meses. Resultados: se evaluó la magnitud del trauma ocular secundario a paintball encontrándose muy baja recuperación de la agudeza visual y la necesidad de cirugía ocular en todos los pacientes Se determinó la severidad y extensión del daño ocular asociado a esta lesión.


Objective: to show the eye consequences of a group of patients who suffered eye trauma because of paintball. Design: case series. Materials and methods: through revision of medical records, information from patients treated at the ophthalmology area of the Hospital Militar Central, who presented a diagnosis of eye trauma caused by playing paintball, during 2011. We followed up the cases through 12 months of treatment. Results: We evaluated the magnitude of eye trauma produced by paintball, finding a very low final visual recovery and also the need for surgery in all patients. Determining the extent and severe eye damage caused by the trauma of paintball.

Résumé

Paciente masculino de 27 años de edad, trabajador agrícola, que fue ingresado en el Hospital Militar Central "Dr. Carlos J. Finlay" por haber sufrido una herida penetrante por arma de fuego, producida por la impulsión retrógrada del percutor al disparar una escopeta artesanal de cacería, que provocó estallido del globo ocular izquierdo, fractura de la pared externa de la órbita y heridas palpebrales. Se retiró el cuerpo extraño, se completó la evisceración del ojo izquierdo, se recolocó el fragmento fracturado de la órbita en posición anatómica, se regularizaron los tejidos blandos intra y periorbitarios, preparando la cavidad orbitaria para la colocación posterior de la prótesis; y se realizó la hemostasia, la exploración de la cavidad neoformada por el cuerpo extraño en los tejidos blandos de la región fronto-temporal y la plastia palpebral. Se logró con éxito la reconstrucción palpebral y con posterioridad, la rehabilitación con fines estéticos mediante prótesis ocular.


A 27 year-old male farmer patient was admitted to "Dr. Carlos J. Finlay" Central Military Hospital suffering a penetrating gunshot wound, caused by the retrograde hammer when shooting a handmade hunting shotgun. This injured caused his left eyeball burst, fracture of the outer wall of the orbit and eyelid injuries. Foreign matter was removed. His left eye evisceration was completed. The fractured orbit fragment was repositioned in anatomical position. Intra and periorbital soft tissues were regularized, preparing the orbital cavity for the subsequent placement of prosthesis; and hemostasis was performed, as well as exploration the newly formed cavity by foreign matter in the soft tissues of the frontal-temporal region and eyelid plasty. Eyelid reconstruction and further rehabilitation for aesthetic purposes by ocular prosthesis was successfully achieved.

Résumé

Cyclodialysis is a relatively rare condition usually caused by ocular injury; however, it can also be caused iatrogenically during intraocular surgery. Hypotony maculopathy is the most important complication and the primary reason for visual loss. Clinical diagnosis using gonioscopy may be difficult, and ultrasound biomicroscopy (UBM) can be an alternative. There are different kinds of treatments, and the optimal one remains controversial. Here we describe a case of traumatic cyclodialysis with persistent ocular hypotony treated by direct cyclopexy, as illustrated by UBM performed before and after surgery.


Ciclodiálise é uma condição relativamente rara, geralmente devido a um trauma ocular, mas também pode ser causada iatrogenicamente como consequência de cirurgia intraocular. A maculopatia hipotônica é a complicação mais importante e a principal razão para a perda visual nessa situação. O diagnóstico clínico por gonioscopia pode ser difícil e a biomicroscopia ultrassônica (UBM) pode ser uma alternativa. Existem diferentes tipos de tratamentos e algumas controvérsias sobre a melhor opção. Neste relato, nós descrevemos um caso de ciclodiálise traumática com hipotonia ocular persistente tratado por cyclopexia direta avaliado por UBM antes e depois da cirurgia.

PURPOSE: In this study, we examined the stability of the lens-angle supporter (LAS) for accommodation restoration by comparing intraocular lens (IOL) location, after-cataract and ciliary body damage after cataract surgery in rabbits. METHODS: Eight rabbits were divided into experimental and control groups of four rabbits each. Phacoemulsification and irrigation and aspiration were performed in all rabbits. This was followed by an LAS and IOL insertion in the four experimental rabbits. In the four control rabbits, only an IOL insertion was performed. Six months after the surgery, the location of the IOL, the conditions of the lens capsule and ciliary body were evaluated using a slitl-amp examination and Miyake-Apple view. RESULTS: For the experimental group, the ultrasound biomicroscope results showed normal LAS and IOL positioning in all four cases. According to the slitlamp examination and Miyake-Apple view, the IOL was positioned at the center, with less after-cataract and damage to the ciliary body. For the control group, ultrasound biomicroscope results indicated a higher IOL position than normal, as well as a single case of IOL decentering. According to the slit-lamp examination and Miyake-Apple view, the IOL was decentered with more severe after-cataract and ciliary body damage. CONCLUSIONS: The LAS has the potential to maintain a stable IOL position while producing less after-cataract when used in lens-angle reconstruction for correction of presbyopia. Moreover, LAS implantation incurs less damage to the ciliary body.

Objetivo: Identificar los factores pronóstico de resultado visual final en niños con traumatismo ocular a globo abierto. Método: Estudio retrospectivo en 25 menores de 14 años con traumatismo ocular a globo abierto intervenidos quirúrgicamente en el Instituto Nacional de Oftalmología, Perú. Resultados: La edad promedio de los niños fue 6,7 años, el sexo más afectado fue el masculino (68,2 %). Los objetos punzocortantes ocasionaron el 52,0 % de las injurias oculares. El 72,0 % presentó lesión del cristalino asociada. El 16,0% de los niños requirió un segundo procedimiento quirúrgico. La agudeza visual final de los niños mejoró al movimiento de manos en 24 % de los pacientes , cuenta dedos en 20 % de casos y 20/200 o mejor en 56 % de casos. Conclusión: Los factores que sugieren mal pronóstico después de traumatismos a globo abierto en niños son: agudeza visual inicial pobre, longitud de la herida >10 mm y prolapso vítreo.


Objective: to identify prognostic factors for visual acuity after repair of open globe injuries in children. Methods: retrospective study by reviewing records of 25 patients, aged 14 years or younger at the Instituto Nacional de Oftalmología Perú. Results: the mean age was 6,7 years. 68,2 % were male children. Objects most often causing penetrating injury were scissors, glasses and needles (52,0 %). 72,0 % of children developed traumatic cataract. Additional surgery was performed in 16,0 % of cases. Final visual acuity was: light perception/ hand motion in 24 % of cases, count fingers acuity in 20 %, and 20/200 or better in 56% of cases. Conclusions: factors suggesting unfavourable final visual acuity after open globe injuries in children are: poor initial visual acuity, wound length >10 mm and vitreous prolapsed at presentation.

Objetivo: Determinar el impacto del tiempo en que fue instaurado el tratamiento quirúrgico y el resultado visual final postoperatorio, en pacientes con trauma ocular a globo abierto en el Instituto nacional de Oftalmología en el periodo 2011. Tipos de estudio: Estudio observacional, descriptivo, retrospectivo. Métodos y procesamientos: Se revisaron 111 historias clínicas de pacientes que acudieron al servicio de Emergencia del Instituto nacional de Oftalmología en el periodo 2011, por trauma ocular a globo abierto. Variables: Edad, sexo, hora en la que sucedió el trauma, momento en que se realizó el procedimiento quirúrgico, tiempo que transcurrió entre el trauma y el procedimiento quirúrgico, agudeza visual al ingreso hospitalario y la del último control postoperatorio, área anatómica comprometida. Resultados: El 82,9 por ciento fue de sexo masculino, 60 por ciento está en el grupo etario de 11 a 30 años; grupo de edad económicamente activo; el 36 por ciento de pacientes mejoro la AV inicial, el 55,9 por ciento permaneció estable; el 24,3 por ciento fue operado dentro de las 12 primeras horas, 35,1 por ciento entre la 13 a 24 horas; el 80,2 por ciento de los traumas comprometió Zona I y II. Conclusiones: No existe diferencia significativa entre el tiempo de espera quirúrgico y la agudeza visual final al operar un paciente durante las primeras 36 horas posteriores al trauma; el sexo masculino presento mayor incidencia de trauma ocular a globo abierto en edad laboral activa y la mayoría de los procedimientos quirúrgicos se realizaron entre las 13 y 24 horas posteriores al trauma.


Objective: To determine the impact of the time it was established surgical treatment and postoperative final visual outcome in patients with open globe ocular trauma at the National Institute of Ophthalmology in the period 2011. Type of study: Observational, descriptive and retrospective. Methods: We reviewed medical records of 111 patients attending the Emergency Service of the National Institute of Ophthalmology in the period 2011, open globe ocular trauma. Variables: age, sex, time at which the trauma occurred, at which time the surgical procedure performed, time elapsed between trauma and surgical procedure, visual acuity at admission and the last monitoring postoperative involved anatomical area. Results: 82.9 per cent were male, 60 per cent are in the age group 11-30 years economically active age group, 36 per cent of patients improved the initial AV, 55.9 per cent remained stable, with 24.3 per cent had surgery within the first 12 hours, 35.1 per cent between 13-24 hours, 80.2 per cent of trauma pledged Zone I and II. Conclusions: No significant difference between the surgical waiting time and the final visual acuity to operate a patient during the first 36 hours after trauma, males had higher incidence of open globe ocular trauma of working age and most surgical procedures were performed between 13 and 24 hours after trauma.

Los traumatismos de los anejos oculares son frecuentes, debido a la ubicación anatómica de estas estructuras y cumplir con una de sus principales funciones de protección del globo ocular. Se pueden clasificar en afecciones traumáticas de párpados, conjuntiva, órbita y traumas del aparato lagrimal. Según los datos aportados por el United States Eye Injury Registry, las lesiones de los anejos oculares constituyen el 5 porciento de todos los traumas oculares y las más frecuentes con 81 porciento son las laceraciones canaliculares....


The ocular adnexal traumas are common due to the anatomical location of these structures, and the fulfilling of one of its main duties, that is, the protection of the eyeball. OBJETIVO: Avaliar os resultados anatômicos e visuais de pacientes com catarata traumática atendidos no Hospital de Olhos do Paraná, apresentando ou não alterações oculares associadas e que foram submetidos ao tratamento cirúrgico, além de traçar o perfil analisando tipo de trauma, relação entre ocupação e trauma, relação entre tipo de trauma e prognóstico visual, correlacionando acuidade visual (AV) pré e pós-operatória. MÉTODOS: Após minuciosa avaliação oftalmológica foram coletados os seguintes dados: sexo, idade, ocupação, tipo de trauma, AV pré e pós-tratamento, alterações oculares associadas e dos pacientes que foram submetidos à cirurgia as complicações pós-operatórias mais frequentes. RESULTADOS: Foram atendidos 45 pacientes com catarata traumática. Destes, 25 submeteram-se à cirurgia. A maioria dos pacientes era do sexo masculino (88%), sofreu trauma contuso (52%) e apresentou idade média de 43,16 anos, atingindo uma faixa etária economicamente ativa. Estes traumas foram classificados como: laboral (14), recreacional, doméstico e automobilístico. Subluxação de cristalino foi a alteração ocular associada mais observada. Vinte e dois pacientes (88%) apresentaram visão inicial ≥ 20/400, entre os submetidos à cirurgia, 64% apresentaram melhor acuidade visual corrigida pela tabela de Snellen (MAVC) ≤ 20/40. Opacidade cápsula posterior foi a complicação pós-operatória mais frequente. CONCLUSÃO: O perfil dos pacientes atendidos, no HOP, com catarata traumática é, na grande maioria, homens com idade média de 43 anos, vítimas de trauma contuso ocorridos quando desenvolviam atividades laborais, expostos ao risco de acidente sem equipamentos de proteção, atingindo principalmente agricultores, pedreiros e serralheiros. Esses, após o tratamento cirúrgico apresentaram melhora significativa da visão.


OBJECTIVE: To evaluate the anatomic and the visual results of patients with traumatic cataract who sought care in Hospital de Olhos do Paraná (HOP), that were subjected to surgical treatment in this service. To describe their epidemiological and clinical aspects, and to analyze the post operative functional results. METHODS: There were included in the study all patients subjected to a surgical treatment and the following characteristics were analyzed: sex, age, pre and postoperative visual acuity, mechanism of trauma, associated ocular alterations and most frequent postoperative complications. RESULTS: Forty five patients were treated with clinical diagnosis of traumatic cataract during the study period. From the total, 25 were submitted the surgical treatment. The majority of patients were male (88%), a history of blunt trauma could be ascertained (52%) middle age 43±16 years reaching an economically active group. Manual laborers were the most frequent (14), followed by recreational accidents, home accidents and automobilistic accidents. Traumatic subluxation lens were the most frequent ocular alterations associated. Twenty two patients (88%) presented pre-operative visual acuity ≥ 20/400. Among the patients that were submitted to cataract surgery, 64% showed better visual acuity corrected by table of Snellen ≤ 20/40. Cause of poor post operative visual acuity posterior capsule opacity CONCLUSION: In this study, the majority of patients were male, with mean age of 43±16 years and victims of blunt trauma during their job without equipments of protection, reaching principally farmers, masons and locksmiths. It was observed a clear improvement of postoperative visual acuity among these patients.

Résumé

Purpose: To study the outcome of in-the-bag implanted square-edge polymethyl methacrylate (PMMA) intraocular lenses (IOL) with and without primary posterior capsulotomy in pediatric traumatic cataract. Materials and Methods: The study was undertaken in a tertiary care center. Thirty eyes of 30 children ranging in age from 4 to 16 years with traumatic cataract which underwent cataract extraction with capsular bag implantation of IOL were prospectively evaluated. Group A included 15 eyes of 15 children where primary posterior capsulotomy (PPC) and anterior vitrectomy with capsular bag implantation of square-edge PMMA IOL (Aurolab SQ3602, Madurai, Tamil Nadu, India) was performed. Group B comprised 15 eyes of 15 children in which the posterior capsule was left intact. Postoperative visual acuity, visual axis opacification (VAO) and possible complications were analyzed. Results: Best corrected visual acuity (BCVA) of 20/40 or better was achieved in 12 of 15 eyes in both groups. Amblyopia was the cause of no improvement in visual acuity in the remaining eyes. Visual axis opacification was significantly high in Group B as compared to Group A (P=0.001). Postoperative fibrinous uveitis occurred in most of the eyes in both groups. Pupillary capture was observed in one eye in each group. Conclusion: Primary posterior capsulotomy and anterior vitrectomy with capsular bag implantation of square-edge PMMA significantly helps to maintain a clear visual axis in children with traumatic cataract.

Résumé

This report describes the only case in the literature of globe luxation due to traumatic cerebrospinal fluid fistula to the orbit caused by fire gun with ocular globe maintenance. E.N., female, white, 7 months, admitted with left orbitocranial injury by fire gun. Ocular globe luxation was detected with complete ocular motility restriction and absence of pupillary reflex in the left orbit. Computed tomography showed fracture of the medial orbital wall; bone fragments near the apex of the orbit and a stretched optic nerve. Surgical exploration was performed, showing liquor fistula through the ethmoid-sphenoid wall that was blocked with sponge (Gelfoam®) plus organic glue in the left orbit posterior wall, with immediate resolution of the proptosis and ocular integrity maintenance. Although controversial, maintenance of the ocular globe instead of enucleation was performed due to the integrity of the globe in this case. Despite the blindness, we considered the result to the proposed treatment excellent, once the maintenance of the ocular globe provides a good appearance and will contribute to an adequate facial bone development.


O presente estudo apresenta o único caso de luxação de globo ocular devido fístula liquórica traumática em vítima de trauma órbito-cerebral por projétil de arma de fogo com a preservação do globo ocular. E.N., feminino, branca, 7 meses, admitida com ferimento por projétil de arma de fogo em região órbito-craniana esquerda. Apresentava luxação de globo ocular esquerdo com restrição completa da motilidade ocular e reflexo fotomotor ausente à esquerda. À tomografia: fraturas da parede medial da órbita; fragmentos ósseos próximos ao ápice da órbita. Realizou-se exploração cirúrgica, evidenciando-se fístula liquórica através de fratura etmoido-esfenoidal a qual foi tamponada com esponja (Gelfoam®) e cola orgânica na parte posterior da parede medial da órbita esquerda, com regressão imediata da luxação do globo ocular, sendo o mesmo mantido íntegro. Embora controversa na literatura, optou-se pela preservação do globo ocular à enucleação, visto que o mesmo estava íntegro. Apesar da cegueira, consideramos um excelente resultado ao tratamento proposto, tendo em vista que a preservação do globo ocular provém uma boa aparência, favorecendo também o desenvolvimento ósseo da face.
