Résumé

Benign tumors of the parotid gland accounts for the majority of salivary gland rumors. Surgical treatment of benign parotid tumors requires complete removal of the mass with preservation of the facial nerve, but is a traditional conservative superficial parotidectomy with complete facial nerve exposure required for success? The aim of this study is to evaluate the results of benign parotid rumors treated by limited superficial parotidectomy versus the traditional conservative superficial parotidectomy with complete facial nerve exposure. All patients with benign tumors treated by the author were included. Fourteen cases were identified and treated with a median follow up period of 2 years [average 1-3 years]. Limited superficial parotidectomy was done on 6 patients. None developed facial weakness. Traditional superficial parotidectomy were performed on 8 patients. Temporary facial weakness developed in 2 patients and one developed permanent weakness in one branch of cervicofacial division of the facial nerve. Recurrence was not reported in any of both groups. Complete superficial parotidectomy is unnecessary in the treatment of localized parotid tumors. Limited parotidectomy is associated with low morbidity and revealed significant clinical advantage in term of decreasing operative time, blood loss, and significantly reducing the volume of normal parotid tissues excised .There was no reported recurrence among either group. Preoperative investigation [including fine needle aspiration cytology] should be used selectively

Résumé

One of the most common complications of trauma to maxillofacial region is nerve injuries of sensory and motor branches of the nerve V and VII in the face. The aim of this article was to study the relative prevalence of injuries of sensory part of nerve V and motor part of nerve VII. The research was descriptive analytic and information were collected by clinical observation. Samples consisted of 384 individuals refereing to Alzahra and Kashani hospitals, collected information were described by descriptive statistics methods and chi statistical test. Motor and sensory nerve involvement in maxillofacial trauma were evaluated in 384 trauma patients. The most frequent nerve damage was sensory part of the nerve V [51.3%] and the next frequent nerve damage was motor part of the nerve VII five patients [1.3%] had both of these two type of injuries. Maxillo-facial nerve injuries specially nerves V and VII were the frequent complications of maxillofacial region. Infra-orbital branch of the nerve V and buccal branch of the nerve VII were most affected. Vigilant clinical examination and early diagnosis of the nerve injuries will guide patients towards proper treatment and prevention of complications

La tarsorrafia fue el método alternativo para el tratamiento de las complicaciones post-lesión completa del nervio facial. El implante de una lámina de oro en el párpado permite obtener una solución funcional y cosmética a la falta de oclusión palpebral. Cuando la lesión lleva mucho tiempo de evolución, se suele agregar ectropion de párpado inferior que puede ser resuelto con la técnica de Kuhnt-Szymanovsky en el mismo momento quirúrgico en que se coloca el implante. En nuestra experiencia de 10 pacientes tratados desde Marzo del '93 a Diciembre del '96, hemos colocado en 9 de ellos el injerto de oro, en un caso corregido el ectropion quirúrgicamente y en otro solo se realizó kinesioterapia de rehabilitación (por negativa de la paciente a colocarse el implante quirúrgicamente). En todos realizamos kinesioterapia de rehabilitación precoz. Igual que otros autores, este es un método sencillo y eficaz, alternativo a la tarsorrafia

Résumé

To study the various otogenic conditions responsible for facial nerve palsy in all age groups. Design: It is a retrospective study completed in three years. Settings: Department of E. N. T., King Edward Medical College and Mayo Hospital, Lahore. Subjects: We studied twenty two patients with facial nerve paralysis and cause was somehow related with ear. Otological Trauma was found to be the commonest cause [41%]. Inflammatory pathologies were responsible for 36% cases and malignant tumours in 23% cases. Otological conditions must be thoroughly looked in every patient with lower motor neuron type of facial nerve palsy

Résumé

O presente estudo tem como objetivo analisar as lesöes do nervo facial e suas localizaçöes, de acordo com o tipo de fratura do osso temporal e da importância das vias de abordagem cirúrgica (transmastóidea, fossa média e descompressäo total do nervo). Foram estudados 40 casos de paralisia facial periférica (PFO) pós-traumatismos do crânio e do osso temporal com avaliaçäo clínica, audiológica, topográfica, radiológica e com testes prognósticos. Apresentamos os resultados desta pesquisa e fazemos comentários à luz dos conhecimentos já adquiridos e os comparamos com os da literatura

Total facial nerve decompression via a combined transmastoid transtemporal approach in a child whose compressible skull was partially crushed in a traffic accident causing a floating petrous bone fracture is contemplated. The peculiarity of the fracture line and the type of fracture [floating petrous bone], as well as the operative findings and post-operative results encouraged us to present this case to be added to the previous three cases reported in the literature

É relatado um caso de diplegia facial traumática, acompanhado de surdez bilateral, em consequência de grave traumatismo crâneo-encefálico, que provocou otorragia-bilateral, fraturas do ocipital e temporal direitos, e fratura bilateral da mandíbula.
