Résumé

RESUMO Os aneurismas intracranianos são dilatações em segmentos arteriais que irrigam o sistema nervoso central. Acometem 2% da população e as alterações oftalmológicas podem ser as primeiras manifestações do quadro. O objetivo deste relato foi descrever um caso de aneurisma de artéria carótida interna que cursou com restrição da movimentação ocular, alteração do reflexo fotomotor, ptose palpebral, dor facial e cervical. O diagnóstico foi confirmado pela identificação do aneurisma por meio do exame de angiografia cerebral. Foi realizado teste de oclusão por balão, cujo resultado positivo possibilitou a oclusão total da artéria carótida interna por meio de ligadura cirúrgica, procedimento este realizado com sucesso.


ABSTRACT Intracranial aneurysms are dilations in segments of the arteries that irrigate the central nervous system. They affect 2% of the population and the ophthalmologic disorders may be the first evidence in the clinical examination. The aim of the report is to describe a case of an internal carotid artery aneurysm that showed restrictions of ocular movements, change of pupillary light reflex, palpebral ptosis, facial, and cervical pain. This diagnosis was confirmed by the identification of the aneurysm through angiography. A balloon occlusion test was performed, and its positive result made a complete occlusion of the Internal Carotid Artery possible through surgery ligation, procedure that was successful.

Résumé

Abstract We present a case study of Tolosa-Hunt syndrome, a rare idiopathic disease, that is characterized by painful ophthalmoplegia of strong intensity, generally affecting the third cranial nerve, and, less frequently, the fourth or the sixth cranial nerves. Usually, there is no visual impairment. The treatment is based on corticosteroids with satisfactory results in most cases although recurrences can occur at intervals from months to years. In our case, the patient presented sudden pain periorbital associated with cranial nerves involvement, which have an excellent outcome after treatment with corticosteroids, with no relapses until today.


Resumo Nós apresentamos um caso de Síndrome de Tolosa-Hunt, uma doença idiopática rara, caracterizada por oftalmoplegia dolorosa, de forte intensidade, geralmente afetando o terceiro par craniano, e, menos frequentemente, o quarto e/ou o sexto par. Geralmente, não há acometimento visual. O tratamento é feito com base em corticóides com resultados satisfatórios na maior parte dos casos, embora recorrências possam ocorrer após meses a anos. Relatamos caso de paciente masculino de 36 anos, com diagnóstico prévio de sífilis congênita e esquizofrenia, com dor periocular súbita associada com envolvimento de pares cranianos, que teve melhora total após vigência de corticoterapia, sem recorrências até a presente data.

Résumé

Paralisia da mirada lateral por hemorragia pontina, secundária a cavernoma, o qual apresenta prevalência estimada de 0,4% a 0,6%. O risco de sangramento de tal entidade é considerado baixo (0,1 a 3,1% ao ano). Relatamos o caso de paciente feminino, 38 anos, admitida no setor de emergência com quadro de cefaleia, vertigem, hipertensão (PA 200/120mmHg), rebaixamento do nível de consciência (Glasgow 13) e paralisia do olhar conjugado lateral à direita, com 24 horas de evolução. A tomografia de crânio revelou hemorragia pontina e a angiorressonância evidenciou a presença de cavernoma no tegmento pontino. Foi optado por tratamento conservador e a paciente evoluiu com síndrome do encarceramento (Locked-in syndrome) por piora da hemorragia e edema perilesional. Os cavernomas são malformações vasculares que podem cursar assintomáticas e passar despercebidas pelos exames de imagem até o evento hemorrágico. Apesar de raro, quando este ocorre no tronco encefálico pode apresentar alta morbimortalidade. Isso reforça a importância de se avaliar a chance de sangramento dessas lesões e instituir a melhor abordagem para cada caso. (AU)


Horizontal gaze palsy due to hemorrhage of a pontine cavernous malformation, which prevalence ranges from 0.4% to 0.6%. The risk of bleeding is considered low (0.1 to 3.1% per year). It is reported a case of a 38-year-old woman admitted to the emergency department with headache, vertigo, hypertension (200/120mmHg), decreased level of consciousness (Glasgow 13) and horizontal gaze palsy to the right side, that started suddenly 24h before admission. CT scan revealed a pontine hemorrhage and MRI showed the presence of a cavernous malformation in the pontine tegmentum. Conservative treatment was chosen and the patient developed locked-in syndrome due to worsening bleeding and perilesional edema. Cavernoma are vascular malformations that can be asymptomatic and remain undetected by imaging until the hemorrhagic event. Although rare, when bleeding occurs in the brain stem, it can cause high morbidity and mortality. This report reinforces the importance of evaluation these injuries' bleeding risk and establish the best approach for each case. (AU)

Résumé

PURPOSE: To evaluate the correlation between hypertropia and excyclotorsion in acquired superior oblique palsy (SOP). METHODS: Thirty-one patients with acquired unilateral SOP were recruited for this study. The torsional angle of each patient was assessed via one objective method (fundus photography) and two subjective methods (double Maddox rod test and major amblyoscope). The patient population was divided into two groups (concordance group, n = 19 and discordance group, n = 12) according to the correspondence between the hypertropic eye (paralytic eye) and the more extorted eye (non-fixating eye), which was evaluated by fundus photography. RESULTS: The mean value of objective torsion was 5.09degrees +/- 3.84degrees. The subjective excyclotorsion degrees were 5.18degrees +/- 4.11degrees and 3.65degrees +/- 1.93degrees as measured by double Maddox rod test and major amblyoscope, respectively. Hypertropia and the excyclotorsional angle did not differ significantly between the groups (p = 0.257). Although no correlation was found in the discordance group, the concordance group showed a significant and positive correlation between hypertropia and excyclotorsion (p = 0.011). CONCLUSIONS: Torsional deviation was not related to hypertropia. However, in the concordance patients in whom the hypertropic eye showed excyclotorsion, a significant positive correlation was found between hypertropia and excyclotorsion.

El clínico frente a una oftalmoplejia dolorosa, debe encarar diferentes alternativas diagnósticas, donde la disciplina semiológica y los estudios imagenológicos, son de capital importancia para arribar al diagnóstico. El síndrome de Tolosa-Hunt es una probable etiología, donde el uso de critérios diagnósticos, el estudio por resonancia magnética y la remisión con tratamiento corticoideo, son fundamentales para llegar al mismo, sabiendo que el seguimento evolutivo es de suma importancia para descartar los diagnósticos diferenciales.

Résumé

To describe the clinical and neuro-radiological patterns of orbital invasion by the sino-nasal diseases. Descriptive, Retrospective study. 2004 to 2009. We retrospectively reviewed fifty four cases of nasal and paranasal sinus diseases invading the orbit. The medical charts were analyzed. The data considered for the study was age, sex, ocular presentation and associated systemic problems of the patients. The neuro-radiological results were correlated with the clinical picture. The age range was from 6 to 85 years [mean 45.5]. Male to female ratio was 3.5: 1. The initial clinical presentation was Proptosis [66.66%], disturbance of vision [25.9%], ophthalmoplegia [11.11%], diplopia [9.26%] and ptosis [9.26%]. 79.63% patients had inflammatory etiology and 20.4% had neoplastic lesions in the nasal and paranasal sinuses extending into the orbit

Résumé

An eight-year-old male child presented with drooping of the left eyelid with a history of penetrating injury of hard palate by an iron spoon seven days ago, which had already been removed by the neurosurgeon as the computed tomography scan revealed a spoon in the left posterior ethmoid and sphenoid bone penetrating into the middle cranial fossa. On examination, visual acuity was 20/20 in each eye and left eye showed total ophthalmoplegia. Oral cavity revealed a hole in the left lateral part of the hard palate. We managed the case with tapering dose of systemic prednisolone. The total ophthalmoplegia was markedly improved in one month. Cases of foreign bodies in the orbit with intracranial extension are not unusual, but the path this foreign body traveled through the hard palate without affecting the optic nerve, internal carotid artery or cavernous sinus makes an interesting variation.

Résumé

The purpose of this article is to highlight an uncommon combination of supranuclear downward gaze paralysis with bilateral eyelid ptosis in a 53-years-old man with a radiation induced midbrain tumor and to discuss the aspects regarding the centers and pathways that mediate supranuclear vertical gaze movements.


O objetivo deste artigo é ressaltar uma rara condição caracterizada por paralisia supranuclear do olhar conjugado para baixo associada a ptose palpebral bilateral em um homem de 53 anos, causada por tumor mesencefálico radio-induzido, e discutir os aspectos relacionados ao controle supranuclear dos movimentos oculares verticais.

Résumé

La fístulas carótido-cavernosas (FCC) son comunicaciones anómalas entre la arteria carótida interna o sus ramas y el seno cavernoso, pueden ser de origen espontáneo o posterior a traumatismos. Los pacientes pueden presentar una combinación de síntomas y signos por hipertensión y congestión ocular; como proptosis, edema periorbitario, quemosis, hemorragia conjuntival, disminución de la agudeza visual, papiledema, glaucoma, entre otros. Se diagnóstica sólo entre 0,17 al 0,27 por ciento de los casos; el ultrasonido, la TAC, la RMN, y la angiografía, son útiles para su diagnóstico. Se presenta, caso clínico de paciente masculino de 40 años quien posterior a traumatismo craneoencefálico presentó proptosis de ojo derecho y quemosis; se realiza TAC y angiografía cerebral que reportan FCC derecha, por lo que se somete a embolización endovascular transarterial en la que se evidencia FCC de pequeño calibre que impide paso de catéter por lo que se decide esperar resolución espontánea bajo observación.

Résumé

Pontine cavernous hemangioma presenting as horizontal gaze palsy is extremely rare. A 32-years-old patient presented with left horizontal gaze palsy with left esotropia. A large pontine mass was present which was removed in-toto using a sub-occipital craniotomy. Post- operatively, the gaze palsy showed recovery and the diplopia decreased. Follow-up MRI showed no residual mass.

Objetivo: Analizar los resultados quirúrgicos de las paresias unilaterales del oblicuo superior utilizando un esquema terapéutico basado en dos parámetros principales: desviación en posición primaria de mirada (ppm) y posición de máxima desviación, modificado según torsión, test de ducción forzada perioperatorio y test de tracción del oblicuo superior. Pacientes y métodos: Estudio retrospectivo de 68 paresia unilaterales de un grupo total de 108 pacientes con paresias de oblicuo superior operados entre 1980 y 2002. 63,2 por ciento eran hombres, con edad promedio al momento de la cirugía de 15,7 ± 16,5 años. 53 por ciento eran congénitos, 22 por ciento adquiridos de origen traumático o postquirúrgico y 25 por ciento adquiridos de origen no precisado. La desviación promedio en posición primaria de mirada (ppm) fue 12,7 A (0-40 A). El seguimiento promedio fue 33,5 meses. Resultados: 54 casos fueron sometidos a cirugía de 1 solo músculo, logrando una desviación en ppm promedio de 1,7 ± 2,7 A. El porcentaje de corrección fue 81,9 por ciento. 14 pacientes recibieron cirugía sobre dos músculos obteniendo una desviación promedio en ppm de 5,0 ± 8,2 A, Con un 79 por ciento de corrección. El éxito global fue 83.6 por ciento definido como la desaparición de las molestias que generaron la consulta. El tortícolis preoperatorio disminuyó de 60,3 a 17 por ciento en el postoperatorio, siendo leve en todos los casos. Hubo necesidad de una reoperación en 5 casos (7,5 por ciento). Los tipos de paresias más frecuentes de acuerdo a la clasificación de Knpp fueron los tipo I (22,3 por ciento), III (32,8 por ciento) y IV (26,8 por ciento), por lo cual los procedimientos más frecuentes fueron: debilitamiento del oblicuo inferior ( n = 42), pliegue del oblicuo superior (n = 9) y retroceso del recto inferior contralateral (n = 2). En las desviaciones > 16 A el procedimiento más utilizado fue el debilitamiento de oblicuo inferior ipsilateral combinado con retroceso del recto inferior contralateral (n = 10). Conclusiones: El uso de un esquema terapéutico basado en los parámetros anteriormente mencionados permite una corrección satisfactoria en la gran mayoría de los casos de paresias de oblicuo superior y define el número y los músculos que deben ser intervenidos. Los resultados son comparables a otras series publicadas.

BACKGROUND AND AIMS: Snake bite is an important cause of mortality and morbidity in certain parts of Sri Lanka. This study was designed to determine the offending snakes, neurological manifestations, disease course, and outcome in neurotoxic envenomation. METHODS AND MATERIAL: Fifty six consecutive patients admitted with neurological manifestations following snake bite were studied prospectively. Data were obtained regarding the offending snakes, neurological symptoms, time taken for onset of symptoms, neurological signs, and time taken for recovery. RESULTS: The offending snake was Russell's viper in 27(48.2%), common and Sri Lankan krait in 19(33.9%), cobra in 3(5.4%), and unidentified in 7(12.5%). Ptosis was the commonest neurological manifestation seen in 48(85.7%) followed by ophthalmoplegia (75%), limb weakness (26.8%), respiratory failure (17.9%), palatal weakness (10.7%), neck muscle weakness (7.1%), and delayed sensory neuropathy (1.8%). Neurological symptoms were experienced usually within 6 hours after the bite. Following administration of antivenom, the signs of recovery became evident within a few hours to several days. The duration for complete recovery ranged from four hours to two weeks. CONCLUSIONS: Complete recovery of neuromuscular weakness was observed in all patients except for one who died with intracerebral haemorrhage shortly after admission.

Résumé

Upward gaze palsy developed in a 23-year-old man without any other abnormal findings except pupillary light-near dissociation in ocular examination. The neuroradiologic examination was consistent with cerebral toxoplasmosis--the most frequent opportunistic infection of the brain in AIDS patients. The serologic work-up revealed a positive HIV test. Symptoms and neuroradiologic abnormality improved after treatment for cerebral toxoplasmosis. HIV infection should be considered in patients who have upward gaze paralysis and neuroimaging compatible with cerebral toxoplasmosis. Empirical treatment may alleviate the upward gaze palsy.

Résumé

Two cases of eosinophilic meningitis who presented with headache and strabismus are reported. Pertinent physical examination revealed unilateral horizontal conjugate gaze palsy with absence of doll's eye maneuver and hemiparesis. The etiologic agent of eosinophilic meningitis is presumed to be Angiostrongylus cantonensis and the infected location that produce horizontal conjugate gaze palsy was a pontine lesion.

Résumé

The authors present the clinical findings of a 30-year-old female and a 29-year-old male who both had isolated unilateral lateral rectus muscle palsy in neuro-Behcet's disease. The clinical feature related to isolated abduscens nerve palsy was identified by CT, systemic assessment and extraocular examination. These patients' constellation of findings appear to be unique: it does not follow any previously reported pattern of ocular manifestations of neuro-Behcet's disease.
