Résumé

To analyze the prevalence and characteristics of late postoperative complications of orthopedic surgeries by video arthroscopy.This was a descriptive cross-sectional study that evaluated, through its own instrument, local and systemic postoperative complications of patients undergoing orthopedic surgeries by video arthroscopy. The study included 270 patients, who were evaluated on days 30(without prosthesis) and 90(with prosthesis placement) of the postoperative period, by telephone service. The selection of participants occurred sequentially and population-based, within the data collection period, from February to July 2020, in a large hospital for medium and high complexity surgeries. Of the 270 procedures performed in the period, 4.4% (n = 12) presented late postoperative infection. The most frequent complications were erythema (83%), edema (75%) and secretion (67%) in the surgical wound. Most used antibiotic therapy (92%) and anti-inflammatory drugs (67%). Hospital readmission was not necessary concerning the complications. Only 50% required medical evaluation before the scheduled time.The need for practices that ensure the quality of perioperative care and improve the active search to assess surgical outcomes is reinforced.

Introdução: Quando a criança é submetida à cirurgia, a presença do familiar transmite segurança e tranquilidade nos momentos que perpassam o período perioperatório. Objetivos: Descrever as experiências do familiar/acompanhante da criança submetida à cirurgia ortopédica no período perioperatório; analisar a presença do familiar/acompanhante da criança submetida à cirurgia no período perioperatório; e discutir a presença do familiar/acompanhante no período perioperatório e suas implicações para a prática assistencial de enfermagem. Método: estudo qualitativo baseado nos conceitos de família saudável e de presença como elemento constitutivo do cuidado familial. O cenário foi uma unidade de pediatria de um hospital especializado em ortopedia no município do Rio de Janeiro. Os participantes foram nove familiares/acompanhantes das crianças internadas e que foram submetidas à cirurgia. Os procedimentos metodológicos foram um formulário de caracterização dos acompanhantes e das crianças e a entrevista não-diretiva em grupo. Os dados foram analisados por meio da análise temática. O projeto foi aprovado no Comitê de Ética em Pesquisa da Escola de Enfermagem Anna Nery e Instituto de Atenção à Saúde São Francisco de Assis e no Comitê de Ética em Pesquisa da instituição coparticipante. Resultados: No pré-operatório, as reações dos familiares/acompanhantes, das crianças e da equipe foram bastante diversificadas, tendo em vista as informações da criança e seu acompanhante e também dos conhecimentos técnico-científicos da equipe. Na sala de cirurgia, os depoimentos apontaram a segurança da criança diante da presença do familiar/acompanhante. No pós-operatório, as depoentes destacaram o êxito da cirurgia e a expectativa de vida das crianças. A religiosidade e a distância da família permeiam as experiências do familiar/acompanhante durante o período perioperatório. Conclusões: A presença do familiar/acompanhante no perioperatório resulta na ampliação da interação família- criança-equipe, estabelecendo situações propícias ao cuidado centrado na criança e sua família em situações cirúrgicas, reafirmando a relevância da presença da família e a garantia do direito da criança de ter um acompanhante.


Introduction: When the child undergoes surgery, the presence of the relative conveys safety and tranquility during the perioperative period. Objectives: describe the experiences of the family member / companion of the child submitted to orthopedic surgery in the perioperative period; presence of the family member / companion of the child submitted to surgery in the perioperative period, to discuss the presence of the family member / companion in the perioperative period and its implications for nursing care practice. Method: qualitative study based on the concepts of healthy family and presence as a constituent element of family care. The setting was a pediatric unit of a hospital specialized in orthopedics in the city of Rio de Janeiro. Participants were nine relatives / companions of hospitalized children who underwent surgery. The methodological procedures were a form of characterization of the companions and the children, non-directive interview in the group. The data were analyzed by means of the thematic analysis. The project was approved by the Research Ethics Committee of the Anna Nery School of Nursing and the São Francisco de Assis Health Care Institute and the Research Ethics Committee of the co-participating institution. Results: In the preoperative period, the reactions of the family / companions, the child and the team were quite diversified considering the information of the child and his / her companion, as well as the technical-scientific knowledge of the team. In the operating room, the testimonies pointed to the child's safety in the presence of the relative / companion. In the postoperative period, the deponents highlighted the success of the surgery and the children's life expectancy. The religiosity and the experiences of the family membercompanion during the perioperative period. The presence of the family membercompanion in the perioperative results in the expansion of the family-child-team interaction, establishing situations propitious to care centered on the child and his family in surgical situations, reaffirming the relevance of family presence and the guarantee the right of the child to have an accompanying person.

O objetivo deste estudo foi caracterizar as ações ao paciente com disfunções musculoesqueléticas prestada pelos profissionais de enfermagem atuante na ortopedia, num Hospital do município de Caxias-Ma. O trabalho segue uma abordagem quantitativa com caráter descritivo, os dados foram coletados por meio da observação dos procedimentos executados com a aplicação de um check list, e através de uma entrevista estruturada com questões fechadas referentes à atuação da enfermagem os critérios de seleção dos sujeitos foram: ser profissional de enfermagem atuante na ortopedia, ser da escala definitiva do setor e aceitar participar da pesquisa. Os resultados mostraram que as ações de enfermagem ao paciente ortopédico em sua maioria são de delegação e supervisão do ortopedista, o enfermeiro raramente participa deste processo. Conclui-se que existe a necessidade de uma atualização dos profissionais que compõe a ortopedia tendo em vista as lacunas existentes frente aos diversos aspectos que regem a enfermagem traumato-ortopédica.

Estudo de validação de conteúdo, descritivo, transversal, com abordagem quantitativa, realizado num hospital universitário. Objetivou-se validar os indicadores de quatro resultados de enfermagem da Classificação dos Resultados de Enfermagem - NOC, para o diagnóstico de enfermagem Déficit no autocuidado: banho/higiene, apresentados por pacientes em pós-operatório de cirurgia ortopédica. Construiu-se um instrumento contendo os indicadores dos resultados Autocuidado: Atividades da Vida Diária, Autocuidado: Banho, Autocuidado: Higiene e Autocuidado: Higiene Oral, e uma escala Likert de cinco pontos (1=não relevante; 5=extremamente relevante). Os peritos foram enfermeiros que atendem esses pacientes há pelo menos um ano e utilizam diagnósticos de enfermagem. Para análise dos dados empregou-se estatística descritiva. Dos 34 indicadores pesquisados, 2 (6 por cento) foram considerados indicadores principais provisórios, 22 (65 por cento), secundários provisórios e 10 (29 por cento) foram descartados. Os indicadores principais e secundários provisórios serão utilizados na observação do banho de pacientes em pós-operatório de Artroplastia Total de Quadril e terão suas evoluções monitoradas.


This is a descriptive, cross-sectional content validation study, with a quantitative approach, carried out in a university hospital. The objective of this study was to validate four nursing outcomes' indicators of the Nursing Outcomes Classification - NOC for the nursing diagnosis Selfcare Deficit: Bathing/Hygiene presented by patients in postoperative of orthopedic surgery. An instrument containing the outcomes indicators Selfcare: Activities of Daily Living, Selfcare: Bathing, Selfcare: Hygiene and Selfcare: Oral Hygiene was built, along with a Likert scale ranging from 1 to 5 (1=not important, 5=extremely important). The experts were nurses who care for these patients for at least a year and make use of nursing diagnoses. Descriptive statistics were used for the data analysis. Out of the 34 indicators studied, 2 (6 percent) were considered as main temporary indicators, 22 (65 percent) as secondary temporary indicators and 10 (29 percent) were discarded. The main and secondary temporary indicators will be used during the bathing observation of patients in postoperative of Total Hip Replacement, and their development will be monitored.


Estudio de validación de contenido, descriptivo, transversal, con abordaje cuantitativo, realizado en un hospital universitario. Se objetivó validar los indicadores de cuatro resultados de enfermería de la Clasificación de Resultados de Enfermería - NOC para el diagnóstico de enfermería Déficit en el auto-cuidado: baño/higiene presentado por pacientes en post operatorio de cirugía ortopédica. Se construyó un instrumento conteniendo los indicadores de los resultados Auto-cuidado: Actividades de la Vida Diaria, Auto-cuidado: Baño, Autocuidado: Higiene y Auto-cuidado: Higiene Oral y una escala Likert de cinco puntos (1=no relevante, 5=extremadamente relevante). Los peritos fueron enfermeros que atienden a tales pacientes desde hace al menos un año y utilizan diagnósticos de enfermería. Para el análisis de los datos se empleó la estadística descriptiva. De los 34 indicadores investigados, 2 (6 por ciento) fueron considerados indicadores principales provisorios, 22 (65 por ciento) considerados secundarios provisorios, y 10 (29 por ciento) fueron descartados. Los indicadores principales y secundarios provisorios serán utilizados en la observación del baño del paciente en el post operatorio de Artroplastia Total de Cadera, y se monitorearán sus evoluciones.

Résumé

The study aimed to evaluate knowledge and practices of nursing staff at the orthopedic units regarding the existing care of patient with skin traction. The sample consists of [40] nurses, [20] of them from Emergency Teaching Hospital in Duhok and the other [20] of them from Erbil Teaching Hospital in Erbil from 1st Dec. 2004 to the end of June 2005 in Kurdistan Region. Two instruments were constructed to evaluate knowledge and practices. Evaluation of knowledge was done by using of multiple choice questions composed of [25] questions, and evaluation of practice was done by using the observational check list which consist of four main category [pre skin traction, during skin traction, post skin traction and follow up of patient have skin traction]. Validity of questionnaire was determined through the expert. Indicated that the nurses knowledge scores was satisfied about patient have skin traction in Duhok and Erbil. In relation to the evaluation of nurses' practice, it was found that the practices of the nurses were inadequate in Duhok and Erbil Hospitals. The study founded there was no significant difference between Duhok and Erbil of knowledge and practices. The finding indicated that there was no significant relationship between nurses' knowledge and practices and years of experience in orthopedic units. The number of staff should be increased in orthopedic units, education and special training programs for these nurses in orthopedic units should be designed and presented through how to provide nursing care, how to prevent complications and management of complications if present

Résumé

PURPOSE: The purpose of this study was to develop and to apply the NANDA, NOC, and NIC (NNN) linkages into a computerized nursing process program using the classification systems of nursing diagnoses, nursing outcomes and nursing interventions. METHOD: The program was developed with planning, analysis, design and performance stages. The program was applied to 117 patients who were admitted to orthopedic surgery nursing units from January to February, 2004. RESULTS: Thirty-five of fifty-three nursing diagnoses were identified. Five nursing diagnoses in order of frequency were: Acute pain (28.4%), Impaired physical mobility (15.6%), Impaired walking (8.7%), Chronic pain (5.5%) and Risk for disuse syndrome (5.0%). The nursing outcomes of the 'Acute pain' nursing diagnosis tended to have higher frequencies (cumulative) in order of Pain management (95.2%), Comfort level (35.5%) and Pain level (17.7%). The nursing interventions of the 'Acute pain' nursing diagnosis tended to have higher frequencies (cumulative) in order of Pain management (71.0%), Splinting (24.2%) and Analgesic administration (17.7%). In comparison of outcome indicator scores between before and after the intervention according to the 61 nursing outcomes, the mean scores of 52 outcome indicators after the intervention were significantly higher than before the intervention. CONCLUSION: It is expected that this program will help nurses perform their nursing processes more efficiently.

Este es un estudio del área de la Enfermería Pediátrica y forma parte de una línea de investigación desarrollada por la Escuela de Enfermería de la Pontificia Universidad Católica de Chile, en conjunto con la Universidad de Otawa de Canadá. Es de tipo descriptivo, retrospectivo y transversal y se realizó en el servicio de traumatología infantil del Centro de diagnóstico terapéutico del Área Metropolitana Sur Oriente. Tiene como finalidad conocer las necesidades de apoyo en la toma de decisiones en salud y los conocimientos que tienen los padres de niños menores de un año con diagnóstico de DLC que se atienden en este servicio, con respecto al tratamiento ambulatorio y cuidados que requiere el niño en el hogar. El Universo del estudio esta compuesto por todos los niños menores de un año con diagnóstico de Displasia Luxante de Caderas (DLC) y en tratamiento ortopédico ambulatorio que consultan entre los meses de Septiembre-Diciembre de 1999, teniendo los siguientes criterios de inclusión:ser padres o tutores de niños menores de un año con diagnóstico de DLC, estar cursando con el tratamiento ortopédico y tener 2 o más controles en el policlínico de traumatología el día de la encuestra, la muestra fue intencionada con un n=34, lo que corresponde al 100 por cientos del universo
