Résumé

ABSTRACT This prospective analysis assessed the effect of histological prostatitis on lower urinary tract functions and sexual function. The patients were separated into two groups as histologically observed prostatitis (Group A) and no prostatitis (Group B) according to the biopsy outcomes. International prostate symptom score, international index of erectile function-5 scores, maximal and average flow rate, and residual urine volumes were compared statistically between groups. There was no significant difference (P>0.05) in baseline age (t=0.64), body mass index value (t=0.51), prostate volume (t=0.87), prostate-specific antigen levels (t=0.43), maximal (t=0.84) and average flow rate (t=0.59), and post-void residual urine volume (t=0.71). Mean international prostate symptom score in patients with prostatitis was numerically but not significantly higher than that in those without prostatitis (t=0.794, P=0.066). Mean international index of erectile function-5 score in the prostatitis group was significantly lower than that in those without prostatitis (t=1.854, P=0.013). Histological prostatitis notably affected sexual function of patients and may serve as a major risk factor for sexual dysfunction while having little effect on lower urinary tract symptoms.

Résumé

To compare the extent and the degree of aggressiveness of inflammation on the total prostate specific antigen [t-PSA], free PSA [f-PSA], free / t-PSA ratio, and PSAD values in patients with chronic prostatitis. Retrospective, descriptive study. Yuksek Ihtisas Training and Research Hospital, Turkey. Fifty-five patients with serum t-PSA levels higher than 4 ng / ml and with chronic prostatitis were included. Transrectal ultrasound guided biopsy was taken from patients with serum PSA levels higher than 4 ng / ml. Histological sections of the prostatic tissues were scored using the extent and aggressiveness of inflammation seen, on a four point scale from 0 to 4. Serum PSA levels in different inflammation grades were compared. Kruskal-Walls variance analysis was used for inter-group assessment. In both groups scoring; no patient was in grade O. On the extent of inflammation group scoring; 2, 36, and 17 patients had grade 1, 2, 3, respectively. In this group, unlike the t-PSA values, the extent of inflammation was significant in PSA-D [p = 0.004] with a negative correlation in f / t PSA values. On the aggressiveness of inflammation group scoring; 18, 23, and 14 patients had grade 1, 2, 3, respectively. In this group, no statistical difference was found in terms of t-PSA, but statistical differences were monitored in terms of f-PSA, f / t PSA, and PSA-D values [p = 0.03, 0.002, 0.01, respectively]. High serum PSA levels in patients with asymptomatic chronic prostatitis may correlate with the degree of extent and aggressiveness of inflammation

INTRODUCTION: Studies that compare the impact of different infectious entities of the male reproductive tract (MRT) on the male accessory gland function are controversial. MATERIAL AND METHODS: Semen analyses of 71 patients with proven infections of the MRT were compared with the results of 40 healthy non-infected volunteers. Patients were divided into 3 groups according to their diagnosis: chronic prostatitis NIH type II (n = 38), chronic epididymitis (n = 12), and chronic urethritis (n = 21). RESULTS: The bacteriological analysis revealed 9 different types of microorganisms, considered to be the etiological agents, isolated in different secretions, including: urine, expressed prostatic secretions, semen and urethral smears: E. Coli (n = 20), Klebsiella (n = 2), Proteus spp. (n = 1), Enterococcus (n = 20), Staphylococcus spp. (n = 1), M. tuberculosis (n = 2), N. gonorrhea (n = 8), Chlamydia tr. (n = 16) and, Ureaplasma urealyticum (n = 1). The infection group had significantly (p < 0.05) lower: semen volume, alpha-glucosidase, fructose, and zinc in seminal plasma and, higher pH than the control group. None of these parameters was sufficiently accurate in the ROC analysis to discriminate between infected and non-infected men. CONCLUSION: Proven bacterial infections of the MRT impact negatively on all the accessory gland function parameters evaluated in semen, suggesting impairment of the secretory capacity of the epididymis, seminal vesicles and prostate. These findings were associated with an infectious related significant increase of semen pH. None of the semen parameters evaluated can be suggested as a diagnostic tool for infection.

Résumé

A infecção do trato urinário (ITU) é a infecção bacteriana mais freqüente nos seres humanos. É responsável por grande parte das consultas de emergência e internações hospitalares na prática médica. Apesar da maioria dos casos ser de fácil manuseio, a identificação e o tratamento adequado dos casos complicados são um desafio na prática clínica. Este artigo visa principalmente orientar quanto aos fatores de risco, bem como na identificação e no tratamento dos casos de infecção urinária complicada.


The urinary tract infection (UTI) is the most frequent infection of the humans and it is responsible for a great part of emergency consults and hospital commitments. Although that the majority of the cases are easy to treat, the identification and the adequated treatment of the complicated cases is a challenge on the medical practice. This article has the purpose to orientate for the risk factors as well as on the identification and treatment of those cases of complicated urinary tract infections.

Se dan a conocer dos casos de una rara variante de prostatitis granulomatosa de la que hasta la fecha se han comunicado seis casos en la literatura mundial. Esta afección poco frecuente se caracteriza por el reemplazo del tejido prostático normal por nódulos formados por histiocitos con infiltración grasa, al parecer como reacción a un estímulo inflamatorio crónico. Se comunican estos dos casos por su escasa prevalencia, así como por la dificultad que representan para la diferenciación clínica con el carcinoma de la próstata

This study included 41 infertile cases and 25 normal controls. ALT and AST activity was measured in semen of all cases, together with study of various semen parameters and number of pus cells in their prostatic secretion. Statistical analysis of results was done. The study showed highly significant rise in ALT activity in cases of infertility with associated prostatic infection

La distribución de las poliaminas es universal y su concentración varía de un tejido otro. Su presencia está asociada con los procesos decrecimiento, multiplicación y diferenciación celulares normales. El metabolismo de estas moléculas se ha estudiado en el semen humano, testículos, epidídimo y próstata. Las poliaminas se han asociado con los procesos de capacitación espermática y fertilización. Algunos estudios de inhibición sugieren que las poliminas pueden estar involucradas en la regulación de procesos de posfertilización. Su detección oportuna en orina puede conducir al tratamiento de casos de prostatitis en una etapa curable
