Résumé

Introdução: Exposições químicas podem variar em populações geograficamente distintas, com diferentes hábitos, estilos de vida e características individuais. Alguns elementos químicos encontrados no ambiente são capazes de alterar a expressão gênica humana. Objetivos: a) quantificar as concentrações de elementos potencialmente tóxicos (EPTs: As, Cd, Cr, Cu, Hg, Mn, Ni, Pb, Sb, Sn, e Zn) na urina da população de Limeira, e nas peças de joias e pó de solda; b) quantificar as concentrações de EPTs no sangue dos participantes de Limeira e Volta Redonda; c) avaliar os riscos de doenças associadas à exposição ocupacional; d) avaliar o impacto da exposição ocupacional na expressão gênica de trabalhadores formais e informais. Métodos: O grupo Exposto foi composto por trabalhadores informais que realizam soldagem de joias e bijuterias em ambiente domiciliar na cidade de Limeira, SP; e por trabalhadores formais da Companhia Siderúrgica Nacional, em Volta Redonda, RJ. O grupo Controle incluiu moradores dos mesmos bairros dos trabalhadores, mas que não desenvolviam nenhuma atividade diretamente relacionada à exposição química. Amostras de sangue foram coletadas para quantificação de glicose, insulina, perfil lipídico, EPTs e para análise transcriptômica. Em Limeira, também foi quantificada a concentração de EPTs na urina. Para transcriptômica, o RNA foi extraído e hibridizado com Agilent SurePrint G3 Human Gene Expression 8x60K v2 Microarray. O pré-processamento, análise estatística e de vias de interesse foram realizados no software GeneSpring GX. Todos os participantes preencheram questionários sobre hábitos, percepção de risco, morbidade referida e exposição ocupacional. A associação entre exposição a EPTs e desfechos de saúde foi testada por modelo de regressão de Poisson multivariado. Resultados: Nos trabalhadores informais, foram detectados 16 genes superexpressos e 33 subexpressos em comparação com os controles (fold-change > 2). A análise de vias indicou genes enriquecidos em vias do processo inflamatório (quimiocinas MAPK, receptor Toll-like e NF-kappa B). Nos trabalhadores formais, foram encontrados 20 genes superexpressos e 50 subexpressos, com vias relacionadas à resposta imune e ao processo de aterosclerose. O único gene diferencialmente expresso (DEG) em comum nas duas populações foi o IFI27 relacionado na literatura a diferentes tipos de câncer. A produção informal de joias de Limeira foi associada a exposição dos trabalhadores ao Cd, com concentrações significativamente maiores na urina e no sangue dos trabalhadores comparado aos controles. Além disso, foi observada uma associação positiva entre as concentrações de Cd no sangue e a glicemia. As concentrações de As e Pb também foram maiores no sangue dos trabalhadores informais comparado aos controles, sendo que participantes com concentrações de Pb superiores a 2,6 µg dL-1 apresentaram prevalência de manifestações neurológicas 2,3 vezes maior (IC 95%: 1,17 - 4,58; p = 0,02). Não foram observadas diferenças significativas nos EPTs entre os grupos de Volta Redonda, provavelmente devido ao uso de equipamentos de proteção individual e à poluição ambiental na região. Conclusão: As diferenças na expressão gênica relacionadas à exposição ocupacional estão associadas, principalmente, à inflamação e à resposta imune. Os resultados sugerem que a exposição ocupacional prolongada a elementos tóxicos pode levar a consequências negativas para a saúde, como por exemplo, um aumento da prevalência de manifestações neurológicas. Os resultados exploratórios desta tese são um ponto de partida para estudos em populações sensíveis e pouco estudadas, especialmente, de países em desenvolvimento. Análises adicionais devem ser realizadas para investigar efeitos diretos e validar associações causais.


Introduction: Chemical exposures may vary in geographically distinct populations, with different habits, lifestyles, and individual characteristics. Objectives: a) to determine potentially toxic elements' (EPTs: As, Cd, Cr, Cu, Hg, Mn, Ni, Pb, Sb, Sn, and Zn) in the urine of the population of Limeira, and in jewelry pieces and soldering powder; b) determine the PTEs concentrations in the participants' blood from Limeira and Volta Redonda municipalities; c) investigate disease risks associated with occupational exposure; d) to evaluate the impact of occupational exposure on gene expression profile. Methods: The exposed group was composed of informal workers who perform soldering of jewelry inside their houses in the city of Limeira, SP; and formal workers from a steel company in the city of Volta Redonda, RJ. Control participants were recruited from the same neighborhoods without occupational chemical exposure. Blood samples were collected for blood glucose, insulin, lipid profile, and PTE determinations, and for transcriptomic analysis. In Limeira, PTE concentration in urine was also determined. RNA was extracted and hybridized to Agilent SurePrint G3 Human Gene Expression 8x60K v2 Microarray for transcriptomics analysis. Pre-processing, statistical, and pathway analysis were performed in GeneSpring GX software. All participants completed questionnaires about household risk, reported morbidity, and occupational exposure. The association between PTEs exposure and health outcomes was tested by a multivariable robust Poisson regression model. Results: 16 up- and 33 down-regulated genes (fold-change > 2) were observed in the informal workers. Pathway analysis revealed genes enriched in inflammatory process (MAPK, Toll-like receptor, and NF-kappa B chemokine signaling pathways). In formal workers, 20 up- and 50 down-regulated genes were found with pathways related to immune response and atherosclerosis development. The gene IFI27 which has been associated with various types of cancer was the only one commonly differentially expressed between informal and formal workers. Informal jewelry production in Limeira increased exposure to Cd, with significantly higher concentrations in the urine and blood of informal exposed workers compared to controls. Furthermore, a positive association was observed between blood Cd concentrations and glycemia. The blood concentration of As and Pb were also Participants with Pb concentrations higher than 2.6 µg dL-1 showed a prevalence of neurological manifestations 2.3 times higher (95% CI: 1.17 - 4.58; p = 0.02) than those with lower lead concentrations. No significant differences were observed between formal workers from Volta Redonda and their control group, probably, because of the use of individual protection equipment and the environmental pollution in the region. Conclusion: Differences in gene expression related to occupational exposure are mainly associated with inflammation and immune response. The results suggest that prolonged occupational exposure to toxic elements could lead to negative health outcomes, such as higher prevalence of neurological manifestations. These exploratory results are a starting point for analysis in sensitive and understudied populations, especially in developing countries. Further analysis should be carried out to investigate its direct effects and to validate causal associations.

OBJECTIVE@#To observe the toxicokinetic parameters, absorption characteristics and pathomorphological damage in different parts of the gastrointestinal tract of rats poisoned with different doses of diquat (DQ).@*METHODS@#Ninety-six healthy male Wistar rats were randomly divided into a control group (six rats) and low (115.5 mg/kg), medium (231.0 mg/kg) and high (346.5 mg/kg) dose DQ poisoning groups (thirty rats in each dose group), and then the poisoning groups were randomly divided into 5 subgroups according to the time after exposure (15 minutes and 1, 3, 12, 36 hours; six rats in each subgroup). All rats in the exposure groups were given a single dose of DQ by gavage. Rats in the control group was given the same amount of saline by gavage. The general condition of the rats was recorded. Blood was collected from the inner canthus of the eye at 3 time points in each subgroup, and rats were sacrificed after the third blood collection to obtain gastrointestinal specimens. DQ concentrations in plasma and tissues were determined by ultra-high performance liquid chromatography and mass spectrometry (UPHLC-MS), and the toxic concentration-time curves were plotted to calculate the toxicokinetic parameters; the morphological structure of the intestine was observed under light microscopy, and the villi height and crypt depth were determined and the ratio (V/C) was calculated.@*RESULTS@#DQ was detected in the plasma of the rats in the low, medium and high dose groups 5 minutes after exposure. The time to maximum plasma concentration (Tmax) was (0.85±0.22), (0.75±0.25) and (0.25±0.00) hours, respectively. The trend of plasma DQ concentration over time was similar in the three dose groups, but the plasma DQ concentration increased again at 36 hours in the high dose group. In terms of DQ concentration in gastrointestinal tissues, the highest concentrations of DQ were found in the stomach and small intestine from 15 minutes to 1 hour and in the colon at 3 hours. By 36 hours after poisoning, the concentrations of DQ in all parts of the stomach and intestine in the low and medium dose groups had decreased to lower levels. Gastrointestinal tissue (except jejunum) DQ concentrations in the high dose group tended to increase from 12 hours. Higher doses of DQ were still detectable [gastric, duodenal, ileal and colonic DQ concentrations of 6 400.0 (1 232.5), 4 889.0 (6 070.5), 10 300.0 (3 565.0) and 1 835.0 (202.5) mg/kg respectively]. Light microscopic observation of morphological and histopathological changes in the intestine shows that acute damage to the stomach, duodenum and jejunum of rats was observed 15 minutes after each dose of DQ, pathological lesions were observed in the ileum and colon 1 hour after exposure, the most severe gastrointestinal injury occurred at 12 hours, significant reduction in villi height, significant increase in crypt depth and lowest V/C ratio in all segments of the small intestine, damage begins to diminish by 36-hour post-intoxication. At the same time, morphological and histopathological damage to the intestine of rats at all time points increased significantly with increasing doses of the toxin.@*CONCLUSIONS@#The absorption of DQ in the digestive tract is rapid, and all segments of the gastrointestinal tract may absorb DQ. The toxicokinetics of DQ-tainted rats at different times and doses have different characteristics. In terms of timing, gastrointestinal damage was seen at 15 minutes after DQ, and began to diminish at 36 hours. In terms of dose, Tmax was advanced with the increase of dose and the peak time was shorter. The damage to the digestive system of DQ is closely related to the dose and retention time of the poison exposure.

El loxoscelismo es ocasionado cuando el veneno dermonecrótico producido por las arañas del género Loxosceles, conocidas como «arañas violinistas», ingresa al organismo de una persona a través de su mordida. En México ocurre un subregistro de los casos de loxoscelismo por la ausencia de pruebas de laboratorio para su diagnóstico y la dificultad del cuadro clínico. El objetivo de este trabajo es describir un caso de loxoscelismo cutáneo ocasionado por mordedura de Loxosceles yucatana en un residente de Yucatán, México. El loxoscelismo cutáneo es el tipo más frecuente y menos severo. El presente caso se diagnosticó por medio de la sintomatología registrada en la historia clínica, la lesión inicial y la identificación de arañas L. yucatana. Este trabajo representa la primera descripción de un caso de loxoscelismo cutáneo con resolución favorable en Yucatán.


Loxoscelism occurs when the dermonecrotic venom produced by spiders of the genus Loxosceles, known as "violin spiders," enters a person's organism through their bite. In Mexico there is an underreporting of loxoscelism cases due to the absence of laboratory tests for its diagnosis and the complexity of the clinical picture. The aim of this paper is to describe a case of cutaneous loxoscelism caused by the bite of Loxosceles yucatana in a resident of Yucatan, Mexico. Cutaneous loxoscelism is the most frequent and less severe type. This case was diagnosed by means of the symptomatology registered in the medical records, the initial lesion, and the identification of L. yucatana spiders. This study represents the first description of a case of cutaneous loxoscelism with favorable outcome in Yucatan.

Introducción. Las intoxicaciones pediátricas son un problema de salud pública a nivel mundial. El objetivo de este estudio fue caracterizar las intoxicaciones pediátricas que fueron atendidas en la unidad de cuidados intensivos pediátricos (UCIP) de un hospital en Chile. Población y métodos. Se revisaron las fichas clínicas de pacientes diagnosticados con intoxicación e ingresados a la UCIP entre los años 2013 y 2017. Resultados. Un total de 105 casos fueron identificados, lo que representa un 3 % del total de ingresos registrados en el período estudiado. La mediana de edad de los pacientes resultó ser de 10 años. El 73,3 % de los casos correspondieron a pacientes de sexo femenino. El 51 % de los casos se asociaron a intoxicaciones intencionales y el 83 % fue causado por exposición a medicamentos. Los medicamentos identificados con mayor frecuencia fueron los antidepresivos (11,2 %), analgésicos no esteroides (10,7 %). La ingesta fue la vía de exposición más común (93 %). El promedio de estadía de los pacientes en UCIP fue de 1,3 días. Dos pacientes ingresaron en la UCI: uno requirió intubación y otro hemodiálisis. Se determinaron relaciones estadísticamente significativas entre el sexo del paciente y la circunstancia de exposición, y entre la condición psiquiátrica del paciente y el número de sustancias tóxicas ingeridas. Conclusión. La mayoría de las intoxicaciones atendidas en la UCIP fueron intencionales y correspondieron a pacientes de sexo femenino, a quienes se les asoció alguna patología psiquiátrica. Los grupos de medicamentos identificados con mayor frecuencia fueron los antidepresivos y los antiinflamatorios no esteroides.


Introduction. Pediatric poisoning is a public health problem worldwide. The objective of this study was to establish the characteristics of pediatric cases of poisoning seen at the pediatric intensive care unit (PICU) of a hospital in Chile. Population and methods. The medical records of patients diagnosed with poisoning and admitted to the PICU between 2013 and 2017 were reviewed. Results. A total of 105 cases were identified, who account for 3% of all admissions recorded in the study period. Patients' median age was 10 years. In total, 73.3% of cases were female patients; 51% of cases were associated with intentional poisoning; and 83% were caused by drug exposure. The most common drugs identified were antidepressants (11.2%) and non-steroidal anti-inflammatory drugs (10.7%). Intake was the most frequent route of exposure (93%). The average length of stay in the PICU was 1.3 days. One patient required intubation and another required hemodialysis in the PICU. Statistically significant relationships were established between patient sex and the circumstance of exposure and between the patient's psychiatric condition and the number of toxic substances ingested. Conclusion. Most poisoning cases seen at the PICU were intentional and occurred in female patients, who had a psychiatric condition. The most common drugs identified were antidepressants and non-steroidal antiinflammatory drugs.

Résumé

Se describen las características sobre el phoneutrismo a propósito de un caso. El phoneutrismo es el término con el que se conoce al accidente ocasionado por la mordedura de la araña phoneutria spp, la cual tiene una relevancia clínica dada por las características tóxicas de su veneno. Se presenta un caso de mordedura de la araña phoneutria spp de un paciente atendido en un hospital de alta complejidad de la ciudad de Medellín, Colombia, con manifestaciones cardiovasculares y hallazgos compatibles con un síndrome compartimental, lo cual es inusual en este tipo de arañas, por lo que se necesitó vigilancia en unidad de alta dependencia y fasciotomía cutánea. En Colombia este tipo de accidentes no son de reporte obligatorio, por tanto existe un alto riesgo de subregistro. Lo llamativo de este caso está en las manifestaciones cardiovasculares y la presencia de síndrome compartimental que no se ha descrito en la literatura con este subespecie de arañas.


The characteristics of phoneutrism are described in relation to a case. Phoneutrism is the term with which the accident caused by the bite of the phoneutria spp spider is known, which has clinical relevance given by the toxic characteristics of its venom. We present a case of a bite by the phoneutria spp spider in a patient treated at a high-complexity hospital in the city of Medellín, Colombia, with cardiovascular manifestations and findings compatible with compartment syndrome, which is unusual in this type of spiders, and required surveillance in a high dependency unit and cutaneous fasciotomy. In Colombia reporting this type of accident is not mandatory; therefore, there is a high risk of underreporting. What is striking about this case is the cardiovascular manifestations and the presence of compartment syndrome that has not been described in the literatura with this genre of spiders.


As características do fonutrismo são descritas em um relatório de um caso. Fonutrismo é o termo usado para descrever o acidente causado pela mordida da aranha Phoneutria spp, que é clinicamente relevante devido às características tóxicas de seu veneno. Apresentamos um caso de mordida de aranha por uma aranha Phoneutria em um paciente tratado em um hospital de alta complexidade na cidade de Medellín, Colômbia, com manifestações cardiovasculares e achados compatíveis com a síndrome compartimental, o que é incomum neste tipo de aranha, exigindo vigilância em uma unidade de alta de-pendência e fasciotomia cutânea. Na Colômbia, este tipo de acidente não é obrigatório, portanto, há um alto risco de subnotificação. O que é impressionante neste caso são as manifestações cardiovasculares e a presença da síndrome compartimental, que não foi descrita na literatura com esta subespécie de aranha.

Résumé

O aumento da prevalência de doenças respiratórias crônicas coincide com o da exposição aos poluentes atmosféricos pelo crescente processo de industrialização, aumento do tráfego veicular e migração da população para áreas urbanas. A poluição do ar é uma mistura complexa de poluentes e outros compostos químicos tóxicos e não tóxicos, e o efeito na saúde pode derivar dessa mistura e da interação com parâmetros meteorológicos. Apesar disso, busca-se estabelecer o papel de um poluente específico em separado e consideram-se os parâmetros meteorológicos como fatores de confusão. Há evidências de que a exposição aos poluentes contribui para maior morbidade e mortalidade por doenças respiratórias, especialmente nas crianças, mesmo em concentrações dentro dos padrões estabelecidos pela legislação. Identificar os efeitos dos poluentes no sistema respiratório, isoladamente e em associação, é um desafio, e os estudos têm limitações devido à variabilidade de resposta individual, a presença de doenças pré-existentes, aos fatores socioeconômicos, às exposições a poluentes intradomiciliares, ocupacionais e ao tabaco. A maioria das evidências sobre o efeito dos poluentes no sistema respiratório de crianças deriva de estudos que incluem desfechos de função pulmonar. Entretanto, esses estudos têm diferenças quanto ao desenho, ao método de avaliação de exposição aos poluentes, às medidas de função pulmonar, às covariáveis consideradas como capazes de alterar a resposta aos poluentes e aos tipos de modelos utilizados na análise dos dados. Considerar todas essas diferenças é fundamental na interpretação e comparação dos resultados dessas pesquisas com os dados já existentes na literatura.


The increase in the prevalence of chronic respiratory diseases coincides with that of exposure to air pollutants due to the growing industrialization process, increased vehicular traffic and population migration to urban areas. Air pollution is a complex mixture of pollutants and other toxic and non-toxic chemical compounds and its effect on health can derive from this mixture and the interaction with meteorological parameters. Despite this, it seeks to establish the role of a specific pollutant separately and considers the meteorological parameters as confounding factors. There is evidence that exposure to pollutants contributes to greater morbidity and mortality from respiratory diseases, especially in children, even at concentrations within the standards established by legislation. Identifying the effects of pollutants on the respiratory system, alone and in association, is a challenge and studies have limitations due to the variability of individual response, the presence of pre-existing diseases, socioeconomic factors, exposure to indoor, occupational and environmental pollutants as well tobacco. Most of the evidence on the effect of pollutants on the respiratory system of children comes from studies that include lung function outcomes. However, these studies differ in terms of design, method of assessing exposure to pollutants, measures of lung function, covariates considered capable of altering the response to pollutants, and types of models used in data analysis. Considering all these differences is fundamental in interpreting and comparing the results of these researches with data already existing in the literature.

Introdução. O material particulado (MP) emitido pela queima de combustíveis fósseis de origem veicular é a principal fonte de exposição ambiental à elementos potencialmente tóxicos (EPTs) presentes no ar atmosférico dos centros urbanos, entre eles arsênio, níquel e chumbo. Objetivos. Este estudo avaliou os riscos à saúde humana de efeitos não carcinogênicos e efeitos carcinogênicos associados a exposição inalatória de arsênio, níquel e chumbo no MP10 de origem veicular, coletado em uma das estações de monitoramento da qualidade do ar, na cidade de São Paulo, ao longo dos anos 2002, 2006, 2009 e 2012. Métodos. Os dados de concentração dos EPTs foram obtidos pela Companhia Ambiental do Estado de São Paulo (CETESB) e estão disponíveis no website da agência. As amostras semanais foram coletadas em todas as estações do ano, durante 2002, 2006, 2009 e 2012 na estação de monitoramento Cerqueira César, localizada próximo a uma via de intenso tráfego veicular. A análise estatística foi realizada utilizando o Software R (versão 4.0.5) e RStudio (versão 1.2). O software ProUCL foi usado para estimar os valores UCL95%. A avaliação de risco à saúde humana foi realizada de acordo com os métodos da United States Environmental Protection Agency (USEPA). Resultados. As concentrações de arsênio e chumbo no ar apresentaram médias mais elevadas durante o inverno do que nas outras estações (p <0,05). O chumbo apresentou o maior risco de efeitos não carcinogênicos (HQ> 1,0). Arsênio e níquel apresentaram o maior risco de efeitos carcinogênicos, inclusive acima de 1E-06. O risco para ambos os efeitos foi maior no inverno. Conclusão. Esses achados destacam a importância da poluição do ar como fator de risco para a saúde da população, principalmente em centros urbanos com intenso tráfego veicular. Ações para reduzir a exposição à poluição do ar ambiente devem ser priorizadas nas agendas de políticas ambientais e de saúde.


Background. Particulate matter (PM) emitted from the burning of fuels by vehicles is the main source of environmental exposure to potentially toxic elements (PTEs) present in atmospheric air in urban centers, between them arsenic, nickel and lead. Objective. This study evaluated the risk to human health from non-carcinogenic effects and carcinogenic effects associated with airborne arsenic, nickel and lead in PM10 exposure of the vehicular source collected at one of the air quality monitoring stations in the city of Sao Paulo over the years 2002, 2006, 2009 and 2012. Methods. The concentration data of the PTEs were obtained by the Sao Paulo State Environmental Protection Agency (CETESB) and are available on the agency's website. The statistical analysis was performed using the Software R (version 4.0.5) and RStudio (version 1.2). ProUCL software was used to estimate UCL95% values. The risk assessment for human health was carried out according to the methods of the United States Environmental Protection Agency (USEPA). Results. Airborne arsenic and lead concentrations showed higher means during the winter than in other seasons (p<0.05). Lead had the greatest risk of non-carcinogenic effects (HQ>1.0). Arsenic and nickel had the highest risk of carcinogenic effects, including above 1E-06. The risk for both effects was greatest in winter. Conclusion. These findings highlight the importance of air pollution as a risk factor for population health, especially in urban centers with high vehicular traffic. Actions to reduce air pollution exposure should be prioritized in environmental and health policies agendas.

A variabilidade estrutural é uma característica das proteínas de venenos de serpentes, e a glicosilação é uma das principais modificações pós-traducionais que contribui para a diversificação de seus proteomas. Recentes estudos de nosso grupo demonstraram que venenos do gênero Bothrops são marcadamente definidos pelo seu conteúdo de glicoproteínas, e que a maioria das estruturas de N-glicanos dos tipos híbrido e complexo identificados em oito venenos deste gênero contêm unidades de ácido siálico. Em paralelo, em glicoproteínas do veneno de B. cotiara foi identificada a presença de uma estrutura de N-acetilglicosamina bissecada. Assim, com o objetivo de investigar a variação do conteúdo de glicoproteínas, assim como os mecanismos envolvidos na geração dos diferentes venenos de Bothrops, neste estudo foram analisados comparativamente os glicoproteomas de nove venenos do gênero Bothrops (B. atrox, B. cotiara, B. erythromelas, B. fonsecai, B. insularis, B. jararaca, B. jararacussu, B. moojeni e B. neuwiedi). As abordagens glicoproteômicas envolveram cromatografia de afinidade e ensaio de pull-down utilizando, respectivamente, as lectinas SNA (aglutinina de Sambucus nigra) e MAL I (lectina de Maackia amurensis), que mostram afinidade por unidades de ácido siálico nas posições, respectivamente, α2,6 e α2,3; e cromatografia de afinidade com a lectina PHA-E (eritroaglutinina de Phaseolus vulgaris), que reconhece N-acetilglicosamina bissecada. Ainda, eletroforese de proteínas, blot de lectina, e identificação de proteínas por espectrometria de massas foram empregadas para caracterizar os glicoproteomas. As lectinas geraram frações dos venenos enriquecidas de diferentes componentes, onde as principais classes de glicoproteínas identificadas foram metaloprotease, serinoprotease, e L-amino ácido oxidase, além de outras enzimas pouco abundantes nos venenos. Os diferentes conteúdos de proteínas reconhecidas por essas lectinas, com especificidades distintas, ressaltaram novos aspectos da variabilidade dos subproteomas de glicoproteínas desses venenos, dependendo da espécie. Ainda, considerando que metaloproteases e serinoproteases são componentes abundantes nesses venenos e fundamentais no quadro de envenenamento botrópico, e que estas enzimas contêm diversos sítios de glicosilação, o papel das unidades de ácido siálico na atividade proteolítica das mesmas foi avaliado. Assim, a remoção enzimática de ácido siálico (i) alterou o padrão de gelatinólise em zimografia da maioria dos venenos, (ii) diminuiu a atividade proteolítica de alguns venenos sobre o fibrinogênio e a atividade coagulante do plasma humano de todos os venenos, e (iii) alterou o perfil de hidrólise de proteínas plasmáticas pelo veneno de B. jararaca, indicando que este carboidrato pode desempenhar um papel na interação das proteases com seus substratos proteicos. Em contraste, o perfil da atividade amidolítica dos venenos não se alterou após a remoção de ácido siálico e incubação com o substrato Bz-Arg-pNA, indicando que ácido siálico não é essencial em N-glicanos de serinoproteases atuando sobre substratos não proteicos. Em conjunto, esses resultados expandem o conhecimento sobre a variabilidade de proteomas de venenos do gênero Bothrops e apontam a importância das cadeias de carboidratos contendo ácido siálico nas atividades enzimáticas das proteases desses venenos


Structural variability is a feature of snake venom proteins, and glycosylation is one of the main post-translational modifications that contributes to the diversification of venom proteomes. Recent studies by our group have shown that Bothrops venoms are markedly defined by their glycoprotein content, and that most hybrid and complex N-glycan structures identified in eight venoms of this genus contain sialic acid units. In parallel, the presence of a bisected N-acetylglucosamine structure was identified in B. cotiara venom glycoproteins. Thus, with the aim of investigating the variation in the content of glycoproteins, as well as the mechanisms involved in the generation of different Bothrops venoms, in this study the glycoproteomes of nine Bothrops venoms (B. atrox, B. cotiara, B. erythromelas, B. fonsecai, B. insularis, B. jararaca, B. jararacussu, B. moojeni e B. neuwiedi) were comparatively analyzed. The glycoproteomic approaches involved affinity chromatography and pulldown using, respectively, the lectins SNA (Sambucus nigra agglutinin) and MAL I (Maackia amurensis lectin), which show affinity for sialic acid units at positions, respectively, α2,6 and α2,3, and affinity chromatography with PHA-E (Phaseolus vulgaris erythroagglutinin), which recognizes bisected N-acetylglucosamine. In addition, protein electrophoresis, lectin blot, and protein identification by mass spectrometry were employed for glycoproteome characterization. The lectins generated venom fractions enriched with different components, where the main classes of glycoproteins identified were metalloprotease, serine protease, and L-amino acid oxidase, in addition to other low abundant enzymes. The different contents of proteins recognized by these lectins of distinct specificities highlighted new aspects of the variability of the glycoprotein subproteomes of these venoms, depending on the species. Furthermore, considering that metalloproteases and serine proteases are abundant components of these venoms and essential in Bothrops envenomation, and that these enzymes contain several glycosylation sites, the role of sialic acid units in their proteolytic activities was evaluated. Thus, enzymatic removal of sialic acid (i) altered the pattern of gelatinolysis in zymography of most venoms, (ii) decreased the proteolytic activity of some venoms on fibrinogen and the clotting activity of human plasma of all venoms, and (iii) altered the hydrolysis profile of plasma proteins by B. jararaca venom, indicating that this carbohydrate may play a role in the interaction of proteases with their protein substrates. In contrast, the profile of amidolytic activity of the venoms did not change after removal of sialic acid and incubation with the substrate Bz-Arg-pNA, indicating that sialic acid is not essential in N-glycans of serine proteases acting on small substrates. Together, these results expand the knowledge about the variability of proteomes of Bothrops venoms and point to the importance of carbohydrate chains containing sialic acid in the enzymatic activities of venom proteases

Background: Since the start of the coronavirus disease 2019 (COVID-19) pandemic, poison centres worldwide have reported an increase in exposures to chemicals used for infection prevention. Increased availability and use could lead to an increase in exposures. Potential effects on a South African Poison Information Helpline were unknown, therefore a study was performed to describe changes in call volume and profile of poison exposures. Methods: A retrospective analysis was conducted on an observational database of telephone enquiries. All human-related poisoning exposure call data collected from 01 March to 31 August during 2018, 2019 and 2020 were extracted and analyzed. Summary statistics were used to describe all variables. Results: The total number of calls were 5137, 5508, and 5181 in 2018, 2019, 2020, respectively. The monthly call number during 2020 was mostly less than in 2019. More calls were received from the public calls (39.4% vs 33.1%) and for accidental exposures (65.6% vs 62.3%) increased during 2020 compared to 2019. Exposures to pharmaceuticals decreased by 14.8% from 2019 to 2020, while exposures to eucalyptus oil more than doubled from 21 in 2019 to 43 during 2020. Exposures to antiseptics and disinfectants increased by 60.4%, mainly due to hand sanitizers exposure which showed a 26-fold increase from 2019 (n = 6) to 2020 (n = 156). Conclusion: A change in the profile of poison exposures was observed during the COVID-19 pandemic. Lockdown regulations and greater availability of antiseptics and disinfectants probably led to the increase in exposures. Although symptoms were mostly mild, the public should be educated on safe storage and proper use of all chemicals.

Background: Abortion in Uganda is illegal, only permitted when it places the pregnant mother at risk. This study aimed to apply the modified Poisson model in identifying factors associated with the prevalence of pregnancy termination among women of reproductive age in Uganda. Methods: The 2016 Uganda Demographic Health Survey (UDHS) data were used in this study. More than 18,000 women of the age of 15 ­ 49 years participated in this study. A modified Poisson model that incorporated sampling weights was used to establish the factors associated with pregnancy termination. Results: In Uganda, 18,506 (18.1%) had ever had a pregnancy terminated. The results revealed that, the woman's age [APR = 3.15, 95% CI: 2.72-3.63], being married [APR = 1.55, 95% CI: 1.40-1.71], mass media exposure [APR = 1.18, 95% CI: 1.08-1.29], working status [APR = 1.21, 95% CI: 1.09-1.35], and having visited a health facility [APR = 1.20, 95% CI: 1.10-1.31] were positively significantly associated with likelihood of pregnancy termination. Conclusion: There exists a significant proportion of women who have had their pregnancies terminated in Uganda. It is observed that woman's age, marital status, mass media exposure, having visited a health facility in the last 12 months and working status were main predictors. Based on these results, researchers concluded that the emphasis should be put on improving access to post-abortion care, contraceptive use and media exposure

OBJECTIVES@#To retrospectively analyze homicide cases of death after injection and provide reference for forensic identification.@*METHODS@#Six homicide cases of death after injection which were investigated by the Criminal Investigation Team of Shanghai Public Security Bureau were collected and analyzed, including case situation, scene investigation, autopsy and other materials.@*RESULTS@#The 6 cases were premeditated crimes, 5 cases took place in private spaces, and 5 cases involved the victims approached by suspects' decoy. There were no obvious abnormalities at the scene of the 6 cases. In 6 cases, the victim's body surface injury was mild or no, lividity color was abnormal, and the needlestick injury showed different manifestations from conventional medical measures.@*CONCLUSIONS@#Death after injection is a highly concealed crime and easy to be ignored. For the suspected injection injury found in autopsy, it is necessary to carefully examine, identify and analyze, be alert to the situation of injecting poison (drug) and do a good job in evidence fixation and material extraction.

Résumé

Coleopteran insects can produce toxic substances containing multiple components which have so far not been properly described. To report an unusual case of intoxication by excretion from Calosoma alternans Fabricius 1792 (Coleoptera: Carabidae) in a Venezuelan patient from a periurban neighborhood near the mesothermal raining forest. The toxic activity caused a clinical status characterized by digestive symptoms such as nausea, vomiting, epigastralgia, an increase in bowel movements and probable kidney inflammation with intense pain in both lumbar regions, which did not correspond to the classic dermal damage. In conclusion, a unique case is presented of intoxication by a coleopteran species, with a clinical description not previously reported(AU)


Los insectos coleópteros pueden producir sustancias tóxicas que contienen numerosos componentes que aún no han sido descritos adecuadamente. Presentar un caso inusual de intoxicación por excreciones de Calosoma alternans Fabricius 1792 (Coleoptera: Carabidae) en un paciente venezolano residente en un barrio periurbano cercano a la selva tropical mesotérmica. La actividad tóxica provocó un cuadro clínico caracterizado por síntomas digestivos como náuseas, vómitos, epigastralgia, aumento del número de deposiciones y probablemente inflamación renal, con dolor intenso en ambas regiones lumbares, lo que no se corresponde con el daño dérmico clásico. En resumen, se presenta un caso singular de intoxicación provocada por una especie de coleóptero, con una descripción clínica no reportada anteriormente(AU)

Résumé

En los últimos treinta años la forestación se consolidó como sector productivo en Uruguay. A pesar de este crecimiento son escasos los estudios en cuanto a las condiciones de salud de sus trabajadores/as. En función de este problema y de un pedido de formación en salud laboral por parte del sindicato, esta investigación tuvo como objetivo describir el proceso de trabajo en cosecha y viveros forestales, y analizar los procesos peligrosos y positivos para la salud de los/as trabajadores/as. Para esto se diseñó un estudio exploratorio, observacional y descriptivo. Desde un enfoque cualitativo y participativo se realizaron 2 entrevistas en profundidad, 3 talleres de investigación y 2 instancias de restitución de resultados. El trabajo integró a 45 delegados/as en salud laboral y referentes sindicales de distintas empresas, sectores y departamentos del país. Los procesos peligrosos más destacados por los/as trabajadores/as se vincularon con el ambiente de trabajo, la manipulación de sustancias tóxicas y los movimientos estáticos y/o repetitivos. Estos se intensifican a partir del pago por productividad, las presiones en calidad y la diversidad de empresas tercerizadas. En el sector estudiado, los modos de organizar el trabajo son transversales a los procesos peligrosos analizados e inciden en su intensidad. Este diagnóstico colectivo permitió planificar una estrategia conjunta de formación y de promoción de la salud ajustada a las particularidades de la forestación en Uruguay(AU)


Over the past 30 years, forestry has become a consolidated production sector in Uruguay. Despite this growth, there are few studies on health conditions of workers. Based on this problem and a request for occupational health training from the sector's trade union, this exploratory, observational and descriptive study aimed to describe the work processes in forest harvesting and nurseries, and analyze the hazardous and positive processes for workers' health. Using a qualitative and participatory approach, we conducted two in-depth interviews, three research workshops and two sessions on results dissemination. Our study involved 45 occupational health delegates and trade union representatives from different companies, sectors and departments in the country. Most of the hazardous processes were linked to the work environment, handling of toxic substances and static and/or repetitive movements. These were intensified by productivity payments, pressures on quality and the variety of subcontractor companies. In this sector, the ways of organizing work cut across the hazardous processes identified and impact their severity. This collective diagnosis allowed us to plan a joint training and health promotion strategy, adapted to the Uruguayan forestry sector(AU)

Objetivo: La incidencia de intoxicaciones en el servicio de urgencias viene en aumento en Colombia. En el abordaje inicial se solicitan paraclínicos de rutina, en ocasiones sin una correlación entre el xenobiótico, la fisiopatología y el riesgo clínico. El objetivo de esta investigación es describir el uso de las ayudas diagnósticas en el paciente intoxicado en el servicio de urgencias de un hospital de alta complejidad. Metodología: Estudio descriptivo transversal con disponibilidad de datos para el periodo 2014-2016. Se recopiló información de los registros de las historias clínica de los pacientes que acudieron al servicio de urgencias con diagnósticos relacionados con intoxicaciones. Resultados: El 55.4% de la población fue de sexo femenino, el 54.5% eran menores de 25 años y la mayoría pertenencia al área urbana (77.7%). La intencionalidad en el 68.9% fue suicida. Se reportó el toxídrome en el 17.6% de los casos, donde el sedante fue el más común (9.2%). Las sustancias más usadas fueron los psicofármacos (22.8%), siendo los antidepresivos tricíclicos más comunes con un 10.9%; seguidos de los plaguicidas (19.1%), donde los organofosforados fueron los más comunes (8.8%). Las ayudas diagnósticas más solicitadas fueron el hemograma en un 94.3% de los casos, seguido de la creatinina en un 90.2%. El electrocardiograma se realizó en el 49.7% de los casos y los tóxicos en orina en el 7.8%. Conclusión: Se evidencia un uso rutinario de ayudas diagnósticas en el servicio de urgencias; se requieren estudios adicionales que evalúen la pertinencia de ayudas diagnósticas en este escenario.


Objective: The incidence of poisoning in the emergency department is increasing in Colombia. In the initial approach, paraclinical routines are requested, sometimes without a correlation between xenobiotic, pathophysiology and clinical risk. The objective of this research is to describe the use of diagnostic aids in poisoned patients in the emergency department of a high-complexity hospital. Methodology: Descriptive cross-sectional study with retrospective data availability for the 2014-2016 period. Information was collected from the medical records of patients who came to the emergency department with diagnoses related to poisoning. Results: 55.4 % of the population was female, 54.5 % were under the age of 25, the majority belonging to the urban area (77%). The intentionality in 68.9% of them was suicidal. Toxidrome was reported in 17.6% of cases, the sedative being the most common (9.2%). Psychotropic drugs were the most commonly used substances (22.8%), the tricyclic anti-depressant being the most used with 10.9%, followed by pesticides with 19.1%, among which organophosphates were the most common (8.8%). The most requested diagnostic aids were the hemogram in 94.3% of the cases, followed by creatinine in 90.2%. The electrocardiogram was performed in 49.7% of the cases and the toxic in urine in 7.8%. Conclusions: A routine use of diagnostic aids is evidenced in the emergency department. Therefore, additional studies are required to evaluate the relevance of diagnostic aids in this setting.

As informações foram coletadas nos prontuários dos pacientes que compareceram ao pronto-socorro com diagnóstico de intoxicação. Resultados: 55,4% da população era do sexo feminino, 54,5% tinham menos de 25 anos e a maioria pertencia à zona urbana (77,7%). A intencionalidade em 68,9% foi suicida. Toxidromia foi relatada em 17,6% dos casos, sendo o sedativo o mais comum (9,2%). As substâncias mais utilizadas foram os psicotrópicos (22,8%), sendo os antidepressivos tricíclicos mais comuns com 10,9%; seguido por agrotóxicos (19,1%), onde os organo-fosforados foram os mais comuns (8,8%). Os meios diagnósticos mais solicitados foram o hemograma em 94,3% dos casos, seguido da creatinina em 90,2%. O eletrocardiograma foi realizado em 49,7% dos casos e a urina tóxica em 7,8%. Conclusão: Evidencia-se o uso rotineiro de meios auxiliares de diagnóstico no pronto-socorro; Estudos adicionais são necessários para avaliar a relevância dos auxiliares de diagnóstico neste cenário.

Objetivo: analisar as evidências científicas existentes até o presente momento que descrevem os efeitos tóxicos causados à saúde humana e ao ambiente, devido à liberação dos rejeitos de minério, decorrentes do rompimento da barragem de Fundão em Mariana - MG. Métodos: a revisão narrativa da literatura baseou-se na busca de artigos em quatro bases de dados: PubMed, SciELO, Science Direct e LILACS, utilizando os descritores "Dam", "Fundão" e "Mariana" junto com o operador booleano "AND". Resultados: trinta e quatro das oitenta e cinco publicações encontradas atenderam aos critérios de inclusão e foram utilizadas para compor a presente revisão. Foram observados os seguintes efeitos tóxicos: contaminação da água e do solo, principalmente, por metais pesados, potencial genotóxico, citotóxico e mutagênico da água, diminuição da diversidade microbiana, contaminação de peixes, perda da vegetação e prejuízos ao metabolismo de crescimento de plantas. Entre esses metais, foi relatado aumento na concentração de ferro, manganês, cobre, cádmio, chumbo, arsênio, cromo, magnésio, alumínio, mercúrio, cobalto, bário, níquel e selênio. Além disso, o desastre promoveu o aumento no número de casos de doenças humanas, como dengue, doenças respiratórias, dermatológicas, intestinais, psicológicas e comportamentais. Conclusão: todas as evidências científicas expostas na presente revisão sugerem que houve a contaminação ambiental, bem como da população local, por agentes tóxicos presentes nos rejeitos de minério, em especial metais pesados, liberados após o rompimento da barragem de Fundão. Assim, torna-se essencial a realização de mais estudos que visem esclarecer os efeitos tóxicos crônicos decorrentes dos rejeitos de minério liberados ao ambiente.


Objective: to analyze the scientific evidence existing to date that describes the toxic effects caused to human and the environmental health, due to the release of ore tailings, resulting from the rupture of the Fundão dam in Mariana-MG. Methods: the narrative literature review was based on a search for articles in four databases: PubMed, SciELO, Science Direct and LILACS, using the descriptors "Dam", "Fundão" and "Mariana" together with the Boolean operator "AND". Results: thirty-four, of the eighty-five publications found, met the inclusion criteria and were used to compose the present review. The following toxic effects were observed: contamination of water and soil mainly by heavy metals, genotoxic, cytotoxic and mutagenic potential of water, decrease in microbial diversity, contamination of fish, loss of vegetation, and impairment of plant growth metabolism. Among these metals, increased concentrations of iron, manganese, copper, cadmium, lead, arsenic, chromium, magnesium, aluminum, mercury, cobalt, barium, nickel and selenium were reported. In addition, the disaster promoted an increase in the number of cases of human diseases, such as dengue, respiratory, dermatological, intestinal, psychological and behavioral diseases. Conclusion: all the scientific evidence exposed in the present review that there was environmental contamination, as well as of the local population, by toxic agents present in the ore tailings, especially heavy metals, released after the collapse of the Fundão dam. Thus, further studies aimed at clarifying the chronic toxic effects resulting from the ore tailings released into the environment are essential.

Las alteraciones del equilibrio hidroelectrolítico pueden ser primarias o secundarias al proceso mismo de una enfermedad, a la terapia farmacológica, o en algunos casos a la exposición intencional o no a sustancias tóxicas. El diagnóstico de los trastornos electrolíticos se establece por medio del análisis de laboratorio clínico, pero su interpretación debe ser correlacionada con el contexto clínico del paciente. Los medicamentos pueden interferir con la absorción de los electrolitos, alterar las respuestas hormonales que afectan la homeostasis, o bien impactar de forma directa la función del órgano responsable de mantener el balance electrolítico [1-2]. Entender los mecanismos fisiopatológicos de estas potenciales anormalidades, permite al clínico anticipar, vigilar y corregir de forma temprana, evitando alteraciones graves en el paciente

Résumé

Abstract We report a case of envenomation by Dendrobates tinctorius in the northwest of the Amazon Forest. The patients were two men, who presented with numbness in the right arm and slight numbness in the lower lip, respectively. Dendrobates tinctorius secretions contain pumiliotoxin, one of several toxins found in the dendrobatidis skin, which interferes with muscle contraction and causes locomotor difficulties. Although Dendrobatidae is a family of anurans known for their venom, few studies describe the symptoms of envenomation in humans.
