Manifestaciones clínicas, diagnóstico y tratamiento de la vasculitis de Wegener / Wegener vasculitis: clinical manifestations, diagnosis findings and treatment

La granulomatosis de Wegener es una vasculitis sistémica rara y, como consecuencia del compromiso de múltiples órganos, los pacientes afectados pueden consultar con diversos especialistas. En su patogenia se relaciona con un proceso inflamatorio que afecta la pared vascular. El cuadro clásico de la enfermedad muestra compromiso tanto respiratorio como renal, aunque también pueden estar comprometidos otros órganos. Esta revisión describe el valor diagnóstico de los ANCA y la importancia de las imágenes en el diagnóstico y seguimiento de la enfermedad. Se describen los criterios diagnósticos y el tratamiento, así como los efectos adversos de los fármacos utilizados. Además, se enfatiza en la necesidad de un diagnóstico rápido, para lo cual es necesaria la alta presunción diagnóstica de la enfermedad.

Vasculitis primaria del sistema nervioso central, diagnóstico diferencial de demencia subaguda: Caso clínico / Primary vasculitis of the Central Nervous System: report of one case

Primary central nervous system vaculitis is an uncommon and invalidating disease, which has a fatal course if left untreated. We report a 63 year-old woman presenting with a history of two months of cognitive impairment, dysarthria, gait instability and tremor. After four months of evolution a right hemianopsia and a flaccid paresis of upper right limb appeared. A brain biopsy was performed and the histological findings confirmed the suspicion of primary cerebral vasculitis. The patient was treated with cyclophosphamide and prednisone, observing a partial recovery of cognitive and motor function.

Granulomatous angiitis of the central nervous system associated with herpes zoster.

Granulomatous angiitis of central nervous system (CNS) is a rare inflammatory disease of blood vessels mostly confined to CNS. We describe a case which presented with right sided hemiplegia with aphasia, after herpes zoster ophthalmicus. CT scan and MRI brain showed a large left sided infarct in the left middle cerebral artery (MCA) territory. MRI angiography revealed narrowing and thinning of left internal carotid artery (ICA) and to a lesser extent, left MCA suggestive of granulomatous vasculitis. Herpes zoster is often associated with major CNS involvement and a vascular etiology was previously postulated. Recent pathological reports suggest that cerebral angiitis secondary to herpes virus infection may be more common than realised.
