Resumo Fundamento: A amiloidose por transtirretina (ATTR) é uma doença infiltrativa causada pela deposição anormal de proteína principalmente no coração e no sistema nervoso periférico. Quando acomete o coração, a doença manifesta-se como uma cardiomiopatia restritiva e, quando afeta o sistema nervoso periférico e autônomo, apresenta-se como uma polineuropatia, podendo ser chamada de Polineuropatia Amiloidótica Familiar (PAF). Existem dois subtipos de ATTR, a ATTR selvagem, em que não há variantes genéticas, e a ATTR hereditária, caracterizada por uma variante no gene que codifica a proteína transtirretina (T/TR). Em ambos os subtipos, o envolvimento cardíaco é o principal marcador prognóstico. Objetivos: Avaliar a prevalência do envolvimento cardíaco subclínico em uma amostra de pacientes com variantes genéticas no gene TTR usando a cintilografia com pirofosfato e o ecocardiograma com strain; comparar os achados cintilográficos e as medidas de strain; avaliar a associação entre PAF e o envolvimento subclínico; e analisar se existe uma associação entre uma variante genética específica e o envolvimento cardíaco. Métodos: Estudo transversal com carreadores de variantes no gene TTR sem sintomas cardiovasculares e sem alterações nos parâmetros da eletrocardiografia ou do ecocardiograma convencional. Todos os pacientes foram submetidos à cintilografia com pirofosfato e à ecocardiografia com análise de strain. O envolvimento cardíaco subclínico, definido como um escore de Perugini ≥ 2, razão Coração (C)/ Hemitórax Contralateral (CL) ≥ 1,5 em uma hora, C/CL ≥ 1,3 na terceira hora, ou um strain longitudinal global (SGL) ≤ −17%. Realizadas análises descritiva e analítica, e aplicados o teste exato de Fisher e o teste de Mann-Whitney. Um valor de p<0,05 foi considerado significativo. Resultados: Os 23 pacientes avaliados apresentavam uma idade mediana de 51 (37-57) anos, 15 (65,2%) eram do sexo feminino, 12 (52,2%) eram pardos, nove (39,1%) apresentavam hipertensão arterial sistêmica, e nove (39,1%) tinham um diagnóstico prévio de PAF. Dos nove pacientes com PAF, oito (34,8%) usavam tafamidis. As variantes genéticas identificadas foram Val142IIe, Val50Met e IIe127Val. O valor mediano do SGL foi −19% (-16% - −20%). Dos 23 pacientes, nove (39,1%; 95% CI = 29-49%) preencheram os critérios de envolvimento cardíaco, seis (26%) somente pelo critério do SGL. Não houve associação entre PAF e um carreador assintomático avaliado por ecocardiograma com análise de strain e pela cintilografia com pirofostato (p=0,19). A prevalência de hipertensão arterial sistêmica, diabetes mellitus, dislipidemia, tabagismo e SGL reduzido não foi diferente entre os grupos. A velocidade da onda e' septal foi a única variável que apresentou diferença significativa entre os indivíduos com e sem SGL reduzido, com uma área sob a curva ROC de 0,80 (IC95% = 0,61-0,98, p = 0,027). A melhor acurácia diagnóstica foi alcançada com uma velocidade e' septal ≤ 8,5 cm/s. Não houve associação entre o tipo de variante genética e o envolvimento cardíaco pré-clínico, nem entre o uso de tafamidis e este mesmo envolvimento (37,5% versus 40,0%, p = 0,90). Conclusão: O envolvimento cardíaco subclínico foi frequente em uma amostra de carreadores da variante genética do gene TTR. Um valor do SGL reduzido foi o achado mais comum. Não houve associação entre a presença de polineuropatia amiloidótica e o envolvimento subclínico. O tipo de variante genética não foi associado com envolvimento cardíaco precoce. Nesta amostra, o uso de tafamidis (20mg/dia) não foi associado com uma menor prevalência de envolvimento cardíaco subclínico.


Abstract Background: Transthyretin amyloidosis (ATTR) is an infiltrative disease caused by abnormal protein deposition mainly in the heart and peripheral nervous system. When it affects the heart, the disease presents as restrictive cardiomyopathy; when it affects the peripheral and autonomic nervous system, it manifests as polyneuropathy, and is called familial amyloid polyneuropathy (FAP). There are two ATTR subtypes: wild-type ATTR, where there is no mutation, and mutant ATTR (ATTRm), which is characterized by a mutation in the gene encoding the transthyretin protein (TTR). In both subtypes, cardiac involvement is the major marker of poor prognosis. Objectives: To assess the prevalence of subclinical cardiac involvement in a sample of patients with TTR gene mutation by using pyrophosphate scintigraphy and strain echocardiography; to compare scintigraphy and strain findings; to evaluate the association between neurological manifestations (FAP) and subclinical cardiac involvement; and to analyze whether there is an association between any specific mutation and cardiac involvement. Methods: This is a cross-sectional study with carriers of the TTR gene mutation, without cardiovascular symptoms or changes in electrocardiographic or conventional echocardiographic parameters. All patients underwent pyrophosphate scintigraphy and strain echocardiography. Subclinical cardiac involvement was defined as a Perugini score ≥ 2, heart-to-contralateral lung (H/CL) ratio ≥ 1.5 at 1 h, H/CL ≥1.3 at 3 h, or global longitudinal strain (GLS) ≤ −17%. Descriptive and analytical analyses were performed and Fisher's exact test and Mann-Whitney test were applied. A value of p < 0.05 was considered significant. Results: The 23 patients evaluated had a median age of 51 years (IQR 37-57 years), 15 (65.2%) were female, 12 (52.2%) were Pardo, nine (39.1%) had systemic arterial hypertension, and nine (39.1%) had a previous diagnosis of FAP. Of the nine patients with FAP, 8 (34.8%) were on tafamidis. The associated mutations were Val142IIe, Val50Met, and IIe127Val. The median GLS in the sample was −19% (−16% to −20%). Of the 23 patients, nine (39.1%; 95% CI = 29-49%) met criteria for cardiac involvement, six (26%) by the GLS-based criteria only. There was no association between having FAP and being an asymptomatic carrier, as assessed by strain echocardiography and pyrophosphate scintigraphy (p = 0.19). The prevalence of systemic arterial hypertension, diabetes mellitus, dyslipidemia, smoking, and reduced GLS did not differ between groups. Septal e' wave velocity was the only variable that significantly differed between individuals with and without reduced GLS, with an area under the ROC curve of 0.80 (95% CI = 0.61-0.98, p = 0.027). The best diagnostic accuracy was achieved with a septal e' velocity ≤ 8.5 cm/s. There was no association between mutation type and preclinical cardiac involvement, nor between tafamidis use and lower degree of cardiac involvement (37.5% versus 40.0%, p = 0.90). Conclusion: Subclinical cardiac involvement was common in a sample of TTR mutation carriers without cardiac involvement. Reduced left ventricular GLS was the most frequent finding. There was no association between the presence of amyloid polyneuropathy and subclinical cardiac involvement. Type of mutation was not associated with early cardiac involvement. In this sample, the use of tafamidis 20 mg/day was not associated with a lower prevalence of subclinical cardiac involvement.

Arthritis has important cardiovascular repercussions. Phenylephrine-induced vasoconstriction is impaired in rat aortas in the early phase of the adjuvant-induced arthritis (AIA), around the 15th day post-induction. Therefore, the present study aimed to verify the effects of AIA on hyporesponsiveness to phenylephrine in rat aortas. AIA was induced by intradermal injection of Mycobacterium tuberculosis (3.8 mg/dL) in the right hind paw of male Wistar rats (n=27). Functional experiments in isolated aortas were carried out 15 days after AIA induction. Morphometric and stereological analyses of the aortas were also performed 36 days after the induction of AIA. AIA did not promote structural modifications in the aortas at any of the time points studied. AIA reduced phenylephrine-induced contraction in endothelium-intact aortas, but not in endothelium-denuded aortas. However, AIA did not change KCl-induced contraction in either endothelium-intact or denuded aortas. L-NAME (non-selective NOS inhibitor), 1400W (selective iNOS inhibitor), and ODQ (guanylyl cyclase inhibitor) reversed AIA-induced hyporesponsiveness to phenylephrine in intact aortas. 7-NI (selective nNOS inhibitor) increased the contraction induced by phenylephrine in aortas from AIA rats. In summary, the hyporesponsiveness to phenylephrine induced by AIA was endothelium-dependent and mediated by iNOS-derived NO through activation of the NO-guanylyl cyclase pathway.

Mesenchymal stromal cells (MSCs) have therapeutic potential due to their abilities of differentiation, immunomodulation, and migration to injured tissues, potentiating such effects when cells are activated. Guarana (Paullinia cupana) is a tropical plant species found in South America that is known for its antioxidant, stimulant, and cicatricial effects. The guarana extract is composed of many substances and caffeine is the main component. The objective was to evaluate the effects of guarana and caffeine on MSCs. After the initial characterization, MSCs were treated with Paullinia cupana (10, 100, and 1000 μg/mL) or caffeine (0.4, 4, and 40 μg/mL) for 24 h. MSCs treatment with 1000 μg/mL guarana increased cell polarity, viability, cell migration to chemoattractant, antioxidant potential, and liberation of extracellular vesicles (EVs), while it reduced the levels of autophagy. MSCs treated with 100 and 1000 μg/mL guarana or 40 μg/mL caffeine showed a decrease of cell proliferation. No treatment affected the cellular area and cell cycle of MSCs. The study shows in vitro evidence that guarana could be a promising alternative for activating MSCs to promote better cellular products for future clinical therapies.

The aim was to investigate the long-term effects of metformin ingestion on high-intensity interval training on performance, glycogen concentration (GC), GLUT-4 content, and metabolomics outcomes in rats. Fifty male Wistar rats were randomly divided into baseline, metformin (500 mg daily), and control groups. Training consisted of 4 sets of 10 jumps with 30 s of passive recovery per day, 5 days/week for 8 weeks. The intensity equivalent was 50% of body mass (BM) in the first four weeks and 70% of BM in the last four weeks. The animals were submitted to a weekly jump test until exhaustion at 50% of BM. Serum and tissues were collected at baseline and after 4 and 8 weeks for biochemical and metabolomics analysis. The number of jumps increased in the Control group without a significant difference between groups at 4 and 8 weeks. GLUT4 was lower in the gastrocnemius muscle in the Metformin at the fourth week compared to Control (P=0.03) and compared to Metformin (P=0.02) and Control (P=0.01) at eight weeks. Hepatic and soleus GC were not altered by metformin. Gastrocnemius GC was lower after 8 weeks in the Metformin group compared to Control (P=0.01). Significantly lower levels of pyruvate and phenylalanine and higher levels of ethanol, formate, betaine, very low-density lipoprotein, low-density lipoprotein, and creatine were found in the Metformin compared to the Control. Although chronic administration of metformin decreased food intake and negatively influenced the synthesis of muscle glycogen, it did not significantly change physical performance compared to the Control.

Background@#No recent meta-analysis has holistically analyzed and summarized the efficacy and safety of omarigliptin in type 2 diabetes mellitus (T2DM). We conducted a meta-analysis to address this knowledge gap. @*Methods@#Electronic databases were searched to identify randomized controlled trials (RCTs) that included patients with T2DM who received omarigliptin in the intervention arm. The control arm consisted of either a placebo (passive control group [PCG]) or an active comparator (active control group [ACG]). The primary outcome assessed was changes in hemoglobin A1c (HbA1c), while secondary outcomes included variations in glucose levels, achievement of glycemic targets, adverse events (AEs), and hypoglycemic events. @*Results@#From 332 initially screened articles, data from 16 RCTs involving 8,804 subjects were analyzed. Omarigliptin demonstrated superiority over placebo in reducing HbA1c levels (mean difference, –0.58%; 95% confidence interval, –0.75 to –0.40; P<0.00001; I2=91%). Additionally, omarigliptin outperformed placebo in lowering fasting plasma glucose, 2-hour postprandial glucose, and in the percentage of participants achieving HbA1c levels below 7.0% and 6.5%. The glycemic efficacy of omarigliptin was similar to that of the ACG across all measures. Although the omarigliptin group experienced a higher incidence of hypoglycemic events compared to the PCG, the overall AEs, serious AEs, hypoglycemia, and severe hypoglycemia were comparable between the omarigliptin and control groups (PCG and ACG). @*Conclusion@#Omarigliptin has a favorable glycemic efficacy and safety profile for managing T2DM.

Background/Aims@#It has been suggested that STW5 (Iberogast) reduces heartburn symptoms in patients with functional dyspepsia, but underlying mechanisms of action are unclear. The aim of this study is to investigate whether STW5 affects esophageal sensitivity or esophageal motility, thereby reducing occurrence and perception of reflux events. @*Methods@#We performed a double-blind, randomized, placebo-controlled, crossover trial in patients with functional dyspepsia (Rome IV) and reflux symptoms. After 4 weeks of treatment with either placebo or STW5, patients were studied with an esophageal acid perfusion test and ambulatory 24-hour pH-impedance monitoring. @*Results@#A total of 18 patients (7 men, median age 54, range [19-76]), were included in the study. Although we found no statistical difference in our primary outcome the total Reflux Disease Questionnaire score 2.33 (0.25-4.33) vs 2.67 (1.17-4.00), P = 0.347, “gastroesophageal reflux disease” and “regurgitation” subscale scores were lower after STW5 treatment compared to placebo (P = 0.049 and P = 0.007). There was no statistical difference in number of reflux events, acid exposure time and acid sensitivity scores between STW5 and placebo. In a subgroup analysis of patients with pH-metry confirmed gastroesophageal reflux disease, treatment with STW5 significantly reduced the total number of acidic reflux events (P = 0.028). Moreover, in patients with reflux esophagitis, the median lag time to acid perception increased after STW5 treatment (P = 0.042). @*Conclusions@#We found some indications pointing towards a beneficial effect of STW5 on reflux symptoms in dyspeptic patients, with reduction of esophageal hypersensitivity as a potential underlying mechanism. Our findings will have to be confirmed in larger studies.

Le diabète sucré est l'une des pathologies chroniques les plus rependues chez l'enfant à cause de son apparition dans la tranche d'âge pédiatrique. Les lésions gingivales liées à la micro-angiopathie et la présence du tartre sont exceptionnelles chez l'enfant. La parodontopathie peut rendre plus difficile le contrôle de la glycémie. Les données actuelles de la littérature ont révélé qu'un enfant diabétique est plus susceptible de développer une pathologie bucco-dentaire qu'un enfant non diabétique. L'objectif de cette étude était d'évaluer l'état de santé bucco-dentaire des enfants diabétiques suivis dans le service d'Endocrinologie à l'hôpital National Donka. Il s'agissait d'une étude transversale de type descriptif et analytique, portant sur les affections orales et métaboliques des enfants diabétiques suivis ou hospitalisés dans le service d'Endocrinologie par un recrutement exhaustif sur une période de 6 mois. La population cible était composée de 72 enfants âgés de 6 à 18 ans au moment de l'étude. Un questionnaire a été utilisé pour obtenir des données sur le niveau socio-économique, le niveau sociodémographique, le niveau comportemental, les habitudes d'hygiène bucco-dentaire et les antécédents familiaux associés à un examen oral. Les paramètres diabétiques sont contenus dans le dossier médical. L'impact du diabète sur l'état oral des jeunes patients a été étudié par des variables qualitative et quantitative. Une corrélation entre l'ancienneté du diabète et l'indice d'hygiène orale simplifiée (p<0,05) a été observée avec p=0,03. IC95% [0,00 ; 0,06]. Mais aucun lien statistique n'a été enregistré entre la tranche d'âge et l'IHOS, d'autre part entre et aussi l'IHOS et la présence de variable clinique (p>0,05). La plupart des bouches présentaient des lésions carieuses avec un taux de 91,98% et un indice C.A.O moyen de 2,94 ± 2,82. Une gencive inflammatoire était présente chez 84,73% des patients et presque tous (95,83%) avaient une affection buccale au moins. La maladie bucco-dentaire est un marqueur social, les enfants diabétiques issus de milieux défavorisés doivent donc être ciblés plus particulièrement par les programmes de prévention


Diabetes mellitus is one of the most common chronic pathologies in children because of its appearance in the pediatric age group. Gingival lesions linked to microangiopathy and the presence of tartar are exceptional in children. Periodontal disease can make it more difficult to control blood sugar. Current data from the literature has revealed that a diabetic child is more likely to develop an oral pathology than a non-diabetic child. The objective of this study was to assess the oral health status of diabetic children followed in the endocrinology department at the national donka hospital. This was a descriptive and analytical cross-sectional study on the oral and metabolic disorders of diabetic children followed or hospitalized in the endocrinology department by exhaustive recruitment over a period of 6 months. The target population consisted of 72 children aged 6 to 18 at the time of the study. A questionnaire was used to obtain data on socioeconomic level, sociodemographic level, behavioral level, oral hygiene habits and family history associated with an oral examination. The diabetic parameters are contained in the medical file. The impact of diabetes on the oral status of young patients was studied by qualitative and quantitative variables. A correlation between the duration of diabetes and the simplified oral hygiene index (p0.05). Most of the mouths presented carious lesions with a rate of 91.98% and an average c.a.o index of 2.94 ± 2.82. Inflammatory gingiva was present in 84.73% of patients and almost all (95.83%) had at least one oral condition. Oral disease is a social marker, diabetic children from disadvantaged backgrounds must therefore be targeted more specifically by prevention programs.

La drépanocytose est une maladie génétique due à une anomalie de l'hémoglobine. Elle est la pathologie génétique la plus répandue dans le monde, elle touche plus de cinq millions de personnes. Les Parodontopathies sont l'ensemble des lésions inflammatoires ou dégénératives qui affectent le parodonte. Cette étude avait pour objectifs de déterminer la fréquence de l'atteinte parodontale et d'identifier ses lésions afin de recenser le traitement antérieur reçu chez les enfants atteints d'une maladie sanguine d'origine génétique. C'est une étude transversale descriptive qui s'est étalée sur une période de 06 mois allant du 01mars au 31 août 2016. L'enquête a été réalisée dans le service de pédiatrie à l'hôpital national d'Ignace Deen. La population cible était constituée de tous les enfants et adolescents ayant consulté au service de pédiatrie pendant la période d'étude. Au terme de cette enquête une fréquence de 73% des patients drépanocytaires associés aux Parodontopathies a été enregistrée dont 33,63% de l'échantillon présentaient des lésions profondes avec des poches infra osseuses. Le sexe masculin a été le plus dominant dont une fréquence de 66% contre 34% du sexe féminin avec une sex-ratio égale à 1,97. Les résultats de cette étude révèlent que la formation de base des pédiatres en dentisterie est nécessaire est que la collaboration interdisciplinaire est indispensable pour la prise en charge parodontale des enfants drépanocytaires


Sickle cell disease is a genetic disease caused by an abnormality in hemoglobin. It is the most widespread genetic pathology in the world, it affects more than five million people. Periodontal diseases are all inflammatory or degenerative lesions that affect the periodontium. The objectives of this study were to determine the frequency of periodontal disease and to identify its lesions in order to identify the previous treatment received in children with a blood disease of genetic origin. This is a descriptive cross-sectional study that spanned a period of 6 months from march 1 to august 31, 2016. The survey was carried out in the pediatric department at Ignace Deen national hospital. The target population was made up of all children and adolescents who consulted the pediatric department during the study period. At the end of this survey, a frequency of 73% of sickle cell patients associated with periodontal disease was recorded, of which 33.63% of the sample presented deep lesions with infra-bony pockets. The male sex was the most dominant with a frequency of 66% against 34% of the female sex with a sex ratio equal to 1.97. The results of this study reveal that the basic training of pediatricians in dentistry is necessary, and that interdisciplinary collaboration is essential for the periodontal management of children with sickle cell disease.

But.Décrire la présentation clinique, les modalités thérapeutiqueset l'évolution de l'insuffisance cardiaque du sujet âgé à Bamako. Méthodologie.Il s'agissait d'une étude rétrospective allant du 1ejanvier 2007 au 30 novembre 2009. Un total de 166 sujets âgés de 60 ans et plus atteints d'IC ont été recrutés parmi 1300 malades hospitalisés en cardiologie au CHU du Point G à Bamako. Résultats.La prévalence hospitalière était de 12,8%. Le sex-ratio était de 1,72 en faveur des hommes. La tranche d'âge la plus touchée était celle de 60-69 (48,2 %). Outre l'âge avancé et la ménopause (pour les femmes), les facteurs de risque cardiovasculaire (FRCV) ci-après ont été retrouvés: l'hypertension artérielle (49,4%), le tabagisme(34,3%), le diabète (9,0%), l'obésité (7,2%) et l'alcoolisme(5,4%). La dyspnée était sévère,aux stades III de la NYHAdans 11,8%des cas et IV dans 83 %des cas. Le groupe nosologique le plus important était l'insuffisance cardiaque globale (66,1%), suivi de l'insuffisance cardiaque gauche (27,1%) et l'insuffisance cardiaque droite (7,8%). La fraction d'éjection était conservée chez 59 % des patients. Les causes identifiéesétaient hypertensives (38,6%), primitives(15,7%), ischémiques(14,5%), rythmiques(9,6%) et le cœur pulmonaire chronique(7,8%). Le traitement était le plus souvent classique. Le taux delétalité hospitalière était de10, 2%. Conclusion.L'IC du sujet âgé atteintle plus souvent les hommes. L'HTA et le tabagismesont les FRCV les plusfréquents. L'IC est en règle globale ou gauche. Les principales étiologies sont l'hypertension artérielle, la cardiomyopathie primitive et la cardiopathie ischémique. Le taux de mortalité hospitalière est de 10.2%. Le suivi régulier, l'éducation et une meilleure observance thérapeutique pourraient améliorer laqualité de vie et réduire la mortalité.


Purpose. To describe the clinical presentation, therapeutic modalities and evolution of heart failure (HF) in elderly subjects in Bamako. Methodology. This was a retrospective study from January 1, 2007 to November 30, 2009. A total of 166 subjects aged 60 and over with heart failure were recruited from 1300 inpatients at the cardiology department of CHU du Point G in Bamako. Results. In-hospital prevalence was 12.8%. The sex ratio was 1.72 in favor of men. The age group most affected was 60-69 (48.2%). In addition to advanced age and menopause (for women), the following cardiovascular risk factors (CVRFs) were found: arterial hypertension (49.4%), smoking (34.3%), diabetes (9.0%), obesity (7.2%) and alcoholism (5.4%). Dyspnea was severe, NYHA stage III in 11.8% of cases and stage IV in 83%. The largest nosological group was congestive heart failure (66.1%), followed by left heart failure (27.1%) and right heart failure (7.8%). Ejection fraction was preserved in 59% of patients. The causes identified were hypertensive (38.6%), primary (15.7%), ischemic (14.5%), rhythmic (9.6%) and chronic pulmonary heart disease (7.8%). Treatment was usually conventional.The hospital case-fatality rate was 10.2%. Conclusion. HFin the elderly most often affects men. Hypertension and smoking are the most frequent CVRFs. HFis usually global or left-sided. The main etiologies are arterial hypertension, primary cardiomyopathy and ischemic heart disease. The in-hospital mortality rate is 10.2%. Regular follow-up, education and better compliance with treatment could improve quality of life and reduce mortality

Introduction : la survenue du cancer du sein chez l'homme est une indication du conseil génétique et doit être systématiquement recherchée. L'objectif de ce travail était de décrire les aspects histopathologiques et moléculaires des cancers de sein chez l'homme. Matériel et Méthodes :il s'agissait d'une étude transversale avec collecte rétrospective de Janvier 2007 à Septembre 2021, incluant tous les cas de cancers du sein masculin diagnostiqués à l'histologie avec examen immunohistochimique. Les variables étudiées étaient histopathologiques et moléculaires. Les prélèvements étaient fixés dans le formol à 10% et traités selon la technique standard d'hématoxylineéosine et d'immunohistochimie. Résultats : Au total 8 cas de cancers du sein masculin ont été colligés. Chaque cas a bénéficié d'examen histologique et immunohistochimique. L'âge variait de 8 à 77 ans avec un âge moyen de 50,63±20,79. Le délai de consultation inférieur à 12 mois était fréquent avec comme motif la présence d'un nodule mammaire. Le sein gauche était aussi atteint que le sein droit. La taille tumorale T3 (5cm et plus) prédominait avec 3 cas.La nature du prélèvement reçu était : 04 biopsies et 04 tumorectomies. Le carcinome infiltrant de type non spécifique était fréquent : 7 cas (soit 87,5%) avec le grade II de Elston et Ellis dans 75% des cas. Le type luminal A et le triple négatif étaient prédominants avec 3 cas chacun (37,5%). Conclusion : Le cancer du sein est de plus en plus diagnostiqué chez l'homme à tout âge.


Introduction: the occurrence of breast cancer in men is an indication for genetic counseling and must be systematically sought. The objective of this work was to describe the histopathological and molecular aspects of human breast cancer. Material and Methods: This was a cross-sectional study with retrospective collection from January 2007 to September 2021, including all cases of male breast cancer diagnosed by histology with immunohistochemical examination. The variables studied were histopathological and molecular. The samples were fixed in 10% formalin and processed using the standard hematoxylin-eosin and immunohistochemistry technique. Results: A total of 8 cases of male breast cancer were collected. Each case underwent histological and immunohistochemical examination. Age ranged from 8 to 77 years with a mean age of 50.63±20.79. Consultation time of less than 12 months was common with the presence of a breast nodule as the reason. The left breast was as affected as the right breast. Tumor size T3 (5 cm and more) predominated with 3 cases. The nature of the sample received was: 04 biopsies and 04 lumpectomies. Infiltrative carcinoma of non-specific type was common: 7 cases (or 87.5%) with Elston and Ellis grade II in 75% of cases. Luminal A and triple negative were predominant with 3 cases each (37.5%). Conclusion: Breast cancer is increasingly diagnosed in men at all ages.

Abstract The current focus is on the overall pattern of seed storage present in the fragments of the soil of piedmont and alluvial landscapes of the environment. The present study predicted the seed banks of both soils of alluvial and piedmont zones in different ecological conditions and evaluate the potential of seeds in the restoration of both environments. The composition of the seed bank of soil is mainly affected by the alluvial environment and the structure of cleared area shows that more species of germinating annual grasses and growable seeds with the higher total number. Extant vegetation structures have an important role in the diversity of soil seed reservoirs, whose composition corresponded with the openness of the site. When in situ soil seed bank is recruited, it helps to restore only some components of the plant community in an alluvial environment. In our current research, it was confirmed that seed richness was higher in number at lower elevation (alluvial) than that at high elevation (piedmont). Seed richness showed a significant negative correlation with anions, cations, while significantly positive with altitude that suggests the richness pattern of the overall seed bank of the area is influenced by various environmental factors.


Resumo O foco atual está no padrão geral de armazenamento de sementes presente nos fragmentos de solo do Piemonte e nas paisagens aluviais do meio ambiente. O presente estudo previu os bancos de sementes de ambos os solos das zonas aluvial e piemontesa em diferentes condições ecológicas e avaliou o potencial das sementes na restauração de ambos os ambientes. A composição do banco de sementes do solo é afetada principalmente pelo ambiente aluvial e a estrutura da área desmatada mostra que mais espécies de gramíneas anuais em germinação e sementes cultiváveis ​​apresentam o maior número total. As estruturas da vegetação existente têm um papel importante na diversidade dos reservatórios de sementes do solo, cuja composição correspondeu à abertura do local. Quando o banco de sementes do solo in situ é recrutado, o mesmo ajuda a restaurar apenas alguns componentes da comunidade de plantas em um ambiente aluvial. Em nossa pesquisa atual, foi confirmado que a riqueza de sementes era maior em número em altitudes mais baixas (aluviais) do que em altitudes elevadas (Piemonte). A riqueza de sementes mostrou uma correlação negativa significativa com ânions, cátions, enquanto significativamente positiva com a altitude, o que sugere que o padrão de riqueza do banco geral de sementes da área é influenciado por vários fatores ambientais.

This study investigated the effects of a foot core intervention on the coordination of foot joints in recreational runners. This was a secondary analysis from a randomized controlled trial conducted with 87 recreational runners allocated to the control group (CG), which followed a placebo lower limb stretching protocol, or the intervention group (IG), which underwent an 8-week (3 times/week) foot core training. The participants ran on a force-instrumented treadmill at a self-selected speed (9.5-10.5 km/h) while the foot segment motion was captured. The vector coding technique was used to assess inter-joint coordination for four selected coupled segment and joint angles. The coordination patterns of the calcaneus and midfoot (CalMid) and midfoot and metatarsus (MidMet) joint pairs were affected. In the frontal plane, IG showed an in-phase with proximal dominancy coordination at heel strike, with a decrease in its frequency after the training (P=0.018), suggesting a longer foot supination. Additionally, IG showed an anti-phase with distal dominancy pattern at early stance compared to CG due to a smaller but earlier inversion of the CalMid-MidMet pair (P=0.020). The intervention also had an effect on the transverse plane of the CalMid-MidMet pair, with IG showing a significantly greater frequency of anti-phase coordination with proximal dominancy during propulsion than CG (P=0.013), probably due to a reduction in the CalMid abduction. Overall, the results suggested that the foot core intervention reduces the occurrence of running-related injuries by increasing the resistance to calcaneus pronation and building a more rigid and efficient lever during push-off.

Aims: Ready-to-eat [RTE] animal products like ponmo are preferred by consumers due to its palatability and quality. However, foodborne pathogens particularly Staphylococcus aureus are sources of concern due to cross-contamination of raw and cooked cowhide. This study aimed to investigate the incidence of enterotoxigenic S. aureus in ready-to-eat locally processed cowhide. Methodology: Sixty (60) RTE cowhide samples were collected from different locations in Lagos, Nigeria and analyzed using conventional microbiological and molecular techniques for the detection of toxigenic S. aureus contamination. Suspected S. aureus isolates were confirmed by the presence of thermostable endonuclease [nuc] gene in their genome. Results: Result showed that 25 (41.67%) and 20 (33.50%) samples harbored coagulase-positive S. aureus and 20 other bacterial species different from S. aureus, respectively while 15 (24.83%) of the tested ponmo samples yielded no bacterial growth. Thirteen of the 15 randomly selected from the 25 suspected isolates were confirmed as S. aureus by the presence of thermostable endonuclease [nuc] gene in their genome. Enterotoxigenic genes were confirmed in all the 13 PCR detected S. aureus. Enterotoxin B gene is most prevalent in ponmo. Multiplex PCR detection of S. aureus enterotoxins [SE] genes revealed the molecular detection of different isolates carrying staphylococcal enterotoxin types A and B, mixed strain carrying both staphylococcal enterotoxins type A and type D. Antibiotic susceptibility of 20 S. aureus isolates revealed varying degrees of susceptibility patterns against the antimicrobial agents. Generally, gentamicin 70% (14/20), azithromycin 75% (15/20), co-trimoxazole 85% (17/20), levofloxacin 95% (19/20) were the most effective antibiotics to S. aureus. A low, ?50% susceptibility was recorded to chloramphenicol 55% (11/20) and nitrofurantoin 65% (13/20). A higher resistance to streptomycin (90%; 18/20) and ceftazidime (95%; 19/20) was identified, with resistance to ceftazidime being the highest (95%; 19/20). Conclusion: It can be concluded that RTE ponmo vended in the study sites is of low hygienic quality and may be of health risk to consumers. High level hygiene practice and good manufacturing practices are required during the production, distribution and marketing of ponmo to curb the potential health consequences of eating ponmo.

Background and Objective: Cardiovascular disease (CVD) is a significant cause of morbidity and mortality worldwide, with high-risk patients requiring effective management to reduce their risk of cardiovascular events. Bempedoic acid is a novel therapeutic agent recently approved as an add-on therapy to statins in patients with uncontrolled LDL-c. Bempedoic acid inhibits cholesterol synthesis in the liver, which ultimately reduces the risk of cardiovascular events. Therefore, the present study aims to assess the efficacy and safety of bempedoic acid in patients with uncontrolled LDL-c (Previously on moderate or high-intensity statins) with a high risk of CVD in real-world settings. Methods: This is a multicenter, retrospective, observational study on the data of high-risk-CVD patients collected from Bempedoic Acid on Efficacy and Safety in patients (BEST) Registry. The clinical data of 140 patients who were already on statin therapy and were receiving Bempedoic acid at a dose of 180 mg, along with measurements of the level of LDL-c, HbA1c, HDL, TG, TC, PPPG, FPG, AST, ALT, serum creatinine was taken into consideration. The primary outcome includes a change in LDL-c level, and secondary outcomes involve a change in the level of HbA1c, HDL, TG, TC, PPPG, FPG, AST, ALT, and serum creatinine at week 12 and 24. Adverse events were reported at both time points. Results: A total of 140 patients were included in the present study with a mean age of 51.8 ± 9.2 years and had primary confirmed diagnosis of dyslipidemia with uncontrolled LDL-c. The mean levels of LDL-c decreased from the mean baseline value of 142.67 ± 46.49 mg/dL, to 106.78 ±33.92 mg/d; a statistically significant reduction by 23.23% (p < 0.01) at week 12. Similarly, at week 24, the mean LDL-c value reduced to 90.39 ± 38.89 mg/dL. A 33.38 % decrease was observed (p < 0.01). Other parameters such as non-HDL, FPG, PPPG, AST and serum creatinine also showed statistically significant reduction at week 12 and week 24. Conclusion: The present study demonstrates that bempedoic acid is an effective add-on medication in lowering LDL-c levels in high-risk CVD patients with uncontrolled LDL-c.

Dermatophytosis is a very common public health problem with high prevalence. Dermatophytes are a highly specialized set of filamentous fungi, which are adapted to keratinized tissues of humans and animals. Dermatophytosis is the most common fungal infection worldwide, affecting approximately 20–25% of the world’s population. The etiological agents of dermatophytosis, called dermatophytes, change with geography and socioeconomic status. Trichophyton rubrum (T. rubrum) is the prime species for skin and nail infections followed by T. mentagrophytes/ T. interdigital complex. There is a shift from T. rubrum to T. mentagrophytes in India for superficial fungal infections. In order to deal with fungal infections, treatment strategies involve the use of systemic antifungals and/or topical antifungal agents. Naftifine is a synthetic allylamine antifungal first reported in 1974 and in 1985 became the first commercially available allylamine. The highly lipophilic nature of allylamine allows efficient penetration and reasonably high concentrations in the stratum corneum (SC) and hair follicles. Naftifine is fungicidal as well as fungistatic. The higher efficacy rates of allylamines over imidazoles for the treatment of fungal infections, even for months after cessation of treatment, is thought to be due to their fungicidal effect, as well as to potentially greater keratin binding and slower release from the SC. The effectiveness of naftifine is also demonstrated against various bacteria belonging to both gram-negative and gram-positive classes. The antiinflammatory property of naftifine has been reported in various preclinical studies where it has been shown to target the prostaglandin pathway. Naftifine 1 and 2% gel and cream is approved by The United States Food and Drug Administration (USFDA), recently naftifine has been approved in India by the Indian regulatory authority Drug Controller General of India (DCGI) for the treatment of dermatophytosis. Naftifine 2% also appears to be a promising treatment, requiring fewer applications than the 1% formulation. Naftifine appears to be effective in a single dose and has a shorter treatment duration than azoles. Naftifine demonstrated its efficacy and safety in various clinical studies of tinea infections. Naftifine offers a very useful and promising option for treating dermatophytosis.

Background & objectives: Sandflies are implicated as vectors of Chandipura virus (CHPV) (Vesiculovirus: Rhabdoviridae). The virus is prevalent in central India including Vidarbha region of Maharashtra. CHPV causes encephalitis in children below 15 yr of age with case fatality rates ranging from 56 to 78 per cent. The present study was undertaken to determine the sandfly fauna in the CHPV endemic Vidharba region. Methods: A year round survey of sandflies was conducted at 25 sites in three districts of Vidarbha region. Sandflies were collected from their resting sites using handheld aspirators and identified using taxonomical keys. Results: A total of 6568 sandflies were collected during the study. Approximately 99 per cent of the collection belonged to genus Sergentomyia, which was represented by Ser. babu, Ser. bailyi and Ser. punjabensis. Genus Phlebotomus was represented by Ph. argentipes and Ph. papatasi. Ser. babu was the predominant species (70.7%) collected during the study. Ph. argentipes was detected in four villages with 0.89 per cent, whereas Ph. papatasi was detected in only one village with 0.32 per cent of the total collection. CHPV could not be isolated despite processing all the sandflies for virus isolation in cell culture. Interpretation & conclusions: The present study showed influence of higher temperature and relative humidity on sandfly population dynamics. An important observation during the study was the absence or decline in the population of Ph. papatasi and Ph. argentipes in the study area. Surge in Sergentomyia population and their breeding/resting in close vicinity to humans pose a concern as they are known to harbour CHPV and other viruses of public health importance
