RESUMO

There are few studies about the relationship between platelet indices and the activity of rheumatoid disease and it is still a controversial issue. Therefore we decided to study the relationship between platelet indices and severity of rheumatoid arthritis according to DAS28 criteria. In this Historical cohort study, 250 patients with rheumatoid arthritis and 250 people who accompanied the patients were selected via convenient sampling method. After written consent; 2cc of blood was taken from each patient and was sent to our laboratory. All patients were examined by a rheumatologist for diagnosis of disease activity. DAS28 formula was used to evaluate the severity of disease activity. Data were entered into SPSS 16 software and analyzed by independent t-test, Pearson correlation and multiple regression analysis. The mean values of hemoglobin [p<0.001], hematocrit [p<0.001], MCH [p<0.001], and PLcr [p=0.002] were lower in the patient group compared to the control group. The mean values of platelet [p<0.001], RDW [p=0.001], and PDW [p= 0.03] were higher in the patient group in comparison to the control group. There was no statistically significant differences between the two groups in relation to the mean values of MCV [p= 0.22] and MPV [p= 0.21]. A significant negative correlation was observed between DAS28 scores of hemoglobin [p=0.005] and hematocrit [p=0.02]. Increased severity of the rheumatoid arthritis led to decreased hemoglobin and hematocrit and increased platelet count. Therefore, these blood parameters can be used as a primary prognostic factor. Also ESR can be regarded as a good prognostic factor. However, MCV and MPV are not suitable prognostic factors for the patients with rheumatoid arthritis

Lactobacilli are known as a valuable source of antimicrobial compounds and have a high potential of use in food biopreservation against food related microorganisms. Antimicrobial potency of 63 dairy lactobacilli isolates against four highly important food-related microorganisms were evaluated. In addition, a new way in data organization was introduced, which led to a more informative and rational comparison of indicator microorganisms' susceptibilities to a set of compounds. Correlation of pH and antimicrobial properties was investigated. Microbroth dilution assay was used to evaluate indicator microorganisms' susceptibility to lactobacilli CFCS [cell free culture supernatant]. Results were organized by both the conventional way - demonstrating the minimum inhibitory and lethal concentrations of CFCS - and a new suggested method, representing comparative effectiveness of each CFCS specimen against indicator microorganisms of comparison interest. Susceptibilities of tested strains were in the following order: Escherichia coliO157:H7 > Listeria monocytogenes > Aspergillus parasiticus> Candida parapsilosis. Despite the high susceptibility of L. monocytogenes, it showed the highest resistance to death among the tested microorganisms. Efficiency of Lactobacilli CFCS in killing the tested strains showed the following susceptibility order: E. coli O157:H7 > A. parasiticus> C. parapsilosis> L. monocytogenes. Antimicrobial property was in correlation with the pH value of CFCS. PH had a pronounced impact on susceptibilities of C. parapsilosis and E. coli in pH values of concentrated CFCS lower than 4 and 4.5, respectively. Potency of lactobacilli isolates in growth inhibition of the indicator microorganisms was found promising, and the suggested data organization method provided additional information, leading to more precise comparison of indicator microorganisms

Osteoporosis is one of the most prevalent disease in current century .Estrogen deficiency is the basic cause of osteoporosis in menopaused women. Hormone Replacement Therapy [HRT] can increase the risk of breast and ovary cancers. Medicinal plant are natural source of secondary metabolite and can a reliable source for treating osteoporosis. Ferula gummosa [Galbanum] has been used in traditional medicine since ancient time. This study is focused on determining the effect of Galbanum root ethanolic extract on osteogenesis progress in Human Mesenchymal Stem Cells [hMSCs]. The Bone Marrow hMSCs were seeded at 12 well plates and treated with different amount of Galbanum extracts [0.5 to 100 micro g/ml]. Extract cytotoxicity were measured using MTT method .Effect of extract on osteogenesis was evaluated in time interval 7 and 14 days using_ Alkaline Phosphatase_ enzyme activity method. The data analysis revealed a significant increase in cell proliferation in range of [0.5 to 5] micro g/ml after 24, 48 and 72 hour of treatment with galbanum extract. Analysis of result revealed a significant increase in alkaline phosphatase [ALP] enzyme activity in the range of 1 to 10 micro g/ml compared with control group. Ferula gumossa has been used in Iranian folk medicine for many years. Our in vitro study showed that Frula gummosa extract has osteoprotective effect

Cerebrolysin, a preparation of low molecular weight neuropeptide and free amino acids, has been shown to have a great affect against excitotoxicity, inhibiting free radical formation, microglia activation and additionally neurotrophic action, promoting neuronal sprouting, and improving cellular survival and stimulating neurogenesis following stroke. The aim of this study was to determine the efficacy of Cerebrolysinon acute ischemic stroke. The present study was a randomised double blind clinical trial. The sample was 89 patients with confirmed the diagnosis of acute ischemic stroke. The patients divided in the intervention or control group with simple randomization. Patients in intervention group received 50ml Cerebrolysin diluted in 100ml normal saline solution [totally 150 ml] infused over a time period of 30 minutes for 7 days and for control group patients, administered just 150ml normal saline infusion. Patients were assessed on days 1,30 and 90 by Orgogozo, barthel index, MRS and NIHSS score. The findings indicated that the mean of scores at the end of the first month was increased in comparision with the first day, 1.68 in intervention and -10.95 in intervention and case groups respectively and these differences were meaningful statistically [p<0.05]. The comparison of scores at the end of three month treatment to the first day reported meaningful too [p<0.05]. The findings demonstrated that at the end of the third month as compared with the first day, added 1.68 to case group score and reduced -13.21 from the control group score. Cerebrolysin is efficient in acute ischemic stroke patients and decreases the tissue damage severity in these patients. On the other hand, presumably it can reduces disability of the disease

Tuberculosis is one of the major health problems, especially in developing countries. This study aimed to further understanding of the medical community with this problem in domestic and international air travel, and ICAO has been predicted that by 2015 that is reached number of travel- air to more than 5 / 2 million per year. Ease and duration of air travel, with large numbers of international travelers- international, has increased the likelihood of contact with infectious TB. To access the resources of articles were used PubMed .nejm. Civil aviation, WHO and the papers published in reputable scientific journals of Iranian and foreign research data from 2000 to 2011 and the Ministry of Health and Medical Education. pulmonary tuberculosis is the most common type of tuberculosis in Iron and world and sputum smear-positive pulmonary TB, so that makes up 53 percent of all TB cases. Non pulmonary tuberculosis is 28% of all cases of tuberculosis. Considering that more tuberculosis in pulmonary tuberculosis is spread through the air, air travel time outlast 8 h is danger for travelers. Over 80 percent of the total number of people with TB annually is in developing country, regarding the transmission of tuberculosis, information is necessary required for reduced risk of transmission of TB on flights travelers

Statement of Problem: Researchers are often looking for appropriate treatments while considering esthetic aspects and health. In endodontically treated teeth afflicted with severe damage or complete loss of the coronal structure, a post is usually inserted in the root canal to provide intra-canal retention of the restorations. Therefore, it seems that it is necessary to investigate the stress distribution in the restored teeth with different post algorithms


Purpose: In this study, the effects of post geometry and its dimensions on the stress distributions and levels in the root of molar teeth repaired by post-core crown [Ceramic, Alumina and Nickel-chrome] were studied using finite element method


Materials and Method: An extracted intact mandibular second molar tooth was embedded in a cylindrical acrylic resin mould and then were sliced. The sections generated in this way were photographed and the images were transferred into the Solidworks software. After tooth modeling, Posts and crowns were designed. Then, the samples were exposed to a uniform distributed load of 240 N with the load angle of 45 degree which was applied over the occlusal area. In order to simulate the surrounding area of the tooth and also the periodontal ligament space, the bone was also modeled


Results: Numerical results revealed that in the prefabricated post restoration, there was a stress concentration in the cervical region. The maximum value of normal stress [32.3 MPa] was seen in the parallel post with 1.4 diameter [D1.4], and the minimum value [26.7 MPa] was observed in the double tapered post [N0.1]. It was observed that the increase in the modulus of elasticity from 100 to 300 causes an increase in normal stress from 69.5 to 38.5 MPa and in tensile stress from 69.5 to 38.5 MPa


Conclusion: Numerical analysis showed that the maximum stress concentration in post core crown restoration increased when the posts with 1.4 mm diameter or post with double taper were used. The stress values decreased with the decrease in the post modulus of elasticity. The maximum stress in the residual dentin decreased by an increase in the post diameter. Besides, the maximum stress on the residual dentine changed irregularly by the change in the tapered post diameter. Also, it was found that crowns material did not have any considerable effect on the stress concentration

The neurobrucellosis can affect any part of the central or peripheral nervous system. The clinical syndromes are ultimately diverse and clinical picture may be confused by the coexistence of two or more clinical syndromes in the same patient. The most common neurologic manifestation is a sub-acute or chronic meningoencephalitis. Acute toxic manifestations [e.g., headache, neck ache, backache, insomnia, depression and muscle weakness] are seen during the acute phase of infection. Incidence of neurobrucellosis cannot be exactly estimated. The most important clinical syndromes include: Brucella meningitis and encephalitis, vascular syndromes, myelopathy and spinal disease and psychiatric disturbances. Diagnosis is made by reviewing patient history, physical examination, CSF and serum serological and other laboratory studies. Treatment is difficult. Doxycycline in addition to rifampin and streptomycin are the best-studied regimen which should be continued until CSF is clear. In the year following treatment, serum agglutinins fall to normal. The efficacy of corticosteroids is not proved. Neurobrucella has a better prognosis rather than other forms of chronic meningitis and the lesser of mortality; however, incidence of minor sequella [not limiting the normal life] is high. We presented one case of neurobrucellosis with presentation Acute toxic manifestations infection, include headache, neck ache, backache, insomnia, depression and muscle weakness several days before admission. The serologic study of both serum and CSF will usually reveal normal. But antibody against Brucella demonstrated in CSF by enzyme - linked immunosorbent assay [ELIS]. Doxycycline in addition to rifampin; ceftriaxon cotrimoxazol started for patient and patient cured. The diagnosis of neurobrucellosis require differentiation from other chronic meningitis, encephalitis, and granulomatous disease, most importantly tuberculosis and fungal infectious.early diagnosis and treatment are crucial factors leading to a good outcome

RESUMO

A case report with a review of the litterature The authors report a case of primary malignant lymphoma of the thyroide [PMLT] in a 22 year old woman. Clinical presentation was a cold nodule median and ocupyring the base of the gland. The diagnostic of centroblastic diffuse PMLT was ascertained upon definitive histologic study. No other locallzations were found upon investigations During the fifth chemo-therapy cure; the patient developed a diffusion to the central nervous system. In the light of this case, a review of epidemiological, clinical and pathologic data on PMLT in the literature is presented
