RESUMO

This study determined the prevalence of inherited factor V Leiden mutation in a group of 128 thrombosis patients [102 with venous thrombosis and 26 with arterial thrombosis] attending a hospital in Sousse, Tunisia, and a control group of 100 with no history of thrombosis. Using an allele-specific PCR amplification technique, factor V Leiden was found in significantly more patients [20.3%] than controls [6.0%]. The higher prevalence was significant in the subgroup of venous thrombosis patients but not in arterial thrombosis patients. The allele frequency was 3.5% in the normal Tunisian population. Screening Tunisian patients with venous thrombosis and their relatives for factor V Leiden may be justified

RESUMO

Inflammatory pseudo tumors of the paratesticular tissue are uncommon lesions. They are often reported in the literature by various terms. These lesions are clinically worry some evoking a malignant testicular tumor. We describe a 57-year-old man with a long time history of painless scrotal mass that encased the left testis. Once excised, histology showed fibrosis and inflammatory tissue with lymphoplasmacytic cells

RESUMO

The women with chronic renal failure in advanced stage have a compromised fertility. The hemodialysis improves considerably the fertility of these patients. However, the pregnancies in the dialysed patients are still rare and often lead to spontaneous miscarriage or therapeutic interruption of pregnancy. In our report, we present a case of pregnancy in a dialysed patient and we discuss the outcome, the diagnosis and the management of these pregnancies

RESUMO

We report the case of a 49-year-old diabetic patient with emphysematous pyelonephritis. Taking into consideration the patient's young age, renal failure and the absence of large parenchymatous lesions, we decided on conservative treatment consisting of percutaneous drainage and antibiotics. Based on our case and a review of the literature we discuss the various treatment modalities

RESUMO

We herein report the case of a 38-year-old female patient with crohn's disease who presented with right hydroureteronephrosis associated with extrinsic compression following an episode of acute inflammatory ileocecal disease. Percutaneous nephrostomy was performed then a double-J stent was placed via the antegrade approach and treatment with corticosteroids was started. The patient responded well to the treatment, and the double J stent could be removed 4 months later. At 2 years' follow-up there is no recurrence of the hydroureteronephrosis

RESUMO

To analyze the clinical characteristics and to evaluate the different factors that influences the prognosis of the peripartum cardiomyopathy [PPCM]. A retrospective review was undertaken on records of women who were diagnosed with peripartum cardiomyopathy at Farhat Hached Hospital [Sousse] between January 1992 and December 2004. Clinically, PPCM shows pulmonary symptoms such as dyspnea and tachypnea. The diagnosis is established by echocardiography that showed decreased systolic function of the left ventricular. Both gynecologist and cardiologist must check the patients regularly. No patient died. Three preterm pregnancies occured with 9 health newborns [2 sets of twins]. One miscarriage took place. PPCM is often undetected or misdiagnosed because of the low incidence and the unspecific symptoms. The treatment is also unspecific and similar to dilated cardiomyopathy or acute cardiac failure. Early diagnosis of the peripartum cardiomyopathy is extremely important. Pregnancy in patients with dilated cardiomyopathy is associated with maternal and fetal morbidity. Left ventricular function is a prognostic factor and must be the most parameter when conseling patients with peripartum cardiomyopathy about a new pregnancy
