RESUMO

Multiple approaches exist for the management of impalpable undescended testicle. With the use of diagnostic laparoscopy widely accepted in the setting of the non-palpable testis, laparoscopic orchiopexy seemed to be a logical extension. We report our experience with laparoscopic orchiopexy in treating 36 impalpable testis in 32 children at Tanta University Hospital, three of them had undergone a negative groin exploration at other centers, Twelve children had an absent testis at the side under investigation [33.3%]. Twenty children with 24 intra-abdominal testes underwent laparoscopic assisted orchiopexy. Eighteen of them were submitted to a single - stage standard subdartos orchiopexy, and two-stage orchiopexy for the remaining six with a success rate reaching 87.5%. There were no technical failures or laparoscopy-related complications in all of these children. We concluded that laparoscopic assisted orchiopexy is a satisfactory and reliable technique which has the advantage of being accurate diagnostic and proper therapeutic in one setting

RESUMO

Neonatal gastrointestinal perforations continue to be associated with, high morbidity and mortality rates. The aim of this study was to define causes, risk factors, and management of neonatal gastrointestinal perforations; and to analyze factors relevant to outcome. The medical records of 45 neonates treated at Tanta University hospital for gastrointestinal perforations during the last 6 years were retrospectively reviewed. Forty-five infants [25 males and 20 females] were analyzed. Their birth weight ranged front 1100 to 3500 grams [mean 2200 +/- 750 grams]. Perforations occurred from birth to 26 days [mean 10.5 days]. Main causes of perforations included, necrotizing enterocolitis [NEC] [21, 46.7%], spontaneous gastroduodenal perforation [5, 11%], iatrogenic colorectal perforations [4, 8.8%], spontaneous intestinal perforations [3, 6.6%], Hirschsprung's disease [2, 4.4%], and meconium ileus [2, 4.4%]. The terminal ileum and cecum were the most frequent sites of perforation. All patients underwent laparotmy except one patient, who died prior to surgical intervention. There were 19 deaths [42.2%]. Nearly haIf of deaths [9, 47.4%] occurred in infants with NEC. 1 Septicemia, low birth weight, prematurity and delayed recognition of perforation were responsible for the majority of deaths in our series; 2: Although the overall mortality in this series compares favorably with similar other large series in the literature, there is still a root, for improvement should iatrogenic perforations and delay in diagnosis be avoided: and 3: Simple closure for isolated perforations or resection and primary anastomosis is the treatment of chronic, however, creation of stoma in some instances may be warranted

RESUMO

A total of 1186 Egyptian students from the governorate capital cities of Menoufia, Beheira and Beni Sueif were interviewed between October and December 1993 to determine their knowledge of and attitudes towards human sexuality. The sample comprised 620 male and 566 female students aged 13-20 years [preparatory and secondary schools]. The results indicated that there was a general lack of knowledge of sexual issues. An informal programme is recommended to improve adolescents' knowledge of sexual matters and correct misconceptions

RESUMO

Smooth muscle [SMT] of gastro-intestinal tract [GIT] is rare. Among GIT bleeding cases treated over the last 3 years, five of them had SMT, out of them 3 had leiomyoma and the other 2 had leiomyoblastoma. Bleeding was mainly a sequela of ulceration of the overlying mucosa. On dealing with a case of GIT bleeding, SMT should be considered as cause on studying the etiology to direct the effort to the proper line of treatment

RESUMO

Gallbladder [GB] mucosal ablation is called chemical cholecystectomy [CC], which is proposed as an alternative to removal of the GB. This study assessed the efficacy of two sclerosants [tetracycline and ethanolamine oleate] in producing CC. Two groups of dogs [5 per group] were submitted to this study. After ligation of cystic duct the GB mucosa was treated with 5% tetracycline solution in group A and 5% ethanolamine oleate in group B. Group A animals developed partial mucosal destruction, whereas those of group B developed complete mucosal destruction with fibrosis. No damage was detected in common bile duct in either group. No damage was detected in the liver of group A animals, whereas those of group B developed inflammatory cellular reaction and fibrosis in the pericystic hepatic tissue. One dog of group B developed subphrenic abscess. Ethanolamine oleate was more decisive in producing CC than tetracycline, but with a drawback of inflammation and fibrosis of the surrounding liver tissue

RESUMO

The etiology and pathogenesis of nonulcer dyspepsia [NUD] are not completely understood and consequently, no specific medication can be prescribed to the sufferers. We studied 69 cases, proved clinically, laboratorally, sonographically and endoscopically, to have NUD. Antral biopsy was taken from every case and subjected to CLO test. Out of the 69 patients, 51 [74%] were infected with Helicobacter pylori [HP]. Oral medication, consisting of amoxacillin 500 mg, metronidazol 500 mg, and bismuth mixture, 5 mls three times daily was prescribed for four weeks. Out of the 51 patients at the end of four weeks, 46 cases [90%] were cleared of HP, and only 38 cases [74.5%] experienced complete improvement of their dyspeptic symptoms. We arrived at the conclusion that a good percentage of NUD patients can be cured by the use of the 3-drug combination
