Extended -Highly Selective Vagotomy [EHSV] at present is considered by most authoroties to be the best avenue for the treatment of peptic ulcer disease and to be more spicific for the treatment of duodenal ulcers. The presence of Helicobacter pylori on gastric mucosal biopsies is strongly associated with chronic duodenal ulceration [CDC]. although there are data on the effect of anti-Helicobacter therapy on CDC, the incidence ulcer recurrence postoperatively when the organisms are eradicated has not been ertablished. We studied the incidence of CDC recurrence after EHSV when anti Helicobacter therapy was and was not employed preoperatively. No recurrence occured in Helicobacter negative patients, while two recurrences occured in the positive group. We suggest that in addition to adequate surgical technique, Helicobacter pylori eradication before EHSV may decrease the incidence of ulcer recurrence
