RESUMO

Background The contradiction between science and operability has always existed in the model of classified occupational health supervision and management. Comprehensive risk assessment method for occupational disease hazards of employers provides risk grading and classification for occupational health management, and it's a new attempt to explore classification supervision and law enforcement. Objective To apply a comprehensive risk assessment method for occupational disease hazards of employers to estimate health risk level of wood furniture enterprises, discuss its advantages and disadvantages, and provide a basis for improving the classified management of occupational health. Methods Seven typical factories were selected in counties with highly concentrated wooden furniture manufacturing. Occupational health field investigation and testing were carried out to estimate occupational disease hazard risk level (Ⅰ, Ⅱ, and Ⅲ). A self-rated checklist was applied to score occupational health management status by interviewing employers, one by one, and to determine occupational health management status grade (A, B, and C) by the score. Thus, a comprehensive risk level (Class A, Class B, and Class C) of a specific factory was obtained from a matrix of occupational disease hazard risk level and management status grade. Risk verification was carried out based on any abnormality reported by regular occupational physical examination in past 3 years. Results Defects in occupational health management were identified in all 7 factories, among which 6 were grade C with key nonconformities (poor), and 1 was grade B (medium). Disqualified occupational disease hazards were found in 6 of 7 factories, and the workstation-specific disqualified rates were 26.09% (12/46) for noise, 14.71% (5/34) for wood dust (hard), and 12.50% (1/8) for xylene. Level Ⅱ (medium) of occupational disease hazard risk was estimated in 5 of 7 factories, while level Ⅲ (high) in 2 factories. All 7 factories were class C (high risk) accessed by the comprehensive risk assessment method for occupational disease hazards. The occupational health surveillance documents showed 636 batches of regular occupational physical examination were ordered by the 7 employers, and a total of 37 workers from 5 factories reported abnormalities in physical examination, among which 3 workers reported dust exposure and 34 workers reported noise exposure. Conclusion The comprehensive risk assessment method for occupational disease hazards of employers is not able to perform with satisfaction in occupational health classification of wooden furniture manufacturing factories yet. It is necessary to expand the pilot to improve this assessment method and develop an efficient supervision mechanism to ensure the authenticity of the data before it is popularized and applied in classified occupational health management.

RESUMO

Objective@#To identify the most appropriate meteorological variable for forecasting the health risk of high temperatures.@*Methods@#The surveillance on causes of death, meteorological data and surveillance on air quality among registered residents in Ningbo City, Zhejiang Province during the period between May and October from 2013 to 2019 were collected. The association models of daily minimum temperature, average daily temperature, daily maximum temperature, daily minimum heat index, average daily heat index, daily maximum heat index, average daily apparent temperature and torridity index with deaths and years of life lost (YLL) were created using time series analysis and distributed lag non-linear models, and the model fitting effect was evaluated using the minimum Akaike information criterion (AIC) procedure. The most appropriate meteorological variable for forecasting gender-, age- and mortality-specific health risks of high temperatures was identified.@*Results@#A total of 120 628 deaths were reported during the study period, with daily deaths of 94 cases, and daily YLL rate of 19.74 person-years/105. Except for daily minimum heat index and torridity index, the exposure-response relationships between other six meteorological variables and deaths and overall YLL rate all appeared a “J” shape. The lowest AIC values and the optimal model fitting effects were measured for the association models between average daily temperature and whole populations, females, subjects at ages of 65 years and older, and deaths and YLL rates due to circulatory diseases and respiratory diseases.@*Conclusion@#High model fitting effects are observed between average daily temperature and deaths and YLL rates, which are more suitable for forecasting the health risk of high temperature.

RESUMO

Angiosarcoma is a rare malignant tumor of soft tissue in adults. However, tumor ulcers caused by foot angiosarcoma are rarely reported in the literature. This paper outlines a case of foot angiosarcoma misdiagnosed as diabetic foot ulcer. It was treated as diabetic foot ulcer in other hospitals without notable improvement. In consideration of its special clinical characteristics and treatment process, pathological biopsy was performed after admission, and angiosarcoma was confirmed. The purpose of this article is to emphasize that foot angiosarcoma or other tumor ulcers in diabetes patients are easily misdiagnosed as diabetic foot ulcers. If there is no obvious improvement in treatment, pathological tissue biopsy should be performed as early as possible.

RESUMO

Objective@#To investigate the reference level of surface radiation dose of digital radiography (DR) among adult examinees in Beilun District, Ningbo City, so as to provide insights into the optimization of the parameters of DR systems and rational management of the radiation dose.@*Methods@#Based on the cloud imaging system covering 14 DR systems in 11 public hospitals in Beilun District, the diagnosis of adults receiving DR and DR systems check were collected from May 2020 to April 2021. The air kerma-area product ( PKA ) was selected as a measure, and the local diagnostic reference level ( LDRL ), the regional median dose ( RMD ), the regional lowest dose predicted level ( LDPL ) and the typical dose in the DR imaging room were defined using the quartile method according to the examination site, projection direction and radiography positions. The PKA value was transformed into incident air kerma ( Ka, e ), and compared with the recommended value in the Requirements for Radiological Protection in Diagnostic Radiology ( GBZ 130-2020 ).@*Results@#DR was performed among 133 065 person-times from May 2020 through April 2021 in Beilun District. The greatest RMD was found on abdomen AP DR in the supine position ( 120 μGy·m2 ), and the lowest RMD was seen on chest PA DR in the standing position ( 17 μGy·m2 ). The maximums of typical dose in the DR imaging room on chest PA ( 33 vs. 30 μGy·m2 ), abdomen AP ( 161 vs. 153 μGy·m2 ) and pelvis AP DR ( 164 vs. 162 μGy·m2 ) were greater than LDRL in the supine position, and the minimums of typical dose on chest PA ( 10 vs. 11 μGy·m2 ) and chest LAT DR ( 33 vs. 34 μGy·m2 ) were lower than LDPL in the standing position. Following transformation into Ka, e, the LDRL ranged from 0.27 to 3.80 mGy, and the maximums of typical dose ranged from 0.25 to 3.55 mGy, which was much lower than the recommended dose proposed in the national criteria.@*Conclusions@#The reference level of surface radiation dose of DR is established among adults in Beilun District, which is lower than the recommended dose in the national criteria of China. Such a reference level may be employed by medical institutions for optimization of the parameters of DR systems. Key words: digital radiography radiation dose diagnostic reference level air kerma-area product

RESUMO

Objective:To investigate the death of patients with granulocytopenia complicated with infection after chemotherapy and the changes of vital signs before emergency treatment, and to analyze the prognosis of different vital signs on patients' prognosis and emergency treatment. Methods:This study used a case-control study method to select 211 patients with hematologic malignancies who met the inclusion criteria in two tertiary hospitals in Weifang City. The vital signs of patients were collected and the vital signs were analyzed using SPSS 17.0 software. And the statistical significance and predictive value in the emergency response group. Results:The heart rate, respiratory rate, systolic blood pressure, blood oxygen saturation and urine volume were significantly different between the survival group (112 cases) and the death group (99 cases)( t values were 11.038-177.102, P<0.01). The area under the receiver operating characteristic curve of body temperature, heart rate, respiratory rate, systolic blood pressure, and blood oxygenation saturation and urine volume were 0.547, 0.495, 0.294, 0.899, 0.988, and 0.827, respectively. The highest predictive efficiency (higher level) was observed with the change of blood oxygen saturation, and the corresponding optimal cutoff point. 0.91; between the emergency treatment group (103 cases) and the non-emergency treatment group(108 cases), the difference in heart rate, respiratory rate and oxygen saturation between the two groups was statistically significant ( t values were 5.247, 8.001, 9.066, P<0.01). The area under the receiver operating characteristic curve of body temperature, heart rate, respiratory rate, systolic blood pressure, blood oxygen saturation and urine volume were 0.581, 0.732, 0.813, 0.346, 0.102, and 0.543, respectively. Among them, the predicted value of respiratory frequency change was the highest (medium level), which was the best corresponding. The cutoff point was 27.5. Conclusions:Patients with granulocytic infection after malignant hematologic disease will have abnormal changes in vital signs before death and emergency treatment. However, different vital signs have different effects on predicting disease changes, and should focus on respiratory rate and oxygen saturation. Changes, when the respiratory rate exceeds 27 beats/min, the probability that the patient needs to implement emergency treatment such as rescue will increase. If the condition is not effectively controlled, the blood oxygen saturation is lower than 0.91, the risk of death of the patient is greatly increased.

RESUMO

Objective @#To explore season-specific association between ambient PM2.5 and outpatient visits for asthma in the urban areas of Ningbo.@*Methods @#Based on the platform of population health information,we collected daily outpatient visits for asthma in four general hospitals in the urban areas of Ningbo from 2014 to 2016. We also collected data of meteorological indicators and air pollutants during the same period from local weather bureau and environmental monitoring center. Generalized additive model(GAM)was applied to examining the relationship and lag effects between ambient PM2.5 and outpatient visits for asthma in different seasons. @*Results @#A total of 45 184 outpatients with asthma were reported from 2014 to 2016,with an average of 41.23 cases per day. The annual average concentration of PM2.5 in Ningbo was(43.12± 26.40)μg/m3 ,with the peak as(65.27± 34.51)μg/m3 in winter and the though as(27.34± 12.70)μg/m3 in summer. A 10 μg/m3 increase of PM2.5 were associated with 1.14%(95%CI:0.48%-1.81%),2.40%(95%CI:0.22%-4.63%)and 1.37%(95%CI:0.28%-2.48%)increase of outpatient volume for asthma on the day in the whole year, summer and winter, respectively. In summer, males were at higher risk compared to females(P< 0.05);in winter, people under 65 years old were at higher risk(P< 0.05). Including NO2,the effect of PM2.5 on the excess risk of outpatient visits for asthma in summer was enhanced in two-pollutant models(P< 0.05). @*Conclusion @#PM2.5 pollution might increase the outpatient volume for asthma in summer and winter. NO2 might have synergistic effect with PM2.5 on asthma.

RESUMO

Objective@#To explore the relationship between different temperature indicators and hospital admission for childhood pneumonia.@*Methods@#The hospital admissions for pneumonia in children aged 0-14 years and meteorological data in Ningbo from 2015 to 2017 were collected. A distributed lag non-linear model combined with a generalized linear model was employed to analyze the exposure-response relationships between different temperature indicators(daily average, minimum and maximum temperature;the first percentile as low temperature and the 99th percentile as high temperature)and hospital admission for childhood pneumonia.@*Results@#A total of 4 542 cases of childhood pneumonia were recruited. There were obvious seasonal fluctuations found in the inpatient volume of childhood pneumonia, which peaked in winter and bottomed in summer. After adjusting for potential confounding variables such as relative humidity,PM2.5,long term trend and seasonal trend, the results suggested that after exposed to whether low or high temperature, the inpatient volume of childhood pneumonia would increase. When the daily average temperature and daily minimum temperature were employed, the effect of high temperature on the increase of inpatient volume for childhood pneumonia was statistically significant and the cumulative relative risk for a lag of 0-7 days were 1.52(95%CI:1.04-2.23)and 1.59(95%CI:1.08-2.34),respectively. When the daily maximum temperature was employed, the effect of low temperature on the increase of inpatient volume for childhood pneumonia was statistically significant and the cumulative relative risk for a lag of 0-7 days were 1.30(95%CI:1.02-1.66).@*Conclusion@#Our findings suggested that an increased risk of hospital admission for childhood pneumonia was associated with both low and high temperature.

RESUMO

OBJECTIVE To study the effect of different concentrations of 17-AAG and EGCG monotherapy or in combination on the induced apoptosis in human nasopharyngeal carcinoma, and to explore new molecular targets for the treatment of nasopharyngeal carcinoma. METHODS MTT colorimetric method and fluorescent staining were used to detect the change of CNE proliferation inhibition rate and cell morphology. And furthermore, the expression level of Bcl-2, Bax, Caspase-3 were detected by RT-PCR. RESULTS 1. 17-AAG or EGCG alone had inhibitory effect on the human nasopharyngeal carcinoma CNE cells at 24 h, 48h and 72 h, and it was related with time and dose(P<0.01). The inhibition effect of combination of 17-AAG and EGCG was significantly increased, which was time and dose dependent(P<0.01). 2. RT-PCR was used to detect the mRNA expression level of Bcl-2, Bax and caspase-3. The level of Caspase-3 and Bax mRNA expression after treated by 17-AAG and EGCG was significantly higher, and the level of bcl-2 mRNA expression was lower than that after treated by 17-AAG or EGCG alone. CONCLUSION Our investigation implied that 17-AAG and EGCG in combination can effectively inhibit the proliferation of human nasopharyngeal carcinoma CNE cells. The involved mechanisms may be associated with the upregulation of Bax and Caspase-3 expression.

Objective To observe the differences in functional capacities generated by different simple breathing balloons extruded by different hand types and methods. Methods The lung functional measurement instrument was used to measure the generated functional capacities of two types of simple breathing balloon extruded by different hand methods: using big-, mid- and small-size hands with 5 fingers generally separately open and maximally extended states or with both small-size hands to extrude balloon; the effective generated gas quantity of the above methods were compared. Results ① Comparison between different balloons: the functional capacity generated by black rubber balloon extruded by any hand type was 68 - 132 mL lower than that generated by light blue silicon balloon. ② The comparison between different hand types: the functional capacities generated by any hand at maximally extended state in extruding balloon was 13 - 70 mL higher than that by hand commonly separate state; extrusion by a small size hand was nearly unable to reach 400 mL, while the functional capacity was 520 - 650 mL when the balloon was extruded by a big size hand, and 435 - 635 mL by a mid-size hand; it was necessary to use both small hands when the black rubber balloon was extruded, when the light blue silicon air bag was extruded, the functional capacity could reach 430 - 440 mL with a small size hand. ③ Difference in extruding methods: the functional capacity generated by either big size hand or mid-size hand with 5 fingers maximally extending state to extrude balloon was significantly higher than that with 5 fingers commonly separate state, the functional capacity generated by both small size hands with fingers maximally separating and extending state to extrude balloon was obviously higher than that generated by 5 fingers generally separating and extending state, the functional capacities generated by light blue silicon balloon were obviously higher than those by black rubber balloon, no matter the 5 fingers of big- or mid-size hand being at generally separating or extending state (mL: 623.00±21.11 vs. 522.00±41.85 by big size hand with common 5 fingers separate state, 649.00±26.01 vs. 575.00±58.55 by big sizehand with maximum 5 fingers extending state; 566.00±37.77 vs. 436.00±21.19 by mid-size hand with common 5 fingers separate state, 637.00±30.02 vs. 505.00±37.49 by mid-size hand with maximum 5 fingers extending state); the light blue silicon balloon extruded by small hand with 5 fingers at generally separate state and at maximally extending state could generate functional capacities (mL)432.00±13.02 and 444.00±37.18 respectively, significantly higher than those using the 2 types of hand state extruding a black rubber balloon (the tidal volume < 400 mL), the functional capacities generated by both small hands extruding a light blue silicon balloon was obviously higher than that by using a black rubber balloon (mL: 557.00±54.98 vs. 489.00±40.12, allP < 0.05).Conclusions Different functional capacities will be generated by different hand sizes, different hand extruding methods and types of simple breathing balloon, clinical application should be based on patients' body weights to decide their tidal volumes, and combined with the rescuers' hand sizes and types of simple breathing balloon to choose a proper extruding method for a certain patient, thus sufficient oxygen can be surely provided in time for him/her and rescue successful rate can be elevated.

RESUMO

Objective To explore the relationship between sudomotor function and peripheral artery disease (PAD),and to evaluate the effectiveness of sudomotor function testing to screen diabetic patients at high risk of PAD comparing to ankle-brachial index (ABI) and toe-brachial index (TBI).Methods A total of 263 diabetes mellitus (DM) outpatients in the 306th Hospital of PLA from August 2014 to April 2015 were enrolled in the study.ABI and TBI were measured by the Doppler method.Sudomotor function was evaluated by measuring the electrochemical skin conductance (ESC) of the hands and feet using the Sudoscan instrument.Cardiovascular autonomic neuropathy (CAN) was assessed and recorded as cardiac autonomic neuropathy risk-score (CAN-RS) by Sudoscan. Results ESC values of the feet and hands were positively correlated with ABI and TBI.The diabetes patients with abnornal ABI and TBI had significantly lower hand ESC [(45.63±12.87) μS vs.(68.10±17.40) μS,(59.17±19.58) μS vs.(68.57±17.11) μS;P< 0.05] and feet ESC [(44.54±25.48) μS vs.(70.92±19.46) μS,(59.21±24.52) μS vs.(71.71±19.02) μS;P< 0.05],and higher CAN-RS[(49.17± 15.41)% vs.(36.33±16.25)%,(44.90±16.09)% vs.(35.39±16.05)%;P< 0.05],than diabetes patients with normal ABI and TBI.Using ABI as the gold standard, the areas under the receiver operating characteristics (ROC) curve of the diagnostic performance of hands ESC,feet ESC and CAN-RS to identify PAD were 0.87,0.84 and 0.74,respectively (P<0.001).Conclusion Sudomotor function testing can be helpful and beneficial to identify PAD in patients with diabetes.

RESUMO

Objective@#To investigate the incidence, distribution features, and incidence trend of occupational diseases in Ningbo, China, from 2006 to 2015.@*Methods@#In February 2016, the data on occupational disease report cards in the occupational disease and occupational health information system from 2006 to 2015 were collected to perform a comprehensive analysis of the types, incidence trend, population characteristics, and enterprise characteristics of occupational diseases.@*Results@#There were 845 new cases of occupational diseases in Ningbo from 2006 to 2015, among which 596 (70.53%) were pneumoconiosis; there were 445 (74.66%) cases of stage I pneumoconiosis, 73 (12.25%) cases of stage II pneumoconiosis, and 78 (13.09%) cases of stage III pneumoconiosis; silicosis (59.76%) was the most common type of pneumoconiosis. Of all patients with occupational diseases, 84.97% were male, 64.50%were aged 35-55 years, and 63.67% had 5-20 working years. Most of the patients with occupational diseases worked in small and medium-sized private (or foreign) enterprises; the most common industries were black metal smelting and rolling (38.76%) , construction (17.11%) , and non-metallic ore mining (13.09%) . A total of 51.28%, 10.26%, and 7.69% of the patients with occupational poisoning worked in the electric apparatus manufacturing industry, furniture manufacturing industry, and chemical raw material and chemical product manufacturing industry, respectively. The cases of occupational diseases are mainly distributed in Yuyao City (24.62%) , Yinzhou District (23.91%) , and Ninghai County (15.62%) .@*Conclusion@#Pneumoconiosis is the most common type of occupational disease in Ningbo and most of these patients work in small and medium-sized private (or foreign) enterprises. The supervision of small and medium-sized private enterprises should be enhanced to protect workers’ health.
