RESUMO

Objective:To investigate the protective effect of dapagliflozin on cell damage and in HK-2 cells induced by high concentration of glucose.Methods:HK-2 cells were divided into four groups: control group (NC) , high glucose model group (HG) , dapagliflozin group (CS-179) and metformin group (PC) . After treatment with different drugs for 24 h, CCK-8 assay was applied to determine HK-2 cells viability; ROS, SOD, CAT and MDA levels were measured by a multi-detection reader; The protein expression of Nrf2 was determined by Western blot.Results:The results of CCK8 showed that the cell survival rate of the high glucose model group was 58.0%±0.8%, and that of the dapagliflozin group was 87.0%±0.4%. Dapagliflozin significantly increased the survival rate of HK-2 cells, and the results were statistically significant ( P<0.01) . The microplate reader test found that compared with the high glucose model group (ROS: 3.46 ± 0.05, MDA: 25.37 ± 0.61, SOD: 55.89 ± 4.09, CAT: 10.22 ± 1.67) , dapagliflozin reduced the accumulation of ROS (1.97±0.04) and MDA (9.5±0.4) caused by high glucose in HK-2 cells (both P<0.01) , increasing the vigor of SOD (114.95±4.19) and CAT (32.83±2.01) (both P<0.01) . Compared with the expression of Nrf2 protein in the high glucose model group (0.26±0.03) , the expression of Nrf2 protein (0.48±0.03) in dapagliflozin group was significantly increased ( P<0.01) . Conclusion:Dapagliflozin can alleviate the HK-2 cells damage induced by high concentration of glucose via reducing oxidative damage, and acti-vating Nrf2 antioxidant transcription factor.

Objective:To explore the relationship between sex hormone binding globulin and glycolipid metabolism in patients with obese polycystic ovary syndrome.Methods:From April 2016 to April 2019, 114 patients with obese polycystic ovary syndrome in the Integrated Chinese and Western Medicine Hospital of Wenzhou were selected and divided into two groups according to the level of sex hormone binding globulin, including 66 cases in the low level group and 48 cases in the normal group.The blood glucose, blood lipid and sex hormone levels of the two groups were measured and the results were analyzed.Results:The levels of TC, TG, LDL-C and ApoB in the low level group[(2.32±0.97)mmol/L, (6.45±1.25)mmol/L, (3.57±1.01)mmol/L, (0.93±0.32)g/L] were significantly higher than those in the normal group[(1.87±0.89)mmol/L, (4.88±0.94)mmol/L, (3.12±0.88)mmol/L, (0.76±0.17)g/L]( t=2.477-7.322, all P<0.05). The level of ApoA in the low level group[(1.22±0.34)g/L] was significantly lower than that in the normal group[(1.58±0.47)g/L]( t=-4.748, P<0.05), while there was no statistically significant difference in HDL-C between the two groups[(1.45±0.31)mmol/L vs.(1.43±0.56)mmol/L, t=0.244, P>0.05]. The levels of FPG, HbA1C and FINS in the low level group[(5.53±1.45)mmol/L, (6.42±0.79)% and (18.71±5.46)mU/L] were significantly higher than those in the normal group[(5.09±0.76)mmol/L, (6.01±0.92)% and (15.49±4.98)mU/L]( t=2.136-3.225, all P<0.05), while there was no statistically significant difference in 2hPG between the two groups[(8.79±1.56)mmol/L vs.(8.32±1.84)mmol/L]( t=1.472, P>0.05). The incidence of insulin resistance in the low level group[54.55%(36/66)] was significantly higher than that in the normal group[35.42%(17/48)](χ 2=4.088, P<0.05). The level of T in the low level group[(61.45±21.44)ng/mL] was significantly higher than that in the normal group[(50.39±22.31)ng/mL]( t=2.673, P<0.05), while there were no statistically significant differences in FSH, LH, E 2 and PRL between the two groups( t=0.427-1.714, all P>0.05). Conclusion:The patients with low levels of sex hormone binding globulin are more likely to suffer from hyperglycemia and hyperlipidemia, and low levels of sex hormone binding globulin may be a risk factor for obese polycystic ovary syndrome.

Objective:To explore the risk factors of diabetic nephropathy(DN).Methods:From January 2016 to September 2019, the clinical data of 403 patients with type 2 diabetes in the Integrated Traditional Chinese and Western Medicine Hospital of Wenzhou were analyzed retrospectively.Excluding other factors, they were divided into DM group (315 cases) and DN group(88 cases) according to whether or not they were complicated with DN.The age, course of disease, complications, monocyte to high-density lipoprotein cholesterol ratio(MHR) and other test data were included in the logistic regression model to analyze the independent risk factors of DN.Results:There were statistically significant differences in the course of ≥10 years[95 cases(30.16%) vs.37 cases(42.05%)], complicated with hypertension[141 cases(44.76%) vs.58 cases(65.91%)], insulin application[94 cases(29.84%) vs.39 cases(44.32%)], ACEI/ARB application[132 cases(41.90%) vs.62 cases(70.45%)], FBG[(7.38±2.45)mmol/L vs.(8.26±2.06)mmol/L], HbA1c≥7%[91 cases(28.89%) vs.36 cases(40.91%)], MHR[(10.30±4.38) vs.(17.10±7.18)] and HDL-C[(0.58±0.16)mmol/L vs.(0.52±0.17)mmol/L] between the two groups( t=4.41, 13.31, 6.52, 22.46, 3.08, 4.60, 11.02, 2.65, all P<0.05). Multivariate logistic regression analysis showed that hypertension( OR=2.06, 95% CI=1.36-2.76), HbA1c≥7%( OR=1.32, 95% CI=1.04-1.60), MHR≥11.78( OR=1.65, 95% CI=1.03-2.67) were independent risk factors for DN. Conclusion:Hypertension, HbA1c ≥7%, MHR ≥11.78 are independently correlated with DN, which provide a basis for clinical prognosis.

Objective@#To analyze the current situation and needs of primary and secondary school health clinic services, and to provide a reference for strengthening school health service in China.@*Methods@#Investigation provinces were selected according to China s three major economic zones, 1 or 2 survey centers were selected from each province, 16 provinces and 26 survey centers were selected. In each survey centers, six schools from primary and secondary schools, junior high schools and high schools in urban and rural areas were randomly selected as research sites. Each research site randomly selected 10 students in each grade(grade three, four, five from primary schools) to fill in the questionnaire anonymously. A total of 4 661 valid questionnaires were collected.@*Results@#Among the services provided by the school clinics to students, "trauma treatment" "consulting health problems" "physical examination" accounted for 49.4%, 40.5% and 39.0% respectively. Within the past one year, 45.1% students went to the school clinic for consultation or consultation 1-3 times, 6.4% for 4-5 times, and 3.7% for 6 times or more. When students were sick in school, 65.8%(primary school), 64.0%(junior high school), and 54.4%(high school) of them selected to go to the school clinic. When suddenly suffered trauma in school, 78.2%(primary school), 73.2%(junior high school) and 69.6%(high school) of the students went to the school clinic.@*Conclusion@#Primary and middle school students have a great demand for school health services, and it is necessary to continue to strengthen the construction of primary and secondary school health clinics.

Objective To explore the effect of Buyanghuanwu decoction on blood glucose control and renal function protection in patients with early diabetic nephropathy.Methods 170 cases of patients with early diabetic nephropathy from May 2012 to June 2013 in wenzhou hospital of integrated traditional chinese and western medicine were selected,all patients were divided into observation group and control group,the control group was given routine treatment,the observation group treatment with Buyanghuanwu decoction based on control group .The clinical effective rate was compared between the two groups after treatment,the levels of blood lipids, 24 h urine microalbumin and hemorheology were compared between the two groups before and after treatment.Results After treatment, the total effective rate of the observation group was 90.59%,which was significantly higher than the control group 76.47% (P<0.05).Compared with before treatment,the levels of triglycerides and cholesterol decreased in two groups,high-density lipoprotein levels increased,24 h urine microalbumin decreased,whole blood viscosity,whole blood viscosity trimming and fibrinogen decreased,the difference was statistically significant(P<0.05).Compared with the control group,the levels of triglycerides and cholesterol in observation group was lower,high-density lipoprotein was higher,24 h urine microalbumin was lower,whole blood viscosity,whole blood viscosity trimming and fibrinogen were lower,the difference was statistically significant(P<0.05).Conclusion Buyanghuanwu decoction in the treatment of diabetic nephropathy is effective in treating diabetic nephropathy,it can effectively reduce blood lipids and blood viscosity,significantly reduce urinary protein excretion and protect the kidneys.

Objective To evaluate the bacteriostatic effect of recombinant human lactoferrin(rhLF) on Helicobacter(H .) py‐lori and its influence on CagA ,Ure and gastric mucosal IL‐8 .Methods The minimum inhibitory concentration(MIC)and the influ‐ence of different drug concentrations on the proliferation of H .pylori were detected .The effects of rhLF on the mRNA and protein expressions of CagA and Ure in H .pylori were detected by RT‐PCR and Western blot ,respectively .The animal study :Balb/c mice were adopted and assigned randomly into four groups ,including the standard triple+rhLF(group A) ,rhLF(group B) ,standard tri‐ple(group C) and normal saline(group D) .The histopathological HE staining was used to observe the gastric inflammation and ELISA was used to detect the IL‐8 level of gastric tissue in each group .Results MIC was 0 .5 mg/mL ,moreover rhLF inhibited the bacterial growth and proliferation with a concentration‐dependent manner .rhLF could reduce the expression of H .pylori major viru‐lence factor CagA ,mRNA and protein of Ure .Comparing the group A with the group B ,C and D ,the gastric mucosal inflammation score and the IL‐8 levels of gastric tissue homogenates had statistically significant differences(P<0 .05) .Conclusion rhLF inhibits the growth and proliferation of H .pylori ,moreover inhibit the expression of major virulence factor CagA in H .pylori ,mRNA and protein of Ure in different degrees ,weakens its pathogenicity ,meanwhile reduces the IL‐8 level in mice gastric mucosa ,and allevi‐ates H .pylori related gastric mucosal inflammatory response .

RESUMO

Objective To investigate the treatment of combined subclavian artery and brachial plexus injury. Methods Since 1994, we have treated 5 cases of combined subclavian artery and brachial plexus injury. 2 of them were cut wound, 1 gun shot wound, and 2 blunt trauma. 2 cases had combined aneurysm and 1 had combined aneurysm plus A V fistula. According to the sites of subclavian artery injury, vein graft( 1 case) , suture repair( 1 cases) and ligation (1 case) were performed. The 2 cases of blunt trauma underwent only neurolysis because of masses of hematoma and tissue adhesion. Results The 3 cases of open injury who had underwent early repair of artery and nerve had various degrees of recovery of nerve functions and experienced no pain in the injured extremities. As for the 2 cases of blunt trauma, 1 got only pain relief and 1 had no recovery. Conclusion Dysfunction due to hematoma or progressive pseudoaneurysm compression on the brachial plexus can be relieved and patients can get maximal rehabilitation if hematoma or aneurysm is managed in time and the brachial plexus nerve is decompressed early.
