RESUMO

Objective: to investigate the association between obesity indices, abdominal fat distribution, and lipid profile in patients with stable angina [SA]


Methods: body weight, height, waist circumference [WC], body mass index [BMI], and waist /height ratio [WHtR] of 123 patients with SA who underwent coronary angiography were measured. Fasting blood samples were taken to measure the levels of fasting blood sugar [FBS], total cholesterol [TC], low- and high-density lipoprotein cholesterol [LDL-C, HDL-C], apolipoproteins A and B [apo A and apo B], and triglycerides [TG]. According to angiography reports, the participants were divided into patients with or without coronary heart disease [CHD]. All patients underwent an abdominal computerized tomography [CT] scan to measure the visceral, superficial, and deep subcutaneous fat


Results: the mean ages of the patients with CHD [n = 73] and without CHD [n = 50] were 50.5 +/- 7.6 and 53.7 +/- 7.6 years, respectively [P = 0.03]. The patients with CHD had significantly higher levels of TC, TG, and superficial subcutaneous fat, while the patients without CHD had higher levels of apo A [P = 0.05]. Multivariate analyses showed a significant association of visceral fat with TC, LDL-C, TG, and apo B, in the patients without CHD, while significant inverse associations were found between WC and HDL-C, WHtR, and apo A as well as visceral fat and LDL-C in the patients with CHD


Conclusions: among anthropometrics and imaging indices of obesity, WC and WHtR have shown better association between central obesity with dyslipidemia in the patients with CHD, while CT-measured visceral adipose tissue area was the best correlate of dyslipidemia in the patients without CHD

RESUMO

McCune-Albright syndrome [MAS] is a rare non-inherited disorder characterized by the clinical triad of precocious puberty, cafe-au-lait skin lesions, and fibrous dysplasia of bone. We report a girl with MAS, presenting initially with vaginal bleeding at the age of 17 months. Ultrasonography revealed unilateral ovarian cysts and ureteral and ovarian enlargement. Considering the clinical and paraclinical findings, the patient diagnosed as a case of gonadotropin-independent precocious puberty was treated with medroxy-progestrone acetate [MPA] for three months. During the follow up, recurrent episodes of bleeding, ovarian activation and cyst formation, as well as breast size development were reported. At the age of 5.5 years, fibrous dysplasia was detected, which in coexistence with precocious puberty confirmed the diagnosis of MAS. The patient had no cafe-au-lait skin macles during follow up. Considering that clinical manifestations of MAS appear later in the course of recurrent periods of ovarian activation and cyst formation, a careful clinical observation and follow up of patients is necessary and the diagnosis of MAS must be kept in mind in cases with gonadotropin-independent precocious puberty

RESUMO

Pancreas as the insulin-producing gland is subjected to destruction and change in the diabetes-producing process. Realtime sonography can assess the gland in 95% of cases and its accuracy in diagnosis of pancreatic disease matches that of CT-scan. The purpose of this study was to evaluate pancreatic diameter and echogenicity by sonography and to examine the correlation of these two factors with duration of disease in diabetes types I and II in comparison with controls. In two groups of 60 diabetic patients and healthy controls, diameter and echogenicity of pancreas was determined. Diameter of pancreas was significantly different in diabetic patients and correlated with duration of disease. In type I diabetes, decrease in the size of pancreas was more prevalent than in type II diabetes and these changes become more prominent over time
