RESUMO
Progressive rubella panencephalitis (PRP) is a rare late complication of rubella that affects mainly teenagers. It is characterized by progressive white matter destruction, gliosis and cerebral atrophy, similar to subacute sclerosing panencephalitis (SSPE) and congenital rubella syndrome. This paper examines the complexity of PRP, its clinical manifestations, pathogenesis, diagnostic criteria, treatment and prophylaxis. PRP should be considered in adolescents with progressive dementia with pyramidal and cerebral dysfunction. With respect to the most affected children, survival and recovery is expected to be poor, with high mortality rates, especially in the first six months of life. The development of progressive spasticity, ataxia, mental deterioration and convulsions in late childhood and early childhood with mothers' rubella or stigmata histories is the subject of research on PRP. Therefore, continued efforts to understand and address PRP are important to improve the quality of diagnosis, treatment and ultimately the quality of life of affected persons.
RESUMO
Background: Cardiovascular diseases are a leading cause of morbidity and mortality worldwide. The prevalence and risk factors of CVDs vary across different populations and regions. This study aimed to observe the cardiac profile among the adult population of Sreemangal, Bangladesh, to identify the prevalence and risk factors of CVDs in this population. Methods: This prospective observational study was conducted over a six-month period at a public health camp in Sreemangal, Bangladesh. A total of 137 adult participants were purposively selected. Data were collected through a structured questionnaire and physical examination, including measurements of blood pressure and laboratory analysis of blood samples. Results: The study found that 44.53% of participants had elevated blood pressure, and 21.90% had stage 1 hypertension. Interestingly, 36.50% of participants were unaware of their hypertension status. Furthermore, 26.28% of participants had a known history of diabetes, but blood glucose level analysis revealed an additional 14.85% of participants were prediabetic, and 6.93% were diabetic based on fasting blood glucose levels. A statistically significant association was found between increasing age and the stage of hypertension (p<0.001), and between fasting and normal blood glucose levels and the stage of hypertension (p<0.005 and p<0.05, respectively). Conclusions: The findings of this study underscore the need for targeted interventions to prevent and manage CVDs in the adult population of Sreemangal, Bangladesh. These interventions should include regular screening for CVD risk factors, health education to increase awareness of these risk factors, and strategies to promote healthy lifestyle behaviors.

Background: The efficacy and side-effect profile of ulipristal acetate (UPA) for the treatment of symptomatic uterine fibroids before surgery are unclear. Main objective of the study to see the effect of UPA on leomyoma. Methods: An observational clinical trial was conducted at the department of gynaecology and obstetrics, Sir Salimullah medical college and Mitford hospital, Dhaka, from January to June 2015. Thirty women aged 18-48 with symptomatic fibroid uterus were studied. They received Tablet UPA 5 mg/day for 3 months. Evaluation included history taking, physical examination, pelvic ultrasonography, and necessary investigations. Results: The majority of patients (43.3%) were ?30 years old. Before intervention, heavy bleeding was observed in all patients until the 8th day of menstruation. During intervention, majority had no bleeding from the 5th to 8th day, with only a few experiencing heavy bleeding on the 7th and 8th day. After intervention, most patients had no bleeding from the 5th to 8th day, with a few reporting spotting on the 5th day. Before treatment, 60% had fibroid size >10 cm2. After treatment, 92.3% had fibroid size <10 cm2, with 7.7% showing no fibroids. Mean uterine size decreased post-treatment. Majority (57.7%) had hemoglobin >11 gm/dl. Side effects included headache (11.5%), hot flashes (15.4%), and nausea/vertigo (19.2%). Treatment success rate was 86.7%.Conclusions: In a group of selected patients with fibroid uterus and heavy menstrual bleeding, UPA (5 mg/day for 3 months) successfully decreases blood loss and shrinks fibroid and uterine size by 86.7%.
RESUMO
Background: Endoscopic sinus surgery (ESS) presents challenges in managing intraoperative bleeding and hemodynamic stability. This study evaluates the efficacy of pre-operative oral bisoprolol in improving surgical conditions and outcomes in ESS. Methods: This study was conducted between March 2021 and June 2022 at the department of anaesthesia, Bangabandhu Sheikh Mujib medical University, Dhaka, Bangladesh. This randomized controlled trial was conducted with 50 participants undergoing elective ESS, divided into bisoprolol and placebo groups. Result: The study involved 50 participants undergoing elective ESS, with 25 in the bisoprolol group and 25 in the placebo group. While demographic characteristics, such as age, weight, height, and gender distribution, showed no statistically significant differences between the groups. The placebo group experienced significantly higher estimated blood loss (421.72 ml vs. 156.24 ml, p<0.001) and postoperative hemoglobin levels (12.88 g/dl vs. 11.07 g/dl, p<0.001) compared to the bisoprolol group. Hemodynamic parameters, particularly heart rate, exhibited significant differences at various time points, with the bisoprolol group maintaining a higher heart rate post-premedication, intra-operatively, and post-operatively (p<0.05 for all). In the assessment of intraoperative bleeding using the Fromme-Boezaart scale, the placebo group demonstrated higher incidences of severe bleeding grades (3 and 4) compared to the bisoprolol group, with these differences being statistically significant (p<0.001). Conclusions: Pre-operative oral bisoprolol in ESS patients significantly reduces intraoperative bleeding and anesthetic requirements while maintaining hemodynamic stability. These findings suggest bisoprolol as a beneficial pre-operative medication in ESS, warranting further research to optimize surgical outcomes.
RESUMO
Abstract Increased anxiety and depressive symptoms have reported to be its association with long term illness. Because of having unwanted effects of newly available drugs, patients administering anxiolytic drugs usually discontinue the treatment before they are completely recovered. Therefore, there is a serious need to develop new anxiolytic drugs. The anxiolytic effect of hydro-alcoholic extract of Agaricus blazei in animal models was assessed. 24 male mice (Mus musculus genus) were included in the study. Four groups were prepared and each group contained six animals. The groups were vehicle control, positive control (diazepam 1.0 mg/kg, i.p.) as well as two treatment groups receiving Agaricus blazei hydro-alcoholic extract at a dose of 136.50 mg/kg and 273.0 mg/kg orally. The Marble burying test, Nestlet shredding test and Light and Dark box test used to assess anxiolytic activity. Mice administered with diazepam 1.0 mg/kg, i.p. while hydro-alcoholic extract of AbM (136.50 and 273.0 mg/kg, respectively) was administered via oral route which exhibited marked reduction in number of marbles-burying as compared to vehicle control group. Mice administered with diazepam 1.0 mg/kg, i.p. and Oral administration of hydro-alcoholic extract of AbM (136.50 and 273.0 mg/kg, respectively) exhibited significant decrease in nestlet shredding in comparison to vehicle control group. The oral administration of hydro-alcoholic extract at a dose of 136.5mg/kg and 273mg/kg showed elevation in time spent in light box and was comparable to standard treated group while time spent by mice following oral administration of hydro-alcoholic extract of Agaricus blazei at a dose of 273.0 mg/kg also showed elevation and was found to be more near to standard treated group (diazepam 1 mg/kg, i.p.).
Abstract The present study involves the chemical and bacteriological analysis of water from different sources i.e., bore, wells, bottle, and tap, from Peshawar, Mardan, Swat and Kohat districts of Khyber Pakhtunkhwa (KP) province, Pakistan. From each district, 50 water samples (10 samples from each source), regardless of urban and rural status, were collected from these sources and analysed for sulphates, nitrates, nitrites, chlorides, total soluble solids and coliforms (E. coli). Results indicated that majority of the water sources had unacceptable E. coli count i.e.> 34 CFU/100mL. E. coli positive samples were high in Mardan District, followed by Kohat, Swat and Peshawar district. Besides this, the some water sources were also chemically contaminated by different inorganic fertilizers (nitrates/nitrites of sodium, potassium) but under safe levels whereas agricultural and industrial wastes (chloride and sulphate compounds) were in unsafe range. Among all districts, the water quality was found comparatively more deteriorated in Kohat and Mardan districts than Peshawar and Swat districts. Such chemically and bacteriologically unfit water sources for drinking and can cause human health problems.
Abstract Magnolia biondii Pamp is an important ornamental tree species widely grown and used as a rootstock in the propagation of different Magnolia varieties. In the current studies, anatomical, physiological and endogenous hormones were studied to check the effect of IBA 750 mg/L on the adventitious rooting and to provide theoretical and technical support for the propagation of Magnolia biondii Pamp through stem cuttings. Two thousand stem cuttings were prepared and divided into two groups i.e., IBA treated cuttings and water control. For the evaluation of antioxidant enzyme activities, and endogenous hormones levels, samples were collected on the day of planting and each 5th day and further steps were carried out in the laboratory according to the protocols and proper precautions. For the anatomical observations, samples were collected on the 13th, 15th, and 17th day for IBA treated cuttings while 21st, 23rd, and 25th day for control. Collected samples were preserved in the FAA solution and further observations were carried out in the laboratory. Anatomical observations showed that it took 13 days for the differentiation of root primordia to the appearance of young adventitious roots in IBA treated cuttings, while it took 21 days to develop primordia in the control. Antioxidant enzyme activities involved in ROS were significantly higher in the IBA treated cuttings compared to control. POD showed a peak on the 13th day before the emergence of roots in IBA treated cuttings while it showed a peak on the 21st day in the control. PPO showed a peak on the 21st day in the IBA treated cuttings while it showed a peak on the 29th day in the control. SOD showed a peak on the 17th day in IBA treated cuttings, while it showed a peak on the 25th day in the control. Exogenous application of IBA enhanced the endogenous IAA and GA3 levels compared to CK, while it reduced the levels of ABA continuously at the time of rooting and then increased gradually. Inclusively, our study suggests that IBA 750 mg/L is efficient for the rooting of Magnolia biondii Pamp cuttings, as it enhanced the process of antioxidant enzyme activities, endogenous hormones levels and reduced the time of root formation which is evident from the anatomical observations.

RESUMO
ABSTRACT Pulmonary arteriovenous malformations (PAVMs) are abnormal vascular connections between pulmonary arteries and veins, often associated with hereditary hemorrhagic telangiectasia (HHT). Most PAVMs are asymptomatic, but life-threatening complications like pulmonary hemorrhage, brain abscesses, and paradoxical emboli can emerge, so prompt diagnosis and treatment are crucial. We report a case of sudden pediatric death in a two-year-old female with no past medical history. Initial vomiting and fast deterioration resulted in a sudden cardiac arrest. The postmortem examination found histological evidence of consistent, extensive lung damage. The absence of the characteristic symptoms made for some challenges when it came to diagnosis, showing precisely that in early life, you could well have many difficulties in catching PAVMs. This case highlights the need to take PAVMs into account as a potential cause of sudden death, particularly when there are no conspicuous symptoms. Awareness among forensic pathologists and consideration of genetic analysis for HHT in such cases is crucial for accurate diagnosis and management.

In addition, this study aimed to identify and grade solid organ injuries according to the American association for the surgery of trauma organ injury scale. Results: Prevalence of hepatic injuries (76.8%), solid organ injuries (90.1%), road traffic accidents (71.8%), falls from height (25.4%). Most received conservative treatment, had short hospital stays, and a 7% mortality rate. Gender had no significant impact on organ injuries (p>0.001). Among the causes, 84.5% had hepatic injuries, 15.5% had spleen injuries due to road traffic accidents, and 58.3% had injuries from falls from height, with a highly significant cause-organ injury relationship (p<0.001).Conclusions: pediatric blunt abdominal injuries in Bangladesh affect various organs, primarily in males and school-aged children, primarily due to road accidents. Timely surgical intervention and preventive measures are crucial for improved outcomes.
RESUMO
Co-morbidities are characterized by the presence of multiple chronic diseases within a single individual. They pose an escalating healthcare challenge and managing this growing burden, particularly with non-communicable diseases, has become a major concern. However, a significant impediment to effective management lies in medication nonadherence. The importance of medication adherence has long been a focus of healthcare literature, yet many comorbid patients tend to underestimate its significance, leading to health deterioration. Research studies indicate alarmingly low adherence rates in comorbid patients. To address this issue, patient education and healthcare campaigns have become essential tools in enhancing adherence. The research, which commenced on October 19, 2023, was initiated after an exhaustive review of existing academic literature. Multiple databases, such as PubMed, Web of Science, and Cochrane, were employed for this comprehensive literature review, utilizing a wide array of medical terminology combinations. The study aims to shed light on potential strategies and educational approaches to enhance medication adherence in comorbid patients. Findings from this research have the potential to influence healthcare policy, the design of tailored interventions, and overall healthcare delivery improvements for co-morbid patients, ultimately leading to enhanced health outcomes, reduced costs, and an improved quality of life.
RESUMO
Background: Vitamin D deficiency is a global health concern affecting individuals across all the age groups in both the genders. It is an essential element that is essential for the production, regulation and absorption of calcium and phosphorus by the human body, as well as for the proper maintenance of teeth and bones and to make your immune system work. Vitamin D is not actually a vitamin; it falls under the pro-hormone category. Because the human body is perfectly capable of producing vitamin D on its own when exposed to sunlight, and vitamins are the type of nutrients that the body cannot or cannot produce automatically, that we consume through dietary supplements. Methods: It was a cross sectional study the sample size was 369. Vitamin D level of all the participants was estimated by using an ADVIA Centaur XPT stem designed to perform continuous operations immunoassay methods. The data were processed to undergo statistical analysis using SPSS 23 windows program. Microsoft Word, Microsoft Excel were used to represent the tabular, charts and graphical icon. Results: The prevalence of vitamin D deficiency exhibited a medical condition increment in both the males and females. This study had clear association was observed using bivariate analysis for any of the three chronic conditions such as hypertension, cardiovascular disease and osteomalacia where (p=0.021), (p=0.039) and (p=0.000) explored in the current study with severe deficiency status. Conclusions: Vitamin D deficiency was prevalent even in apparently healthy young individuals; this study is suggestive of a higher prevalence of vitamin D deficiency among young adults, females, Bangladeshi nationality and those with higher BMI.
RESUMO
Inflamed appendix as a cause of incarcerated inguinal hernia is a rare presentation in any age group. Appendix as a content in an inguinal hernial sac was named after the French surgeon Claudius Amyand (c.1681-1740), who performed the first successful appendectomy, on an 11-year-old boy who presented with an inflamed, perforated appendix in his inguinal hernia sac. Treatment of this condition is herniotomy with its repair with or without appendectomy depending on its involvement (inflammation status). We are reporting the case, in which, the inflamed appendix was presented as incarcerated inguinal hernia. The management of Amyand抯 hernia depends on the variability of involvement of the appendix however, in children and neonates, it is better to perform appendectomy during the hernia repair.
RESUMO
Background: Multi-drug resistant organisms, particularly in urinary tract infections, have become a significant concern in developing countries like Bangladesh. Physicians are facing challenges in treating hospitalized cases due to the ineffectiveness of conventional antibiotics and empirical treatment, as well as the emergence of multi-drug-resistant Escherichia coli. The main goal of the study was to observe the prevalence and resistant pattern of multidrug-resistant Escherichia coli urinary tract infections among patients admitted to a teaching hospital. Methods: The cross-sectional study was conducted between August 2011 to February 2012 at Uttara Adhunik Medical College Hospital (UAMCH), Dhaka, Bangladesh. It included 100 cases, and detailed information was obtained through a standardized protocol. Results: In a study of 100 UTI patients, 45 had MDR E. coli while 55 had non-MDR E. coli. Females accounted for 79% of the patients, with a mean age of 44.85±17.81 years. The majority of participants fell into the 31-40 years age group (28%) and 60 years age group (23%). Among the participants, 57% had a history of UTI while 43% had no previous UTI history. High resistance was observed against amoxicillin, amoxiclav, cephradine, cefuroxime, cefixime, ceftriaxone, ceftazidime, and nalidixic acid. However, ciprofloxacin (2.22%), levofloxacin (6.67%), and cotrimoxazole (31.11%) exhibited lower resistance rates among MDR samples. Imipenem and meropenem showed 100% effectiveness against all MDR samples. Conclusion: MDR E. coli rates were alarmingly high in a teaching hospital in Bangladesh. Excessive antimicrobial drug consumption globally has led to antibiotic-resistant E. coli isolates, posing challenges for effective UTI treatment worldwide. Antibiotic therapy remains crucial in controlling these invasive agents.
RESUMO
Paraphenylenediamine (PPD) is a widely used chemical in various hair dye formulations. Due to its cheap price and easy availability, it emerges as a major cause of suicidal poisoning. It carries high morbidity and mortality and does not have any specific antidote. We reported an unusual type of poisoning of a 44-year-old man with PPD and how he was managed with an excellent outcome.
RESUMO
Background: Colorectal cancer (CRC) is one of the lethal cancers all over the world. Early detection of colorectal cancer has been shown to reduce incidence and mortality. Primary care physicians have a crucial role in early detection of cancer. This study aims to explore the knowledge, attitude and practice of primary care physicians in Bahrain towards CRC screening and to determine the barriers related to CRC screening. Methods: A cross sectional study was carried out among primary care physicians working at primary health care centers in Bahrain. It included 174 physicians. A self-filled questionnaire that includes demographic information, knowledge scale, attitude scale, practice scale items and barriers of not performing CRC screening was used as the study tool. Results: The overall knowledge score revealed that 51.7% had poor knowledge score and 48.3% had adequate knowledge score. Majority of the participants (93.7%) agreed to have a structured screening program for colorectal cancer rather than an opportunistic one. Most of the physicians (60%) reported that less than 25% of those eligible patients truly receive a screening. Physician抯 lack of time was the top barrier of not performing CRC screening. Conclusions: This study showed that most primary care physicians have poor knowledge regarding CRC screening and the majority do not screen their eligible patients for CRC.
