A importƒncia do novo desing da overlay provis¢ria na est‚tica do planejamento da reabilita‡Æo oral / The importance of the new design of overlay splint in aesthetics of oral rehabilitation planning

O desgaste dent rio quando excessivo ou patol¢gico pode levar a uma altera‡Æo da dimensÆo vertical de oclusÆo, gerando preju¡zos na est‚tica, fun‡Æo e fon‚tica do paciente. A reabilita‡Æo com a dimensÆo vertical de oclusÆo perdida ‚ complexa e trabalhosa para o cirurgiÆo dentista. Necessitamos primeiramente determinar se houve a perda dessa dimensÆo vertical e o quanto ela ocorreu. Para isso avaliamos se houve perda de suporte posterior, a hist¢ria de desgaste dental, distƒncia interoclusal, aparˆncia facial e fon‚tica. Detectada a perda da dimensÆo vertical de oclusÆo, o tratamento de elei‡Æo para restabelecˆ-la ‚ a confec‡Æo de uma overlay provis¢ria. Terapˆutica que apresenta reversibilidade, baixo custo e tempo operacional reduzido comparado a outros tratamentos reabilitadores. Neste trabalho mostramos que al‚m dessas vantagens a overlay provis¢ria pode apresentar um novo design que recobre nÆo s¢ a oclusal dos dentes remanescentes, mas h  o recobrimento de toda a face vestibular desses dentes at‚ a cervical, desaparecendo com a linha de uniÆo dente-placa, o que comprometia a est‚tica

Dental wear when excessive or pathological can lead to an alteration on vertical dimension of occlusion compromising aesthetics, function, and phonetic. The rehabilitation with loss of vertical dimension of occlusion is complex and laborious for the dentist. Therefore were assessed the presence of loss of posterior support, patients’ history of wearing, interocclusal distance, facial appearance and phonetics. Detected the loss of vertical dimension of the occlusion the chosen restorative treatment was an overlay splint. This therapeutics is reversible, and has reduced costs and operating when compared to other treatments. In addition this study presents the This paper shows that beside these advantages overlay splint also has a new design that covers not only the occlusal tooth remaining, coating the entire buccal surface of these teeth extending to the cervix. Such design makes the line uniting teeth and splint indistinguishable optimizing aesthetics

Planejamento intelectual da seleção de pilares protéticos na reabilitação implantorretida / Intellectual planning of the selection of abutments for screw-retained implant rehabilitation

O tema seleção de pilares protéticos é, sem dúvida, um pré-requisito para o sucesso da reabilitação do paciente. Para se realizar um planejamento intelectual da seleção dos pilares protéticos ‚ fundamental conhecer as opções protéticas disponíveis e suas respectivas indicações e contraindicações. Entretanto, determinadas situações clínicas dificultam sua aplicação, devido ás limitações de certos componentes, …ás vezes estéticas, …ás vezes mecânicas. Com isso, vão sendo desenvolvidos novos componentes de acordo com as necessidades práticas, surgindo assim uma gama de pilares com o objetivo de facilitar a resolução de problemas clínicos. Este trabalho tem como finalidade demonstrar, através de um caso clínico, a importância do planejamento intelectual na seleção dos intermediários para se obter sucesso estético e mecânico em uma reabilitação metalocerâmica implantorretida parafusada com sistema tubo-parafuso, lateral ou transversal em implantes com inclinações vestibulares mantendo a reversibilidade

The selection of abutments is a requirement for a successful rehabilitation. To make an intellectual plan for the selection of prosthodontic abutment is extremely important to know the options available, as well as their indication and contraindication. However, some clinical situations may complicate its application, because of aesthetic or mechanical issues and other limitations of some components of the abutments. Therefore, new prosthodontics abutments are developed in order to meet practical needs and facilitate the solution of clinical issues. The present study aimed to present the importance of the intellectual plan for the selection of abutment in order to achieve aesthetic and mechanical success in a rehabilitation using metal ceramic screw retained implant, with lateral or transversal screw-tube system, in buccal angulation keeping the reversibility

Limitações clínicas nas próteses implantorretidas com conexões tipo cone morse (sem indexador) / Clinical limitations of cone morse (no index) implant-supported dental prostheses

Na atualidade, os implantes que apresentam interfaces protéticas tipo Morse têm sido preconizados como os de eleição para a reposição de dentes ausentes, em função de inúmeros benefícios discutidos na literatura. Contudo, apesar da variedade de componentes protéticos disponíveis no mercado, algumas situações clínicas dificultam a sua seleção devido às limitações estéticas e/ou mecânicas peculiares a cada um deles. Este artigo visa apresentar as dificuldades e limitações das próteses implantorretidas com conexões tipo cone Morse (sem indexador) e uma das alternativas propostas para a resolução destes casos conhecida como técnica do encaixe tubo-parafuso, parafuso transversal ou parafuso lateral. Muito embora a fase cirúrgica tenha sido mal conduzida, foi possível a confecção e adaptação satisfatória da prótese fixa implantorretida, permitindo atender às necessidades da paciente, devolvendo a função e a estética.

Currently, dental implants that present the Morse type prosthetic connections have been suggested as the first choice for the replacement of missing teeth, according to numerous benefits discussed in the literature. However, despite the variety of abutments available, some specific clinical situations difficult the selection of these prosthetic components, due to aesthetic and/or mechanical limitations of each one. This article presents the difficulties and limitations of cone Morse (no index) implant-supported dental prostheses and one of the alternatives proposed to solve these cases. This technique became known as tube screw technique, lateral fixation screw or screw-secured out- block. In spite of an unfavorable surgical phase outcome, it was possible to make a successful implant-supported prosthesis, satisfying patient’s expectations by retrieving their aesthetic and functional aspects.
