RESUMO

The hemorrheological changes were observed in 7 cases with portal hypertension after they were operated on with distal splenocaval shunt (DSCS). Similar cases with splenecto-my served as controls. It was found that blood hyperviscosity was slightly decreased after DSCS and the deformability of erythrocytes was improved from 7 to 14 days after DSCS. But blood hyperviscosity was significantly increased in the cases after splenectomy. Blood hyperviscosity was lower in the cases with DSCS than in those with splenectomy. Blood hyperviscosity , expressed mainly by the aggregation rate and deformability of erythrocytes, platelet count, etc, is dependent on the spleen. Since the spleen is preserved in DSCS so blood hyperviscosity is slightly decreased and complications due to blood hyperviscosity may be less in number after DSCS.

RESUMO

24 dogs for experiments were divided into 3 groups: total splenectomy, sham-operation and splenic autologous transplantation (SAT). Results of the study showed that in dogs the SAT (each splenic piece size 110.5 cm) into the omental pouch did survive well,and restore a part of function. The function of neutro-phil chemoluminescence and the phagocytosis rate of neutrophils, after splenectomy, in vitro, were lower than that of sham-operation group. The dogs of SAT could give some protective action, but the number of T-lymphocytes in splenic corpuscles and mononuclear phagocytes in splenic red pulps in regenerated splenic tissue were lower that of normal splenic tissue.

RESUMO

One hundred and forty-five mice were randomized into 4 groups:Group Ⅰ consisted of 25 mice with sham operation,group Ⅰ of 40 mice with hemisplenectomy,group Ⅲ of 55 mice with hemisplenic autotransplantation,and group Ⅳ of 25 mice with splenectomy.Twelve weeks after operation,it was found under optical microscopy,transmission electron microscopy and scanning electron microscopy that the autografted hemispleen was viable.The contents of alpha-naphthol acetylate esterase (ANAE) and acid phosphatase (ACP) in the splenic tissue were measured with microspectrophotometer.ACP level decreased continuously in group Ⅰ (0.462?0.013),group Ⅱ (0.304?0.009),and group Ⅲ(0.180?0.014) (P

RESUMO

In order to clarify the limitative factors in the management of severe acute pancreatitis (SAP) especially the problems of the complicated multiple organ failure (MOF),70 cases of SAP,of which 17 were complicated with MOF,admitted to our hospital in the past 16 years were reviewed.The overall mortality rate of the series was 20.0% (14/70),and the death of 12 cases out of the 14 was associated with MOF (85.7%) and that of the other 2 with single organ failure (14.3%).MOF occurred in 17 cases with a mortality rate of 70.0%.Two patterns of MOF manifestations identified:A rapid single-phase of MOF was found in 7 cases.It developed rather early after the onset of SAP and was associated with severe pancreatic necrosis and early shock.And a delayed two-phaseof MOF was seen in 10 cases.It developed progressively with a lagging interval and from the pancre-atic necrosis and subsequent infection.The clinical manifestations of MOF were characterized by a severe and systemic inflammatory response.It is believed that the systemic inflammatory response trigged by the septic material released from the pancreatic necrotic focus seems to be the pathological channel to link SAP with MOF.
