Insuficiencia renal aguda inducida por mordedura de serpiente Bothrops / Acute renal failure induced by Bothrops snake bite

Mujer de 58 años de edad, remitida a urgencias por presentar cuadro clínico de insuficiencia renal aguda (IRA) secundaria a mordedura de serpiente (Bothrops Atrox). Ingresa hipotensa con elevación de azoados e hiperkalemia, ecografía renal dentro de parámetros normales. Se maneja terapia dialítica con lo cual presenta mejoría clínica. En este reporte se detallan aspectos del diagnóstico, manejo clínico y posibles mecanismos fisiopatológicos que explican el daño renal.

A 58 years old woman was referred to the emergency room with acute renal failure (ARF) having suffered a snake bite (Bothrops Atrox). On examination she was found with hypotension, elevated kidney function, hyperkalemia and a renal ultrasound with no abnormalities. Clinical improvement was achieved with dialytic therapy. In this report, certain aspects of the diagnosis are highlighted such as clinical management and possible pathophysiological mechanism of urderlying kidney damage.

Insuficiencia renal aguda con necrosis tubular aguda secundaria a picadura masiva de abejas / Acute renal failure and acute tubular necrosis due to bee sting

Leñador de 46 años consulta al servicio de nefrología, de la Clínica Renal de la Costa en Barranquilla,con episodio de insuficiencia renal aguda 48 horas después de haber sufrido múltiples picaduras porabejas africanizadas. Durante su estancia hospitalaria presentó incremento de enzimas musculares(AST LDH), y de pruebas de función renal, motivo por el cual fue dializado en varias ocasiones. Conmejoría total, se decide egresar y manejar ambulatoriamente. Se concluye que el caso se trata de unainsuficiencia renal por necrosis tubular aguda por rabdomiolisis debida a la picadura múltiple deabejas africanizadas. / A farmer, 46 years old, consults the service of nephrology of “Clinica Renal de la Costa”, in Barranquillawith an episode of acute renal failure 48 hours after suffering multiples Africanized bee’s stings. Hepresented a rise in the muscular enzymes (AST LDH), and tests of renal function, therefore he wasdialyzed in several occasions with a further improvement. It is concluded that the case was due to anacute tubular necrosis by rabdomyolysis caused by multiple bee stings.
