RESUMO

Fifty-three patients diagnosed with degenerative lumbar canal stenosis were prospectively reviewed following limited segmental decompression. After a mean follow-up of 14.6 months, the subjective improvement in leg pain and walking distance was seen in 46 patients and 48 patients respectively. There were 4 dural tears and three superficial wound infection. Limited segmental decompression is safe and reliable method in treating elderly patients with symptoms of degenerative lumbar canal stenosis

RESUMO

Recovery of functionally acceptable digital motion after severance of flexor tendons has historically challenged surgeons dealing with upper extremity trauma of special surgical challenge is the area beyond the distal palmar crease, Zone II of fingers, and Zone II of the thumb. The main target of surgeons and researchers was to find the most suitable reparative method to avoid the occurrence of postoperative adhesions that present the principal cause of adhesions for the majority of authors. Since the beginning of the sixties, a constant improvement of results is continuously observed due to the continuous evolution of operative techniques, the better understanding of the pathophysiological nature of tendon healing, and the amelioration of postoperative therapeutic care. Despite this remarkable progress, flexor tendon lesions continue to present a difficulty. In our study, we use the modified Kessler technique for primary repair of the flexor tendons, and a modification of Duran's technique of postoperative passive motion stress protocol. Our clinical series contains 60 tendon lesions in 54 patients with an average follow-up of 8.7 months. Lesions are studied in all zones of the flexor tendons of the hand. Evaluation of the results is based oh the Strickland formula for the fingers and on the Tubiana system for the thumb. A final result of 70% of digits are excellent and good, while 30% are fair and poor. modified Kessler technique together with immediate immobilization based on controlled passive motion conducted by competent therapist is recommended to provide effective postoperative flexor tendon repair
