RESUMO

A 70- years old female referred with left upper quadrant pain since about 72 hours prior to hospital admission. In physical examination the patient was pale, hypotensive, tachycardiac and a vague left upper quadrant [LUQ] mass palpated. LUQ mass and free intraperitoneal fluid declared by ultrasound investigation there was no history of abdominal trauma and with impression of intraabdominal bleeding; of unknown origin the patient underwent laparotomy. A huge fundal gastric mass identified with surrounding rupturea retroperitoneal hematoma and intraperitoneal hemorrhage. No intragastric penetration or hemorrhage was seen, so complete excision of the mass with safe margin was done with subsequent splenectomy. She had an uneventful post operative course and was discharged at the 7th post- op day. A microscopic analysis of the excised tumor demonstrated gastric leiomyoma

RESUMO

Neural tube defects [NTD] are one of the most important malformations of newborns. The aim of this research is to determine the rates of NTD, anencephaly and spina bifida cystica and the relations of these abnormalities to the factors including sex, race, ethnicity, maternal age and familial marriage in the south-east of the Caspian Sea border. A three-year descriptive-analytic and cross-sectional study was carried out on 19545 newborns delivered in Dezyani hospital in Gorgan during 1998-2000. From the total sample population [19545], the NTD rate was 3.12 per 1000 deliveries. In males the rate was 2.49 per 1000, in females 3.79 per 1000, and the female to male ratio was 1.44:1. The rate of anencephaly and spina bifida was 1.25 per 1000 and 1.74 per 1000 respectively. In regard to different races the NTD rate was 7.48 per 1000, 9.5 per 1000, and 2.35 per 1000 among Turkmans, Sistanis and Farses, respectively. It can be concluded that there is a higher rate of NTD in this region that may be due to the race-ethnicity and the ecological variations

RESUMO

A 70 year old male presented with right lower quadrant pain and anorexia for about 72 hours prior to hospital admission. He underwent laparotomy with impression of acute appendicitis. The operative finding was an inflamed tumoral appendix without appendicular base involvement, and appendectomy was done. The patient had an uneventful post -op course. Due to the pathology report of prostatic carcinoma he underwent cystoscopy and biopsy of prostate on the 4th post-op day. Histopathology of the appendix was consistent with metastatic adenocarcinoma of the appendix with prostatic origin. Histopathology of the prostatic specimen was consistent with high grade prostatic adenocarcinoma
