RESUMO

Ticks are obligate parasites on animals and sometimes humans. They usually suck the blood of the hosts and can carry various infectious diseases as a vector. Otoacariasis is the presence of ticks and mites within the ear canal and relatively common in domestic and wild animals. However, tick infestations of human ear canals are rarely reported in the scientific literature and hardly occur in developed countries. Herein, we report a rare case of otoaracariasis involving Haemaphysalis longicornis . A 9-year-old girl living in a suburb presented with otalgia of left ear for 1 day. Otoscopic examination revealed a huge insect occluding the tympanic membrane. Tick removal and washing of external auditory canal was done successfully. The causative tick was identified as the H. longicornis. To our knowledge, this is the first reported case of human otoacariasis by a H. longicornis in Korea.

RESUMO

A 53-year-old woman presented with asymptomatic pedunculated mass on her buttock. She was attacked by the tick while climbing the Jungbyung mountain located in Changwon City, the southern coastal area of the Korean Peninsula. It was identified as male Amblyomma testudinarium according to its morphological characteristics. Histological examination showed dense inflammatory infiltration of neutrophils and eosinophils, and dilated vessels on the dermis beneath the biting site of the tick. There have been only few reports of the tick bite by A. testudinarium in the Korean literature. To the best of our knowledge, our patient is the second case of tick bite from this species and the first tick bite from the male tick recorded in Korea. Here, we report a case of tick bite by male A. testudinarium in a woman, including a review of relevant literature.

RESUMO

Onychomatricoma is a rare tumor of the nail matrix. Until now, few cases of onychomatricoma have been reported in the literature. Immunohistochemically, CD10, a marker of the onychodermis, is expressed in the stroma of the onychomatricoma. In the present case, a 27-year-old woman presented with an 8-year history of a yellowish, thickened, and overcurved nail plate of the right index finger, mimicking onychomycosis. She had been treated for 4 years with antifungal agents by general physicians, without improvement. The nail was surgically removed, and the tumor at the nail matrix was excised. The nail plate continued to grow in the 2 months after the excision. This is a case of onychomatricoma in South Korea, which was initially misdiagnosed as onychomycosis. In addition, we present a review of the literature regarding clinical, sonographic, and histological features, differential diagnoses, and treatment of onychomatricoma.

Onychomatricoma is a rare tumor of the nail matrix. Until now, few cases of onychomatricoma have been reported in the literature. BACKGROUND: Delusions of parasitosis (DOP) is a psychiatric disorder in which patients have a fixed, false belief that they are infested by parasites. Management of patients with DOP presents a challenge to dermatologists, because such patients usually refuse to see a psychiatrist. OBJECTIVE: The aim of this study was to investigate the clinical manifestations and responses to treatment in DOP patients. METHODS: Between January 2010 and December 2012, the medical records of 32 DOP patients were reviewed to obtain data on clinical presentation, family history, accompanied psychiatric disorders, and treatments. RESULTS: The male-to-female ratio was 1:4.3. Twenty-two patients (68.8%) had symptoms for over 6 months. Seven patients (21.9%) had apparent skin lesions. Eighteen patients (56.3%) had a positive specimen sign. Three patients had depression, 4 had insomnia, and 2 had other delusional disorders. Eight patients (25%) reported that their family members were also experiencing itching. Six patients underwent skin biopsies, the results of which were consistent with chronic dermatitis. With pimozide treatment, 6 patients (18.8%) achieved complete remission, 17 (53.1%) showed a partial response, and 9 (28.1%) showed no response. CONCLUSION: Because DOP patients depend on dermatologists for treatment, it is crucial for dermatologists to be prepared for the proper management of this condition. This study expands our understanding of the disease and suggests the optimal methods of treatment. Further studies on socioeconomic data and selection of effective antipsychotics are needed.

BACKGROUND: There have been a few reported cases of congenital great toenail dystrophy (GTND), described as a congenital malalignment of the great toenails. However, acquired GTDN is rare, and has not been documented extensively. This study aimed to describe the clinical features of 21 patients with acquired GTND. METHODS: Twenty-one patients with acquired GTND who visited Yeouido St. Mary's Hospital between June 2005 and August 2012 were retrospectively reviewed. RESULTS: The mean patient age was 43.1 years (range, 17 to 88 years), and the cohort predominantly comprised women (18/21). In our experience, all acquired GTND patients presented with yellow or yellow-brownish chromonychia, onychotrophy, and onycholysis. Conservative treatment with tape methods and grinding, as well as nail extraction, was provided and yielded little improvement in any case. CONCLUSION: This study provides initial data on the nail changes affecting the great toenail, such as yellowish chromonychia, onychomadesis, and onycholysis. These data may help physicians to distinguish various nail disorders, including onychomycosis, congenital malalignment of the great toenails, and yellow nail syndrome.

BACKGROUND: Rosacea is characterized by erythema of the central face that persists for several months or longer. Reports of the histological changes in rosacea are scarce, and few attempts have been made to correlate such changes with clinical findings and pathophysiology. OBJECTIVE: This study aimed to elucidate the clinical manifestations of rosacea and investigate its histological features. METHODS: We performed a retrospective analysis of 278 patients with histologically confirmed rosacea who visited the Department of Dermatology at the Catholic Medical Center between January 2008 and May 2013. Clinical subtypes, disease severity, and precipitating factors were evaluated. In 115 randomly selected patients, histopathological features were evaluated as well. RESULTS: The ratio of males to females was 1:1.8. The age distribution showed a peak incidence in the fifth decade. The most common subtype was papulopustular rosacea (52.9%) followed by erythematotelangiectatic rosacea (34.9%), ocular rosacea (4.0%), and phymatous rosacea (2.9%). Granulomatous rosacea accounted for 5.4% of rosacea cases. Precipitating factors included hot weather (54.7%), stress (51.8%), sun exposure (37.4%), alcohol (37.4%), and hot baths (33.1%). Histological analysis of skin biopsies from 115 patients revealed solar elastosis in 62 patients (53.9%) and telangiectasia in 85 patients (73.9%). CONCLUSION: In this study, Korean rosacea patients were predominantly female with a peak age in the fifth decade and the majority suffered from the papulopustular and erythematotelangiectatic types of rosacea. Histological observations pertaining to each rosacea type were also discussed.
