Objective: To analyze the short-term effect of individual atmospheric PM2.5 exposure on the diversity, enterotype, and community structure of gut microbiome in healthy elderly people in Jinan, Shandong province. Methods: The present panel study recruited 76 healthy elderly people aged 60-69 years old in Dianliu Street, Lixia District, Jinan, Shandong Province, and followed them up five times from September 2018 to January 2019. The relevant information was collected by questionnaire, physical examination, precise monitoring of individual PM2.5 exposure, fecal sample collection and gut microbiome 16S rDNA sequencing. The Dirichlet multinomial mixtures (DMM) model was used to analyze the enterotype. Linear mixed effect model and generalized linear mixed effect model were used to analyze the effect of PM2.5 exposure on gut microbiome α diversity indices (Shannon, Simpson, Chao1, and ACE indices), enterotype and abundance of core species. Results: Each of the 76 subjects participated in at least two follow-up visits, resulting in a total of 352 person-visits. The age of 76 subjects was (65.0±2.8) years old with BMI (25.0±2.4) kg/m2. There were 38 males accounting for 50% of the subjects. People with an educational level of primary school or below accounted for 10.5% of the 76 subjects, and those with secondary school and junior college or above accounting for 71.1% and 18.4%. The individual PM2.5 exposure concentration of 76 subjects during the study period was (58.7±53.7) μg/m3. DMM model showed that the subjects could be divided into four enterotypes, which were mainly driven by Bacteroides, Faecalibacterium, Lachnospiraceae, Prevotellaceae, and Ruminococcaceae. Linear mixed effects model showed that different lag periods of PM2.5 exposure were significantly associated with a lower gut α diversity index (FDR<0.05 after correction). Further analysis showed that PM2.5 exposure was significantly associated with changes in the abundances of Firmicutes (Megamonas, Blautia, Streptococcus, etc.) and Bacteroidetes (Alistipes) (FDR<0.05 after correction). Conclusion: Short-term PM2.5 exposure is significantly associated with a decrease in gut microbiome diversity and changes in the abundance of several species of Firmicutes and Bacteroidetes in the elderly. It is necessary to further explore the underlying mechanisms between PM2.5 exposure and the gut microbiome, so as to provide a scientific basis for promoting the intestinal health of the elderly.

ABSTRACT Introduction: Injuries will always be with an athlete throughout their career. When analyzing the factors that cause athletic injuries, their coaches usually consider environmental and physical training factors, ignoring psychological factors. Objective: Investigate the mental health status of college athletes after serious injuries during competition, discussing the impact of sports injuries on athletes' mental health. Methods: Twelve college athletes were selected as research volunteers. They were divided into two groups by randomization. The control group was followed by traditional psychotherapy while the experimental group received the post-traumatic mental intervention. A comparison was carried out and mainly the general situation of sports injuries and the effect of intervention treatment were analyzed. The impact of psychological intervention methods on athletes' mental health after injuries were also studied using mathematical statistics to analyze the data. Results: There were significant differences in STAI scores in the experimental group before and after the intervention. BFS scores in the experimental group were significantly higher than those in the control group before and after an intervention. There was no significant difference in the indicators in the control group. Conclusion: Post-traumatic psychological intervention positively affects the mental health recovery of college athletes. RESUMO Introdução: Os ferimentos estarão sempre com um atleta durante toda a sua carreira. Ao analisar os fatores que causam as lesões atléticas, seus treinadores geralmente consideram fatores ambientais e de treinamento físico, ignorando os fatores psicológicos. Objetivo: Investigar o estado de saúde mental dos atletas universitários após as lesões graves durante as competições, discutindo o impacto das lesões esportivas sobre a saúde mental dos atletas. Métodos: Foram selecionados 12 atletas universitários como voluntários de pesquisa. Eles foram divididos em dois grupos por randomização. O grupo de controle foi acompanhado pela psicoterapia tradicional enquanto o grupo experimental recebeu intervenção mental pós-traumática. Foi efetuada a comparação e analisada principalmente a situação geral das lesões esportivas e o efeito do tratamento de intervenção. O impacto dos métodos de intervenção psicológica sobre a saúde mental dos atletas após lesões também foi estudado utilizando o método de estatística matemática que é usado para analisar os dados. Resultados: Houve diferenças significativas na pontuação do STAI no grupo experimental antes e depois da intervenção. Os índices de BFS no grupo experimental foram significativamente maiores do que os do grupo de controle antes e depois de uma intervenção. Não houve diferença significativa nos indicadores do grupo de controle. Conclusão: A intervenção psicológica pós-traumática tem um efeito positivo sobre a recuperação da saúde mental dos atletas universitários. Al analizar los factores que causan las lesiones deportivas, los entrenadores suelen tener en cuenta los factores ambientales y de entrenamiento físico, ignorando los factores psicológicos. Objetivo: Investigar el estado de salud mental de los atletas universitarios después de sufrir lesiones graves durante las competiciones, analizando el impacto de las lesiones deportivas en la salud mental de los atletas. Métodos: Se seleccionaron doce atletas universitarios como voluntarios para la investigación. Se dividieron en dos grupos de forma aleatoria. El grupo de control recibió una psicoterapia tradicional, mientras que el grupo experimental recibió una intervención mental postraumática. Se realizó una comparación y se analizó principalmente la situación general de las lesiones deportivas y el efecto del tratamiento de intervención. También se estudió el impacto de los métodos de intervención psicológica en la salud mental de los deportistas después de las lesiones, utilizando el método de la estadística matemática para analizar los datos. Resultados: Hubo diferencias significativas en las puntuaciones del STAI en el grupo experimental antes y después de la intervención. Las puntuaciones del BFS en el grupo experimental fueron significativamente más altas que las del grupo de control antes y después de la intervención. No hubo diferencias significativas en los indicadores del grupo de control. Conclusión: La intervención psicológica postraumática tiene un efecto positivo en la recuperación de la salud mental de los deportistas universitarios. Objective: To investigate whether the contralateral normal external auditory canal (EAC) skin graft can maintain the ear canal health after EAC reconstruction in unilateral congenital aural atresia (CAA) cases. Methods: A Zelen design randomized controlled study was used to collect unilateral CAA patients for EAC reconstruction prospectively (clinical trial registration number: ChiCTR2000032103). The patients were randomly divided into the control group and the trial group. The trial group used the contralateral normal EAC skin graft group (transplant part of the contralateral normal EAC skin to repair the atresia side for unilateral CAA patients), the control group all used scalp blade thick skin. We observed the EAC health and hearing results of the two groups after EAC reconstruction. Results: A total of 13 cases were enrolled from July 2020 to August 2021. There were eight patients in the trial group, including six males and two females, with an average age of 22.3 years (14-36 years). There were two patients with CAA on the left and six patients on the right. The average follow-up time was 8.8 months (4-14 months). There were five patients in the control group, all cases were male with an average age of 16.2 years (12-20 years). There were four patients with CAA on the left and one patient on the right. The average follow-up time was 7.0 months (2-14 months). In the trial group, eight cases of reconstructed EAC epithelium were healthy, one patient had cicatricial stenosis of EAC opening and lateralization of the tympanic membrane. The other patient had cicatricial stenosis of reconstructed EAC, this case also had scar hyperplasia of the contralateral EAC opening but recovered after soft packing and triamcinolone acetonide injection treatment. The healthy side EAC of the rest trial group had no scarring stenosis or local bone hyperplasia during long-term follow-up. In the control group, one patient was lost to follow-up and the other four patients had dry ears of reconstructed EAC, but easily to form crusts and needed to be cleaned repeatedly, one patient had lateralization of the tympanic membrane, the EAC epithelium was not healthy for long-term follow-up. The incidence of complications related to EAC reconstruction was lower than previous studies (χ²=5.55, P=0.018), and the average postoperative Air-Bone Gap increased (18.8±10.0)dB. Conclusion: By optimizing the EAC reconstruction technology, the health of the reconstructed EAC is improved compared with the previous study. After active intervention and treatment, there should be no scarring stenosis or local bone hyperplasia on the contralateral side EAC.

Objective: To investigate the effect and satisfaction of using the postauricular tongue-shaped flap combined with deep fascia suspension in correcting severe cup ear malformation. Methods: Between August 2018 and November 2019, nine cases (10 ears) of Tanzer type Ⅲ severe cup ear malformation were treated with postauricular tongue-shaped flap combined with deep fascia suspension in this study. Results: Nine cases primary healed without hematoma, infection or skin necrosis, except one case with postoperative incision dehiscence. The cup ear deformity was significantly improved after the operation, the helix was intact, and the anti-helix was visible. The length, width, and circumference of the auricle were significantly enlarged three months following surgeries. There were no other complications except 1 patient recurrence 12 months after surgery. The patients were highly satisfied with the results of the surgery. Conclusions: Although the aesthctic of the reconstructed auricle is insufficient, this new technology makes full use of the autologous auricle tissue to correct severe cup ear malformation with good results.

Objective To investigate whether the metabolites in follicular fluid can be used as indicators for predicting oocyte quality and embryos early development in patients with polycystic ovary syndrome (PCOS). Methods The study subjects were divided into four groups: lean control (LC) 30 cases, overweight (OW) 13 cases, lean PCOS (LP) 26 cases, and overweight PCOS (OP) 26 cases based on the Chinese criteria for body mass index (BMI) categories. The metabolic variance of follicular fluid from PCOS and controls was performed by liquid chromatography-mass spectrometry (LC-MS), and clinical characteristics, oocyte outcomes and differences of metabolites in follicular fluid of those patients have been compared by ANOVA. Furthermore, Pearson's correlation analysis was used to explore the correlation between oocyte fertilization rate, top-quality embryos rate and follicular metabolites. Results Compared to the LC group, oocytes retrieved and top-quality embryos rate were significantly increased, while the mature oocytes rate and fertilization rate decreased significantly in the OP group. A total of 236 metabolites were identified and quantified by metabolomics in follicular fluid. Compared with LC and OP groups, 19 metabolites showed statistically significant differences in PCOS group. Additionally, 7 metabolites were significant correlated with the fertilization rate or top-quality embryo rate respectively, most of which were lysophospholipids. Conclusion Several metabolites in the follicular fluid are significantly different between PCOS and healthy women. Among these, lysophospholipids are crucial for oocyte quality and early embryonic development, may serve as potential markers to evaluate the oocyte quality in PCOS patients.

Effective supervision proves vital for the guidance of healthy development of private hospitals in China. Based on study and analysis of the current situation and problems of private hospitals supervision in China, the authors reviewed previous studies, leveraged " system+ mechanism+ technology" resources, and attempted to explore and build an integrated, omnidirectional, efficient and diversified linkage supervision system. Such a system was based on the information technology and fits the development trend of private hospitals in the Internet-plus era. The authors attempted to combine such supervision modes as government supervision, social supervision, industry self-discipline, and institutional autonomy, so as to develop a " four-dimensional supervision system" . This system works on such aspects as the supervision subject, supervision content, supervision mode and means, as well as classified supervision of private hospitals. The paper hence proposed relevant specific measures and supporting mechanisms, and provided policy suggestions, and references for improving the comprehensive and effective supervision of private hospitals.

@#Objective: To investigate the effects of miR-200c on the proliferation, apoptosis and migration of triple negative breast cancer cell (TNBC) MDA-MB-231 and its metabolism-related molecular mechanism. Methods: miR-200c-231 (MDA-MB-231 overexpressing miR-200c) cells, miR-NC-231 (MDA-MB-231 transfected with miRNA-negative control) and the corresponding transplanted tumor models in nude mice were used as the study subjects. qPCR was used to detect the content of miR-200c and other related genes in cells and transplanted tumor tissues. The number of Ki67 positive cells in tumor tissue was analyzed by immunohistochemistry. The migration and apoptosis of cells were examined by Transwell chamber method and Flow cytometry, respectively. The expressions of proteins associated with proliferation, migration, and metabolism related signaling pathways in cells and tissues were confirmed by Western blotting. The changes of oxygen consumption rate (OCR), extracellular acidification rate (ECAR) and metabolic phenotype were detected by Seahorse energy metabolism detector. UPLC/LTQ-Orbitrap-MStechniquewasusedtoprofilethedifference of metabolites in cells. Conclusion: miR-200c targeting TDO2 elevates the level of intracellular anticancer metabolites in TNBC MDAMB-231 cells, promotes the transformation from glycolysis to aerobic respiration phenotype, and inactivates STAT3 and NF-κB pathyway but activates cAMPpathway TNBC MDA-MB-231 cells, thus affects the malignant biological behaviors of MDA-MB-231 cells.

@# Objective: To explore the related factors for efficacy and prognosis of personalized comprehensive treatment for T790Mpositive lung adenocarcinoma patients with bone metastasis. Methods: The clinical data of 68 patients undergoing personalized comprehensive treatment for T790M-positive lung adenocarcinoma with bone metastasis were retrospectively reviewed; chemotherapy, radiotherapy, molecule-targeted agents, Bevacizumab, bisphosphonate and other therapies were chosen for the patients, and the efficacy and prognosis were observed to explore the related factors. Results: Effective rate of personalized comprehensive treatment was 60.3% (41/ 68), with a median survival time of 23 months. Multiple factors showed significant effects on long-term efficacy, such as no radiotherapy, T790M mutation but no KRAS mutation, adjuvant scheme+rescue scheme in prior chemotherapy treatment, N1 stage, isolated bone metastasis, alternative treatment of osimertinib with chemotherapy, less metastasized organs and ECOG scores<2 (P<0.05). Multivariate analysis revealed that T790M mutation but no KRAS mutation (P=0.012), number of metastasized organs =0 or 1 (P=0.000), alternative treatment of osimertinib with chemotherapy (P=0.020), and isolated bone metastasis (P=0.006) were independent protective factors for long-term results of personalized comprehensive treatment for T790M-positive lung adenocarcinoma patients with bone metastasis. Conclusion: Chemotherapy combined with osimertinib, agents of bisphosphonate and other personalized comprehensive treatment prolongs survival time in T790M-positive lung adenocarcinoma patients without KRAS mutation, providing a potential therapeutic model for those patients.

The hospital took the lead in the area to carry out stroke process improvement, including the accountability practice of the president, micro-videos for illness condition description, standardized language communication, CT-room thrombolysis, and interdisciplinary seamless corporation among others. The timespan from admission to thrombolysis ( door to needle time, DNT) has shortened significantly, scoring the median time of 25 minutes and the shortest of 8 minutes. The hospital is also the first in the area to initiate the program to bypass the emergency department and the thrombolytic treatment on the ambulance. Such bypass program keeps the DNT of emergency patients within 15 minutes.

To investigate the prevalence of intestinal parasite infections in Yanbian Prefecture, Jilin Province, China, epidemiological surveys were conducted on a collaboration basis between the Korean Centers for Disease Control and Prevention and the Yanbian Center for Disease Control and Prevention. A total of 8,396 (males 3,737 and females 4,659) stool samples were collected from 8 localities and examined with the formalin-ether sedimentation technique, and additionally examined with the cellotape anal swab to detect Enterobius vermicularis eggs. The overall rate of intestinal parasites was 1.57%. The prevalence of Ascaris lumbricoides was the highest (0.80%), followed by Entamoeba spp. (0.23%), heterophyid flukes (0.15%), Clonorchis sinensis (0.08%), Enterobius vermicularis (0.07%), hookworms (0.06%), Trichostrongylus spp. (0.06%), Giardia lamblia (0.04%), Paragonimus spp. (0.02%), Diphyllobothrium spp. (0.02%), Trichuris trichiura (0.02%). The prevalence by sex was similar, 1.58% (n=59) in males and 1.57% (n=73) in females. By the present study, it is partly revealed that the prevalences of intestinal parasite infections are relatively low among the inhabitants of Yanbian Prefecture, Jilin Province, China.

Emotion regulation is an important ability for individuals which can help them adapt to society better.The characteristics of patients with bipolar disorder are characterized by extremely unstable mood,associated with impaired emotion processing and mood dysregulation.This article reviews emotion regulation strategies of patients with bipolar disorder and their neural mechanisms.During the emotion regulation process,patients often use maladaptive regulation strategies.Their activation of the prefrontal cortex is abnormal,and the functional connectivity between prefrontal cortical regions and the amygdala is attenuated.This could also be the physiological basis of patients'difficulty in regulating emotions.

Objective To investigate the effect of low-dose glucocorticoid on prognosis of critical illness-related corticosteroid insufficient (CIRCI) patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD).Methods A total of 385 eligible patients met the criteria of AECOPD were admitted from January 2010 to December 2012.The AECOPD patients co-morbid with CIRCI screened by an adrenal corticotrophic hormone test within 12 hours after admission were randomly divided into treatment group (n =32) and control group (n =31) for prospective,randomized (random number) and controlled clinical study.Hydrocortison (150mg/d) for treatment group or normal saline instead for control group was injected intravenously for 7 days.The 28-day mortality,shock-free days,length of ICU stay within 28 days and ventilator-free days were evaluated.And the markers of inflammation C-reactive protein,tumor necrosis factor-α,interleukin 6 and procalcitonin were measured before and 7 days after treatment.The variables were analyzed by Student' s t-test,non-parametric statistical test,Chi-square test or KaplanMeier test with SPSS 18.0 statistic software.P < 0.05 was considered statistically significant.Results A cohort of 385 patients with AECOPD was screened,and the prevalence rate of CIRCI was 16.4%.The shock rate was higher in the AECOPD patients co-morbid with CIRCI than that in the AECOPD patients without CIRCI (23.8% vs 8.7%,P <0.01).Compared with the control group,the 28-day mortality was significantly lower in treatment group (2/32 vs 8/31,P < 0.05),and shock-free days within 28 days longer in the treatment group (18.2 ± 9.5 vs 25.8 ± 4.1,P < 0.05).However,there was no difference in the shock rate,days of ICU stay and ventilator-free days between the two groups.After treatment,the levels of infection markers were decreased and obviously lower than those in control group (P < 0.01),such as Creactive protein (13.2 ± 5.5 mg/L vs 8.3 ± 3.1 mg/L for control group; 13.5 ± 5.9 mg/L vs 5.1 ± 2.3mg/L for treatment group),tumor necrosis factor-α (26.1 ± 16.2 μg/L vs 17.5 ± 11.7 μg/L for control group ; 25.0 ± 14.8 μg/L vs 10.4 ± 7.8 μg/L for treatment group) and procalcitonin [3.88 (0.25,8.5) μg/L vs 2.03 (0.15,5.1) μg/L for control group; 3.77 (0.21,8.0) μg/L vs 1.26 (0.10,3.2) μg/L for treatment group],furthermore,the levels of infection markers were decrease more obviously in the treatment group than those in the control group (P < 0.01).Conclusions There was high prevalence rate of CIRCI in the patients with AECOPD in the department of critical medicine,and low-dose glucocorticoid reduced 28-day mortality,shock days and markers of infection and inflammation.

<p><b>OBJECTIVE</b>To search for an effective method of reducing intraoperative blood loss in radical retropubic prostatectomy (RRP).</p><p><b>METHODS</b>We performed RRP for 100 patients with prostate cancer, 50 (group A) with the Walsh or Poor method for handling the dorsal venous complex (DVC), and the other 50 (group B) through the following three additional procedures for hemostasis: first placing a #7 prophylactic suture in the distal position of DVC, then ligating the vascular bundle of the prostatic apex with continuous 4-0 Vicryl sutures, and lastly placing a 4-0 absorbable suture followed by freeing the neurovascular bundle (NVB) or freeing NVB before suturing the remained levator ani myofascia and the deep layer of Denovilliers' fascia above the rectal serosa with 4-0 Vicryl. We assessed the effects of the three hemostatic methods in RRP by comparing the volumes of intraoperative blood loss and transfusion, operation time and perioperative levels of hemoglobin.</p><p><b>RESULTS</b>There were no significant differences between groups A and B in age, PSA, Gleason score, clinical stage, prostate volume, operation time and perioperative hemoglobin levels (P>0.05). The volumes of intraoperative blood loss and transfusion were markedly higher in group A ([1103.00 +/- 528.03] ml and [482.00 +/- 364.60] ml) than in B ([528.00 +/- 258.96] ml and [140.00 +/- 266.28] ml) (P<0.05).</p><p><b>CONCLUSION</b>Intraoperative blood loss in RRP could be significantly decreased by placing a prophylactic hemostatic suture in the distal position of DVC, continuous suture of the vascular bundle of the prostatic apex after cutting off the urethra, and placing a fine absorbable suture above NVB or continuous suture of the remained levator ani mony fascia and the deep layer of Denovilliers'fascia above the rectal serosa with absorbable sutures after freeing NVB.</p>

Objective To observe the effect of transforming growth factor β1 (TGF-β1) on thyroid peroxidase activity and the expression of mRNA in primary porcine thyrocytes, and approach its possible mechanism. Methods In vitro cultured porcine thyrocytes were divided into 0 μg/L(control group), 2, 5, and 10 μg TGF-β1/L groups.After cultured for 72 h, the cell survival rate was measured with methyl thiazolyl tetrazolium (MTT) assay and the thyroid peroxidase (TPO) activity was measured by guaiacol method. The expression of TPO mRNA was determined cell survival rates of 2, 5, 10 μg/L groups(85.26%, 75.14% and 63.21%) were significantly lower(all P < 0.05 ).significantly decreased as compared with the control group [(2.143 ± 0.102), all P < 0.05)], and the TPO of TGF-β1, the lower the expression of TPO mRNA [(0.875 ± 0.078), (0.466 ± 0.044), (0.273 ± 0.007)] compared with the control group[(1.419 ± 0.148), all P < 0.05)]. Conclusions The inhibiting effect of TGF-β1 on primary cultured porcine thyrocytes may be related to its mechanism of inhibiting TPO activity and reduced mRNA expression.
RESUMO
Investigated and analyzed the graphical symbols commonly used in medical equipment, their benefits and drawbacks. From the perspective of human factors engineering, some root causes of poor pictogram as well as their relations with medical equipment adverse events were addressed. Furthermore, how to deal with the problems with current pictogram aiming at reducing use error was also discussed.

Objective To decrease the incidence of acute rejection in renal allograft recipients by monitoring of cyclosporine A (CsA) concentration at 2-hour after dosing(C2). Methods The CsA C2 and CsA trough concentration(C0) were assayed in renal allograft recipients.All patients were followed up for at least 1 year.The correlation of C0 and C2 monitoring with clinical outcomes was analyzed. Results At 1 week and 1 month post-transplantation,the incidence of acute rejection in patients with C2 in target level was 4.41% and 10.29%, respectively,but the incidence of acute rejection in patients with C2 in lower level was 42.37% and 36.20%,respectively. ConclusionBy reflecting the drug exposure of CsA more accurately,C2 monitoring is beneficial for decreasing the incidence of acute rejection after renal allograft transplantation.
