RESUMO

Adult atopic dermatitis is a chronically recurring inflammatory dermatosis which presents in various forms. Some of these forms develop mostly in adults such as head and neck dermatitis and chronic hand eczema. Even though the diagnosis is clinical it frequently requires further investigations to exclude differential diagnosis. Once confirmed, it is crucial to classify its severity and exclude other comorbidities. Treatment includes general measures such as the use of emollients and soap substitutes which are applicable in all cases regardless of its severity. Other therapeutic options include topical corticosteroids, topical calcineurin inhibitors, phototherapy and immunosuppressants. Their use will depend on the degree of severity and specific characteristics of each individual. Newer biologics have proven to be a safe and effective alternative, and seem to be a promising option in cases of adult atopic dermatitis refractory to conventional treatments. (AU)

RESUMO

Pernio (chilblains) is an inflammatory condition classically characterized by localized erythema and swelling of acral sites upon exposure to cool and damp conditions. It is reported a case of a 59-year-old otherwise healthy woman with acute primary perniosis. She had a 3-day history of lesions on the toes brought on by cold, damp weather. On initial presentation, a biopsy sample was taken of a hallux lesion, and the patient was given a trial course of oral pentoxifylline, topical corticoid and nifedipine therapy. Follow-up at 2 weeks showed complete relief of symptoms and the biopsy results confirmed the diagnosis. Pentoxifylline therapy has been shown to be effective and should be considered the standard of care in the treatment of perniosis along with conservative environmental measures.The etiology and pathogenesis of perniosis are reviewed and discussed, as well as differential diagnoses and treatment options. Medical treatments have shown poor outcomes, bariatric surgery has become a frequent treatment alternative for obesity. Obese patients seeking treatment have higher psychiatric comorbidity than not consulting ones. Bariatric surgery has allowed a reduction in medical comorbidity and some improvement in pre surgery psychopathology. Recent evidence reports a decrease in depressive disorders rate, improvement in self-esteem and life quality, in direct association with weight reduction, with no significant anxiety changes. Although weight decrease should help to have a better social life, important difficulties in adapting to the new environment can emerge. An increase in suicides and problematic alcohol use has been observed, as well as a decrease in binge eating disorder with further appearance of compensatory behavior, distorted body image, and marital problems. Pre surgery personality disorder could be a risk factor for a negative outcome. In this context a rigorous post surgery follow up is desirable, in order to optimize mental health benefits of massive weight reduction.


Los tratamientos médicos de la obesidad han demostrado baja efectividad, lo que ha llevado a que la cirugía bariátrica se haya establecido como una alternativa de tratamiento cada vez más frecuente. Las personas obesas que buscan tratamiento presentan tasas significativamente elevadas de comorbilidades psiquiátricas en comparación con las personas obesas que no consultan. La cirugía bariátrica ha permitido una reducción de las comorbilidades médicas y una mejoría de las patologías psiquiátricas presentes antes de la cirugía. Publicaciones recientes reportan una disminución de los trastornos depresivos, mejoría de la autoestima y de la calidad de vida, directamente asociados a la reducción de peso, sin variaciones significativas en los niveles de ansiedad. Si bien la pérdida de peso debería ayudar al mejoramiento de la vida social del paciente, pueden surgir importantes dificultades para adaptarse al nuevo entorno. Se ha visto un aumento de la tasa de suicidios y del uso problemático de alcohol, disminución precoz del trastorno por atracones con aparición posterior de conductas compensatorias, distorsión de la imagen corporal y aparición de conflictos conyugales. La presencia de trastorno de personalidad previo a la cirugía, constituiría un predictor negativo de resultado a largo plazo. Es en este contexto que se hace necesario un seguimiento post operatorio exhaustivo a estos pacientes, de tal forma de intervenir precozmente y optimizar los beneficios en la salud mental obtenidos con la baja de peso.

Las cicatrices atróficas constituyen una complicación frecuente del acné vulgar. Existen múltiples tratamientos quirúrgicos, aunque ninguno ha demostrado ser satisfactorio. Se realizó un estudio transversal retrospectivo de 47 pacientes con cicatrices atróficas de acné intervenidos mediante Cirugía Punch (CP) y/o Láser CO2 10.600 nm (LCO), a quienes se aplicó un cuestionario validado entre Abril y Septiembre de 2014. El 63,8 por ciento recibió tratamiento solo con LCO, el 10,6 por ciento solo con CP y el 25,6 por ciento recibió ambos tratamientos. Respecto al grado de satisfacción, el 9 por ciento refirió un puntaje de 10 luego del procedimiento, el 32 por ciento entre 7-9, el 43 por ciento entre 4-6 y un 17 por ciento entre 0-3. El 66 por ciento de los pacientes refirió que los efectos se mantienen en el tiempo y el 21 por ciento reportó efectos adversos. El tratamiento quirúrgico de las cicatrices atróficas de acné con técnicas como CP y/o LCO consigue grados de satisfacción moderados, con efectos mantenidos a largo plazo y escasos efectos adversos.


Atrophic scars are a common complication of acne vulgaris. There are multiple surgical treatments, but none have proved satisfactory. A retrospective cross-sectional study of 47 patients with atrophic acne scars surgery was performed by Punch Surgery (CP) and / or CO2 Laser 10,600 nm (LCO), a validated questionnaire between April and September 2014 was applied. 63.8 percent received only LCO treatment, only 10.6 percent CP and 25.6 percent received both treatments. Regarding the degree of satisfaction, 9 percent reported a score of 10 after the procedure, 32 percent between 7-9, 43 percent between 4-6 and 17 percent 0-3. 66 percent of patients reported that the effects are maintained over time, and 21 percent reported adverse effects. Surgical treatment of atrophic acne scars with techniques such as CP and / or LCO achieved moderate degree of satisfaction with maintained long-term effects and few adverse effects.

El Síndrome de Ovario Poliquístico es un trastorno endocrino común que afecta a mujeres en edad reproductiva que puede causar problemas metabólicos y deterioro psicosocial importante. Las manifestaciones dermatológicas más frecuentes del hiperandrogenismo incluyen acné, hirsutismo y alopecia, cuyo reconocimiento es esencial para hacer un diagnóstico precoz. Las modalidades de tratamiento incluyen terapia hormonal con el objetivo de modular la producción de andrógenos y su acción, así como tratamientos no hormonales dirigidos a condiciones dermatológicas específicas.


Polycystic ovarian syndrome is a common endocrine disorder that affects women of reproductive age, which can cause metabolic, reproductive and psychosocial impairment. The most common skin manifestations of hyperandrogenism are acne, hirsutism and alopecia, whose recognition is essential for early diagnosis. There are several treatment modalities, including hormonal therapy in order to modulate androgen production and their effects, as well as other non hormonal treatments targeted to specific dermatologic conditions.

RESUMO

La rosácea es una dermatosis facial inflamatoria crónica caracterizada por periodos intermitentes de exacerbación y remisión. La etiología es desconocida y factores genéticos y ambientales están involucrados en su desarrollo. El tratamiento tópico y sistémico controla la rosácea en forma parcial. La Isotretinoína oral es útil por sus propiedades antiinflamatorias,inmunomoduladoras y de reducción del flujo sanguíneo.


Rosacea is a chronic facial inflammatory dermatoses characterized by intermittent periods of clinical exacerbation and remission. The etiology is unkown. Genetic and external factors are involved in the pathogenesis. Topical and oral treatments partially control the disease. Oral Isotretinoin can be useful because of the antiinflammatory, inmumodulating actions and because it reduce the blood flow.
