RESUMO

Althesin(CT 1341), a new steroid anesthetic, was tried clinically as an induction agent. All 24 patients were in the physical status class 1 by A.S.A. classification. Dosage of althesin given intravenously was 50ul/kg B.W. and the injection speed for the given dose was 15 seconds. The following results were obtained. 1) Induction time was 42+/-19. 7 seconds. 2) No significant changes in pulse rate were found after intravenous injection of althesin. 3) Systolic blood pressure was decreased 8mmHg (p after three minutes. 6) As complications, involuntary muscle movement of extremities was observed in two cases and transient apnea in one case.

RESUMO

Autoclaving the local anesthetic ampuls has been suggested as a safer method of sterilization. However the method and time necessary to sterilize the ampul has not been standardizzed, and the amount of deterioration of anesthetic agents by this method is unknown, and the effect of injection of the caramelized dextrose has not been determined. If ampuls are autoclaved with the spinal tray or set up, the ampuls are subjected to sterilization for a prolonged time which in turn causes caramelization of dextrose and possible deterioration. We have studied the effect of multiple autoclavings on pH of the drugs whichart used in spinal anesthesia, 1% tetracaine, 10% dextrose, 2% xylocaine and epinephrine at 134C, 36 psi for 5 minutes. Statistieal significance of the pH changes were observed by multiple autoclavings.

RESUMO

Physiotherapy, psychotherapy, hormone therapy, chemical therapy, traditional surgical methods; amputation of painful extremities, cordotomy, leucotomy, rhizotomy, electrode insertion into the dorsal column of cord and acupuncture have been performed for therapeutic approaches to pain relief, but still there are problems in management of complete relief for the chronic pain today. At present, another new method of eliminating chronic pain is based on expanding the neurophysiologic aspect. Among the above mentioned therapeutic approaches to chronic pain relief, spinal barbotage seems to be least stressful to the patient and very effective in some cases. We have experienced a case of successful control of chronic pain by spinal barbotage for a 62 year old woman with metastatic colon carcinoma. The patient was well controlled chronic pain for 28 hours after the first spinal barbotage and for 3 weeks after the second spinal barbotage. It is considered that spinal barbotage is more simple, convenient and effective than other methods for intractable pain relief.
