@#Objective     To investigate the effect and prognosis of patients with ventricular septal rupture after myocardial infarction treated by surgical repair combining an occluder and a patch. Methods     Clinical data of 42 patients with myocardial infarction complicated with ventricular septal rupture admitted to the First Affiliated Hospital of Zhengzhou University from January 2010 to September 2021 were retrospectively analyzed. According to the surgical methods, 27 patients were divided into a traditional group, including 17 males and 10 females, with an average age of 62.81±6.81 years, who were repaired by patch only, and 15 patients were divided into a modified group, including 11 males and 4 females, with an average age of 64.27±9.24 years, who were repaired by surgery combining an occluder and a patch. Perioperative and follow-up data of the two groups were compared and analyzed. Results     There were statistical differences between the two groups in preoperative Killip grading, rate of intra-aortic balloon pump use, interval from myocardial infarction to operation, and the number of culprit artery (P<0.05). There was no statistical difference in other preoperative data, the cardiopulmonary bypass time, aortic cross-clamping time, postoperative hospital stay or in-hospital death rate between the two groups (P>0.05). No residual shunt occurred in the modified group, and the difference was statistically significant compared with the traditional group (P=0.038). There was no statistical difference in other complications between the two groups (P>0.05). The median follow-up time was 4 years. Two patients in the traditional group and one in the modified group died during follow-up. The follow-up cardiac function grading of patients in the modified group was statistically different from that in the traditional group (P=0.023). Conclusion     The perioperative  mortality of ventricular septal rupture after myocardial infarction is high, but the long-term effect is satisfactory. Surgical repair combining an occluder and a patch is a safe and effective treatment for ventricular septal rupture, which can effectively reduce postoperative residual shunt.

OBJECTIVE@#To investigate the clinical efficacy of the combined application of blistering cupping with thunder-fire moxibustion in treating bronchial asthma of cold-wheezing syndrome, and its influences on airway remodeling, inflammatory factors, lung function, and quality of life on the base of conventional western medicine treatment.@*METHODS@#A total of 76 patients with bronchial asthma of cold-wheezing syndrome were randomly divided into an observation group and a control group, 38 cases in each group. In the control group, the basic treatment was used, i.e. budesonide formoterol powder inhalation. In the observation group, on the basis of the treatment as the control group, blistering cupping combined with thunder-fire moxibustion was supplemented, Dazhui (GV 14), Danzhong (CV 17) and bilateral Feishu (BL 13), Gaohuang (BL 43), and Zhongfu (LU 1) were selected; blistering cupping was administered once a day and thunder-fire moxibustion was given twice a day. One course of treatment was composed of 7 days in both groups, and 2 courses of treatment were required. Before and after treatment, the airway remodeling indexes (matrix metalloproteinase-9 [MMP-9], tissue inhibitor of matrix metalloproteinase-1 [TIMP-1], and transforming growth factor-β1 [TGF-β1]) and inflammatory indexes (interleukin [IL] -1β、IL-25) were detected by using radioimmunoassay in the patients of the two groups. The lung function, traditional Chinese medicine symptom score, and asthma quality of life questionnaire (AQLQ) score were observed in the patients of the two groups.@*RESULTS@#After treatment, the serum levels of MMP-9, TIMP-1, TGF-β1, IL-1β, IL-25, peak expiratory flow (PEFR), traditional Chinese medicine symptom scores, and AQLQ scores were decreased compared with those before treatment in the patients of the two groups (P<0.05), and the results in the observation group were lower than those in the control group (P<0.05). After treatment, the first second forced expiratory volume (FEV1) and peak expiratory flow rate (PEF) were increased compared with those before treatment in the two groups (P<0.05), and the results in the observation group were higher than those in the control group (P<0.05).@*CONCLUSION@#On the basis of the conventional western medicine treatment, the combination of blistering cupping with thunder-fire moxibustion can effectively ameliorate the clinical symptoms of patients, reduce inflammatory levels, inhibit airway remodeling and improve the lung function and quality of life in the patients with bronchial asthma.

Sulfadoxine-pyrimethamine (SP) has been recommended for intermittent preventive treatment of malaria in children and pregnant women, and also used as a compound component of artemisinin based therapy. The mechanisms of SP resistance in P. falciparum involve point mutations in the genes encoding dihydrofolate reductase (DHFR) and dihydropteroate synthase (DHPS), and the drug pressure can also lead to the mutations in the two genes of P. vivax. To provide the information for the formulation of anti-malarial strategies, this article reviews the discovery, application, effect of SP, and the resistance mechanism and research progress of the related genes in P. vivax.

RESUMO

Objective To research the derivatives of panax notoginseng sapogenins and their anti-tumor activities. Methods The ginsenosides Rg1 was treated with Smith degradation. The products were separated and purified by silica gel column chromatography. The structures of the products were determined by NMR spectra. The activity of anti-tumor cells of compound (1) and 1'-hydroxyethanedioxy PT was detected with CDC25B activity assay and fluorescence technique. Results 1'-hydroxyethanedioxy PT , 20 (s)-protopanaxatriol (PT), and 24, 25 - en-3β, 6 α-dihydroxy-12, 20-(1', 2'-isopropylidenedioxy) propanedioxy-dammarane (1) were isolated and identified. Conclusion Compound (1), named 1',2'-isopropylidenedioxy-propanedioxy-pro-panedioxy , is a novel derivative of panax notoginseng sapogenin with better inhibitory activity against CDC25B.

In this study, an endophytic bacteria strain BZJN1 was isolated from Atractylodes macrocephala, and identified as Bacillus subtilis by physiological and biochemical tests and molecular identification. Strain BZJN1 could inhibit the growth of mycelia of Ceratobasidium sp. significantly, and the inhibition rate was more than 70%. The mycelium growth deformity with bulge as spherical and partially exhaustible in apex or central with microscopic observation. The inhibitory rates under 3% and 6% concentrations of the cell free fermentation were 22.7% and 38.7% expectively. The field test proved that the control efficacy of treatment of 1×10⁸ cfu·mL⁻¹ is 75.27% and 72.37% after 10 and 20 days. All the treatments of strain BZJN1 was able to promote the growth of A. macrocephala, the treatment of 1×10⁸ cfu·mL⁻¹ could able to increase the yield to 14.1%.

BACKGROUND:Compared with conventional medications, drug micro-capsule system can control the release of drugs and have wel target properties and biocompatibility. The drugs can be concentrated at the focus and play an important role in clinic. OBJECTIVE:To prepare dacarbazine magnetic micro-capsules with different capsule materials and gelatin complex by coacervation, and to optimize capsule materials and preparation process. METHODS:Fe 3 O 4 RESULTS AND CONCLUSION:The solution complex coacervation method was better than the emulsion coacervation method. As for the solution complex coacervation method, the optimal capsule material was gelatin-sodium alginate, with drug embedding rate 37.90%, the yield rate 72.31%, and the average magnetization intensity 8.53 emu/g. The second material was gelatin-chitosan. As a capsule material, the gelatin was better than chitosan with single coagulation method. Drug embedding rate was 51.58%, the yield rate was 64.50%, and the average magnetization was 6.93 emu/g. Single coagulation method was better than coacervation method. complex coacervation, we prepared the gelatin-Arabic gum magnetic micro-capsule, gelatin-sodium alginate magnetic micro-capsules, gelatin-sodium carboxymethyl cel ulose magnetic micro-capsules, and gelatin-chitosan magnetic micro-capsules. With the emulsion complex coacervation method, we further prepared the gelatin-Arabic gum magnetic micro-capsule, gelatin-sodium alginate magnetic micro-capsules, gelatin-sodium carboxymethyl cel ulose magnetic micro-capsules, and gelatin-chitosan magnetic micro-capsules. The magnetic gelatin micro-capsules and magnetic chitosan micro-capsules were prepared with single coagulation method. The micro-capsules were determined for the embedding rate, the magnetic susceptibility, the micro-capsule size and the release performance, to define the optimal preparation technology of dacarbazine magnetic micro-capsules.

RESUMO

<p><b>OBJECTIVE</b>To explore the changes of CD(14)(+) monocyte human leucocyte antigen DR (HLA-DR) and their relationship with multiple organ dysfunction syndrome (MODS) in severe sepsis.</p><p><b>METHODS</b>Ninety-one patients with a definite diagnosis of severe sepsis in the intensive care unit (ICU) were included. CD(14)(+) monocyte HLA-DR levels were detected by flow cytometry on the first, 4th and 7th days of the study, and Marshall scores and prognosis on day 28 were evaluated.</p><p><b>RESULTS</b>Thirty-four patients died within 28 days following the onset with a mortality rate of 37.4%. Persistently lowered levels of HLA-DR were detected and significantly increased Marshall scores were found in the fatal cases at all the time points (P<0.001). In the surviving patients, the levels of HLA-DR were significantly increased (P<0.01) and Marshall scores were gradually decreased (P<0.001). During the observation period, the levels of HLA-DR decreased significantly as the number of dysfunctional organs and Marshall scores increased (P<0.001). The levels of HLA-DR were significantly increased in severe sepsis patients with 2-4 dysfunctional organs and Marshall score of 5-12 (P<0.05 or P<0.001). No changes in HLA-DR levels in severe sepsis patients with 5-6 dysfunctional organs and Marshall scores of 13-22. The levels of HLA-DR showed a significant inverse correlation to Marshall scores (r=-0.368, P<0.001).</p><p><b>CONCLUSION</b>In patients with severe sepsis, persistent low CD(14)(+) monocyte HLA-DR levels predicts high mortality. The levels of HLA-DR are significantly correlated to the severity of organ dysfunction.</p>

<p><b>OBJECTIVE</b>To evaluate the technique and significance of mastoscopic axillary sentinel lymph node biopsy.</p><p><b>METHODS</b>Sixty-two patients with breast cancer use methylene blue to test axillary sentinel lymph node. Sentinel lymph node was moved with endoscopy, and endoscopic axillary lymph nod dissection was performed. Pathological examination of sentinel lymph node and axillary lymph node was made with HE. To evaluate detection rate and false negative rate in sentinel lymph node.</p><p><b>RESULTS</b>Among the 62 patients, 61 were confirmed by endoscopic axillary sentinel lymph nod biopsy. Detection rate was 98.4%. Thirty-five cases were no metastasis, 27 cases were metastasis, false negative rate was 0.</p><p><b>CONCLUSIONS</b>Mastoscopic axillary sentinel lymph node biopsy has a high detection rate, good efficiency of cosmetic and lower complications. It has higher sensitivity than traditional axillary lymph nod dissection and provide accurate lymph node stages.</p>
