RESUMO

Glomerular obsolescence is a common histologic feature in various renal diseases, of which histogenesis remains unclarified especially in renal tuberculosis, and little has been attributed for the postulation of low prevalence of hypertension in such occasion. One hundred and fifty consecutive nephrectomy specimens with renal tuberculosis under the age of 50 were histopathologically analyzed along with comparison of 18 cases of chronic pyelonephritis, 15 hydronephrosis and 10 acute traumatic kidneys. Glomerular sclerosis following tuberculous glomerulitis seemed little significant in its histogenesis except in one case, which presented epitheliold cell reaction and crescent formation. Wrinkling and thickening of glomerular basement membrane and subsequent simplification of tufts with mesangiolysis(Stage I and II) represented the major alternative glomerulopathy in nodular renal tuberculosis, whereas in confluent ulcero-cavitary lesion the development of hyalin-knot within the Bowman's space and collapse of the tufts (Stage III and IV) comprised the bulk, aside from frequent intraglomerular shunt formation between afferent and efferent arterioles mostly in autonephrectomized cases, Arteriolosclerosis was found in 17.2 % among nodular type, while 26.7% in ulcero-cavitary, variety, partly responsible for the histogenesis of glomerular obsolescence. Those cases with moderate arterio and arteriolar sclerosis showed a high tendency of transformation into stage III-IV obsolescence especially in ulcero-cavitary tuberculosis. Relative prevalence rate of systolic hypertension in ulcerocavitary tuberculosis was 5.8%, two times higher than nodular type, but in only one case the post-operative blood pressure returned to normal range. With the above findings it is assumed that glomerular oboslescence of so-called hypertensive glomerulopathy is a subtype of ischemic nature, and induced not only by vascular is chemic changes but also compromised through the intra-renal tubular obstruction and intraglomerular capillary shunt formaion may attribute to the development of secondary hypertension, though rare, in renal tuberculosis associated with arteriosclerot ic changes as well.

RESUMO

A statistical observation was made on 204 cases of congenital anomalies of the genitourinary tract in patients who had been admitted to Department of Urology, Seoul National University Hospital during the period January 1965 through December 1973 and the following results were obtained. 1. During the period, 204 cases of congenital anomalies of the genitourinary tract were found among 2.910 cases hospitalized, giving a rate of 7.O%. 2. The cases of congenital genitourinary tract anomaly had been increased annually, from 3.2% in 1965 to 9.8% in 1973. 3. The most cases of congenital anomaly of the G-U tract were found in the age group below 19 years old (66.7%). 4. The most vulnerable organ for the G-U tract anomalies were urethra (38. 9%), testis (25.9%) and kidney (11.8%). 5. Hypospadias were observed in 69 cases (33.8%), cryptorchidism in 51 cases (25.O%) and nephroptosis in 13 cases (6.4%). 6. 12 cases of combined G-U tract anomaly were also found.

RESUMO

An unusual case of incomplete double ureter on right side and complete double ureter associated with single ectopic ureteral opening on left side was presented. Distal end of the ectopic ureter was shown cystic dilatation of hen egg size and opened into the vaginal fornix. Diagnosis was made by retrograde ureterography, double dose pyelography and renal angiography and she was treated by ureteroheminephrectomy.

RESUMO

Two cases of spontaneous kidney rupture with literatural review was presented at first in Korea. 1) 56 years male patient and 25 years male patient admitted to our department of Urology through Emergency Room after onset of 2 and 1 day. after treatment at local clinic. 2) Chief complaints were one side flank pain which was followed by total gross hematuria. 3) Preoperative diagnoses were kidney rupture due to necrotic pelvic tumor in 56 years male and hypernephroma in 25 years male patient. 4) Operation was procedured at 8 days and 4 days after admission respectively. 5) The operative procedure were nephrectomy in both cases. 6) The pathological diagnoses were pelvis rupture due to renal stone and parenchymal rupture due to renal infarction in each case.

RESUMO

A clinical observation was made on the patients with renal tumors who had been admitted and treated in the Department of Urology, Seoul National University Hospital during the period from Jan. 1965 to Sept. 1973. The following results were obtained. 1. During these period, a total of 2, 886 patients were admitted and 44 cases were confirmed as renal timor, giving a rate of 1.53%. 2. Of 44 cases, 28 cases were male and 16 cases were female patient. 3. Of 44 cases, renal cell carcinoma was 27(61%), transitional cell carcinoma of renal pelvis 9(20%), Wilms' tumor 7(16%) and hamartoma was 1 case(2.2%). 4. Chief complaints were as follows: hematuria in 48%, loin pain in 34%, abdominal mass in 24% and weight loss with fatigue observed in 18% of patients. 5. Raised ESR in 70%, anemia in 49%, hypertension in 30% and abnormal liver function test was found in 14% of patients as a systemic effects of renal tumors.
