RESUMO

Objective To understand the dietary structure and nutritional characteristics of children aged 6-17 under the mode of collective feeding and self-catering in schools, and their effects on children's health-related indicators. Methods From January to April 2022, stratified sampling method was used to select 2 collective feeding schools and 2 non-feeding schools in Baoshan District of Shanghai City, and finally 1 538 school-age children aged 6-17 were selected. Then, the “3-day 24-hour dietary review method” and “eighing method” were used to carry out the survey, and combined with the “Chinese Residents' Balanced Diet Pagoda (2016)” and “Chinese Residents' Dietary Nutrient Reference Intakes (DRIs 2013)” standards to analyze the Food intake and energy supply ratio of the three major nutrients. At the same time, on-site sampling was conducted on the last day to collect samples of children's height, weight, blood pressure, blood, etc, and analyze the changes of students' obesity, anemia, blood lipids and other routinely available indicators. Results Compared with the school group feeding mode, the students in the self-dining mode had a higher rate of insufficient intake of cereals, potatoes and vegetables; and the latter students had a higher intake of aquatic products. high (P28.0) was 3.8%, the rate of high TG was 7.3%, the rate of hypertension was 1.9%, and the rate of iron deficiency anemia was 2.6%, bone calcium loss rate was 2.8%, hyperglycemia rate was 5.1%. In addition, pearson correlation analysis showed that the intake of cereals and potatoes was positively correlated with BMI (r=0.348) and systolic blood pressure (r=0.438), and the intake of salt was also positively correlated with systolic blood pressure (r=0.472). ), while the intake of aquatic products was negatively correlated with the change of BMI value (r=-0.331). Conclusion Children aged 6-17 have insufficient intake of vegetables , aquatic products, soybeans and nuts in their diets. In addition, they need to pay attention to the intake of salt to prevent the occurrence of adverse health problems such as obesity and hyperlipidemia in children.

Objective:To construct a double-line mixed teaching mode and to explore its application effect on the practice teaching of medical students in the department of surgical oncology.Methods:A total of 80 clinical medicine students were selected and randomly divided into a routine group and a research group, with 40 students in each group. The routine group implemented regular teaching, and the research group developed "double-line mixed teaching" mode. After teaching, the students' theoretical and practical skills assessment results, changes in medical students' abilities and their satisfaction with teaching were compared between the two groups. SPSS 19.0 was used for t test and chi-square test. Results:The theoretical and practical skill assessment scores of the research group were significantly higher than those of the routine group [(91.59±3.65) score vs. (84.67±4.34) scores, (86.05±4.16) scores vs. (74.73± 4.69) scores] ( P<0.05). The scores of medical humanistic spirit, clinical thinking ability, scientific research thinking ability, critical thinking and innovation ability of the two groups after teaching were all higher than those before teaching, and the above-mentioned ability scores of the research group were all higher than those of the routine group after teaching ( P<0.05). The satisfaction rates of the students in the research group regarding teaching content and form, improving self-study ability, strengthening understanding and mastering of knowledge, deepening practical experience, and improving collaboration ability were all higher than those of students in the routine group (87.50% vs. 67.50%; 82.50% vs. 60.00%; 90.00% vs. 67.50%; 92.50% vs. 75.00%; 95.00% vs. 77.50%), and the differences were statistically significant ( P<0.05). Conclusion:Constructing double-line mixed teaching mode in the practice teaching of medical students in the department of surgical oncology can improve the performance of theoretical and practical skills assessment, as well as improve the various abilities of medical students, and can obtain high satisfaction with the teaching mode.

<p><b>OBJECTIVE</b>To compare clinicopathological features and prognosis between patients with carcinoma in the remnant stomach (CRS) and with gastric cancer, and to investigate the prognostic factors in CRS patients.</p><p><b>METHODS</b>A retrospective cohort study was performed on clinicopathological data of 96 CRS patients (CRS group) and selected 440 patients with gastric cancer (GC group) treated at Harbin Medical University Cancer Hospital from January 1977 to December 2017.</p><p><b>INCLUSION CRITERIA</b>(1) undergoing gastrectomy; (2) diagnosed with CRS or gastric cancer through electronic gastroscopies and pathology; (3) without preoperative neoadjuvant radiotherapy or chemotherapy; (4) complete clinicopathological and follow-up data. The patients who died of other reasons or were lost during follow-up were excluded. Chi-square test and independent samples t-test were used to determine differences in clinicopathological factors between two groups. Survival analysis was conducted using the Kaplan-Meier method, and Log-rank test was used to compare survival difference between two groups. The prognosis of CRS patients was analyzed using Cox proportional hazards regression model.</p><p><b>RESULTS</b>As compared to GC group, CRS group had a higher proportion of female [30.2%(29/96) vs. 13.2%(58/ 440), χ=14.095, P=0.000], younger age [(56.4±10.1) years vs. (60.0±9.9) years, t=2.838, P=0.005], more distant metastasis and local organ infiltration [25.0%(24/96) vs. 16.1%(71/440), χ=4.246, P=0.039; 64.6% (62/96) vs. 24.5% (108/440), χ=58.331, P=0.000], lower prognostic nutritional index [(48.0±6.7) vs. (50.4±6.9), t=3.093, P=0.002], lower serum hemoglobin level [(115.0±24.7) g/L vs. (127.9±24.6) g/L, t=4.634, P=0.000], lower serum albumin level [(40.0±4.9) g/L vs. (41.2±5.0) g/L, t=2.038, P=0.042], and earlier occurrence of symptoms [(1.9±1.4) months vs. (3.7±3.2) months, t=5.431, P=0.000]. However, there were no statistically significant differences in TNM staging, postoperative hospital stay, and total hospitalization days between the two groups (all P>0.05). During follow-up, 24(25.0%) patients developed recurrence or distant metastasis and 68 (70.8%) patients died of tumor progression in CRS group, while 71(16.1%) patients developed recurrence or distant metastasis and 378(85.9%) patients died of tumor progression in GC group. The 5-year survival rate of CRS patients was 23.4%, which was higher than 15.0% of gastric cancer patients (P=0.032). Univariate analysis showed that the CRS patients with radical operation (P=0.000), earlier TNM stage (P=0.000), non-distant metastasis (P=0.022), serum hemoglobin level >130 g/L(P=0.013), and serum album level >40 g/L (P=0.042) had better prognosis. Multivariate analysis, enrolling above 5 factors, showed that TNM staging (HR=2.363, 95%CI: 1.478-3.776, P=0.000) and serum hemoglobin level >130 g/L(HR=0.449, 95%CI: 0.244-0.827, P=0.010) were independent factors influencing prognosis of CRS patients.</p><p><b>CONCLUSIONS</b>Although CRS patients have better prognosis than gastric cancer patients, but local organ invasion and distant metastasis occurs more readily. TNM staging and serum hemoglobin level are independent prognostic factors for CRS patients.</p>

RESUMO

OBJECTIVE:To investigate the effects of tirofiban combined with atorvastation before PCI on the miRNA expres-sion of peripheral blood and vascular endothelial function in acute myocardial infarction(AMI)patients. METHODS:A total of 80 patients with AMI selected from our hospital as reaserch objects during Jan. 2015-Jun. 2016 were divided into control group and ob-servation group according to random number table,with 40 cases in each group. Both groups received anticoagulant therapy. Con-trol group was given Aspirin enteric-coated tablets 300 mg+Clopidogrel sulfate tablets 75 mg+ Atorvastatin calcium tablets 20 mg orally 30 min before PCI. Observation group was additionally given Tirofiban hydrochloride for injection with initial dose of 0.5 mg+0.9% Sodium chloride injection 100 mL,iv,then adjusted to pump injection of 0.4 μg/(kg·min),30 min later adjusted to pump injection of 0.1 μg/(kg·min),for consecutive 24 h. The levels of peripheral miRNA (miRNA-1,miRNA-133a,miR-NA-208b,miRNA-499),the levels of brachial artery diameter and vascular endothelial function indexes(vWF,ET-1,NO)were observed in 2 groups before medication and after PCI,and the occurrence of ADR was recorded. RESULTS:Before treatment, there was no statistical significance in above indexes between 2 groups(P>0.05). After treatment,miRNA expression,the levels of vWF and ET-1 were decreased significantly in 2 groups,and observation group was significantly lower than control group,with statistical significance (P<0.05). The brachial artery diameter and NO levels of 2 groups were increased significantly,and observation group was significantly greater or higher than con-trol group,with statistical significance(P<0.05). No obvious ADR was found in 2 groups during treatment. CONCLU-SIONS:For AMI,tirofiban combined with atorvastation before PCI can reduce miRNA expression,increase brachial artery diame-ter and protect vascular endothelial function with good safety.

Objective To explore the CT and MRI manifestations of Kimura disease in head and neck.Methods The CT and MR findings of 20 cases with Kimura disease confirmed by pathology were collected.The lesion location,boundary,size,number,density of CT and signal intensity of MRI were analyzed.Results In 20 cases,multiple lesions were seen in 19 cases.There were 11 cases involving parotid gland,6 cases involving the submandibular area,2 cases in cheek,1 case in palate,2 cases involving the upper arm,1 case involving axillary fossa,1 case involving cervical lymph node alone,and 8 cases with subcutaneous nodules in the front or back of the ear,16 cases accompanied by lymphadenopathy in the neck (8 cases with lymphadenopathy in the parotid gland).The largest diameter of lesion was (3.09± 1.08)cm.The margin was blurred in 19 cases and well defined in 1 case.The isodensity lesions were found in 16 cases and slightly lower density lesions were in 4 cases in non-contrast enhanced CT scan.Calcification was not found in all cases.Homogeneous enhancement was found in 16 cases and inhomogeneous enhancement was found in 1 case.The enhancement degree varies from mild to strong enhancement.Homogenous signal and strong enhancement was found in 2 cases with MR scan.Conclusion The typical manifestations of Kimura disease are involvement of parotid gland and subcutaneous fat,accompanied by lymphadenopathy in the neck,homogeneous density on CT or MRI,which may indicate Kimura disease.

Objective To analyze the recurrence rate of intubation and increase of ventilator support rate within 24 hours after using fiberoptic bronchoscopy (FOB) in critically ill patients with hypoxemia complicated with respiratory failure, and to approach the feasibility of FOB in such patients.Methods A prospective study was conducted, including 200 critically ill patients with acute respiratory failure using FOB [oxygenation index (PaO2/FiO2) ≤ 300 mmHg (1 mmHg = 0.133 kPa)] admitted to the intensive care unit (ICU) of the First Affiliated Hospital of Xinxiang Medical College. The rates of intubation and increased ventilatory support and the reasons for bronchoscopy related complications after using FOB 24 hours were recorded, the main risk factors leading to these changes and complications were analyzed and screened by logistic regression analytic method.Results Within 24 hours after using FOB for 200 patients with respiratory failure, an increase in mechanical ventilatory support was required in 68 patients (34%) of that 28 (14%) led to endotracheal intubation. With the extension of time, the rates of intubation and ventilatory support showed a tendency of elevation, the rise in ventilatory support rate being faster. The reasons for bronchoscopy related complications after FOB consisted of cardiovascular disease (41%), coronary artery disease (17%), chronic obstructive pulmonary disease (COPD, 17%), chronic restrictive pulmonary disease (10%), immunity suppression (54%), malignant neoplastic hematologic disorder (20%), acquired immune deficiency syndrome (AIDS, 12%), solid organ transplantation (3%), solid tumor (10%), corticosteroid therapy (25%), immunosuppressive drug (16%), diabetes (15%), chronic renal failure (14%), swallowing nerve injury (37%), anticoagulant therapy (19%), antiplatelet therapy (13%). In the patients with occurrence of COPD or immunosuppression, the rate of invasive ventilation used was significantly higher than that without using invasive ventilation [COPD: 35% (10/28) vs. 14% (24/172),χ2 = 8.081,P = 0.004; immunosuppression: 75% (21/28) vs. 50% (86/172),χ2 = 6.051,P = 0.014]. The logistic regression analysis showed that the occurrence of COPD or immunosuppression was obviously related to whether the intubation being necessary or not [COPD: odds ratio (OR) = 5.200, 95% confidence interval (95%CI) = 1.500 - 17.700,P = 0.006; immunosuppression:OR = 5.300, 95%CI =1.600 - 17.100,P = 0.004].Conclusions In patients with hypoxemia using FOB, they often require addition of mechanical ventilatory support, but the intubation rate is not high. Under the ventilatory support, FOB has certain feasibility for treatment of critically ill patients with hypoxemia and acute respiratory failure.

Objective To develop an up-converting phosphor technology based lateral flow (UPT-LF) assay for rapid detection of Salmonella paratyphi A, S.paratyphi B, Escherichia coli O157 ∶H7 and Vibrio parahaemolyticus. Methods With up-converting phosphor nano-particles ( UCP-NPs ) as the bio-marker, four double-antibody-sandwich mode based UPT-LF strips for detecting the above mentioned four pathogens were prepared respectively and their sensitivi-ty, accuracy, linearity and specificity were evaluated .Furthermore, the feasibility of detecting bacteria in food samples was evaluated by different food samples artificially contaminated with less than 10 CFU target pathogens .Results The sensitivi-ty of UPT-LF assays for four pathogens was 105 ~106 CFU/ml with excellent specificity .The four strips had a good linear response with the linear fitting coefficient of determination (r) for each target pathogen ranging from 0.985 to 0.996.The positive rate of detecting pathogens from samples was acceptable .Conclusion The four developed UPT-LF strips provide a new choice for rapid , specific and sensitive and quantitative detection of S.paratyphi A , S.paratyphi B, E.coli O157∶H7 and V.parahemolyticus.

<p><b>OBJECTIVE</b>To develop an up-converting phosphor technology-based lateral-flow (UPT-LF) assay for rapid quantitative detection of Burkholderia pseudomallei on site.</p><p><b>METHODS</b>The strip Bps-UPT-LF strip was prepared with up-converting phosphor (UCP) particles as the bio-label using double-antibody sandwich method. Detection performance, including sensitivity, quantitative accuracy, precision, and specificity, were first evaluated using bacterial suspensions of Burkholderia pseudomallei, the related species and the strains which had similar routes of transmission with serial standard concentrations diluted by phosphate buffer, then biological and chemical reagents and simulated samples with series concentrations were employed for sample tolerance evaluation, while the operation error during on site detection was also evaluated through adjusting liquid measure.</p><p><b>RESULTS</b>The whole detection was accomplished within 20 minutes, and the sensitivity was 10(4) CFU/ml with linear quantitative range from 10(4) CFU/ml to 10(7) CFU/ml, which covered four orders of magnitude. Bps-UPT-LF strip demonstrated high specificity with the absence of any false-positive result even at 10(7) and 10(8) CFU/ml of non-specific bacterial contamination. Not only Bps-UPT-LF strip could tolerate to high concentration of the extreme acid and basic matter (pH 1-12), saline matter (≤ 2 mol/L mixture of NaCl and KCl), viscous materials (≤ 50 g/L of PEG 20000 and ≤ 20% of glycerol) and bio-macromolecule (≥ 400 g/L of bovine serum albumin or ≥ 80 g/L of casein), but also it can directly detect animal, environmental and powder specimen, such as ≥ 400 g/L of milk powder, flour powder, fruit juice, fresh and decomposed viscera, and ≤ 200 g/L of putty powder, sucrose, gourmet powder, and soil. Operation errors of liquid measure had few effects on sensitivity and specificity, including -50%-200% of sample, -22%-44% of sample-treating buffer and -30%-30% of loading mixture.</p><p><b>CONCLUSION</b>The good detection performance and tolerance performance bring the bright future for Bps-UPT-LF strip to detect Burkholderia pseudomallei on site rapidly and quantitatively for nature foci surveillance and anti-bioterrorism.</p>

<p><b>OBJECTIVE</b>To develop an up-converting phosphor technology based lateral flow (UPT-LF) assay for rapid and quantitative detection of Yersinia pestis, Bacillus anthracis spore and Brucella spp.and make the comparison with BioThreat Alert (BTA) test strips (Tetracore Inc., USA).</p><p><b>METHODS</b>Using up-converting phosphor nano-particles (UCP-NPs) as the bio-marker, three double-antibody-sandwich model based UPT-LF strips including Plague-UPT-LF, Anthrax-UPT-LF, Brucella-UPT-LF were prepared and its sensitivity, accuracy, linearity and specificity were determined by detecting 10(10), 10(9), 10(8), 10(7), 10(6), 10(5) and 0 CFU/ml series of concentrations of Y.pestis, B.anthracis, Brucella standards and other 27 kinds of 10(9) CFU/ml series of contrations of bacteria strains.Furthermore, the speed, sensitivity and accuracy of bacteria standards and simulated sample detection were compared between UPT-LF and BTA system.</p><p><b>RESULTS</b>The detection limit of Plague-UPT-LF, Anthrax-UPT-LF and Brucella-LF was 10(5) CFU/ml. The CV of series of bacteria concentrations was ≤ 15%, and the r between lg (T/C-cut-off) and lg (concentration) was 0.996,0.998 and 0.999 (F values were 1 647.57, 743.51 and 1 822.17. All the P values were <0.001), respectively. The specificity of Plague-UPT-LF and Brucella-LF were excellent, while that of Anthrax-UPT-LF was a little bit regretful because of non-specific reaction with two isolates of B. subtilis and one B.cereus. On-site evaluation showed the detection time of UPT-LF for all Y.pestis, B.anthracis spore and Brucella spp.was 33, 36 and 37 min, while BTA was 115, 115 and 111 min, which revealed the higher detection speed and sensitivity of UPT-LF comparing with BTA. The negative rate of two methods for blank standard was both 5/5, the sensitivity of UPT-LF for Y.pestis,B.anthracis spore and Brucella spp. was all 10(5) CFU/ml, then BTA was 10(6), 10(6) and 10(5) CFU/ml, respectively. The detection rate of UPT-LF for all three bacteria analog positive samples was 16/16, while BTA for B.anthracis was 7/16 only.</p><p><b>CONCLUSION</b>The good performance including rapidness, simplicity and high sensitivity will bring the bright future of UPT-LF to be broadly used on-site as first response to bio-terrorism.</p>

Objective To develop an up-converting phosphor technology-based lateral-flow ( UPT-LF) assay for rapid detection of Listeria monocytogenes, named LM -UPT-LF.Methods Monoclonal antibodies against p 60, which was the specific virulence factor of L.monocytogenes,were prepared and covalently conjugated with up-converting phosphor nanopar-ticles (UCP-NPs) as bio-label.Then, LM-UPT-LF was established with double-antibody sandwich mode-based LF assay. Detection performance , including sensitivity and specificity , was evaluated .Results The samples with absolute contamina-ted amount of L.monocytogenes cells <10, 10-99, and 100-1000 cfu could be significantly detected as positive after in-cubation at 20 h, 18 h, and 16 h, respectively.Other 13 kinds of food-borne pathogens with concentration of 109 cfu/ml did not caused any non-specific reaction .Conclusion The established LM-UPT-LF assay could detect L.monocytogenes with high sensitivity , specificity and simplicity and provides an alternative method for food safety control .

<p><b>BACKGROUND</b>Herpes simplex virus thymidine kinase phosphorylates ganciclovir to ganciclovir monophosphate, which is then converted to ganciclovir triphosphate by endogenous cellular nucleoside kinases. The ganciclovir triphosphate acts as a DNA chain terminator due to the lack of a functional 3'-OH group and terminates the process of DNA replication, hence leading to cell apoptosis. At present, HSVtk gene usually acts as suicide gene to kill tumor cells. The aim of this study was to investigate the selective cytotoxicity of the herpes simplex virus thymidine kinase/ganciclovir (HSVtK/GCV) suicide gene system controlled by the a-fetoprotein (AFP) promoter on hepatocellular carcinoma (HCC) cells in vitro.</p><p><b>METHODS</b>pAFP-HSVtk-IRES2-EGFP recombinant plasmid vectors driven by the AFP promoter were constructed. HL-7702 liver cells, HUH-7 HCC, and HepG2 HCC were transfected with the recombinant plasmids. HSVtK gene expression was detected using Western blotting analysis. HepG2 cells line stably expressing HSVtk gene was selected by G418 reagent. The cytotoxicity of HSVtK/GCV suicide gene system on hepatoma cells was measured by CCK-8 reagents when different doses of ganciclovir were added.</p><p><b>RESULTS</b>Plasmid pAFP-TK-IRES2-EGFP-expressed HSVtk gene was constructed successfully. HSVtk gene expression level was significantly higher in AFP-positive hepatoma cells than in AFP-negative liver cells. After G418 selection, a HepG2 cells line stably expressing HSVtk gene was acquired. With the increase of the dose of ganciclovir the optical density at 450 nm of HepG2 cells stably expressing HSVtk gene gradually decreased (P < 0.05).</p><p><b>CONCLUSION</b>The HSVtK gene-specific expression in hepatoma cells as well as the cytotoxicity of the suicide gene system in HepG2 cells provided the basis for the targeted gene therapy of HCC.</p>

Objective To study the incidence of ventilator-associated pneumonia(VAP)and antimicrobial resistance of pathogens in an intensive care unit(ICU).Methods The occurrence of VAP in hospitalized patients with mechan-ical ventilation>48 hours between January 2011 and December 2012 were investigated,species and antimicrobial re-sistance of pathogens causing early onset-VAP (E-VAP,mechanical ventilation≤4 d)and late-onset VAP(L-VAP, mechanical ventilation>4 d)were compared.Results A total of 1 76 patients were investigated,incidence of VAP was 44.32% (78 cases);With the prolongation of mechical ventilation,incidence of VAP increased gradually (χ2=52.561,P<0.001).The incidence of L-VAP was significantly higher than E-VAP (58.33% [70/120]vs 14.29%[8/56])(χ2= 30.02,P<0.001).A total of 178 pathogens were isolated,gram-negative bacteria,gram-positive bac-teria and fungi were 104(58.43% ),46(25.84% ),and 28(15.73% )isolates respectively;97(54.49% )multidrug-resistance/pandrug resistance organisms (MDRO)were isolated. MDRO isolation rate in L-VAP patients was high-er than E-VAP patients([58.86% ,n= 93]vs [20.00% ,n= 4]),resistance rate of major pathogens causing L-VAP was significantly higher than E-VAP patients(allP<0.05).Fungi infection only occurred in L-VAP patients,the total antimicrobial resistance rate was 12.14% .Conclusion The prolongation of mechanical ventilation can increase the incidence of VAP,and resistance rate of pathogen in L-VAP is high.

Objective To investigate the effects of different doses Alpha-Linolenic acid ( ALA ) on the expressions of fatty acid synthesis-related genes in HepG2 cells.Methods HepG2 cells were divided into two groups, normal control group (NC) and high fat group (HF) in which cells were firstly cultured for 36h in the medium contained 0.5mmol/L stearic acid.Real-time quantitative PCR was taken to detect mRNA expression of genes SREBP1C, FAS and ACC which are related to fatty synthesis.While there are significant differences in fatty synthesis, 10%, 20%, 50%, 70% and 100%ALA took the place of stearic acid, 36h later, real-time quantitative PCR and Western blotting were taken to detect mRNA and protein expression of genes related to fatty synthesis.Results SREBP1C mRNA expression of ALA substitution groups were significantly lower than the high-fat group ( P <0.001) .The FAS of 0.5 mmol/L ALA group and 0.35 mmol/L ALA group were significantly lower than the high-fat group (P <0.001).ACC genes mRNA level was not significantly different from high-fat group.SREBP1C and FAS protein expression were significantly lower than the high-fat group, but ACC showed no significant difference.Conclusions Saturated fatty acids promote hepatocyte fatty acid synthesis, ALA abates fatty acid synthesis by inhibiting FAS and SREBP1C gene expression.

Objective To evaluate the feasibility of community treatment and management mode in elderly patients with hypertension.Methods A total of 286 elderly patients with hypertension were treated with community therapy and management for 6 months.The improvements in life style and hypertension control were observed before and after the community treatment and management.Results There were significant differences in systolic blood pressure and diastolic blood pressure in the elderly patients before and after the treatment and management [(158.4± 11.3) mm Hg vs.(131.3±5.9) mm Hg,(99.6±8.9) mm Hg vs.(81.5±5.6) mm Hg,respectively,both P<0.05].Compared with pretreatment,the blood pressure control rate,the awareness rate of diagnostic criteria and the drug compliance were significantly increased after the treatment and management [38.5% (110/286) vs.96.9% (277/286),66.4% (190/286) vs.100% (286/286),49% (140/286)vs.91.3% (261/286),x2 =98.87,115.36,122.13,respectively,all P<0.01].Conclusions The community treatment and management can increase the blood pressure control rate and improve the quality of life in elderly patients with hypertension.It is a feasible method to comprehensively prevent and control hypertension.

Objective To study the heart rate variability and arrhythmia characteristics in the elderly patients with unstable angina and negative psychology and to observe the changes before and after treatment. Methods Eighty patients aged ≥ 65 years with unstable angina were recruited in the study, and divided into two groups according to the determination of Hospital Anxiety and Depression Scale: 36 patients in the negative psychology group( NP group) and 44 in the non-negative psychology group ( non-NP group). All patients were monitored by Holter. Results The SDNN, SDANN, SDNNidex, rMSSD and PNN50 in the NP group were (86.8 ±14.1 ) ms, (78.1 ± 11.2 ) ms, (22.9 ± 4.5 ) ms, ( 6.3 ± 1.2 ) % and (40.3 ± 16.4) ms, respectively, which were significantly decreased compared to that in the non-NP group [ ( 138.9 ± 37.1 ) ms, ( 123.3 ± 32.5 ) ms, (25.7 ±12.3)ms,(9.2 ±5.3)% and (47.8 ± 10.7)ms,respectively] (t =4.20,2.52,2.32,2.28 and 2.20,Ps <0.05)The prevalence of ventricular arrhythmia, sinus tachycardia, ischemic ST-T changes and heart rate variability (HRV) (55.56%, 13.89%, 33. 33% and 52. 78%, respectively) significantly increased in the NP group,compared to those in the non-NP group(9.09% ,2.27% ,6.82%, 18.18% ,respectively) ( χ2 = 16.89,4.18,4.33 and 4.99, respectively, Ps < 0.05 ). After treatment with Prozac ( fluoxetine hydrochloride scientific name), SDNN,SDANN,rMSSD,PNN50,SDNNindex significantly increased from (84.7 ± 34.0) ms, (79.2 ± 39.6) ms, (23.4 ±7.5)ms,(5.1 ±2.1)% and (38.1 ± 11.8)ms to (102. 1 ±29.6)ms,(94.2 ±26.7)ms,(35.6 ± 10.2)ms,(9.9 ± 7.5 ) % and (63.3 ± 13.8 ) ms ( t = 3.60,2.43,2.34,2.26 and 2.30, P < 0.05 ). After treatment,ventricular arrhythmia,sinus tachycardia,ischemic ST-T changes and HRV significantly decreased compared to that before treatment in the NP group (χ2 = 5.58,4.33,4.24 and 4.06 ,P < 0. 05 ). Conclusions In patients of coronary heart disease with unstable angina and negative psychologically , HRV decreased , and they are prone to ventricular arrhythmias,sinus tachycardia and ischemic ST-T changes. Treatment with Prozac could decrease arrhythmia and improve cardiac ischemia arrhythmias, sinus tachycardia and ischemic ST-T changes. Treatment with Prozac could decrease arrhythmia and improve cardiac ischemia.

OBJECTIVE To investigate the association between 592C/A and-819C/T of interleukin-10 gene polymorphism in patients with immediate ?-lactam drug allergy in Chinese Han population.METHODS The genotype and allele frequency of interleukin-10 gene were studied by polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP)in 44 Chinese Han patients with evidence of immediate ?-lactam drug allergy and 44 controls subjects.They all matched for sex and atopy,the production was investigated by sequence analysis.RESULTS Our analysis did reveal differences in the distribution of the single nucleotide polymorphism(SNP)between female allergic patients and controls.Among allergy subjects,we found two distinct significant associations between immediate drug allergy women and two linked IL-10 promoter genes polymorphism,-592C/A and-819C/T(P

OBJECTIVE The diagnosis of drug allergy is difficult and is usually based on clinical history,skin tests(for some drugs) and,in a few specialized allergy centers,provocation tests.To establish the method for analyzing basophil activation test(BAT) by flow cytometry(FCM) and evaluate its clinical significance in the diagnosis of drug allergy.METHODS The protocol for FCM analysis of basophil degranulation in allergy dustmite by CD63,CD203c and CD45 combination was established.The clinical significance of activated basophil by FCM was evaluated by comparing with the results of sIgE by fluorescence enzyme-linked absorbent assay(FELISA),regarding the skin prick test as the gold standard.RESULTS Pure basophils were got by CD45 and CD203c gating,CD63 was the best marker for activated basophil;Spearman′s correlation coefficients indicated a moderate positive correlation between SIgE class categorized by Unicap class and activated basophil;there was no significant difference between activated basophil by FCM and sIgE by FELISA in the diagnosis of allergic reaction,but the former was better than the latter in specificity and positive likelihood ratio.CONCLUSIONS Quantification of activated basophil by CD63 expression with FCM is a valuable new and safe method in vitro for diagnosis of immediate type hypersensitization.

RESUMO

OBJECTIVE To determine the value of acidic electrolyzed oxidizing water (EOW) for disinfecting endoscopes in the children's hospitals. METHODS The detection methods of field sampling were used for monitoring the effect of EOW disinfection. Washed after-use endoscopes were disinfected by EOW for 3-5min. Bacteriological tests on samples of pre-and post-disinfection made for 30 samples of laryngoscopes,30 examples of gastroscopes and 16 samples of enteroscopes,which were selected randomly. RESULTS Through EOW disinfection,the bacteria were not found on the laryngoscopes. The total number of bacteria on digestive endoscopes was conformed to the Standards of Disinfection Techniques and no pathogenic bacteria were detected. CONCLUSIONS Applying EOW to disinfection of endoscopes for clinical purpose can fulfill the State Standards completely. EOW has strongly germicidal activity,also with characteristic aslo act rapidly,securely and environment friendly,and without any irritant odor. These rusults indicate a widely potential use of EOW as in children's hospitals,especially,with a positive significance in health care of pediatric patients.
