RESUMO

OBJECTIVES@#This study aimed to investigate the association between depression with atypical features and metabolic syndromes in Korean adults using the 2016 Korean National Health and Nutrition Examination Survey (KNHANES) data.@*METHODS@#We used the 2016 KNHANES data to enroll 277 participants with a score of 10 or higher on Patient Health Questionnaire-9. Depression with atypical features was diagnosed when at least two of the following criteria were met : 1) sleeping more than 10 hours a day ; 2) weight gain of more than 3 kg in a year ; and 3) fatigue/anergia. Depression was divided into two groups based on the presence/absence of atypical features. Physical and mental health, and risk of metabolic syndrome were compared between the groups.@*RESULTS@#Among the 277 participants, 91 had depression with atypical features. We identified significant differences in age, sex, income, and education between the two groups. After adjusting for these variables, depression with atypical features had lower EuroQol-5D index scores (p<0.001) and higher prevalence of metabolic syndromes (p=0.035) compared to the depression without atypical features. Depression with atypical features had higher odds ratio (OR) in association with metabolic syndromes after adjusting for confounding variables (OR=1.923 ; 95% confidence interval : 1.069–3.460).@*CONCLUSIONS@#Depression with atypical features increases the risk of metabolic syndromes and lowers the quality of life.

RESUMO

PURPOSE: The purpose of this study is to evaluate the surgical outcomes of the clavicle lateral end fracture fixed with an oblique T locking compression plate (LCP). MATERIALS AND METHODS: Fourteen clavicle lateral end fractures were fixed with the oblique T-LCP and followed up for at least 1 year after the surgery. Thirteen cases were unstable Neer type II fractures and one case was nonunion of the Neer type I fracture. The mean age was 46 years of age (range, 26~70). In ten cases, augmenting sutures with the absorbable suture material were placed in the coraco-clavicular ligament and around the plate and the clavicle to improve the stability of fracture fixation. Autogenous iliac bone graft was done in four cases. The clinical outcomes were evaluated by using UCLA scoring system and KSS (Korean Shoulder Score). RESULTS: The mean UCLA score was 33.5 and the mean KSS was 94.9. Average time of bone union was 11.9 weeks (range, 6~28), including 1 case with a delayed union. There was no complication such as loss of fixation or nonunion. CONCLUSION: Fixation with the oblique T-LCP is a good option providing reliable functional results in clavicle lateral end fractures.

RESUMO

Lateral ankle sprains are one of the most common injuries to the lower extremity. Most of them well respond to conservative treatments. However, simultaneous peroneal nerve injuries may occur rarely following lateral ankle ligamentous injuries. We report a case presents superficial peroneal nerve injury with dorsal foot pain lasting for more than 2 months after lateral ankle sprain and review the literature.

RESUMO

Stress fractures occur when the loads applied to a bone exceed the mechanical resistance and fall into two groups. Fatigue fractures, in which abnormal mechanical stress is applied to a normal bone, and insufficiency fractures, in which fracture occurs when stress of normal activity is applied to a bone that has decreased elastic resistance. Femoral shaft insufficiency fractures are reported rarely in patients with postmenopausal osteoporosis. We report a case of repetitive insufficiency fractures of the femoral shaft in 70 year-old female with marked osteoporosis.

Ultraviolet (UV) irradiation is an environmental factor that causes skin aging, and is also a major factor leading to cumulative alterations of skin structure, function and appearance. To investigate the effects of Selenium (Sel) on UV-induced skin aging, hairless mice were treated for 4 weeks with UV irradiation and topical application of Sel. Then, the effects of Sel were measured in the skin of these mice via histological analysis and Western blotting. According to the results of wrinkle formation analysis, the topical application of Sel induced a reduction in wrinkling formation in the damaged skin of the UV-irradiated mice. Additionally, our histological analysis demonstrated that the skin thickness in the Sel-treated group was less than in the UV-irradiated group. Furthermore, in an effort to investigate the mechanisms underlying the effects of Sel, the expression levels of matrix-metalloproteinase (MMP) and MAPK protein were assessed in both groups. The application of Sel induced a reduction in MMP-1 expression levels to the levels observed in the non-irradiated group. However, the expression level of MMP-9 was increased slightly in the Sel application group as compared with the vehicle application group. Additionally, the levels of ERK phosphorylation were increased by the application of Sel, but the levels of JNK and p38 were not altered by Sel treatment. These results suggest the possibility that Sel should be considered as a skin aging-protective and therapeutic drug candidate, which functions via the regulation of MMP expression levels.

RESUMO

Atopic dermatitis is an well-known skin disease showing inflammatory, chronically relapsing, non-contagious and pruritic symptoms. The aims of this study were to investigate the effects of Liriope platyphylla (LP) on atopic dermatitis of NC/Nga mice. To achieve this, NC/Nga mice were treated with four different conditions including vehicle, phthalic anhydride (PA), PA+5% LP and PA+10% LP, and the changes of immune-related factors were detected after 2 weeks. The pathological phenotypes of atopic dermatitis such as erythema, ear thickness, edema, scab and discharge were significantly decreased in PA+10% LP cotreated groups compare to PA treated group. Also, the weight of lymph node and thymus in immune organs were gradually decreased in LP treated groups, while the weight of spleen was slightly increased in same group. Furthermore, toluidine blue staining analysis, a method used to specifically identify the mast cell, showed that the decrease of master cell infiltration into the dermis were statistically observed in PA+5% LP and PA+10% LP5% cotreated groups. Especially, the decrease of IgE concentration was detected only PA+10% LP cotreated group, although this level was maintained in PA+5% LP cotreated group. Therefore, these results suggested that the water extracts of LP may contribute the relieve of atopic dermatitis symptoms and be considered as an excellent candidate for a atopic dermatitis-therapeutic drug.

RESUMO

PURPOSE: We report a case of pyogenic arthritis of the shoulder secondary to infective endocarditis. MATERIALS AND METHODS: A 70 year-old male who had suffered from pyogenic arthritis of the left shoulder secondary to infective endocarditis was treated with artificial valvuloplasty, arthroscopic synovectomy and drainage. RESULTS: Infection was cured and the patient achieved a good functional outcome. CONCLUSION: Pyogenic arthritis of the shoulder is rarely associated with infective endocarditis. However, if the symptoms are misdiagnosed as musculoskeletal symptoms associated with infective endocarditis, serious complications may arise. As such, musculoskeletal symptoms associated with infective endocarditis should be paid careful attention.

RESUMO

PURPOSE: This study was performed to evaluate the usefulness of proximal femoral nail antirotation (PFNA) for internal fixation of femur trochanteric fractures. MATERIALS AND METHODS: We operated on 36 femur trochanteric fracture patients with performing PFNA from September, 2006 to November, 2008 and we analyzed the operation time, the blood loss, the union time, the tip apex distance (TAD), the Cleveland index, the sliding distance of the blade and the complications. We also evaluated the clinical results according to the recovery of ambulatory function and the functional recovery score. RESULTS: The mean operation time was 54 minutes and the mean amount of blood loss amount was 119cc. Thirty two cases progressed to union within 4 months and 4 cases also progressed to union within 6 months without a further operation. The mean TAD was 16mm and the mean sliding distance was 3.8 mm. Clinically, the mean loss of ambulation ability was 1.2 grades and the Jensen functional recovery score was 1.8. There was one case of back out of the blade, but there was no skin problem. There were no significant complications. CONCLUSION: The findings from this study indicate that PFNA is a useful and reliable choice for the treatment of trochanteric fracture of the femur.
