RESUMO

Objective: To investigate the characteristics of non-alcoholic fatty liver disease (NAFLD) and its associated factors in rheumatoid arthritis (RA) patients. Methods: This cross-sectional study recruited 385 RA patients [including 72 (18.7%) male and 313 (81.3%) female] who received abdominal sonographic examination from August 2015 to May 2021 at Department of Rheumatology, Sun Yat-Sen Memorial Hospital. There were 28 RA patients at 16-29 years old and 32, 80, 121, 99, 25 at 30-39, 40-49, 50-59, 60-69, ≥ 70 years old, respectively. Demographic and clinical data were collected including age, gender, history of alcohol consumption, disease duration, body mass index (BMI), waist circumference, blood pressure, RA disease activity indicators and previous medications. Logistic regression analyses were used to identify the associated factors of NAFLD in RA patients. Results: The prevalence of NAFLD was 24.2% (93/385) in RA patients, 26.3% (21/80) in 40-49 age group and 33.1% (40/121) in 50-59 age group. There were 22.1% (85/385) and 3.6% (14/385) RA patients with overweight and obese, in which the prevalence of NAFLD was 45.9% (39/85) and 78.6% (11/14) respectively, which was 2.6 folds and 4.5 folds that of RA patients with normal BMI. Although there was no significant difference of age, gender and RA disease activity indicators between RA patients with or without NAFLD, those with NAFLD had higher proportions of metabolic diseases including obese (11.8% vs. 1.0%), central obesity (47.3% vs. 16.8%), hypertension (45.2% vs. 29.8%) and type 2 diabetes mellitus (24.7% vs. 12.0%), consistent with higher levels of total cholesterol [(5.33±1.31) mmol/L vs. (4.73±1.12) mmol/L], triglyceride [(1.51±1.08) mmol/L vs. (0.98±0.54) mmol/L] and low-density lipoprotein cholesterol [(3.37±0.97) mmol/L vs. (2.97±0.78) mmol/L, all P<0.05]. Multivariate logistic regression analysis showed that BMI (OR=1.314) and triglyceride (OR=1.809) were the independent factors positively associated with NAFLD in RA patients. Conclusion: NAFLD is a common comorbidity in RA patients, especially in those with middle-aged, overweight or obese, which is associated with high BMI or high triglyceride. Screening and management of NAFLD in RA patients especially those with overweight, obese or dyslipidemia should be emphasized.

OBJECTIVES@#To analyze the characteristics of sudden death associated with sexual activity to provide recommendations for forensic identification.@*METHODS@#A retrospective analysis was conducted on autopsy cases accepted by Forensic Identification Center of Huazhong University of Science and Technology from 1998 to 2018, and a total of 15 cases of sudden death associated with sexual activity were screened out. The general information, case data and pathological changes of 15 cases were collected to find the relationship between sexual activity and sudden death.@*RESULTS@#The ratio of male to female was 1.5∶1. The average age of males was 50.1 years and that of females was 35.0 years. Coronary artery diseases and brain diseases accounted for most of the cases (12/15). Sexual partners were associated with locations of deaths and body dumping behaviors.@*CONCLUSIONS@#Sudden death associated with sexual activity, although rare, may occur in people over 30 years old with pre-existing heart or brain diseases, which should be paid attention to in forensic practice.

Objective:To investigate the effect of low molecular weight heparin (LMWH) on the inflammatory response of PC12 cells induced by oxygen glucose deprivation (OGD) and its related mechanism.Methods:The PC12 cells were cultured in vitro were randomly divided into sham(control) group, OGD group, LMWH group and blocking agent group. The latter group was divided into six groups: Eritoran+ OGD group, LMWH+ Eritoran+ OGD group, ST2825+ OGD group, LMWH+ ST2825+ OGD group, pyrrolidinedithiocarbamate (PDTC)+ OGD group and LMWH+ PDTC+ OGD group. OGD cell model was established. Cell counting kit-8 (CCK-8) assay was used to detect cell activity. The expressions of toll-like receptor 4 (TLR4), MyD88 and nuclear factor κB (NF-κB) mRNA and protein were detected by real time polymerase chain reaction (qRT-PCR) and Western blot. The concentration of interleukin (IL)-1β, IL-6, tumor necrosis factor-α(TNF-α) and S100β were determined by enzyme linked immunosorbent assay (ELISA). Results:The cell activity of OGD group was significantly lower than that of control group on the first, second, third day ( P<0.05). Compared with OGD group, the activity of LMWH group was increased on the second, third day ( P<0.05), but lower than that of control group ( P<0.01). The mRNA expression of TLR4, MyD88 and NF-κB was significantly increased in OGD group compared with the control group ( F=144.9, F=710.5, 79.51, P<0.01). Compared with OGD group, the mRNA expression of TLR4, MyD88 and NF-κB were significantly decreased after treatment with LMWH ( P<0.01), and the specific inhibitor of TLR4, MyD88 and NF-κB enhanced the anti-inflammatory effect of LMWH. The protein expression of this pathway was consistent with that of the gene. The concentration of IL-1β, IL-6, TNF-α and S100β in OGD group was significantly higher than control group ( P<0.05). After treatment with LMWH, the concentrations of inflammatory factors and S100β were significantly decreased compared with OGD group ( P<0.01). When hinder TLR4, MyD88 and NF-κB respectively by Eritoran, ST2825 and PDTC, the concentrations of inflammatory factors and S100β were significantly decreased, but it was still higher than control group ( P<0.05). Conclusions:OGD can cause pathological damage of PC12 cells, including high expression level of S100β and aggravation of inflammatory reaction. LMWH can improve cell activity, down-regulate inflammatory reaction degree and protect the cells. Using inhibitors of TLR4/MyD88/NF-κB pathway to inhibit the corresponding target, the up-regulation of inflammatory factors by OGD can be inhibited in varying degrees. These suggested that LMWH may regulate inflammatory reaction of PC12 cells induced by OGD through TLR4/MyD88/NF-κB pathway.

The study is aimed to construct high quality Salvia miltiorrhiza cDNA library and obtain the SmJAZ8 gene of S. miltiorrhiza by yeast two-hybrid system. In this study, full-length cDNA was synthesized from roots, stems, leaves, flowers and hairy roots of S. miltiorrhiza. The full-length cDNA library was synthesized by SMART method and constructed with DSN homogenization technique. The results showed that the library capacity was 1.45×10⁶, the recombination rate was 100%, and the average size of the insert was 500-2 000 bp. The recombinant vector of pDEST-pGADT7-SmJAZ8 was constructed and transformed into Y2HGold strain. The interaction protein was screened by yeast two-hybrid system. The DnaJ protein and UBQ protein were screened by yeast two-hybrid system. This study has successfully constructed a full-length cDNA library of S. miltiorrhiza, and laid the foundation for the follow-up study on functional gene screening and gene function of S. miltiorrhiza.

Objective To explore effects of dizocilpine (MK-801) preconditioning on excitatory amino acids and inflammatory response in rats induced by cardiac arrest-cardiopulmonary resuscitation (CACPR).Methods 18 male Sprague Dawley (SD) rats were randomly divided into three groups:control group,CA group and CA + MK-801 group.To establish rat models of CA-CPR and keep samples of serum and specimens of brain tissues for following detection.The injury of neurons was observed by HE staining and expression of N-methyl-D-aspartic acid receptor (NMDAR) in brain tissues was detected by Western blot.The concentrations of interleukin 1 beta (IL-1 β) and tumor necrosis factor (TNF)-α in serum were detected by enzyme linked immunosorbent assay (ELISA).Results Neurons in CA group were disorganized,cells shrank,nuclei pyknosis,and cytoplasmic eosinophilia,accompanied by inflammatory cell infiltration.Preconditioning with MK-801 reduced the pathological damage of neuron and degree of macrophage infiltration.The relative expression of NMDAR protein in CA group were significantly higher than that in control group (907.9 ±24.9 vs 321.6 ± 18.4,P <0.001).Preconditioning with MK-801 significantly decreased the expression of NMDAR in CA + MK-801 group compared with that in CA group (512.4 ± 21.1 vs 907.9 ± 24.9).The CA group showed significantly increased concentrations of IL-1 β and TNF-α than that in control group (P < 0.001),and this effect was abolished by preconditioning with MK-801.CA rats treated with MK-801 showed higher concentrations of IL-1 β and TNF-α than the control group.Conclusions Cardiac arrest causes pathological injury of neurons,up-regulates expression of NMDAR and aggravates inflammatory response.These results induce the apoptosis of nerve cells.Blocking glutamate receptor with MK-801 can inhibit expression of NMDAR,decrease level of cytokines,down-regulate inflammatory reaction degree therefore to protect the brain.

Objective To explore clinical curative effects of superficial temporal artery-middle cer-ebral artery bypass combining with encephalo-duroarterio-synangiosis( STA-MCA+EDMS) in the treatment of moyamoya disease. Methods Thirteen cases were confirmed to be moyamoya disease by digital subtrac-tion angiography ( DSA) , and treated with STA-MCA+EDMS methods. Four cases were ischemic, and the others were hemorrhagic in this group. Results The follow-up was 6-12 months. The clinical symptoms were improved significantly, without cerebral ischemic attack in 4 ischemic cases. Seven hemorrhagic cases got blood from the external carotid artery by means of vascular bypass or temporal muscle, and established effective collateral circulation half a year after operation. Two hemorrhagic cases bleed again in July after operation. However, the family gave up treatment. Ten cases were good, one case was moderate, while two cases got bad. The total effective rate was 76. 9%. Conclusions STA-MCA+EDMS can improve cerebral hemodynamics of moyamoya disease, improve quality of life, and reduce incidence of stroke.

Objective To investigate the effects of knockdown of miR-449a on breast cancer xenograft in nude mice.Methods In this study,we established a model of breast cancer xenograft in nude mice with breast cancer MCF-7 cell line,then stably transfected MCF-7 cells with plasmids containing shRNA-miR-449a precursors;the stable transfected cells were injected subcutaneously into the nude mice by multi-point injection.We used negative plasmid as negative control and PBS as blank control.We detected tumor volume after the treatment of nude mice and tumor growth inhibition rate was calculated.The expressions of miR-449a and Notch1 in xenograft in nude mice were detected by real-time PCR;the levels of Notch 1,β-catenin and E-cadherin in the xenograft tumor tissues were detected by Western blot; and the expression of E-cadherin in tumor tissues was detected by immunohistochemistry.Results In the process of xenograft tumor formation,no nude mice died.Knockdown of miR-449a could significantly inhibit the growth of tumor volume after tumor detection (P <0.05 ).miR-449a was significantly down-regulated in xenograft tumor tissues,and the expressions of Notch 1 and β-catenin were also down-regulated,while the level of E-cadherin was increased.The results of immunohistochemistry showed that the expression of Notch1 protein was decreased and that of E-cadherin protein was increased (P <0.05).Conclusion Knockdown of miR-449a inhibited the normal growth of breast cancer xenografts in nude mice by down-regulating the expressions of Notch 1 andβ-catenin and promoting the expression of E-cadherin.

<p><b>OBJECTIVE</b>Three-dimensional finite method was used to analyze stress and strain distributions of periodontal ligament of abutments under dynamic loads.</p><p><b>METHODS</b>Finite element analysis was performed on the model under dynamic loads with vertical and oblique directions. The stress and strain distributions and stress-time curves were analyzed to study the biomechanical behavior of periodontal ligament of abutments.</p><p><b>RESULTS</b>The stress and strain distributions of periodontal ligament under dynamic load were same with the static load. But the maximum stress and strain decreased apparently. The rate of change was between 60%-75%. The periodontal ligament had time-dependent mechanical behaviors. Some level of residual stress in periodontal ligament was left after one mastication period. The stress-free time under oblique load was shorter than that of vertical load.</p><p><b>CONCLUSION</b>The maximum stress and strain decrease apparently under dynamic loads. The periodontal ligament has time-dependent mechanical behaviors during one mastication. There is some level of residual stress left after one mastication period. The level of residual stress is related to the magnitude and the direction of loads. The direction of applied loads is one important factor that affected the stress distribution and accumulation and release of abutment periodontal ligament.</p>

<p><b>OBJECTIVE</b>To construct and compare the body mass index (BMI) cutoff points for defining overweight and obesity in school-age children and youths with Uygur and Han ethnic backgrounds in Xinjiang, China.</p><p><b>METHODS</b>A total number of 9146 boys and girls in Urumqi, Xinjiang, aged 7-18 years, were recruited by stratifying and clustering sampling from April to June 2004. Demographic and anthropometric data, including body weight and standing height, were collected. Questionnaires and standard methods were used and BMI was calculated by weight/height2. BMI centile curves with age were drawn according to gender and ethnicity, using LMS software (Tim cole and Huiqi Pan) which were making the centile curves passing through 25 kg/m2 and 30 kg/m2, and 24 kg/m2 and 28 kg/m2 at age of 18 for overweight and obesity respectively.</p><p><b>RESULTS</b>The age and sex specific BMI centile curves were drawn for children with Uygur and Han ethnicities. Data showed that BMI increased with age. The centile of BMI curves, P94.46 and P99.58 for Uygur boys, P92.44 and P99.64 for Uygur girls, P85.05 and P97.26 for Han boys, P90.92 and P99.03 for Han girls, which passing through 25 kg/m2 and 30 kg/m2 by IOTF recommendation at age 18, were constructed. Under the basis of WGOC recommendation, the points that passing through 24 kg/m2 and 28 kg/m2 at age 18 were P90.54 and P98.86 for uygur boys, P86.96 and P98.77 for uygur girls, P78.98 and P94.72 for Han boys, P86.15 and P97.56 for Han girls. The BMI cutoff points for defining overweight and obesity were suggested.</p><p><b>CONCLUSION</b>To the best of our knowledge, the present study established the BMI cutoff points for Uygur ethnic school-aged children and youths for identifying overweight and obesity. There was ethnic difference in the BMI distribution with age. We recommended using the BMI cutoff points proposed by the current study to identify those overweight and obese children in Urumqi.</p>

<p><b>BACKGROUND</b>To evaluate the influence of assays with primer labeled with fluorochrome (Cy5) and dUTP labeled with Cy5 on the signal intensity of the chip for detection of hepatitis B virus (HBV) gene polymorphism.</p><p><b>METHODS</b>The P-region and pre-C/C-region of HBV gene were amplified by polymerase chain reaction (PCR) with Cy5 labeled primer or Cy5 labeled dUTP. The amplicons of the two assays were hybridized with chips, scanned and analyzed by computer software for the detection of HBV gene polymorphism.</p><p><b>RESULTS</b>The signal intensity of assay with Cy5 labeled dUTP was slightly higher than that of assay with Cy5 labeled primer, but non?specific signal intensity of the assay with Cy5 labeled dUTP was higher. The result of 42 samples showed that there was no significant difference between the two assays, and that both had a good repeatability and CV value (15%-20%).</p><p><b>CONCLUSIONS</b>The assay with Cy5 labeled primer may replace the assay with Cy5 labeled dUTP as a routine method to detect HBV gene polymorphism, and it is simpler and cheaper.</p>

Objective:To investigate the influences of material properties and damping effect on the dynamic response analysis of human periodontal ligament. Methods: A 3-D finite element model of human teeth and periodontal tissue was built from graphic information obtained by CT scanning. Dynamic analysis was performed to investigate the influence of different damping effects on the stresses of periodontal ligament by the finite element method. Results: (a) No significant difference of stress between small and large deformation assumption was found, the variation was no more than 1.4%.(b) Dynamic response was obtained under ? varies from 0 to 1.2. After one mastication period, the residual stress at interior surface was larger than that at exterior surface, with a maximum value of ? r = 0.34 MPa at labial cervical area. Conclusion: Appropriate assumptions on material properties may reduce the cost of analysis. After one mastication period, some levels of residual stresses are in periodontal ligament and influenced by material damp.

RESUMO

Objective:To improve stress analysis in stress-intensive regions of human periodontal tissue. Methods:Based on coarse analysis on the finite element(FE) model including human teeth and periodontal tissue, the submodel method was introduced to analyze the stresses in interested region such as apical. Results:Submodel analysis showed same stress distribution as FE model while more precision of stress value was obtained by submodel because refined mesh was used.Conclusion:More accurate stresses measurements in local regions can be obtained by submodel method.

RESUMO

By analysing the anatomy of upper cervical segment, the atlas - axis relation in plain X - ray film in 3-1 cases revealed that the joint displacement is one of the important causes for vertebral artery type or sympathetic type cervical spondylopathy. The main manifestations for diagnosing atlas - axis displacement included headache, dizziness, unilateral protrusion and tenderness of the affected vertebra, displacement of tooth protrusion up to 1 mm at anterior mouth -open positions. The most effective therapy was accurate and dexterousmanipulation for reduction.
