RESUMO

Deepening healthcare reform highlights the importance of medical services pricing adjustment, and the imperativeness to strengthen the informationized management of such pricing.With this concept in mind, the authors analyzed the problems and causes of the current management on medical service pricing informatization at public hospitals, and discussed the construction strategy of an intelligent error correction system in this regard. That is, to develop a technical process from the four aspects of software flow, interface flow, project rules, and reporting system. This system is designed with such functions as inpatient cost inquiry, inpatient information, price adjustment analysis, price feedback record statistics and inquiries over the hospitalization expense ceiling, as well as inpatient cost omission inquiry.This paper also discussed the construction of a platform to transform the knowledge of pricing policy into the knowledge base of medical service pricing, in order to improve the level of medical pricing management, promote the efficiency of hospital operation and management, and reduce the burden of patients.

RESUMO

Objective To establish a simple animal model of cough variant asthma(CVA)through sensitizing Brown-Norway(BN)rats with ovalbumin(OVA). Methods A total of 36 BN rats were randomly divided into three groups, including the normal control group,the model control group and the montelukast group. BN rats in the model group and the montelukast group were intraperitoneally administered with 2.0 mg of OVA and 100 mg of Al(OH)3,and the same volume of sterile saline was given to the normal group by intraperitoneal injection. Boosting was carried out by intraperitoneal administration with 0.01 mg of OVA and 100 mg of Al(OH)33 weeks later,and the rats in the normal group were injected with the same dose of physiological saline. Three weeks later,the actively sensitized BN rats were challenged with aerosolized OVA for 7 times on alternative days,and the rats in the normal group were treated with sterile saline instead of OVA. At the same time, the montelukast group was given 1.3 mg/kg of montelukast 30 minutes before atomization by intragastric administration once a day for 2 weeks,and the normal group and the model group were given the same volume of water. The tests of cough sensitivity to capsaicin and bronchial responsiveness were performed 24 h after the last administration. Results Compared with the normal group, the times of coughing(P< 0.01)and the lung resistance(RL)(P< 0.05)in the model group were significantly increased,while the lung compliance(Cdyn)was significantly decreased(P< 0.05). There was a significant difference(P < 0.05)in the times of coughing caused by capsaicin between the model group and the montelukast group. Compared with the model group,RLin the montelukast group was decreased significantly(P< 0.05), and Cdynwas increased significantly(P< 0.05). Conclusions This rat model of CVA is similar to a variety of clinical features of CVA and is easy to operate. Thus it can be used as an effective animal model of CVA.

By analyzing the medical insurance policy, the medical insurance audit business and the intelligent monitoring requirements put forward by the medical reform, the intelligent examination and approval database was established, and the system process reengineering was carried out in conjunction with HIS, LIS and electronic medical record systems. The intelligent medical insurance audit information system was successfully developed and implemented, which realized automatic review of the medical qualifications of trauma patients, automatic audit of blood product items, automatic verification of high-cost fees, automatic review of repeated hospitalizations within 10 days, and automatic update of discharge diagnosis, as well as drug( diagnosis/treatment) project limits( excessive) early warning and other core business functions. The system meets the actual needs of the quality and efficiency of data management in medical insurance audit, providing basic platform and data support for standardizing doctors′diagnosis and treatment behavior and avoiding hospital risks.

Objective: To research the anti-inflammatory and anti-allergic effects of Qifangbimin particle.Methods: The anti-inflammatory effects were observed by the methods of xylene-induced ear-swelling in mice and cotton-ball induced granuloma in rats.The anti-allergic effects were evaluated by the method of passive skin allergy model in rats and ear-heterogeneous passive skin allergy model in mice.Results: In the treatment groups with Qifangbimin particle, the swelling degree of ear edema induced by dimethylbenzene in mice was significantly suppressed when compared with that in the control groups (P<0.01 or 0.05) , however, the particle had no significantly inhibitory effect on granulation tissue hyperplasia induced by cotton-ball in rats.The Qifangbimin particle groups obviously decreased the absorbance value of locus coeruleus on rats' back (P<0.01 or 0.05), and Qifangbimin particle at high dose significantly reduced the absorbance value of locus coeruleus of auricle in mice (P<0.01 or 0.05).Conclusion: Qifangbimin particle has significant anti-inflammatory and anti-allergic effects.

Objective To investigate the clinical efficacy and its mechanism of Yangyin Xiaoying decoction combined with western medicine in the treatment of Graves' Disease.Methods A total of 50 patients suffered from Graves' Disease in our hospital were selected from January 2012 to December 2014,and were randomly divided into observation group and control group using random number table method,25 cases in each group.Other 25 cases of healthy subjects were selected in the same period in our hospital as the healthy control group.The control group received the treatment of methimazole,propranolol and other western medicine.The observation group received Yangyin Xiaoying decoction combined with western medicine.After 2 months of treatment,the levels of free triiodothyronine (FT3),free thyroxine (FT4),thyroid stimulating hormone (TSH),thyroglobulin antibody (TGAb) and thyroid peroxidase (TPOAb),and the percentages of Th1,Th2,Th17 cells in CD4+ cells and their corresponding cytokine levels were detected.Results After treatment,the levels of FT3,FT4,TGAb and TPOAb in the observation group and control group were significantly decreased,and levels of TSH were significantly increased in the two groups (P<0.05);percentages of Th1,Th2 and Th17 cells in the observation group and control group were significantly lower than those before treatment (P<0.05).After treatment,the interferon-γ (IFN-γ),interleukin (IL)-4 levels and IFN-γ/IL-4 ratio,IL-12,IL-5 levels and IL-12/IL-5 ratio in the observation group and control group were significantly lower than those before treatment (P<0.05).The IL-17A,IL-17F and IL-23 levels in the observation group and control group after treatment were significantly decreased,compared with those before treatment (P<0.05).The degree of improvement of above indexes in the observation group were better than those in the control group (P<0.05).Conclusion Yangyin Xiaoying decoction combined with western medicine can significantly improve patients' thyroid function,which suggests that it may have a role in the therapy in the treatment of Graves' disease.

RESUMO

The effects of linking loop structure between guanine ( G) repeats on G-quadruplex formation were investigated. The results show that the unfavorable effects of long linking loops on G-quadruplex formation can be overcome by introducing double-stranded structures in linking loop regions. This finding provides a new way for sensor design. The activity of G-quadruplex DNAzyme can be tuned by utilizing target-mediated formation of double-stranded structures in loops. As an example, T-T mismatches are introduced in loops to destroy double-stranded structures. The stabilization of Hg2+ to T-T mismatches promotes the reformation of double-stranded structures. Correspondingly, the oligonucleotide folds into G-quadruplex, which binds with hemin to form peroxidase-like G-quadruplex DNAzyme. Hg2+ sensor is designed based on this principle. Using this method, Hg2+ quantitation is achieved in the concentration range of 10-700 nmol/L, with a detection limit of 8. 7 nmol/L. Cysteine will compete with T bases to bind with Hg2+, releasing Hg2+from T-Hg2+-T base pairs. Thus cysteine can also be quantified with this system in the concentration range of 20-700 nmol/L, with a detection limit of 14 nmol/L.

BACKGROUND: Studies have demonstrated that intermittent high glucose can have a more severe impact on vascular endothelial function in comparison with persistent hyperglycemia.OBJECTIVE: To investigate the effect of intermittent high glucose on the proliferation and apoptosis of endothelial progenitor cells (EPCs) from human peripheral blood in vitro as well as the production of malondialdehyde (MDA) and antioxidant. METHODS: Total mononuclear cells were isolated from human peripheral blood by Ficoll density gradient centrifugation and then the cells were placed on fibronectin-coated culture dishes. After 7 days of culture, the adherent cells were identified as EPCs by laser scanning confocal microscope. The cells were synchronized and then stimulated with glucose 5.5 mmol/L (normal control group), 20 mmol/L (constant high glucose group), and 5.5/20 mmol/L (intermittent high glucose group, 5.5 and 20 mmol/L glucose culture solution was changed every 8 hours) for 72 hours. EPCs proliferation and apoptosis was measured by MTT assay and flow cytometry, respectively. The content of MDA and the activity of superoxide dismutase (SOD) in culture solution were detected with colorimetry.RESULTS AND CONCLUSION: After EPCs were exposed to constant high glucose (20 mmol/L) and intermittent high glucose (5.5/20 mmol/L) for 72 hours, proliferated cells were significantly reduced and the apoptosis rate was significantly increased compared with those exposed to normal glucose (P < 0.01). Furthermore, there was a significant increase in MDA contents as well as a significant reduce in SOD activities in the constant high glucose and intermittent high glucose group (P < 0.01), especially in the latter group. These findings indicated that both intermittent high glucose and constant glucose could inhibit the proliferation and promote the apoptosis of EPCs; however, intermittent high glucose appears to worsen the effects on EPCs. This is maybe due to the increased oxidative stress.

Objective To establish a pancreatic ?-cell developmet fish model with specific spatial expression patterns.Methods Molecular cloning,microinjection,whole embryo in-situ hybridization(WISH)and fluorescence microscopy in living were used to analyze of ?-cell development through generation of transgenic zebrafish.ResultsScreened and established pancreatic ? cells of transgenic zebrafish,and confirmed the fluorescence protein expression in the same spatiotemporal pattern with endogenous insulin gene to achieve dynamic monitoring islet ?-cell development situation in vivo.Conclusion The pancreatic ? cells of transgenic zebrafish animal model can successfully trace pancreatic ? cell development.
