RESUMO

Tailgut cysts are known to originate from the remnants of the embryonic hindgut. They occur exclusively in the retrorectal and presacral spaces. There have been limited reports of tailgut cysts occurring in the left perirenal space. The present case features a huge tailgut cyst extending from the right perirenal to the perivesical space. We believe that this case report will help to further elucidate the characteristics of perirenal and perivesical tailgut cysts.

This study is a record of ethnomedicinal knowledge in Begumganj province focussed on medicinal plants and their local uses for primary health care. The aim was to document and preserve the ethnomedicinal knowledge used by traditional healers of Begumganj upazila, Bangladesh, to treat human diseases and evaluate the relative efficacy of the medicinal plants. The uses of medicinal plants were documented as an ethnomedicinal data sheet using direct observation, field interview, plant interview and group interview techniques from December 2012 to January 2014 in the study area. Data were collected from 98 traditional healers through a questionnaire survey and analyzed through informant consensus factor and fidelity level. This study revealed comprehensive relationship among various diseases and families, forms and parts of plants and modes of preparation. Overall, 75 plant species under 71 genera of 47 families were documented, which are used to treat 41 diseases. Data analysis revealed that 41.33%, 14.67%, 36% and 8% of the medicinal plant species were herbs, shrubs, trees, and climbers, respectively. Leaves were the most used parts, followed by stem, root, fruit, bark, latex and rhizome. The most frequently treated diseases were dysentery, rheumatism and skin diseases. This is the first ethnobotanical survey, which recorded the importance of medicinal plants in Begumganj upazila, Bangladesh.This study can contribute to preserving the indigenous knowledge on the traditional use of medicinal plants in this region and new drug development with attracting future generations towards traditional healing practice.

Amyloidosis is an all-inclusive disease of deposition of amyloid proteins in the extracellular spaces, which in localized or systemic form cause tissue damage and dysfunction. Herein, we report a case of small bowel involvement of systemic amyloidosis presenting with multiple polypoid wall thickening mimicking small bowel polyposis syndrome in an age 75 male. Interestingly, polypoid wall thickening and amyloidoma showed hypointensity on T2-weighted images. To our knowledge, there has been no literature describing MRI findings of poylpoid wall thickening and amyloidoma. Although the underlying mechanisms are unclear and need validation, hypointensity on T2-weighted images could be valuable in diagnosing small bowel involvement of amyloidosis in patients presenting with poylpoid wall thickening and amyloidoma.

RESUMO

Desmoplastic fibroma of bone is a rare locally aggressive, but non-metastatic tumor. In this case report, we present a desmoplastic fibroma in an unusual location, the clavicle. Desmoplastic fibroma involving the clavicle is extremely rare, with only 2 reported cases before 1985. We report the imaging findings of a desmoplastic fibroma of the clavicle with a review of the relevant literature.

Primary breast leiomyosarcoma is extremely rare. The associated imaging findings are generally not pathognomonic and can mimic those of other breast sarcomas and phyllodes tumors. The prognosis of leiomyosarcoma is better than that of other breast sarcomas. Here, we report on the case of a 36-year-old woman with a large, rapidly growing mass in the left breast, which was diagnosed as leiomyosarcoma. The lesion appeared as a circumscribed hyperdense mass on mammography, and a lobulated circumscribed mixed echoic mass with surrounding edema on ultrasonography, and it showed peripheral and multi-septated enhancement on computed tomography. We discuss the imaging findings and clinical outcome.

Average anteversion of the femur is 15.3 degree. Ultrasound, computerized tomogram, and three dimensional reconstruction from CT or MRI have been used for more accurate measurement of the anteversion. There are two methods in measuing anteversion angle from CT scan:one is by drawing a mid line through long axis of the femoral neck (conventional method). Another is by drawing a line from the center of the head to that of the neck at the base of the trochanter using several cut slices (Murphy’s method). We compared these methods with fluoroscopic measurement of the Anteversion. We traced the change of the anteversion before and after closed femoral intramedullary (IMO) nailing to evaluate the origin of malrotation of the femur. 1. Normal anteversion angles were measured in 15 cases. Average anteversion angle was 6.9 degrees by conventional method, 12.3 degrees by Murphy’s method, and 12.2 degrees by fluoroscope. 2. Anteversion angles were measured after IM nailing in 18 cases. Average was 17 degrees by Murphy’s method and 15.2 degrees by fluoroscope. Mean of difference between these two methods sea 6.3 degrees. That was 1.7 degrees in normal side. 3. Change of the anteversion angle between before and after IM nailing was measured in 17 cases by fluoroscope. Average anteversion angle before the operation was 11.9 degrees and it was change to 15.8 degrees after operation. Mean of these change was 7.1 degrees. 4. Pereperative traction provides important information on change of anteversion. Reduction excessive flexion of proximal fragment was a origin of change of anteversion during nailing procedure. Conclusion : Malrotation of the femur after IM nailing must be keep in mind and it may be preventable by fluoroscopic control of the rotation in nailing procedure.

Soft tissue related complications are quite frequent in limb lengthening. Muscle fibers may proliferate or regenerate after stretching injury over 10-20% of their original length. However, the cells which are engaged in this phenomenon are not confirmed yet. We chased the S-phase cells (phase for DNA replication) in the posterior leg muscle during limb lengthening by immunohistochemical technique. We lengthened the tibiae of fifteen New Zealand white rabbits. We divided them into three groups and each group is consisted of five rabbits. In group 1, we lengthened the left tibiae by 10% of their original length, in Group 2, 20%, and in group, 3, 25%, respectively, At the end of lengthening posterior muscles of lengthened left side and of controlled right side were fixed and processed for Immunohistochemical staining which could detect the incorporation of bromodeoxyuridine(BDU). Labelling index(LI:% of positively stained S-phase nuclei) of group 1 was zero. LI of groups for more than 20% lengthening (sum of group 2 and 3) was statistically significant. In conclusion, nuclei around or within the muscle tissue are in S-phase during limb lengthening which means proliferation of the muscle fivers or of the certain cells that abut muscle fibers.
