RESUMO

Objective:To evaluate the clinical efficacy of acupuncture combined with extracorporeal shock wave therapy in the treatment of piriformis syndrome.Methods:Eighty patients with piriformis syndrome who received treatment in Huaihe Energy Occupational Disease Prevention Hospital Rehabilitation Medicine Center from February 2021 to December 2022 were included in this study. They were divided into a treatment group ( n = 40) and a control group ( n = 40) according to different treatment methods using a retrospective cohort study design. The treatment group was treated with acupuncture combined with extracorporeal shock wave therapy, once every 7 days for 3 consecutive weeks. The control group was treated with extracorporeal shock wave therapy alone once every 7 days for 3 consecutive weeks.Pre- and post-treatment visual analogue scale scores and clinical efficacy were compared between the two groups. Results:There was no significant difference in baseline data between the two groups (all P > 0.05). After treatment, the visual analogue scale score decreased in each group. After adjusting for covariates, there was no significant difference in visual analogue scale score between the two groups ( F = 112.38, P < 0.001). After adjusting the pre-treatment visual analogue scale score, the post-treatment visual analogue scale score in the treatment group was lower than that in the control group [1.417 (95% CI: 1.151-1.683)]. The total response rate in the treatment group was 97.5% (39/40), which was significantly higher than 82.5% (33/40) in the control group ( Z = -2.03, P = 0.042). Conclusion:Acupuncture combined with extracorporeal shock wave therapy is highly effective on piriformis syndrome.

RESUMO

This paper presents a systematic review of extant research investigating the effectiveness of Internet-based group cognitive behavioral therapy (IGCBT) for depression and anxiety symptoms with an emphasis on the treatment modalities, treatment protocols, efficacy and mechanism, together with a summary of research limitations and suggestions for future research. Cognitive behavioral therapy (CBT) is a structured, short-term and present-oriented psychotherapeutic treatment, its Internet- and group-delivered modalities have led to its wider application, and has proven to be clinically effective in alleviating depression and anxiety symptoms caused by different diseases, whereas some drawbacks have been reported. Thus this paper elaborates the clinical application, therapeutic effect and underlying mechanisms of IGCBT intervention for depression and anxiety symptoms, so as to provide references for its future application. Objective:To develop an ex vivo normothermic mechanical perfusion(NMP)and compare the effect of different portal perfusion pressures on attenuating hepatic injury from donor after cardiac death(DCD).Methods:All rat livers were subjected to in situ warm ischemia for 30 min after cardiac attest and thereafter stored for 8 h under cold preservation. Six livers were harvested and regarded as static cold storage(group CS, n=6). In experimental group, liver received an ex vivo dual NMP with oxygenated perfusion via hepatic artery for 2 h after cold storage. Hepatic injury was assessed and compared from perfused livers with full portal vein pressure(group M1, n=6)and low portal vein pressure(group M2, n=6). The evaluation parameters included perfusion flow, liver enzymes of perfusate, pathological changes by hematoxylin-eosin staining, Suzuki histological criteria, expression of activation markers of polymorphonuclear neutrophils and macrophages, myeloperoxidase (MPO)and CD68 by immunohistochemistry, level of malondialdehyde(MDA)and activity of superoxide dismutase(SOD). Results:In experimental group during NMP, perfusion flows tended to increase when portal pressures were stabilized in groups M1 and M2.Perfusion flow during NMP 60~120 min was significantly higher than during NMP 0~20 min.After NMP with full portal pressure, hepatic sinusoidal congestion, hepatocyte necrosis, steatosis and Suzuki criteria were lower in group M1 than those in group CS( P<0.05). Compared with group M1, lower hepatic injury was characterized with a lower change of liver enzymes in perfusate( P<0.05), a better histological evaluation( P<0.05), a lower level of MDA and a higher activity of SOD( P<0.05), lower expressions of CD68 and MPO ( P<0.05)and lower levels of TNF-α and IL-6( P<0.05)in perfused liver. Conclusions:The ex vivo dual NMP with oxygenated perfusion via hepatic artery mimics liver perfusion under the physiological conditions.NMP with a lower portal pressure can attenuate hepatic ischemia-reperfusion injury and confer a better protection against liver damage from DCD.

Objective@#To analyze the mechanism of metformin in improving oxaliplatin resistance of colorectal cancer cells.@*Methods@#The expression levels of EIF3G in 71 cases of colorectal cancer and adjacent tissues were detected by RT-qPCR. The survival curve was established by Kaplan-Meier method. The survival rate difference between the two groups was compared by Log-rank test.HCT-116 cells were infected with shEIF3G lentivirus, HCT-116 L-OHP-resistant cell line was constructed.After treated with metformin, sensitivity to L-OHP was detected by CCK-8 assay, RT-qPCR and Western blot were used to analyze the EIF3G mRNA and protein levels. The subcutaneous tumor model of nude mice was constructed to observe the effect of metformin on tumor growth.@*Results@#The expression level of EIF3G mRNA in colorectal cancer tissues was up-regulated by (3.24±0.43) times compared with paraneoplastic tissues (P<0.01). EIF3G level was significantly correlated with the prognosis of colon cancer patients. Knockdown EIF3G could enhance the L-OHP sensitivity of HCT116/L-OHP cells (P<0.05); Metformin can reduce the expression level of EIF3G and improve the drug resistance of HCT116/L-OHP cells (P<0.05), and inhibit the development of colorectal cancer in vivo (P<0.05).@*Conclusion@#Metformin can inhibit the expression of EIF3G and improve the L-OHP resistance of HCT-116 cells.

Objective: To investigate the regulation effects of fibrous-actin (F-actin) on senescent of the human bone marrow mesenchymal stem cells (hBMSCs), and to elucidate the molecular mechanisms of senescence the of hBMSCs. Methods: The hBMSCs were cultured in vitro and divided into control group (P2 hBMSCs), F-actin inhibitor group (P2 hBMSCs were treated with 2. 5 μmol · L-1 F-actin inhibitor Latrunculin B for 1 h) and Pll hBMSCs group (Pll hBMSCs passaged from P2 hBMSCs continuously). The hBMSCs in various groups were induced by osteogenic, adipogenic and chondrogenic induction mediums and the induction effects were identified by Alizarin red staining, SO staining and oil red O staining. The number of Ki67 positive cells, polymerization and morphology of F-actin and YAP subcellular localization were detected by immunofluorescence staining. SA-β-Gal staining was uesd to detect the SA-β-Gal staining positive cells in the hBMSCs in various groups. Results: The Alizarin red staining, SO staining and oil red O staining results showed that the hBMSCs in various groups had the osteogenic, adipogenic and chondrogenic abilities. The immunofluorescence staining and SA-β-Gal staining results showed that the microfilament bundles in the hBMSCs in control group were more thick, the F-actin length was longer, and the YAP mainly concentrated in the nucleus (YAP was activated in the nucleus). Compared with control group, the number of Ki67 positive cells in Pll hBMSCs group was less and the number of SA-β-Gal positive cells was more; the Factin was shorter and thinner, the YAP mainly concentrated in the cytoplasm, and the high cytoplasmic YAP cells was positive for SA-β-Gal staining; in F-actin inhibitor group, the high cytoplasmic YAP inactivation was found in the hBMSCs and the SA-β-Gal staining positive hBMSCs had more senescent cells. Conclusion: To inhibit the entering of YAP into the nucleus can promote the senescence of hBMSCs and F-actin can affect the senescence of hBMSCs by regulating the YAP activity.

OBJECTIVE: The aim of this study was to investigate ziprasidone plasma concentration, daily dose and clinical efficacy and safety in Han Chinese and Mongolian patients with first-episode schizophrenia. METHODS: A total of 123 inpatients affected by schizophrenia were recruited from the Mental Health Center of Inner Mongolia in China. Ziprasidone plasma concentration, clinical efficacy and side effects were systematically evaluated at baseline, and at 1, 2, 4, and 6 weeks. Metabolic measures such as changes in weight, body mass index (BMI), fasting blood glucose (FBG), triglycerides, and cholesterol, were also recorded. RESULTS: 90 patients completed the study. Compared with Han patients, on average, Mongolian patients received a significantly higher ziprasidone dosage for adequate symptom control during the 6-week period and had a lower plasma concentration-to-dose ratio. The Mongolian patients also experienced greater increases in weight and BMI. No significant differences between the two ethnic groups were found in the rate of reduction in the Positive and Negative Syndrome Scale (PANSS) score, Treatment Emergent Symptom Scale (TESS) total score, FBG, triglycerides, cholesterol or Q-Tc interval. CONCLUSION: Compared to Han Chinese patients, Mongolian patients appeared to have increased ziprasidone clearance and require higher doses to achieve effective treatment for schizophrenia.

RESUMO

Cleidocranial dysplasia (CCD) is a rare congenital disorder, typically characterized by persistently open skull sutures, aplastic or hypoplastic clavicles, and supernumerary teeth. Mutations in the gene encoding the runt-related transcription factor 2 (RUNX2) protein are responsible for approximately two thirds of CCD patients. We report a 20-year-old CCD patient presenting not only with typical skeletal changes, but also complex dental anomalies. A previously undiagnosed odontoma, 14 supernumerary teeth, a cystic lesion, and previously unreported fused primary teeth were discovered on cone-beam computed tomography (CBCT) scans. Mutation analysis identified the causal c.578G>A (p.R193Q) mutation in the RUNX2 gene. At 20 years of age, the patient had already missed the optimal period for dental intervention. This report describes the complex dental anomalies in a belatedly diagnosed CCD patient, and emphasizes the significance of CBCT assessment for the detection of dental anomalies and the importance of early treatment to achieve good outcomes.

<p><b>OBJECTIVE</b>To investigate the effects of all-trans retinoic acid (atRA) on the function of bone morphogenetic protein receptor 2 (BMPR2) expression in embryonic palate.</p><p><b>METHODS</b>Cleft palate mice model was established by atRA. On gestation day (GD) 15 and GD 17, the pregnant mice were killed to obtain the embryos from the uteri. The embryonic palates were stained with hematoxylin-eosin, and the remaining sections were used for the immunohistochemistry of BMPR2 detection. Reverse transcription-polymerase chain reaction was performed to detect the expression levels of Bmpr2 mRNA.</p><p><b>RESULTS</b>In the atRA-treated group, short extensions and failure to fuse with each other were observed. The positive expression of BMPR2 was detected in developing palatal process from GD 15 to GD 17 in the control group. Compared with those of the control group, BMPR2 protein and Bmpr2 mRNA decreased in the atRA-treated group (P<0.05).</p><p><b>CONCLUSION</b>The treatment of pregnant mice with retinoic acid produces small palatal shelves in their fetuses and down-regulates BMPR2 expressions.</p>

Objective To explore the changes of PH values of N,O-CMC/β-TCP compositive materials with different mass fractions in simulated body fluids (SBF)and their influence in the growth of MG63 cells, and to illustrate their mechanisms, and to provide reference for the further research on the bone repair materials. Methods The N,O-CMC/β-TCP with mass fractions of 2/1,1/1 and 1/2 were used as experimental groups,and the collagen nano calcium phosphate bone repair material as control group. The materials with different mass fractions were immersed in SBF and the pH values were measured by pH meter after soaking for 7,14,21 and 28d,respectively.The MG63 cells with the concentration of 1 × 105 mL-1 were inoculated and co-cultured in experimental and control groups,the adhesion and morphological changes of MG63 cells in each group were observed by scanning electron microscope and the cell proliferation was detected by MTT method after co-culturing for 2,4 and 6 d.Results The pH values were 6.70-7.25 in N,O-CMC/β-TCP (1/2)group and N,O-CMC/β-TCP (2/1)groups and the pH value in N,O-CMC/β-TCP (1/1)group was basically 7.15. The cells in N,O-CMC/β-TCP (2/1)group formed owe full,spreading face small and less secretion,but the cells in N,O-CMC/β-TCP 1/2 and 1/1 groups formed in full, pseudopodia interconnection, widely spreading and more secretions under electron microscope. The proliferation rate of the cells in N,O-CMC/β-TCP with (1/1 ) and N,O-CMC/β-TCP (2/1)groups had no statistical difference compared with control group (P>0.05),but there was significant difference between control group and N,O-CMC/β-TCP (1/2)group (P<0.05).Conclusion The changes of pH values of N,O-CMC/β-TCP materials with different mass fractions in SBF are small and the pH values are neutral;the order of the mass fraction of N,O-CMC/β-TCP to promote the growth of MG63 cells is 1/1,2/1,and 1/2.

OBJECTIVE: To evaluate the short-term effect of fangchinoline, an antiinflammatory drug widely used in Asia, on root resorption that is associated with orthodontic tooth movement. METHODS: Twenty-four Wistar rats were randomly divided into 6 groups. Mesial forces of 0, 50, or 100 g were applied to the maxillary first molar of the rats in each group for 14 days by activating nickel-titanium closed-coil springs. One-half of the rats receiving each of these treatments also received injections of 200 microL fangchinoline every 2 days. Finally, movement of the maxillary first molars was measured using digitized radiographs. The molars were extracted and the surfaces of the root resorption craters were recorded using a scanning electron microscope. The distance the molars moved and resorptionarea ratio was measured, and results were analyzed using 2-way ANOVA tests. RESULTS: There were no statistical differences in the distances the first molars moved under 50 or 100 g force, regardless of treatment with fangchinoline. However, the resorption area ratios were significantly smaller in those rats that were treated with both tension and fangchinoline than in those rats treated by tension alone. CONCLUSIONS: Fangchinoline reduced the resorption area ratio in rats and is therefore an important means of alleviating root resorption.

Based on the latest development of the education of liberal arts in Lingnan,Harvard,Tokyo,through the analysis of the relationship among knowledge,subjects and system and the role of college curriculum reform,the author thinks that in the new era the change of the knowledge development has brought about the corresponding changes in the subjects,giving rise to the new changes in the organization of the system of colleges and departments of the universities as well as the changes in the education of liberal arts itself.The new goals and tasks of the educational reform of universities are to integrate knowledge,subjects and system by implementing the education of liberal arts.
