@#Introduction: During the early phase of Coronavirus disease (COVID-19), there were various uncertainties, which had a detrimental impact on the prevalence of burnout among critical care personnel worldwide. This study aims to investigate the prevalence of burnout and its associated factors in critical care personnel involved in the COVID-19 pandemic. Methods: This is a single-center, cross-sectional study with 81 critical care personnel for a survey using Copenhagen Burnout Inventory. Binary logistic regression analysis was conducted to identify factors associated with personal burnout. Results: More than half of the respondents were female (60.5%) over the age of 30 (61.7%), and 54.3% were medical doctors. A large number (72.8%) of the respondents experienced personal burnout, with twothirds of them experiencing work-related (65.4%) and client-related burnout (59.3%). Personal burnout was found to be associated with those who had children [OR: 11.31 (1.90, 67.37), p = 0.008], stayed with family, relatives, or friends [OR: 9.40 (1.27, 69.46), p = 0.028], were medical doctors [OR: 26.52 (2.79, 252.22), p = 0.004], worked more than 45 hours per week [OR: 8.68 (1.45, 58.09), p = 0.018], and previously never had COVID-19 viral test [OR: 6.93 (1.17, 40.89), p = 0.033]. Conclusion: Overall, more than half of the critical care personnel experienced burnout. There were possible associations between personal burnout with social characteristics such as having children and living with family, relatives, or friends, and occupational characteristics such as being a medical doctor, long working hours, and previously never had COVID-19 viral test.

【Objective】 To understand the epidemiological characteristics of COVID-19 in Shaanxi Province from December 9, 2021 to January 20, 2022, and analyze the factors influencing the interval from isolation to diagnosis. 【Methods】 We collected the data of local COVID-19 cases from December 9, 2021 to January 20, 2022 published on the official website of Health Commission of Shaanxi Province. Descriptive statistical method was used to analyze the epidemiological characteristics of COVID-19 in Shaanxi Province. Mann-Whitney U test and Kruskal-Wallis H test were used to compare the differences between groups. The unconditional Logistic regression model was applied to analyze the factors influencing the interval between isolation and diagnosis. 【Results】 The outbreak of COVID-19 in Shaanxi Province started on December 9, 2021 and ended on January 20, 2022. The overall change trend of the outbreak showed an "inverted V" shape. A total of 2,080 confirmed local cases were reported, and the main type of disease was mild, with an incidence rate of 5.26/100,000. Xi’an had the most cases, accounting for 98.69% of the total. The reported cases were mainly concentrated in people aged 21 to 55 years old, with a male-to-female sex ratio of 1.19∶1. The median interval from isolation to diagnosis was 3 days, the shortest interval being 0 day and the longest interval being 21 days. Unconditional Logistic regression model analysis showed that the way of finding cases was the factor influencing the interval from isolation to diagnosis. Compared with the way of isolation of the key population, the way of the nucleic acid screening could reduce the risk of late detection of confirmed cases by 89% (OR=0.11, 95% CI: 0.07-0.16). 【Conclusion】 The way of finding cases is the factor influencing the interval from isolation to diagnosis. In the face of the recent intensification of the spread of Omicron variant in mainland China, accurate and rapid identification and detection of confirmed cases can not only reduce the risk of the spread of the epidemic, but also endeavor more time and initiative for the treatment of patients, which is the key to curbing the spread of the epidemic.

Objective:To compare the postoperative efficacy and complications of frontalis suspension and levator resection in the treatment of blepharoptosis.Methods:According to the Cochrane systematic review method, we searched PubMed, Medline, Cochrane Library, Wanfang database, VIP Database, and China Knowledge Network database. The randomized and non-randomized controlled trials for comparing frontalis suspension and levator resection in the treatment of blepharoptosis were included. Revman 5.3 Meta analysis software was used.Results:A total of 13 clinical trials were involved in this study, including 1308 patients. Frontalis suspension was performed in the experimental group and levator resection was performed in the control group. The OR value of postoperative efficacy between the 2 groups was 2.91, 95% CI (1.57-5.39), P<0.01, the difference was statistically significant. Postoperative lagophthalmos after frontalis suspension was better [ MD=-1.05, 95% CI (-1.43--0.68), P<0.01], the difference was statistically significant. The OR value of undercorrection between the 2 groups was 0.24, 95% CI(0.16-0.36), P<0.01, the difference was statistically significant. Conclusions:The postoperative efficacy after frontalis suspension is higher. Postoperative complications after frontalis suspension are less.

Objective:To explore the application effect of "secret room escape" game teaching combined with situational cases on the teaching of standardized training of neurological nursing.Methods:A total of 102 nurses who underwent the standardized training in the Department of Neurology of Xinjiang Uygur Autonomous Region People's Hospital from January 2019 to September 2020 were divided into control group ( n=51) and observation group ( n=51). The control group used situational case teaching, and the observation group used "secret room escape" game teaching combined with situational case teaching. The scores of critical thinking and learning engagement of the two groups were evaluated, and the theoretical assessment scores, clinical comprehensive ability scores and teaching satisfaction of the two groups were compared. SPSS 22.0 was performed for chi-square test and t test. Results:Before teaching, there was no significant difference in the scores and total scores of truth seeking, mind opening, analytical ability, systematic ability, self-confidence of critical thinking, thirst for knowledge and cognitive maturity between the two groups. After teaching, the scores and total scores of the observation group in above aspects were higher than those of the control group ( P<0.05). There was no significant difference in the scores and total scores of behavioral engagement, cognitive engagement and emotional engagement between the two groups before teaching; after teaching, the scores and total scores of these aspects in the observation group were higher than those in the control group ( P<0.05). The scores of theoretical examination [(82.02±3.27) vs. (79.66±3.45)] and clinical comprehensive ability [(83.30±3.54) vs. (80.82±3.73)] in the observation group were higher than those in the control group. The teaching satisfaction of the observation group was 98.04% (50/51), higher than 82.35% (42/51) of the control group. Conclusion:The "secret room escape" game teaching combined with situational cases can improve the critical thinking ability of standardized training nurses, stimulate them to participate in learning, improve their theoretical examination results and clinical comprehensive ability, and improve their satisfaction with teaching and the recognition rate of teaching effect.

Objective To analyze the relationship between follow-up service, personal characteristics, lifestyle and blood sugar management of diabetic patients in Gansu Province, and put forward scientific suggestions on influencing blood sugar management of diabetic patients. Methods Based on the data of the 6th National Health Service Questionnaire, 273 subjects were included. Chi-square test and unconditional logistic regression were used to analyze the blood glucose control and its influencing factors of diabetic patients. Results A total of 39.56% patients' blood sugar status was well controlled. 76.92% patients signed up for family doctor service, and 77.66% patients took hypoglycemic drugs according to the law of doctor's advice. There were significant differences in blood sugar control among patients in different regions, educational levels and occupational types (P <0.05) , logistic regression analysis showed that Hui patients (OR=0.21), doctors without family contract (OR=2.86) and patients taking hypoglycemic drugs intermittently (OR = 6.58). Conclusion The blood sugar control rate of diabetic patients in Gansu is low, and the nationality, contracted family doctors and medication plan affect the blood sugar control level of patients. In order to ensure the high efficiency of follow-up treatment, the related follow-up services provided by primary medical institutions, it is necessary to provide self-management programs that meet the individual characteristics and meet the needs of the disease.

OBJECTIVE To sum marize the pro cedure and informatization construction of centralized volume-based procurement(VBP)in our hospital ,in order to give references for normal development of centralized VBP. METHODS The standardized workflow system was established ,including using flow chart method to establish standardized workflow ,carrying out procedure training and inspection of procedure implementation , and continuously conducting procedure optimization. The information system was developed for the task links that needed a lot of calculation to improve the automation level of information processing. RESULTS & CONCLUSIONS Eight specific work procedures were established in our hospital ,including the work procedures of submitting the demand data of centralized VBP and the implementation of centralized VBP ,and has taken measures such as procedure training ,establishing supervision and inspection system and using auxiliary means to promote the implementation of the procedure ,so as to optimize the procedure and work form. An informatization platform for the clinical task allocation of the agreed purchase quantity of centralized VBP and a supervision platform for the daily use of VBP were also established in our hospital,then the two tasks with a large amount of calculation could be finished. Standardized workflow system and informatization platform construction has improved the operation and supervision efficiency of centralized VBP in our hospital ,ensured the completion of centralized purchase tasks and saved human resources ,which has a certain promotion value.

@#The number of patients requiring intensive care has surged since the outbreak of the SARS-CoV-2 virus. This had rendered the intensive care unit (ICU) a huge challenge not only to provide care for the existing patients but also to support the COVID-19 patients. The ICU was restructured to ensure strict adherence to the infection control guidelines. The aspects of change in the ICU had been ranging from the clinical operation, medication equipment and facilities, medications supply, and staffing. Strategies required upon implementation of change include having contingency plans, being innovative, getting the collaboration from other ICUs, exchanging information, getting support from the health policymakers, and ensuring the safety of the healthcare workers. This article aimed to share the experience of challenges and strategies in managing an ICU for the COVID-19 pandemic in Malaysia.

OBJECTIVE Right ventricular (RV) remodeling is one of the essential pathological features in pulmonary arterial hypertension (PAH). RV hypertrophy or fibrosis are the leading causes of RV remodeling. Magnolol is a com?pound isolated from Magnolia officinalis. It possesses multiple pharmacological activities, such as anti-oxidation and anti-inflammation. This study aims to evaluate the effects and underlying mechanisms of magnolol on RV remodeling in hypoxia-induced PAH. METHODS ① Male SD rats (220 g) were randomly divided into 5 groups (n=10): the normoxia group, the hypoxia group, the hypoxia plus Magnolol (10 and 20 mg·kg-1·d-1) group, and the vehicle group. Rats in the normoxia group were kept in a normoxia environment for 4 weeks, while rats in the hypoxia group were kept in a hypoxic chamber (10% O2). The rats in the hypoxia plus magnolol groups were administered with magnolol at 10 or 20 mg·kg-1 (ip) once a day for 4 weeks. At the end of 4 weeks, the heart function was assessed by Doppler echocardiography, and then the rats were anesthetized with sodium pentobarbital (30 mg·kg-1, ip). The RVSP was measured by the right heart catheterization method. The heart tissues were collected and dissected to calculate the index of RV remodeling (RV/LV+IVS, RV/tibial length, or RV/body weight). Part of the RV samples was fixed with 4%paraformaldehyde for morphological analysis, while other samples were frozen at-80℃for molecular studies (measurements of ANP, BNP,α-SMA, and col?lagen Ⅰ/Ⅲ mRNA expression as well as p-JAK2/JAK2 and p-STAT3/STAT3 protein levels). ② To evaluate the effect of magnolol on hypoxia-induced myocardial hypertrophy and fibrosis, H9c2 or cardiac fibroblasts were divided into 7 groups: the control group, cells were cultured under normal conditions; the hypoxia group, cells were cultured under hypoxic condition (3% O2);the hypoxia plus magnolol 10 mg·kg-1 group, magnolol10μmol·L-1 was added to the culture medium before the hypoxia treatment;the hypoxia plus magnolol 30 mg·kg-1 group, magnolol 20μmol·L-1 was added to the culture medium before the hypoxia treatment;the hypoxia plus TG-101348 group, TG-101348 (a specific inhibitor of JAK2) 1μmol·L-1 was added to the culture medium before the hypoxia treatment;the hypoxia plus JSI-124 group, JSI-124 (a specific inhibitor of JAK2) 1μmol·L-1 was added to the culture medium before the hypoxia treatment;and the hypoxia plus vehicle group, an equal volume of vehicle (DMSO) was added to the culture medium before the hypoxia treatment. At the end of the experiments, the cells were collected for morphological and molecular analysis. RESULTS In vivo, male Sprang-Daley rats were exposed to 10% O2 for 4 weeks to establish an RV remodeling model, which showed hypertrophic and fibrotic features (increases of RV remodeling index, cellular size, hypertrophic and fibrotic marker expression), accompanied by an elevation in phosphorylation levels of JAK2 and STAT3;these changes were attenuated by treating rats with magnolol. In vitro, the cultured H9c2 cells or cardiac fibroblasts were exposed to 3% O2 for 48 h to induce hypertrophy or fibrosis, which showed hypertrophic (increases in cellular size as well as the expression of ANP and BNP) or fibrotic features (increases in the expression of collagenⅠ, collagenⅢandα-SMA). Administration of mag?nolol and TG-101348 or JSI-124 (JAK2 selective inhibitors) could prevent the process of myocardial hypertrophy and fibrosis, accompanied by the decrease in the phosphorylation level of JAK2 and STAT3. CONCLUSION Magnolol can attenuate RV hypertrophy and fibrosis in hypoxia-induced PAH rats through a mechanism involving inhibition of the JAK2/STAT3 signaling pathway.

OBJECTIVE:To optimize the formul ation of Polygala japonica cream,and to evaluate the quality of prepared cream. METHODS :With centrifugal stability ,heat resistance stability and viscosity as evaluation indexes ,the weight coefficient was determined by analytic hierarchy process (AHP),criteria importance through intercriteria correlation (CRITIC)method and mixed weighted AHP-CRITIC ,and the comprehensive score was calculated. The amount of octadecyl alcohol ,hexadecanol and mixed emulsifier (polysorbate 60 mixed with glycerin monostearate at the mass ratio of 2.82)in the formulation of P. japonica cream were screened by central composite design-response surface methodology. The optimized formulation was validated. P. japonica cream prepared by the optimal preparation was evaluated in terms of apperance ,particle,pH,stability and rheological characteristics. RESULTS :The weight coefficients of centrifugal stability ,heat resistance stability and viscosity were 0.428 5, 0.415 6 and 0.155 9 respectively,according to the mixed weighted AHP-CRITIC. The optimal formulations were 1.96 g of hexadecyl alcohol ,5.17 g of octadecyl alcohol ,2.48 g of mixed emulsifier ,1.83 g of polysorbate 60,0.65 g of glyceride monostearate,1 g of benzyl alcohol ,5 g of propylene glycol ,6 g of isopropyl myristate and 5 g of P. japonica extract,and then added water to 100 g. Prepared cream was a light yellow fluid paste with particle size of 0.5-2.5 μm and pH value of 6.5;the results of centrifugal test ,heat resistance test and cold resistance stability test showed that the cream had no oil-water separation or thickened paste. The prepared cream was shear-thinning non-Newtonian fluid. CONCLUSIONS :P. japonica cream is prepared successfully,which shows good apperance ,particle,acidity and stability.

Objective To investigate the expression and significance of bone morphogenetic protein?2 (BMP?2) and phosphatase and tensin homolog deleted on chromosome ten (PTEN) in incipient,recurrent and malignant salivary gland pleomorphic adenoma??Methods A total of 93 cases of salivary gland pleomorphic adenoma in Kailuan general hospital from 2008 to 2017 were collected,including 54 in incipient group (group A,47 cases in benign group A1,7 cases in malignant group A2),39 cases in recurrent group (group B,26 cases in benign group B1,13 cases in malignant group B2)??The expression of BMP?2 and PTEN were detected by immunohistochemical detection and western blot,the correlation of BMP?2 and PTEN was analyzed by Spearman analysis??Results The immunohistochemical and western blot analysis both showed expression of BMP?2 in recurrent group was significantly higher than that in incipient cases((129??03 ±15??52) vs??(87??88±18??11),t=-8??094,P=0??000),and it was lower in malignant cases than that in benign cases(( 100??24 ± 25??07) vs ( 116??66 ± 26??19), t=2??125, P=0??040)??There was no significant difference in PTEN expression between incipient and recurrent groups (( 89??15 ± 13??92 ) vs??( 96??19 ±28??02),t=1??055,P=0??279),but lower PTEN expression was found in malignant cases than benign cases ((76??06±11??16) vs??(109??28±17??05),t=7??543,P=0??000)??BMP?2 was positively correlated with PTEN expression (r=0??313,P<0??05)??Conclusion BMP?2 is associated with the recurrence of salivary gland pleomorphic adenoma, and both BMP?2 and PTEN are associated with malignant in the salivary gland pleomorphic adenoma??

Objective: To explore effect and mechanism of Jiaotaiwan (JTW) on cognitive impairment in diabetic mice. Method: The 40 db/db mice of spontaneous diabetes were randomly divided into model group, JTW 1 group (1.68 g·kg-1), JTW 2 group (3.36 g·kg-1), and JTW 3 group (8.40 g·kg-1), with 8 mice in each group, and 8 C57BL/6J mice were included into normal group. After 8 weeks of treatment, behavioral test,fasting blood glucose (FBG), fasting insulin (Fins), insulin resistance index (HOMA-IR), total triglyceride (TG), total cholesterol (TC), free fatty acids (FFA), high density lipoprotein (HDL), and low density lipoprotein (LDL) were detected. Western blot was used to detect protein expressions of pSer199, pSer202, pSer214, p-Akt, glycogen synthase kinase-3β(GSK-3β), phospho-GSK-3β(p-GSK-3β). Result: Compared with model group, FBG, Fins, HOMA-IR, TG, TC, HDL, FFA in JTW group decreased significantly (P0.01), with a significant improvement in cognitive function (P0.01), protein expressions of taupSer199, pSer202, pSer214 and GSK-3β in hippocampus of mice decreased significantly (PP0.01), and expressions of p-Akt and p-GSK3 beta significantly increased (P0.01). Conclusion: JTW 3 (Coptidis Rhizoma-Cinnamomi Cortex 10:1) can alleviate insulin resistance and cognitive impairment. The mechanism may be related to the inhibition of over-phosphorylation of tau protein, the down-regulation of GSK-3β protein expression and the up-regulation of p-Akt and p-GSK-3β expressions.

Objective To analyze the death cases of the oncology inpatients of the First Hospital of Hunan University of Chinese Medicine (hereinafter referred to as "this hospital") during 2007-2016; To provide the basis for the prevention and treatment of cancer, prognosis improvement and rational allocation of TCM health resources. Methods The death cases of the oncology inpatients from 2007 to 2016 were analyzed by retrospective medical history method from the aspects of causes of death, hospitalization costs and survival time. Results There were 830 death cases of the oncology inpatients in the hospital during 2007-2016, with 548 males and 282 females, and the mortality rate decreased year by year. The average hospitalization costs for patients was 39 630.86 yuan, of which TCM treatment cost 6206.98 yuan (15.66%). Patients with colon cancer had the highest proportion of TCM treatment costs (20.10%), followed by lung cancer, breast cancer, and liver cancer. The top five causes of death were lung cancer, liver cancer, breast cancer, colon cancer, and rectal cancer. The sex ratio of patients who died of lung cancer was 1.61:1. The average survival time of male patients was 446.3 days, which was far lower than the 738.3 days of female patients. The cancers with long-term survival periods were cervical cancer, breast cancer, and rectal cancer. The cancers with shorter survival periods were pancreatic cancer, liver cancer, and lung cancer. Conclusion The structure of death causes of the oncology inpatients in the hospital has a certain degree of representation. The prevention and control of lung cancer, especially male lung cancer should be strengthened; TCM health resources and prevention should be utilized rationally; the prognosis of patients with liver cancer should be improved, to improve the overall survival and quality of life of patients with breast cancer or cervical cancer, which is important for the prevention and treatment of malignant tumor.

This study was carried out by the methods of typical plots investigation and laboratory test aiming at analyzing the survival state and fruiting characteristics of three Sinopodophyllum hexandrum populations in different environmental habitats. Meanwhile, it could provide scientific basis for enhancing wild population quantity recovery. The results showed that more population quantity grow in the habitats of large-area gap (Population A) and bushes (Population C) with a majority of the young individuals, while the minor-area gap (Population B) was the opposite. The development tendency of S. hexandrum populations would be the recession in the future. Spatial distribution pattern of populations was clumped at small scales but random at large scales. The indexes of population A and C, as fruit size,the quantity and quality of seeds,germ inability,were all superior to those of population B. Comparing the mainly environment factors of three populations, that favorable environmental factors for vegetative and reproductive growth of S. hexandrum populations were found,especially certain lighting intensity and fertile soil. Therefore, the favorable environmental habitats for S. hexandrum individuals growth could be artificial to promote the recovery and quantities of S. hexandrum populations in the future.

AIM To study the programmed death effect of trametenolic acid B (TAB) against human gastric cancer HGC-27 cells.METHODS The proliferation of cells was examined by MTT assay;cell apoptosis rate and cell cycle were detected by flow cytometry;cell autophagy was observed by acridine orange staining and transmission electron microscope;expressions of autophagy-related proteins were detected by Western blot.RESULTS 10,20,40 μmol/L Trametenolic acid B had good growth-inhibitory effects on HGC-27 cells,blocked in G0/G1 phase,in a dose-dependent and time-dependent manner,but had no obvious effect on cell apoptosis.After treatment with 20 μmol/L trametenolic acid B for 24 h,a large number of autophagy vacuoles and autophagy bodies were observed under transmission electron microscope,and the proportion of cell autophagy was significantly increased with higher dose of TAB;TAB promoted the transformation of type Ⅰ microtubule-associated protein 1 light chain 3 (LC3 Ⅰ) to type Ⅱ microtubule-associated protein 1 light chain 3 (LC3 Ⅱ),up-regulated the expressions of myosin-like B-cell lymphoma-2 (Bcl-2) interacting protein (Beclin-1) and cyclin-dependent kinase inhibitor 1A (p21),and inhibited the CCND1 gene protein (CyclinD1) expression.CONCLUSION Trametenolic acid B can induce the autophagic death of HGC-27 cells,but it has no significant effect on cell apoptosis.

Objective To explore the anti-inflammatory action of Heshi Yangshen Recipe on rat model of nephrotic syndrome induced by adriamycin. Methods Rat model of nephrotic syndrome was established by tail venous injection of one-dose of adriamycin. SD rats were divided into normal group(N = 10),model group(N =11), positive control group (N = 12), and high-, middle- , low-dose Chinese medicine groups (N = 12, respectively). Two weeks after modeling,rats in the normal group and model group were given gastric gavage of same volume of normal saline, rats in positive control group were given gastric gavage of benazepril aqueous solution at a dose of 10 mg·kg-1·d-1 in the former period of 15 days and at 10 mg·kg-1·d-1 in the later period of 20 days, and rats in high-,middle- ,low-dose groups were given gastric gavage of Heshi Yangshen Recipe at a dosage of 33.26, 16.63, 8.32 g·kg-1·d-1 respectively, for 30 continuous groups. During the treatment period, the general situation of rats was observed. After the completion of medication, the levels of 24-hour urinary protein, urine total proteins, urine creatinine were measured by biochemical analyzer, serum high-sensitivity C-reactive protein (Hs-CRP) level was detected by enzyme-linked immunosorbent assay(ELISA),and the expression levels of intercellular adhesion molecule 1 (ICAM-1)and vascular cell adhesion molecule-1 (VCAM-1)in rat renal tissue of various groups were determined by immunohistochemistry. Results The increase of body mass in all of the modeled rats was significantly slowed down (P < 0.01 as compared with the normal group). Compared with the normal group, the levels of 24-hour urinary protein, serum hs-CRP, and the expression of ICAM-1 and VCAM-1 in rat renal tissue were increased in the model group (P < 0.05 or P <0.01). Compared with the model group, the level of 24-hour urinary protein in the positive control group and high-dose Chinese medicine group was decreased (P < 0.05), and the levels of serum Hs-CRP, and the expression of ICAM-1 and VCAM-1 in rat renal tissue of the positive control group and high-,middle-,low-dose Chinese medicine groups were decreased (P < 0.05 or P < 0.01). Conclusion Heshi Yangshen Recipe exerts certain anti-inflammatory action, and its mechanism may be related with the decrease of ICAM-1 and VCAM-1 expression. High dosage of Heshi Yangshen Recipe has the strongest therapeutic effect.

Objective To establish the animal model of traumatic myositis ossificans through striking the quadriceps muscle of rabbits repeatedly.Methods Nine white adult New Zealand rabbits were selected,as their left lower limbs were used as the model group and the right lower ones served as controls.The left hind limbs were stricken by a 0.25 kg ball falling from a height of 100 cm every 3 days for and then immobilized with the knee in extension,while the right knees were immobilized in the same way without striking.The rabbits were sacrificed at the 4th,6th and 8th weeks respectively.The swelling,local physical signs and pathological changes of the heterotopic ossification had been assessed.Results The swelling of left quadriceps was obvious,with progressively stiffness of the left knees accompanied by distinctly palpable indurations,while in the right hind limbs,the joint was stiff but without indurations.The imaging examination showed the ossification began to appear in the impact sites of the model group at the 4th,6th and 8th weeks.The hematoxylin-eosin staining demonstrated that there was obvious cartilage or bone formation in the muscle tissues of the left quadriceps at the 8th weeks.Conclusion An animal model of post-traumatic myositis ossificans can be successfully established in rabbits through beating their quadriceps repeatedly.

Objective To investigate the effects of enteral nutrition via the nasojejunal and nasogastric feeding on stroke-associated pneumonia (SAP) and other complications in patients with severe acute stroke.Methods A total of 60 patients with severe acute stroke who needed tube feeding were randomized to either a nasojejunal tube group or a nasaogastric tube group for enteral nutrition support.The occurrence of SAP,diarrhea,vomiting,and gastrointestinal bleeding within 2 weeks were observed.The nutrition indices (total protein,prealbumin,and hemoglobin) before and at day 7 and 14 after the tube placement were documented and compared.Results There were 30 patients in the nasojejunal tube group and the nasaogastric rube group,respectively.There were no significant difference in demographic and baseline clinical data between the 2 groups.The incidence of SAP (43.3% vs.70.0%;x2 =4.340,P=0.037),vomiting (13.3% vs.43.3%;x2 =6.648,P =0.010),and hyponatremia (6.7% vs.40.0%;x2 =4.022,P =0.045) in the nasojejunal rube group were significant lower than those in the nasogastric rube group,however,there was no significant differences in the incidences of diarrhea and gastrointestinal bleeding.Compared with before indwelling tube,there were no significant difference in the concentrations of total plasma protein and prealbumin at day 14 after indwelling tube in the nasojejunal tube group,but the concentration of hemoglobin were decreased significantly (P =0.001);compared with before indwelling tube,the concentrations of the total plasma protein (P =0.001),prealbumin (P =0.036),and hemoglobin (P =0.001) at day 14 after indwelling tube in the nasogastric rube group were significantly decreased.Conclusions Nasojejunal nutrition may effectively prevent SAP,volmiting,and hyponatremia in patients with severe acute stroke,and contributes to maintain the nutritional status.

Objective To investigate the effects of an ar?turmerone derivative(ATD)on the proliferation and apoptosis of A375 human melanoma cells. Methods Both A375 cells and human skin fibroblasts (HSFs) were cultured with different concentrations(5, 10, 20, 40 and 80μmol/L)of ATD, vincristine and ar?turmerone, separately, for 48 hours in vitro. Subsequently, cell counting kit?8 (CCK?8) was used to evaluate cell proliferation, inverted microscopy to observe cell morphology after acridine orange/ethidium bromide (AO/EB) staining, and a colorimetric method to estimate caspase?3 activity. DNA fragmentation assay and flow cytometry were performed to assess cell apoptosis, and flow cytometry was conducted to analyze cell cycle. Results ATD, vincristine and Ar?turmerone all inhibited the proliferation of A375 cells in a dose?dependent manner(ATD:R2=0.99, F=340.96, P<0.05;vincristine:R2=0.99, F=349.19, P<0.05;ar?turmerone:R2=0.89, F=25.41, P<0.05). The fifty percent inhibitory concentra?tions(IC50s)of ATD, vincristine and ar?turmerone against A375 cells were 15.96 ± 0.02μmol/L, 77.00 ± 0.04μmol/L and 356.95 ± 0.01μmol/L respectively. When the drug concentrations were 5 and 10μmol/L, the proliferation of HSFs was inhibited by 8%± 0.06%and 25%± 0.02%respectively by ATD, by 49%± 0.09%and 34%± 0.07%respectively by ar?turmerone, and by 33%± 0.04%and 29%± 0.08%respectively by vincristine, and the proliferation of A375 cells was inhibited by 26%± 0.06%and 39%± 0.02%respectively by ATD, by 6%± 0.09%and 10%± 0.07%respectively by ar?turmerone, and by 8% ± 0.04% and 17% ± 0.08% respectively by vincristine, with the inhibitory effects of the three drugs being significantly different from that of dimethyl sulfoxide(all P<0.05). ATD showed stronger inhibitory effects on the proliferation of A375 cells, but weaker cytotoxic effects on HSFs compared with ar?turmerone and vincristine(all P<0.05). Meanwhile, ATD, vincristine and ar?turmerone all induced the apoptosis of A375 cells(P<0.05), and caspase?3 activity increased with the increase in drug concentrations(ATD:R2=0.98, F=162.30, P<0.05;vincristine:R2=0.96, F=94.39, P<0.05;ar?turmerone:R2=0.95, F=57.35, P<0.05). The effect of ATD on caspase?3 activity was strongest, followed by that of vincristine and ar?turmerone. As flow cytometry showed, all the three drugs induced cell apoptosis to different degrees, and ATD showed a relatively strong effect on cell apoptosis, especially late apoptosis, compared with the other two drugs. In the ATD group, the number of A375 cells in G1 phase gradually increased, while that in G2 phase and S phase significantly decreased with the increase in drug concentrations. Conclusions ATD exhibited proliferation?inhibiting and apoptosis?inducing effects on A375 cells, and the effects were stronger than those of vincristine and ar?turmerone. It is quite possible that ATD affects cell proliferation and differentiation by activating caspase?3 and arresting cell cycle in the G1 phase.

Objective To observe the efficacy of self-made Arnebiae oil by our hospital on phaseⅠandⅡpressure ulcers. Methods A total of 324 pressure wounds of 283 cases collected from March 2014 to August 2015 were randomly divided into the observation group (145 cases) and the control group (138 cases). The control group received Kangfuxin solution in conjunction with conventional pressure ulcer care treatment, the observation group were treated with Arnebiae oil and conventional pressure ulcer care treatment. The therapeutic effect and healing time of both groups were compared. Results Arnebiae oil was effective in 96.97% (160/165) of the pressure ulcer patients and cured 84.85% (140/165) patients in the observation group. Nearly 84.91%(135/159) patients were effective and 50.94%(81/159) patients were cured in the control group. There was statistical significance (χ2=42.93, 14.46, P<0.01). The healing time was (8.78±4.27) d in the observation group and (10.46±3.99) d in the control group with significance(t=2.88, P< 0.05). The healing rates of Arnebiae oil sacrococcygeal region, hips, legs and other parts were 87.96%(95/108), 9/10, 12/16, 9/9, significantly higher than those of the control group, 46.43%(39/84), 6/12, 3/9, 9/15, the difference had statistical significance (χ2=4.02-44.35, P<0.05). Conclusion Arnebiae oil com-bined with the overall care can improve the rate of wound healing in aged pressure ulcer, which is suitable for clinical application.

Objective To analyze the use of hypoglycemic agents of diabetic adult patients who were managed in the Community Health Service Center of Huazhong University of Science and Technology in Wuhan .Methods By analyzing of fol-low-up record of 647 adults with diabetes of the Community Health Service Center outpatient in 2014 to understand about their drug use .Results The ages of prevalence of adult diabetes were 50 to 79 years old (79 .29% ) ,of which the majority of pa-tients aged 60 or older (73 .11% ) ,men are more than women ;the using of antidiabetic monotherapy accounted for 61 .36% , the two drugs together accounted for 34 .93% ,and three or more drugs accounted for 3 .71% ;α-glucosidase inhibitors ,met-formin ,insulin secretagogues are the highest frequency of use ;outpatient diabetics administered orally based .Conclusion The medication use of diabetes in this community was consisted with the requirement of ChinaGuideline for Type 2 Diabetes (2013 edition) .
