RESUMO

Pseudomyxoma peritonei [PMP] can occur in the pleural cavity presenting as pleural effusion, either as a primary disease of mucin producing adenocarcinomas of the lung or as a metastasis from PMP originating in the peritoneum. We report the case of a 64-year-old male with metastatic poorly differentiated mucinous adenocarcinoma of the lung who progressed and developed a pleural effusion that proved to be hemorrhagic pseudomyxoma in the pleura in the absence of abdominal manifestations

RESUMO

To evaluate the value of depending on pleural fluid glucose level of less than 40 mg/dl for differentiating whether malignant pleural effusion is due to intrathoracic or extrathoracic tumors. This is a retrospective cross-sectional study in which a total of 69 patients, admitted to our hospital, having malignant pleural effusion due to varying malignancies, were included in this study. According to the final diagnosis, these 69 patients were divided into two groups: intrathoracic tumors [23 patients], and extrathoracic tumors [46 patients]. Chi-square was applied and p-value and confidence intervals were obtained. Sensitivity, specificity and accuracy values were calculated. Glucose level of lower than 40 mg/dl was observed in 8 out of 23 cases of intrathoracic tumors. None of 46 cases of extrathoracic tumors had a value of pleural fluid glucose of less than 40 mg/dl. Accordingly sensitivity, specificity and accuracy values were 35%, 100% and 67% respectively for a cut-off value of 40 mg/dl for pleural glucose. From this study, it is concluded that pleural fluid glucose is a useful test in helping to differentiate the origin of pleural effusion whether it is due to intrathoracic or extrathoracic tumors

RESUMO

Twelve [good death] principles have been identified that apply to Westerners. This study aimed to review the TFHCOP good death perception to determine its validity for Muslim patients and health care providers, and to identify and describe other components of the Muslim good death perspective. Participants included 284 Muslims of both genders with different nationalities and careers. We used a 12-question questionnaire based on the 12 principles of the TFHCOP good death definition, followed by face-to-face interviews. We used descriptive statistics to analyze questionnaire responses. However, for new themes, we used a grounded theory approach with a [constant comparisons] method. On average, each participant agreed on eight principles of the questionnaire. Dignity, privacy, spiritual and emotional support, access to hospice care, ability to issue advance directives, and to have time to say goodbye were the top priorities. Participants identified three main domains. The first domain was related to faith and belief. The second domain included some principles related to self-esteem and person's image to friends and family. The third domain was related to satisfaction about family security after the death of the patient. Professional role distinctions were more pronounced than were gender or nationality differences. Several aspects of [good death] as perceived by Western communities, are not recognized as being important by many Muslim patients and health care providers. Furthermore, our study introduced three novel components of good death in Muslim society
