RESUMO

Thyroglossal duct carcinoma is uncommon, occurring in approximately 1% of all thyroglossal duct remnants. This rare neoplasm is characterized by relatively nonaggressive behavior with infrequent lymph nodal spread. Another rare neoplasm of the head and neck region is a carotid body tumor. A 78-year-old woman with a 3-year history of midline and bilateral neck masses was referred to us. Fine needle aspiration biopsies and a computed tomography scan suggested the diagnosis of thyroglossal duct carcinoma with cervical lymph node metastasis. Interestingly, the left-side neck mass was found to be splaying the carotid bifurcation, on computed tomography imaging. Carotid arteriography demonstrated a highly vascular mass in the bifurcation of the carotid artery that was compressing the internal and external carotid arteries. To our knowledge, this is the first reported instance of a thyroglossal duct carcinoma with neck metastasis accompanied by a carotid body tumor. In addition, the carotid body tumor in this case mimicked neck metastasis from the thyroglossal duct carcinoma.

RESUMO

Pulmonary sclerosing hemangioma is a relatively rare neoplasm of the lung with polymorphic histologic features of 2 unifying cellular components including surface cuboidal cells and interstitial round cells. Pulmonary sclerosing hemangioma typically occurs in middle aged women with asymptomatic, peripheral, solitary, well-circumscribed lesions. Although it is pathologically benign, it reveals size growing and chest symptom. We here report a case of pulmonary sclerosing hemangioma in a 72-year-old woman. She presented chest discomfort. A chest radiography and a chest computed tomography scan showed growing size from 3.2x3.1 cm to 6.0x5.3 cm in left upper lung during 10 years' follow-up period. Surgical resection of lung revealed a distinct constellation of findings including 2 epithelial cell types, surface cells, and round cells, which form 4 architectural patterns, papillary, sclerotic, solid, and hemorrhagic. She was diagnosed as pulmonary sclerosing hemangioma and chest discomfort disappeared.

RESUMO

A nevus which is a benign melanocytic neoplasm rarely occurs within the external auditory canal (EAC). A dysplastic nevus presents atypical features both clinically and histologically, and is important as a potential precursor for melanoma. We present a case of a 33-year-old female patient with a dysplastic nevus in her EAC. Physical examination revealed a protruding mass arising from the posterior wall of the left cartilaginous EAC. The mass showed clinically characteristic findings of a melanocytic nevus. The patient underwent excisional biopsy via a transcanal approach under local anesthesia. Histopathological examination revealed an intradermal nevus with atypical melanocytes without pleomorphism. There was no evidence of recurrence two years after surgical excision.

RESUMO

Fibroepithelial polyps are a benign neoplasm of mesodermal origin that usually occur in the urinary tract. However, a giant fibroepithelial polyp of the glans penis is very rare and is strongly linked with long-term condom catheter use. In this article, we report a case of a 45-year-old man who presented with a giant fibroepithelial polyp originating from the glans penis. Physical examination showed a 6x3 cm sized mass lesion overlying the ventral surface of the glans near the urethral meatus without urethral communication. For the previous 10 years, the patient had been required to use a condom catheter secondary to paraplegia sustained during a T12-L1 spinal cord injury. He was successfully treated by wide local excision of the mass and suprapubic cystostomy placement. There was no evidence of recurrence after 12 months of follow-up.

RESUMO

Necrotizing Sialometaplasia (NS) is a benign, self-limiting inflammatory disease of the mucus-secreting glands, and this illness mainly involves the minor salivary glands. The significance of NS resides in its clinical and histopathological resemblance to malignancy. We present here a case of necrotizing sialometaplasia on the soft palate, and this was accompanied by adenoid cystic carcinoma. We report here on this case to draw attention to the difficulty for deciding the extent of resecting a malignancy, and especially when the malignancy is simultaneously accompanied by necrotizing sialometaplasia.

RESUMO

Urothelial neoplasms in patients younger than 20 years old are extremely rare, and there is no established etiology, treatment guidelines, or surveillance for urothelial carcinoma in the pediatric population. A 17-year-old boy who presented with gross hematuria showed a smooth, round mass on the left lateral wall of the urinary bladder on cystoscopy. Histologic examination showed low grade papillary urothelial carcinoma of the urinary bladder without muscle layer invasion. We report a case of papillary urothelial carcinoma of the urinary bladder in a pediatric patient treated with transurethral resection.

RESUMO

We describe here a case of minimal deviation endometrioid adenocarcinoma (MDEA) of the uterine cervix that was initially suspected according to the abnormal cytologic findings in a 39-year-old woman. The Papanicolaou (pap) smear showed many neoplastic glandular cells in monolayered sheets, rosettes, and clusters with palisading and feathering borders. The tumor cells had oval, hyperchromatic nuclei, with chromatin clumping and small nucleoli. Histologic examination disclosed endometrial-type glands with a bland, isolated, mainly rounded appearance and these glands were widely scattered deep into the cervical stroma with only scant stromal reaction. An association of MDEA with tubo-endometrioid metaplasia or cervical endometriosis has been suggested by identifying the tubo-endometrioid glands in the vicinity of the MDEA.

BACKGROUND AND OBJECTIVES: The accurate measure of salivary flow rate requires a variety of clinical and experimental protocols. Several methods have traditionally been used to collect and measure the whole mouth saliva. The objective of this study was to verify usefulness of a newly developed oral Schirmer test for detecting salivary gland hypofunction. SUBJECTS AND METHOD: The control group consisted of 85 healthy patients, while another group consisted of 30 patients with salivary grand resection or post-head and neck irradiation and a third group of 30 patients who suffered from subjective xerostomia caused by other pathologies. Oral Schirmer test was carried out in all subjects. The main outcome was the wetted length after 5 minutes. RESULTS: The mean saliva flow was 59.3+/-16.99 mm/5 min in the control group, 47.0+/-13.50 mm/5 min in the 2nd group and 39.9+/-11.65 mm/ 5 min in the third group. The differences between the control group and the other two groups were statistically different (p<0.001). A cut-off value 40 mm/5 min showed sensitivity of 83.7% and specificity of 62.5%. CONCLUSION: The oral Schirmer test is valid and reliable for identifying subjects with xerostomia.

RESUMO

A 40-year-old woman was admitted to the hospital due to a palpable tumor on an anterior chest wall. The tumor was diagnosed with a nodular fasciitis. It is a rare benign soft-tissue tumor which has a characteristic referred to as proliferation of fibroblast, and a surgical removal is the best effective treatment. Therefore, we report this case with documents and considerations after the surgical removal.

RESUMO

The specific diagnosis in diffuse interstitial lung disease may be obtained through open lung biopsy. Diffuse interstitial lung disease is often associated with lung cancer. We report one case of lung adenocarcinoma with idiopathic pulmonary fibrosis in whom previous open lung biopsy had been performed. We need general concepts about sites of open lung biopsy in these patients. Therefore, we report this case and document other references.
