RESUMO

Introduction: diabetes mellitus is the most common metabolic disorder. This study, conducted in Arak in 2014, aimed to determine the effects of education, based on a health belief model on the adherence to medication among patients with diabetes


Materials and Methods: for this research, a quasi-experimental study, with a before and after design was used for which 130 patients with diabetes of Arak were selected by sampling method and randomly assigned to two groups [65 cases and 65 controls]. The data collection tool used was a questionnaire, the validity and reliability of which had been confirmed. Before the training, the questionnaire was completed for both groups. The intervention group received 4 sessions for one month. Three months after the intervention, both case and control groups completed questionnaires and data were analyzed using the appropriate tests


Results: three months after the educational intervention, perceived susceptibility, perceived benefits, self-efficacy, internal and external guidelines and performance in the field of medication adherence were all significantly increased with pre-intervention [p<0.05] while the structures, perceived barriers and perceived severity, did not differ significantly to before intervention status [p>0.05]


Conclusion: teaching diabetic patients skills in the context of medication adherence can raise their compliance with medication beneficially affect their health

RESUMO

Due to the high rate of road accidents in Iran, rescue and relief operations in road are important in health system. Applying some techniques in order to reduce failure in these processes will lead to an increased quality of services; also cost reduction and health improvement will be some consequences. Some of the main Safety Management System elements including hazards and risk identification, assessment, and control them. This paper aimed to determine the optimal processes and relief and rescue operations in roads and also to assess and analyze the failure of this filed; and finally provide some solutions to reduce them. In this applicable and qualitative study, it was used of Health Care Failure Mode and Effect Analysis [HFMEA] in order to identify and analyze the potential errors and failure in relief and rescue operations in road accidents. The relief and rescue operations in road accidents were divided into two processes, 12 sub-processors, and also 53 activities. Each activity probable errors and failures was analyzed and identified; a total of 147 errors were determined and analyzed in both processes and their potential reasons were investigated. Among identified errors, 48 percent were related to manpower; 24 percent equipment malfunction; 11 percent method and process, and the rest was related to other cases such as system, environment, etc based on the results. The issues raised in the article can preserve basic information for future safety studies in relief and rescue operations in road accidents; provide preventive measures and also minimize the likelihood of errors and their consequences

RESUMO

Epiphora and dacryocystitis following nasolacrimal duct obstruction arc considered as two widely common complaints among the patients and dacryocystorhinostomy [DCR] is the treatment choice for such conditions. To determine success rate of DCR with endoscopic and mechanical techniques. In this prospective analytic study 67 patients were operated during 2000-2003. Based on clinical history, results of regurgitation and irrigation tests and also dacryoscintigraphy, the patients with primary nasolacrimal duct obstruction, were operated using nasal endoscopes and mechanical instruments. Intraoperative bleeding, postoperative pain and bleeding, and periobital ecchimosis were recorded and success rate after a follow up of 6 and 12 months was determined. After a follow up of 6 and 12 months duration, success rates were 91.4% and 88.5%, respectively. Severe intraoperative bleeding was observed in 11.4% of patients and periorbital ecchimosis found in 7.1% of cases. Three days following the surgery, mild and moderate pain was seen in 32 patients whereas moderate nose bleeding observed in only 2 cases. Endoscopic mechanical DCR without expensive equipments and with high success rate could be substituted for external DCR

RESUMO

One of the most important factors in increasing patient satisfaction and improving healthcare outcomes is appropriate nurse-patient relationship. This quasi-experimental study was performed to determine the impact of training communication skills to nurses on patients' satisfaction with nurse-patient relationship. All 60 nurses working in medical and infection wards of Imam Khomeini and Vali-e-Asr hospitals were chosen and were divided into two control and experimental groups. Similarly, 80 patients were chosen from the same wards by convenience sampling and divided into two control and experimental groups at each stage of the study. A questionnaire was used for gathering data collected before and 1 month after the intervention. After the first stage, nurses in the experimental group took part in communication-skill workshop for two days, each lasted 8 hours. Findings showed a significant difference between patients' satisfaction before and after the intervention in the experimental group [P<0.001]. There was also a significant difference in patients' satisfaction after instruction between experimental and control groups [P<0.001]. It can be concluded that communication skill training in nurses has positive effects on patients' satisfaction. Thus, communication skill training programs is recommended for nurses to improve the quality of nursing care and increase patients' satisfaction
