RESUMO

Mesenteric cysts are rare intra-abdominal masses. To prevent complications, recurrence and possible malignant transformation, complete excision is considered to be the best option. We report a case of a 25-year-old woman with a giant mesenteric cyst which was completely excised by laparoscopy. A review of the literature is presented

RESUMO

To assess the severity of All-Terrain Vehicle [ATV] injuries and its impact on public health in Kuwait. Retrospective study conducted between January 2002 and August 2003. Accident and Emergency [A and E] department of Adan Hospital, Kuwait. Medical records of 462 consecutive ATV accident victims seen at the A and E department of Adan Hospital were retrospectively analyzed. Out of 462 patients, 364 [78.8%] were male whereas 98 [21.2%] were female. Their ages ranged from 3.5 to 63 years [mean = 17.8 years]. Out of 462 patients, 107 [23%] required admission to the hospital. 18.6% of these admissions [20/107] required ICU care. Fractures of extremities were common among admitted cases [44/107, 41%]. Tibia, fibula and femur were the most common bones involved in these accidents. The face was the commonest site of injury in all patients. The ATV accidents result in significant head injuries, fractures of extremities and skin injuries. Imposing an age limit and implementing state licensing, safety programs and the compulsory use of safety gear are required to reduce mortality and morbidity from this form of recreational riding

RESUMO

To report a case of splenic artery aneurysm that ruptured during labor with both maternal and fetal survival. Clinical Presentation and Intervention: A 33-year-old primigravida at 42 weeks of gestation was admitted to Adan Hospital for induction of labor. Three days after induction, the patient suddenly collapsed and was found to be hypotensive. Abruptio placentae was mistakenly diagnosed, despite the absence of vaginal bleeding, and the patient was immediately taken to the operating room for emergency cesarean section. A female infant was delivered without any evidence of abruptio placentae but with 2,000 ml blood noted in the abdominal cavity. A vertical midline incision was performed and a ruptured splenic artery aneurysm was found. Proximal ligation of the splenic artery was performed followed by splenectomy. Both mother and newborn did well and were discharged on the 7th postoperative day. This case illustrates the need to consider ruptured splenic artery aneurysm as part of differential diagnosis of hemoperitoneum in pregnant women. Immediate surgical intervention is needed to ensure survival of both mother and fetus.
