RESUMO

A comparative study between the use of local muscle flaps versus cancellous bone grafting in management of infected bone cavities, Knowing that osteomyelitis is a disease once established, is extraordinarily difficult to cure even with long term antibiotic treatment. The study included 24 patients, divided into two groups. Group I, included 12 patients for which, cancellous bone grafts were used to fill in the bone defects after adequate debridement. While group II included another 12 patients for which local transposition muscle flaps were used to fill in the bone defects. Both methods were emphasized as regards; the technique, prerequisites and stages. A comparison between the results of both techniques is highlighted with special emphasis to the advantages and disadvantages of each. It was concluded that the use of muscle flap was found to be advantageous over cancellous bone grafting regarding the improvement in the local environment following proper debridement of all necrotic bone as well as better control of infection

RESUMO

Description and evaluation of the use of endoscope in surgery for lower limb lymphoedema. A new technique for surgical treatment of lymphoedema is used in a series of 24 patient. The idea is to make use of the surgical endoscope in fulfilling the goals put by Kondoleon and Sistrunk previously. The endoscope allows the surgeon to excise both the deep fascia and subcutaneous tissue of the diseased lower limb through a 5 cm incision in the leg. Thus, reducing the morbidity and simplifying the procedures. Follow-up showed significant improvement in the leg circumference measurements. Starting third post-operative day and for the whole period. The introduction of endoscopic surgery in our field stimulated many plastic surgeons to use it in different regions of the body. Since lower limb lymphoedema is endemic in Egypt we found that lymphoedema surgery can become much easier with less morbidity using the endoscope. One of the biggest advantages was the very small scar compared to the very large and ugly scars in traditional lymphoedema surgery
