RESUMO

Objective@#To evaluate the effectiveness of health belief model-based health education intervention in improving blood pressure control of patients with hypertension in community settings.@*Methods@#From September 2016 to September 2017, 400 newly diagnosed patients with hypertension were recruited from 6 community healthcare centers with comparable population size and health services in the Shunyi District of Beijing. All community healthcare centers were randomly assigned to the intervention group (206 patients) and the control group (194 patients). Patients in the intervention group received 3 lectures (20-30 min for each) of health belief model-based health education. Patients in the control group received usual care. The basic characteristics, health beliefs, and health literacy were collected, and blood pressure was measured before and after the intervention, respectively. The difference-in-difference model was used to analyze the change of blood pressure and the influencing factors between two groups before and after the intervention.@*Results@#A total of 134 patients in the intervention group and 129 patients in the control group completed the study. After adjusting for the age, gender, family income, medical insurance, chronic diseases and family history, the score of perceived barriers was increased by 1.65 (P=0.016), and perceived seriousness was decreased by 0.73 (P=0.018). The systolic blood pressure of patients was decreased by 7.37 mmHg (1 mmHg=0.133 kPa, P=0.001) and diastolic blood pressure was decreased by 4.07 mmHg (P=0.014), respectively. The β (95%CI) values were -7.37 (-11.88,-2.86) and -4.07 (-7.30, -0.84). The perceived susceptibility and self-efficacy had a significant influence on the blood pressure of patients (P<0.05).@*Conclusion@#Health belief model-based health education intervention could significantly improve the blood pressure control of patients with hypertension in the community settings.

RESUMO

Objective To explore the scientific development direction in medical institutions under the new health care reform,and provide a reference for promotion of scientific innovation and development of medical institutions in Beijing.Methods Through analyzing the fund of Natural Science Foundation of China (NSFC) in 2016,scientific competitive power of medical institutions in Beijing was explored from the aspect of project type,subject classification and talent training.Results The NSFC by medical institutions in Beijing mainly focused on medical science department (92.68%),of which corresponding subjects were mainly oncology,neural and mental disease and traditional Chinese medicine.The project approved rates of tertiary hospitals subordinate to country and Beijing were 43.46% and 29.22%,respectively.Only a few dispersing talent projects were obtained.Conclusions Funds and approved rate of NSFC increased.Medical institutions in Beijing had relatively strong scientific competitive power in the fundamental researches of major and difficult diseases,and traditional Chinese medicine.Fundamental researches,talent training,research team building and international cooperation should be improved in the further.

Objective:To study the status quo of directive and nondirective support from family or friends among patients with diabetes in community and to explore the association between directive, nondirective support from family or friends and self-management behaviors.Methods: The data of the subjects were from the Community Diagnosis in Tongzhou District, Beijing survey.Questionnaire-based investigation, physical examination and glycated hemoglobin test were conducted on 474 eligible patients in community.The summary of diabetes self-care activities measure (SDSCA) was adopted to investigate the self-management behaviors, and directive and nondirective support scale among patients with diabetes (DNSS-PD) was adopted to measure the directive and nondirective support among patients with diabetes.Results: A total of 452 patients with diabetes were included in the analysis.The average score of directive support was 2.53±1.07, and only 27.6% patients had high level directive support.The average score of nondirective support was 3.13±1.12 and 50.8% patients had high level nondirective support.The average SDSCA total score was 35.38±14.21 and the corresponding scoring percentage was 45.95%.Only 20.6% patients had optimal self-management.The total score, score of diet and physical activity, general self-management situation, diet self-management situation and physical activity self-management situation of the patients in high level directive support group were significantly better than in low level directive support group.The total score, score of self-management behaviors in five aspects (diet, physical activity, blood glucose self-monitoring, foot care and smoking), general self-management situation, self-management situation of five aspects of patients in high level nondirective support group were significantly better than in low level nondirective support group.Gender (OR=2.729), nondirective support level (OR=4.890), age (OR=0.969) and body mass index (BMI) (OR=0.819) were important influencing factors of self-management behavior.Conclusion: The status quo of self-management among patients with diabetes is not optimistic.Patients obtain more nondirective support than directive support from family and friends.However, the support level of both nondirective and directive support should be improved in the future.Both high level directive support and high level nondirective support had positive correlation with better self-management behaviors.Nondirective support had positive correlation with five aspects of self-management behavior, while directive support had positive relation with self-management behavior in aspects of diet and physical activity.In addition, more consideration and support of self-management should be provided for older obese male patients with diabetes in community, especially nondirective support.

Objective To investigate the effect of genistein on the expression of Bax and Bcl-2 protein in PC12 cells transfected with mutant APP695 Type.Methods PC12 cells were transfected with pIRES2-EGFP plasmid or pIRES2-EGFP/APP695MT expression plasmid,and then divided into normal control group,Unladen group,APP695 transfected group,GST treatment group(5 μmol/L,15 μmol/L).The Bax and Bcl-2 protein expression in PC12 cells was measured by Immunocytochemistry SABC and Western-blot.Results Immunocyto-chemistry SABC showed that:compared with the normal control group,the expression of bax protein in APP695 transfected group was strongly increased((78.02 ± 1.26) %,P<0.01)) and the expression of bcl-2 protein was very weak oppositely ((15.40 ± 0.72) %,P<0.01)).Compared with APP695 transfected group,the expression of bax protein in cells of 15umol/L GST treatment group was significantly decreased((22.35 ± 0.49) %,P<0.01))and the expression of bcl-2 protein in these cells was strongly increased ((68.11 ± 0.24) %,P <0.01)).Western-Blot showed that:compared with the normal control group,the expression of bax protein in APP695 transfected group was markedly increased and the expression of Bcl-2 protein was significantly decreased.Compared with the APP695 transfected group,the expression of Bax protein was markedly decreased and the expression of Bcl-2 protein was significantly increased in the cells of the GST treatment groups.Conclusion GST can reduce the expression of bax protein and the increased expression of Bcl-2 protein has protective effect on PC12 cells transfected with APP696MT.

To explore the major risk factors for hepatitis B (HBV) in Shanghai Jinshan district.A one-to-two matched case control study was performed among the residents of Shanghai Jinshan district in 2010.The data were analyzed by single factor and multivariate analysis performed with SAS 9.13.Single factor analysis showed that the risk factors for hepatitis B were family member with hepatitis B,no vaccination history of hepatitis B virus vaccine and oral treatment.Muhivariable analysis showed that family member with hepatitis B (OR =1.91),no vaccination history of HBV vaccine (OR =0.49) and oral treatment (OR =2.06) were independent risk factors for HBV infection in Shanghai Jinshan district.Strengthening health education for HBV prevention,modifying unhealthy living habits and standardizing the treatment of oral diseases are important measures for lowering infectious rate of HBV.

ObjectiveTo investigate the effect of Genistein (GST) on cell cycle and apoptosis in PC12 cells transfected App695MT gene.MethodsPC12 cells were transfected with pIRES2-EGFP plasmid or pIRES2-EGFP/APP695MT expression plasmid,and then were divided into control vectortransfected group,APP695 transfected group and GST treatment group.Flow cytometry was applied to detect cell cycle and apoptosis,laser confocal microscope was used to observe morphological changes of cell apoptosis.ResultsCompared with control vectortransfected group,PC12 cells in APP695 transfected group increased significantly in G0 and G1 phase,and less into S phase,cell proliferation index was decreased significantly( (55.6 ±0.57)%,P<0.0l ),apoptosis rate was increased significantly( (77.10 ± 12.53)%,P<0.01 ).Emitted red fluorescence increased significantly when cell death,cell body swelling,organelle disintegration,nuclear condensation or fragmentation.Compared with APP695 transfected group,PC12 cells in GST( 15μ mol/L) treatment group,decreased significantly in G0 and G1 phase,and more into S phase,cell proliferation index was increased significantly ( ( 61.57 ± 0.47 ) %,P < 0.01 ),apoptosis rate was decreased significantly ( (46.00 ± 8.43 ) %,P < 0.01 ).Cell death was significantly reduced red fluorescence,emitted green fluorescence was significantly enhanced,compared with APP695 transfected group cell morphology improved.ConclusionGST can improve APP695MT gene caused cell cycle arrest,promote cell to S phase transition,reduce apoptosis rate in PC12 cells,and have a protective effect on transfected cells.

RESUMO

The ubiquitin-related modifier Urm1 can be covalently conjugated to lysine residues of other proteins, such as yeast Ahp1 and human MOCS3, through a mechanism involving the E1-like protein Uba4 (MOCS3 in humans). Similar to ubiquitination, urmylation requires a thioester intermediate and forms isopeptide bonds between Urm1 and its substrates. In addition, the urmylation process can be significantly enhanced by oxidative stress. Recent findings have demonstrated that Urm1 also acts as a sulfur carrier in the thiolation of eukaryotic tRNA via a mechanism that requires the formation of a thiocarboxylated Urm1. This role is very similar to that of prokaryotic sulfur carriers such as MoaD and ThiS. Evidence strongly supports the hypothesis that Urm1 is the molecular fossil in the evolutionary link between prokaryotic sulfur carriers and eukaryotic ubiquitin-like proteins. In the present review, we discuss the dual role of Urm1 in protein and tRNA modification.

RESUMO

Objective:To observe the protective effects of genistein(GST) on the medulla oblongata neurons in ovariectomized rats. Methods:Rats with bilaterally ovariectomized were injected genistein 200 ?g/(kg?d) for 6 weeks. The madlondialdehyde(MDA)and superoxide dismutase(SOD)activity of medulla oblongata were measured, and the ultrastructure of medulla oblongata neurons were observed. Results:In the GST group,the MDA activity was reduced and SOD activity was increased compared with the OVX group. The ultrastructure of medulla oblongata neurons showed pathological changes in OVX group. These ultrastructure was not find in GST group. Conclusion:GST could protect neurons of medulla oblongata, which might be related to eliminate oxygen free radical.

Abstract Objective: To study the relationshiop between the HBV pre-c mutant and the clinical the, the host immunity. Methods: Mu-tation specific PCR(msPCR) was employed for detecting pre-c mutant in 92 cases with HBV-DNA positive;T subpopulations were determinedby APAAP immune-bridge assay; the cytokines (TNF?、 sIL-2R ) levels by ELISA. Results: 57 cases had Pre-c mutant, the rate was ahaut 62 %;pre-c mutant existed popularly in different e systems status, mainly in anti-HBe(+) serum. The reat increased with the deterioration of liverfunction, and was highest in fulminant hepatitis, the percentage of CD4 T lymphocyte and the ratio of CD4/CD8 were aignificantly lower in mu-tational group than in wild group,but CD8 T lymphocyte was obviously higher, the quantity of sera TNF? and sIL-2R in mutational group wereobviously higher than in wild group, both groups were obviously higher than control group. Conclusion:PreC 1896 mutant existed popularly inpatients with HBV infection. There were more serious disturbance in host immunity in mutant group. The mutant may be associated with the pre-ssare of host immune system.The mutant type virus could provoke host immunity resulting in severe liver damage.

RESUMO

Objective In the present study we used real-time quantitative RT-PCR to measure the relative expression levels of CGRP,CRLR,RDC-1,RAMP1-3 in periaqueductal gray(PAG)of intact rats. Methods With ?-actin as an endogenous control,write down the C T value and calculate the relative amount of every sample against its endogenous control. Results The results showed the specific amplification of CGRP,CRLR,RAMP1-2 in PAG of rats.Furthermore,the expression of CGRP and RAMP1 is the highest,that of CRLR is higher and the RAMP2 is low;whereas no band is detected in RDC-1 and RAMP3. Conclusion These findings suggest the specific expression of CGRP,CRLR,RAMP1 and RAMP2 mRNA and indicate their functional role in the modulation in PAG of intact rats.
