RESUMO

Objective:To reduce the occurrence of false alarm of vital signs during the use of ECG monitor by using stress ring to fix the lead wire of ECG monitor. Methods 200 patients who used ECG monitor in our department from March 2018 to February 2019 were intervened by two methods. The control group ( n=100) was connected and observed by routine method. The experimental group ( n=100) was added with lead wire to form "stress loop" on the basis of this method. Methods:The connection and fixation of ECG monitors were carried out. The differences of vital signs, heart rate/rhythm and respiratory false alarm between the two groups during ECG monitoring were compared, and the changes of nurses' workload, Hamilton anxiety score and satisfaction were compared.Results:There were significant differences in heart rate/rhythm and respiratory false alarm between the two groups ( P < 0.05). The scores of anxiety, nurses' workload and satisfaction in the experimental group were significantly improved. Conclusion:During the application of ECG monitor, the lead wire coiled into "stress ring" can effectively reduce the ECG monitor vital signs monitoring center rate/rhythm, respiratory false alarm phenomenon, reduce the workload of nurses, improve the satisfaction of nurses, and reduce anxiety of patients.

RESUMO

Objective To investigate the current status and existing problems in the implementation of stroke care continuity in China, and to collect relevant suggestions for solving these problems. Methods Focus group interviews were used, to survey thirty-six nursing managers and senior nurses in neurology departments or rehabilitation programs in 24 cities of 13 provinces. The interview team members presided over the meetings, with full-time staff taking notes and recordings, and the results of the interviews were summarized and organized in a timely manner after the interview. Results At present, most of the general hospitals surveyed are doing their best to continue supporting nursing care for patients discharged from hospitals in different forms following their rescue of the acute phase. However, they are faced with such challenges as insufficient nursing manpower, and discontinuity between different medical institutions, which result in oversimplified nursing care and fragmented care. Conclusions Stroke nursing practitioners are working on continuous nursing care, with quite some challenges as well. Nursing care continuity is an important step towards establishing and improving a scientific and rational grading nursing care system.

Objective To improve the effect on oral hormone compliance with discharged neurological patients. Methods The convenience sampling method was used by selecting 230 hospital patients as control group between January to December, 2015 and another 152 patients in the hospital between January and December 2016 as experimental group. After discharge, the experimental group was treated with auxiliary hormone drugs,"hormone calendar"method, and the control group with the conventional education. The two groups were compared in terms of compliance in hormone taking, error in medication error and disease recurrence. Results About 2 months and 4 months after discharge, compliance with medication of the control group was higher than that of the experimental group. The error rate of medication and the recurrence rate were significantly higher than those of the control group (all P<0.05). Conclusion"hormone calendar"can enhance the medication compliance of hormonal medication, reduce the error rate of medication and recurrence rate, thus worth promoting.

Objective To study the effects of different temporal resolution of spiral computed tomography (CT)on perfusion parameters and perfusion curve of cervical cervical cancer.Methods Ten cases of cervical cancer were clinically confirmed with CT perfusion scanning.The original data were acquired using temporal resolution of 0.75 s.Then the original data were grouped according to different temporal resolution,namely,1.5 s group,2.25 s group,3 s group,3.75 s group,4.5 s group,5.25 s group,and 6 s group (experiment group). According to the same mathematical model and ROI of the same part,perfusion parameters (BF,BV,MTT,and PS)in each group were calculated respectively and compared with the original data.Results BF and MTT were relatively sensitive to the change of temporal resolution.When the temporal resolution was 3 s,it had a significant impact.PS and BV were not so sensitive to the change of temporal resolution.Temporal resolution of 4.5 s had a significant effect on PS. There was a significant effect on BV until the temporal resolution was 5.25 s. Conclusion Changing the temporal resolution will lead to corresponding changes of perfusion curve and perfusion parameters.Under the premise that it does not affect the diagnosis,properly decreasing temporal resolution (circu-lar scanning temporal ≤2.25 s)of CT perfusion scanning of cervical cancer can reduce the radiation dose effectively.

Objective To explore the changes of blood fat in patients with cerebral infarction complicated by metabolic syndrome (MS) receiving community path intervention treatment.Methods A total of 116 cases of cerebral infarction complicated by MS were selected and given comprehensive intervention treatment after risk assessment.Patients were divided into intervention group (60 cases) and control group (56 cases) according to their difference in compliance.Results After intervention,the total cholesterol (TC),triglyceride (TG) and low density lipoprotein cholesterol (LDL-C) in two groups were significantly decreased,high-density lipoprotein cholesterol (HDL-C) was significantly increased.There were significant differences in intervention group before and after intervention (P< 0.05).Compared with those in control group,differences of all index in each time point in intervention group were statistically significant (P <0.05).There was no significant difference in carotid plaque integral before intervention between two groups (P> 0.05).The carotid plaque integral 12 and 24 months after intervention in intervention group was significantly lower than that before intervention (P < 0.05).The carotid plaque integral 12 and 24 months after intervention in intervention group was significantly lower than that in control group [(3.20 ± 2.01) cm vs.(4.71 ±2.87) cm,(2.98 ±2.61) cm vs.(4.60 ±2.43) cm,P<0.05].Twelve and 24 months after intervention in intervention group,TC and carotid plaque integral was significantly positive correlation (r =0.304 and 0.317,P < 0.05),TG and carotid plaque integral was significantly positive correlation (r =0.229 and 0.128,P < 0.05),LDL-C and carotid plaque integral was significantly positive correlation (r =0.654 and 0.518,P < 0.05),and HDL-C and carotid plaque integral was significantly negative correlation (r =-0.495 and-0.528,P < 0.05).Conclusion For patients of cerebral infarction complicated by MS,great importance should be attached to early prevention and control of their major components so as to reduce the incidence of acute cerebrovascular recurrence and mortality.

BACKGROUND: During plasty of breast, innervation in nipple and areola of breast should be comprehended particularly in order to avoid sensory and motor disorder induced by nerve injury. OBJECTIVE: To observe the courser and distribution of nerve in nipple and areola of breast and provide anatomic data for plasty of breast. DESIGN: Single sampling study. SETTING: Department of Human Anatomy, Medical College of Qingdao University; Department of Pathology, Affiliated Hospital of Medical College of Qingdao University. PARTICIPANTS: The experiment was carried out in the Department of Human Anatomy, Medical College of Qingdao University and Department of Pathology, Affiliated Hospital of Medical College of Qingdao University from September 1999 to April 2001. Sixteen breasts of 8 adult female corpses aged 20-40 years and 10 breasts (5 right and 5 left) of 10 adult females with cancer of the breast aged 43-58 years were selected in this study. All patients or relations were consent. METHODS: ① Breast samples of 16 adult female corpses were scanned layers by layers with stereo anatomic micro scope to chase courser and distribution of nerve in subcutaneous fascia of mammary cancer. ② Ten fresh breast sam pies were frozen, cut into sections and stained with haematine-eosin. The typical transects were selected to observe dis tributed density of nerve fiber in 4 quadrants at various layers with stereology. MAIN OUTCOME MEASURES: ① Innervation in nipple and areola of breast,, ② numbers of nerve fiber of the same sample in various transects. RESULTS: ① Nerve which was distributed in nipple and areola of breast was derived from the lateral cutaneous branch and anterior cutaneous branch of the 3rd, 4th and 5th intercostal nerves and deep branch of mammary cancer. ② The lat eral cutaneous branch of the 4th intercostal nerves went into nipple and areola of breast with the directions like 4 o'clock in the left and 8 o'clock in the right, and nerves dominated nipple were derived from the lateral cutaneous branch of the 4th intercostal nerves. ③ Numbers of nerve fiber in extra-inferior quadrant of nipple and areola of breast were more than those in intra-superior, intra-inferior and extra-superior quadrants (8.06±0.18, 4.63±0.14, 4.43±0.16, 2.13±0.11, P < 0.01). ④ Density of innervation in nipple and areola of breast was decreased centripetally. CONCLUSION: Operation of chest, especially plasty of breast, had better not damage lateral cutaneous branch and ante rior cutaneous branch of the 3rd, 4th and 5th intercostal nerves. Extraordinarily, the lateral cutaneous branch of the 4th inter costal nerves and nerve in extra-inferior quadrant of nipple and areola of breast should be protected carefully in order to avoid sensory disorder.

@# ObjectiveTo study the relationship between the incidence of cervical osteophytosis and the age.MethodsThree hundred X-ray films (male 159, female 141) on lateral cervical position were observed. The patients were divided as group A (age <30), group B (age 30~39), group C (age 40~49), group D (age 50~59), group E (age 60~69) and group F (age 70~90). The osteophytosis on superior or inferior cervical vertebral body were marked.ResultsThe incidences of osteophytosis were group A 0%, group B 4.0%~4.7%, group C 20.0%~23.3%, group D 38.7%~39.2%, group E 56.7%~60.0%, group F 72.7%~80.9%. There was significant differences between the group B and group C, group C and group D, group D and group E, group E and group F (P<0.05~0.01). The common places of the osteophytosis were C5 and C6, while often on C6 in male and on C5 in female.ConclusionThe incidence of cervical osteophytosis increases following age increasing. The most place of the osteophytosis is on C5 and C6.

Objective To provide credible anatomic data and information for clinical diagnosis and surgery of hand by measuring the length of metacarpal bone and phalanges of hands on X-ray films and to study their mathematical relations.Methods The X-ray films of hands on posteroanterior position were taken in 186 healthy college students (M90,F96).The length of each metacarpal and phalange were measured.All the data were calculated by SPSS software.Results The mathematical relations of each metacarpal and phalange bone were obtained .It was found that there were sexual differences in the ratio of first to second,second to fourth,second to fifth and third to fourth metacarpal length,the ratio of first to second,first to fourth and second to fifth proximal phalangeal length,the ratio of second to fifth,third to fifth and fourth to fifth middle phalangeal length and the ratio of first to second and first to third distal phalangeal length.Conclusion The results of this mathematical relations are of great significant in orthopaedic surgery of hand.
