RESUMO

Objective:To study the correlation between serum homocysteine (Hcy) and the staging of gastric cancer,by comparing the concentrations of patients with gastric cancer at different pathological staging.Methods:90 patients with benign gastric diseases and 138 patients with gastric cancer were selected and admitted by Shandong University Qilu Hospital during the date from Mar 2014 to Jun 2015.The patients with gastric cancer were divided into 4 groups,according to the 7th AJCC Cancer Staging.To compare the difference of the concentration levels of Hcy and Tumor marks in different groups and analyze the relationship between benign disease and gastric cancer,and analyze it correlation with different pathological stagings of gastric cancer.Enzymatic cycling assay was used for detecting serum Hcy.Results:The serum Hcy concentration level in benign disease was (1 2.31 ± 3.22) μ mol/L,and significantly lower than cancer group(1 6.19 ± 4.84) μ mol/L,the difference was statistically significant (P<0.05):The serum Hcy concentration levels increased gradually from staging I to staging Ⅳ,and the concentration level in staging Ⅰ was (13.94 ± 4.07) μ mol/L,in staging Ⅱ was (15.49 ± 4.09) μ mol/L,in staging Ⅲ was (17.10 ± 4.79) μ mol/L,in stagingⅣ was (19.81 ± 5.77) μ mol/L,the differences among the four groups were statistically significant(P<0.05).Spearman Rank Correlation analysis confirmed that,the Hcy concentration level was positively related with pathological staging(r=0.503,P< 0.001).Logistic regression analysis showed that,the serum Hcy concentration level is significantly correlated with gastric cancer,after the adjustment of other risk factors (OR=1.208,P=0.003).Conclusions:The serum Hcy concentration level is closely correlated with gastric cancer staging,and increase significantly with the cancer staging (from staging Ⅰ to staging Ⅳ),so itcan be used to evaluate the severity of gastric cancer.

Objective To investigate the diagnostic value of blood routine and inflammatory factors in children with acute fever.Methods 137 children of acute fever were divided into infection group(including bacterial infection and non bacterial infection) and non infection group.And 50 healthy children(control group) were collected.Results were compared with blood routineand inflammatory factor.Results White blood cell(WBC),platelet(PLT) and neutrophil percentage(N%) were significantly increased in the bacterial infection group(P<0.05),and there was significant difference between the control group,the non-infection group and the non-bacterial infection group(P<0.05),and the percentage of PLT and N% was statistically significant(P<0.05) compared with non-infected group.The levels of interleukin(IL)-2 in the bacterial infection group were significantly lower than those in the nonbacterial infection group,the non-infection group and the control group(P<0.05).The levels of IL-6,IL-10 and TNF-α in bacterial infection group were significantly higher than those in control group and non-infected group(P<0.05).Misdiagnosis rate of inflammatory factors is the highest (18.25 %),followed by blood 14.60%,and blood combined with inflammatory factors is the lowest (8.76 %).Conclusion Blood combined with inflammatory factors can effectively reduce the misdiagnosis rate,the children with the blood unclear caused of should be combined with inflammatory factors detection in order to further clarify the cause and early treatment.

Objective To analyze the clinical characteristics,diagnosis,differential diagnosis and treatment of gastrointestinal schwannoma.Methods Clinical data of 3 patients with gastrointestinal schwannoma were collected and retrospectively analyzed.Results Gastrointestinal bleeding or melena,anemia and epigastric pain were the most common presenting symptoms.The symptoms,physical signs and auxitiary examinations (such as X-ray,ultrasonography and gastrointestinal endoscopy) of gastrointestinal schwannoma had no value in differentiation.In all the 3 patients definite diagnosis was achieved only by postoperative pathology.Conclusion Gastrointestinal schwannoma are derived from the Schwann cells of nerves in gastrointestinal wall and are usually benign.Benign schwannomas can only be distinguished from the malignant ones on the basis of histological and immunohistochemical criteria.Surgical resection is the most effective treatment.

Objective To investigate the features of epileptic seizures and eletroencepalogram (EEG) in patients with mitochondrial myopathy encephalopathy,lactic acidosis and stroke-like episodes (MELAS).Methods Forty-four patients with MELAS were diagnosed at the First Hospital of Peking University from November 2007 to August 2013.EEG and head MRI were performed on all patients.The types of epileptic seizure and EEG changes were compared between patients in and outside stroke-like episodes.Results Epileptic seizures occurred in 39 of 44 patients (88.6%) with MELAS,while multitype epileptic seizures were presented in 26 cases (66.7%).In stroke-like episodes,22 patients presented with partial seizures,30 with generalized seizures and 17 with status epilepticus.In nonstroke-like episodes,7 patients presented with partial seizures,14 with generalized seizures and 2 with status epilepticus.The frequency of partial seizures,generalized tonic-clonic seizures,status epilepticus were 47.7% (21/44),68.2% (30/44),38.6% (17/44) in stroke-like episodes and 13.6% (6/44),27.3% (12/44),4.5% (2/44) in nonstroke-like episodes,which have statistical significance (x2 =12.022,14.758,15.103;P =0.001,0.000,0.000,respectively).Abnormal EEGs appeared in all patients.The rates of slow alpha frequency,diffuse δ or θ wave,epileptic discharges were 6.8% (3/44),43.2% (19/44),25.0%(11/44) in stroke-like episodes and 31.8% (14/44),59.1% (26/44),22.7% (10/44) in nonstroke-like episodes,respectively.Slow alpha frequency were significantly different between patients in and outside stroke-like episodes (x2 =8.822,P =0.003).Conclusions Epileptic seizures with different types are more common during stroke-like episodes in patients with MELAS.While the rates of epileptic discharges are also common outside the stroke-like episodes.

This study sought to investigate the in vivo antiviral effect of amantadine (AM) and biphenyl dimethyl dicarboxylate (DDB) on hepatitis B virus (HBV) in HBV replication mice. HBV replication-competent plasmid was transferred into male BALB/c mice by using hydrodynamics-based in vivo transfection procedure to develop HBV replication mouse model. The model mice were matched by body weigh, age and serum levels of hepatitis B e antigen (HBeAg) and were divided into four groups: AM group, DDB group, AM+DDB group and NS group, with the last one as control, and the mice of each group were administered corresponding agent orally twice a day, in a medication course lasting 3 d. On the third day, the mice were sacrificed 4-6 h after the last oral intake. HBV DNA replication intermediates in liver were analyzed by Southern blot hybridization. The serum hepatitis B surface antigen (HBsAg) and HBeAg were detected by enzyme linked immunosorbent assay (ELISA). Compared to the animals in the control group, HBV DNA replication intermediates in liver and HBsAg and HBeAg in serum from the AM and AM plus DDB group of mice decreased, and there was no difference between these two groups of mice. The levels of HBV DNA intermediate from liver and the serum HBsAg and HBeAg between the control and DDB group, however, were not obviously different. In conclusion, the inhibition effect of AM on HBV was detected, but treatment with DDB for 3 days did not influence the viral replication and expression of HBV in the HBV replication mice.

Objective The present study was designed to investigate the efficiency of simvastatin therapy for experimental pul‐monary hypertension (PH) in rat ,and the effects on the number and function of circulating endothelial progenitor cells (EPC) . Methods Twenty four Sprague Dawley rats were divided into 3 groups randomly :model group ,treatment group and control group , 8 rats in each group .Rats were treated with a single subcutaneous injection of monocrotaline to induce PH (PBS used as control) . The rats in the experimental group were administrated with simvastatin 3 days following subcutaneous injection of monocrotaline .In the 21st day ,the number of circulating EPC ,the right ventricle systolic pressure of rat ,pulmonary vascular structural changes and the quantity of cultured EPC were measured .At the same time ,EPC in each group were cultured in vitro ,then the ability of prolif‐eration and function were analyzed and compared .Results The number of circulating EPC in model group was decreased signifi‐cantly compared to both control and model groups (P< 0 .01) .Compared with model group ,simvastatin treatment markedly de‐creased the RVSP and the ratio of media thickness to eternal diameter (P<0 .01) ,but the ratio of vessel area to total arterial area (VA/TAA) was definitively increased(P<0 .01) .After 7 days of culture in vitro ,both the output of EPC and the ability of prolif‐eration ,conglutination and migration of EPC in treatment group were up -regulated compared with those in model group (P<0 .01) .Conclusion This study confirmed that simvastatin effectively treat experimental PH through improving quantity and func‐tion of circulating EPC .

Objective To investigate the clinical and radiological features of the patients with cryptogenic stroke as the first manifestation of pulmonary arteriovenous malformation (PAVM),and to explore the mechanism and summarize the diagnostic strategy.Methods Transcmnial Doppler (TCD) bubble tests,non-contrast or contrast-enhanced transthoracic and transesophageal echocardiography,contrast-enhanced thoracic CT and pulmonary angiography were performed in 2 patients with cryptogenic stroke for the detection of right to left shunt (RLS) and the diagnosis of PAVM.Then interventional catheter embolization of PAVM and postoperative follow-up were arranged.Results TCD bubble tests in these 2 patients showed the following characteristics indicating RLS:spontaneous continuous shunt at rest,earlystage shunt appeared rapidly after injection,considerable large amount of shunt volume as a shower of microbubble,the extent of shunt volume not attenuated by Valsalva Maneuver (VM).One patient was diagnosed as PAVM,the other was PAVM plus patent foramen ovale (PFO).Both patients of PAVM were successfully treated with interventional catheter embolizations.Then TCD bubble test was repeated and showed significantly decreased amount of RLS.They remained asymptomatic during the 3-year and 1-year follow-up.Conclusions In patients with cryptogenic stroke due to suspected paradoxical embolism,TCD bubble test should be performed for screening RLS.Besides PFO,a traditional access to intracardiac shunt,PAVM should be considered as a new access to extracardiac shunt in paradoxical embolism.

Objective To study the wavelet entropy of intracranial electrode recording electroencephalogram (EEG) during the seizure onset,and explore the prospect of applying it for automatic detection of EEG.Methods Twenty-eight seizure events of 9 epilepsy patients whose outcome were Engel class Ⅰ were retrospective analyzed,by comparing the pre-ictal,initial ictal and ictal wavelet energy entropy variations of both seizure onset zone (SOZ) contacts and non-SOZ contacts.Results The wavelet energy entropy of SOZ contacts increased from pre-ictal (0.053 ± 0.047 ) to initial ictal (0.326 ± 0.250),while it declined during the ictal (0.138 ± 0.097,F =30.904,P =0.000).There was no obvious change of the wavelet energy entropy of non-SOZ contacts in different periods.By further applying time-window wavelet entropy to the detection of intracranial EEG,the results showed that the time-window wavelet energy entropy could be sensitive to reflect the seizure onset time and electrodes.Conclusion The wavelet energy entropy of contacts inside SOZ increased significantly at scizurc onset,and this conclusion can be used to assist in presurgical localization of epilepsy.

Objective:To study the relationship of expressions of E-cadherin and ?-catenin proteins and gastric mucosa intestinal metaplasia and gastric carcinoma.Methods:The immunohistochemical Envision method was used to determine the expressions of E-cadherin and ?-catenin in 30 cases of simple intestinal metaplasia(SIM) ,38 cases of atypia intestinal metaplasia(AIM) and 55 cases of gastric carcinoma.Results:The reduced expression rates of E-cadherin and ?-catenin in gastric carcinoma were significantly higher than that in SIM(P0.05) .Conclusion :E-cadherin and ?-catenin proteins take part in the transformation from intestinal metaplasia to carcinoma in gastric mucosa,AIM is a kind of precancerosis of gastric carcinoma.

RESUMO

In addition to immune regulation, interferon could suppress hepatitis B virus (HBV) replication through direct antiviral effect. After binding with the receptors on cell membrane, interferon directly inhibits HBV at different steps in HBV replication cycle by activating cell signaling cascades such as JAK-STAT pathway, interferon regulatory factor (IRFs) signaling pathway, and so on, followed by inducing a series of cytokines which are involved in regulation of the function of HBV enhancer I / X promoter (Ehn I / Xp). Also, interferon could induce the host cells to produce anti-viral proteins. This review describes the direct antiviral mechanism of interferon on HBV.

Objective To evaluate the efficacy of three-dimensional conformal radiotherapy (3DCRT) combined with concurrent chemotherapy for loco-regionally recurrent or mastastatie rectal cancer. Methods Between June 2004 and January 2007,47 patients with loco-regionally recurrent or nmatastatic rectal cancer were treated by 3DCRT of 55-65 Gy in 1.8-2.0 Gy fractions. Chemotherapy was given concurrenfly using oxaliplatin(100 mg/m2 ,iv drop,d1 ) and capecitabine(1500 mg/m2,orally,dl-14,21 days per cycle). Results After the follow-up of 12-35 months, the total response rate, complete response rate and partial response rate were 79% (37/47) ,19% (9/47) and 60% (28/47) ,respectively. The pain-alleviation rate and the mean pain-alleviation time were 85% and 6 months. The 1- and 2-year survival rates were 83% and 51%. Quality of life was improved without any treatment related death. Conclusions 3DCRT combined with concurrent chemotherapy is effective and well-tolerated in patients with post-operatively locoregionally recurrent or mastastatic rectal cancer.

To make a universal gene targeting vector fitting for most gene and delete positive selection gene after targeting successfully, a vector named pA2T was constructed by inserting one neomycin gene (neo) for positive selection and two same herpes simplex virus thymidine kinase gene HSV-tk1 and HSV-tk2 for negative selection into the vector of pGEM-3Z, and two locus of crossing-over (x) in P1 (LoxP) and two different multiple cloning sites (MCS) were inserted into two flanks of neo separately. There were eight rare cloning sites between neo and HSV-tk1 and five rare cloning sites between neo and HSV-tk2, and neo, HSV-tk1 and HSV-tk2 could be translated respectively in the pA2T. Transfection of the pA2T into goat fetus fibroblast cells with Lipofectamine 2000 conferred resistance to geneticin (G418) and resistance to ganciclovir (GAC) in the cells, which suggested the positive and negative selectable markers could express in the cells and thus the vector pA2T could be used as a universal gene targeting vector. Transformation of the pA2T into the BM25.8 expressing Cre recombinase conferred neo was deleted in the pA2T, which suggested the LoxP was active. Thus, this vector can be inserted by most gene sequences as homologous sequences and positive selection gene can be deleted after targeting successfully, which is very convenience for the production of transgenic animals using gene targeting method.

@# ObjectiveTo investigate the feasibility of transcranial Doppler (TCD) in evaluating the effect of drugs on reducing the intracranial pressure.Methods20 patients who were treated with 20% mannitol (10 cases) and 10% glycerin (10 cases) separately were monitored the changes of blood flow velocity of middle cerebral artery (MCA) with TCD before and after treatment. ResultsIn mannitol group, The peak time presented at 30 min (P<0.001) after drug application and continued 4~6 h after treatment. In glycerin group, the peak time presented at 2 h (P<0.001) after drug application and continued 6~8 h after treatment.ConclusionTCD can monitor the changes of of blood flow velocity of middle cerebral artery (MCA) and thereby evaluate the effects of drugs on reducing intracranial pressure.

BACKGROUND:Apolipoprotein E is one of the important plasma apolipoproteins with polymorphism and involved in lipid metabolism and cholesterol balance modulation, playing also a role in the normal growth and repair of the nervous system.OBJECTIVE: To analyze apolipoprotein E gene polymorphism in patients with vascular dementia (VaD) in comparison with patients with cerebral infarction (CI) patients and healthy controls, so as to elicit the relationship between apolipoprotein E gene polymorphism and VaD.DESIGN: Case-control study.SETTING: Department of Neurology, General Hospital of Jinan Military Area Command of Chinese PLA.PARTICIPANTS: The participants were recruited from the inpatients and outpatients with cerebrovascular diseases and healthy subjects for routine physical examination in the General Hospital of Jinan Military Area Command of Chinese PLA between August 2001 and October 2003, including 35 cases of VaD, 36 CI cases and 40 healthy controls.METHODS: Four milliliters of venous blood was collected from the elbow vein form all participants after fasting for 12 hours for detection of apolipoprotein E genotype using polymerase chain reaction-restriction fragment length polymorphism (PCR-RFLP). Blood lipids and apolipoprotein contents were also examined.of triglyceride, total cholesterol, high-density lipoprotein cholesterol (HDLC), low-density lipoprotein cholesterol (LDL-C), as well as apolipoprotein A and aoplipoprotein B.RESULTS: Data of 35 VaD 36 CI patients and 40 healthy controls were all VaD group and CI group had higher ε4 allele frequency were higher (22.86% and 22.22% vs 7.5%, respectively, P < 0.05) but lower ε3 allele frequency (70% and 69.45% vs 86.25%, respectively, P < 0.05); the frequencies of all alleles were basically comparable between VaD and CI groups er in E ε4 allele carriers [(5.85±L03), (4.73±0.29), (4.96±0.87) mmol/L, respectively] than in E ε2 [(3.91±0.87), (3.12±0.65), (3.06±0.33) mmol/L] and ε3 [(1.34±0.21), (1.00±0.28), (0.94±0.32) g/L] allele carriers in VaD group (P < 0.05).CONCLUSION: Apolipoprotein E gene polymorphism is closely associated with the onset of VaD, for which ε4 allele might be a risk factor with similar impact in CI.

Objective:To study the expressions of c-erbB-2 and ki67 proteins in simple intestinal metaplasia(SIM),atypical intestinal metaplasia(AIM) and gastric adenocarcinoma,and the relationships among them.Methods:The immunohistochemical EnVision method was used to determine the expressions of c-erbB-2 and ki67 in 27 cases of SIM,26 cases of AIM and 37 cases of gastric adenocarcinoma.Results:The positive rates of c-erbB-2 and ki67 in gastric adenocarcinoma were significantly higher than that in SIM(P0.05).Conclusion:The high expression of c-erbB-2 and ki67 in AIM and gastric adenocarcinoma suggested that AIM may play an important role in gastric adenocarcinoma.The AIM is one kind of precancerosis of gastric adenocarcinoma.

Objective To explore the possibility of using HBV as a gene delivery vector, and to test the anti-HBV effects by intracellular expression of antisense RNA. Methods Two parts of HBV genome were reversedly recombined back into overlength HBV genome, which can produce HBV particle, to express antisense RNA complementary to S or S promoter region respectively. HepG 2.2.15 cell lines were transfected with these constructs and the empty vector pMEP4, then positive clones were selected and mixed in respective groups with hygromycin in the culture medium. HBsAg and HBeAg, which exist in the culture medium, were tested by ELISA method and intracellular HBc related HBV DNA was examined by dot blot hybridization. The existence of recombinant HBV virion in the culture medium was examined by PCR. Results The mean inhibitory rates of HBsAg were (2.74?3.83)%、(66.54?4.45)%(P

RESUMO

Objective: To study the relationship of expression of CDX2 and PTEN proteins and gastric metaplasia. Methods: Immunohistochemistry Envision method was used to detect the expression of CDX2 and PTEN proteins in simple intestinal metaplasia (SIM), atypical intestinal metaplasia (AIM) and gastric adenocarcinoma. Results: The positive rates of CDX2 in SIM, AIM and gastric adenocarcinoma was 82.6%, 51.5% and 36.4% respectively. The expression of CDX2 in gastric adenocarcinoma was significantly lower than that in SIM(P0.05). The positive rates of PTEN in SIM, AIM and gastric adenocarcinoma were 86.9%,75.8%and 47.7% respectively and the difference between SIM and gastric adenocarcinoma was significant (P
