Introducción. Un factor fundamental en el envejecimiento de la piel es la exposición acumulativa a la luz solar. Al avanzar el proceso de fotoenvejecimiento, el colágeno y las fibras elásticas experimentan mayores daños, generando patologías asociadas a cáncer. Se plantea como objetivo de esta revisión, analizar la evidencia que existe en relación a las diferentes situaciones del fotoenvejecimiento cutáneo y su asociación con el cáncer de piel. Metodología. Se elaboró una revisión sistemática, exploratoria, a través de una búsqueda bibliográfica en gestores de bases de datos en salud. Se seleccionaron 18 artículos completos entre los años 2019 y 2020. Resultados. Se agruparon los hallazgos de los artículos en dos matrices de información, la primera que indica los temas principales del fotoenvejecimiento y la relación con los factores asociados a cáncer de piel, y una segunda matriz que presenta la interrelación de la asociación con la conclusión más relevante del artículo. Conclusiones. El fotoenvejecimiento es un problema que crece día a día impactando en la vida del ser humano, creando repercusiones a nivel físico y mental que afectan la calidad de vida. Es necesario crear conciencia acerca de los mecanismos de fotoprotección, como el uso de bloqueadores solares, antioxidantes, evitar la luz solar y uso de lentes solares, entre otros; y, en los casos que ya exista algún daño por fotoenvejecimiento, elegir la mejor alternativa e individualizar cada paciente según su tipo de piel y enfermedades asociadas con la edad, para un mejor enfoque de tratamiento, y así impactar de manera positiva en el crecimiento exponencial que existe hoy en día en diversos tipos de cáncer de piel y lesiones precancerosas


Introduction. A fundamental factor in skin aging is cumulative exposure to sunlight. As the photoaging process progresses, collagen and elastic fibers experience greater damage, generating pathologies associated with cancer. The aim of this review is to analyze the evidence that exists in relation to the different situations of cutaneous photoaging and its association with skin cancer. Methodology. A systematic, exploratory review was prepared through a bibliographic search in database managers in health. A total of 18 articles were selected between the years 2019 and 2020. Results. The articles information were grouped into two matrix, the first one that indicates the main themes of photoaging and the relationship with the factors associated with skin cancer, and a second matrix that presents the interrelationship of these factors with the most relevant conclusion of the study. Conclusions. Photoaging is a problem that grows day by day impacting on human life, creating repercussions on a physical and mental level affecting the quality of life. It is necessary to raise awareness about photoprotection mechanisms such as the use of sunscreen, antioxidants , avoid sunlight, use of solar lenses, among others; and in cases where there is already some damage caused by photoaging, choose the best alternative and individualize each patient according to their skin type and diseases associated with age, for a better treatment approach, and thus positively impact the exponential growth that exists today in various types of skin cancer and precancerous lesions

Abstract Introduction: To date, many studies have validated the Hematopoietic Cell Transplantation Specific Comorbidity Index (HCT-CI) scoring system in allogeneic hematopoietic stem cell transplantation (allo-HSCT), but studies from developing countries remain scarce. Objective: The aim of this study was to evaluate and categorize Mexican patients using the HCT-CI at a referral center. Methods: One hundred and nineteen consecutive patients undergoing allo-HSCT at the National Institute of Medical Sciences and Nutrition in Mexico City were included. Patients were classified according to the HCT-CI scores. Results: The median age was 31 years and most were males (56%). Most patients had hematological malignancies (73%) and a low HCT-CI score (72%). The non-relapse mortality and survival were predicted according to the score. Conclusions: This is one of the few studies to evaluate the HCT-CI in adults with HLA-matched donors in a developing country and our findings suggest that the high percentage of patients with a low HCT-CI scores, contrary to international reports, could be explained by different comorbidities and demographics, but mainly due to stricter filters applied to HSCT candidates and consequently, a potential selection bias caused by limited resources.

Introducción: El riesgo de infección tuberculosa se asocia a contacto estrecho y prolongado. En las escuelas el contacto diario, aulas con poca ventilación o retraso diagnóstico del caso índice, podrían contribuir al contagio. Objetivo: Conocer la incidencia de infección tuberculosa latente (ITBL) en contactos escolares comparando dos puntos de corte en la prueba de tuberculina (PPD): ≥ 10 mm y ≥ 5 mm. Determinar el grado de cumplimiento en la realización de estudios de catastro y de quimioprofilaxis (QP). Materiales y métodos: Se realizó el análisis retrospectivo de contactos escolares de TB bacilífera, en adolescentes de 12 a 19 años, correspondientes al área programática del Hospital Parmenio Piñero entre febrero 2016 a diciembre 2017. La evaluación incluyó radiografía de tórax (RxT), laboratorio y PPD. Se analizaron 2 puntos de corte de PPD: ≥ 10mm y ≥ 5mm, repitiéndose a los 3 meses en aquellos con resultado negativo. Se indicó QP primaria con isoniazida a todos los contactos, y secundaria en aquellos con PPD+ basal o conversores al tercer mes. Se estimó cumplimiento de QP si tomaban > 80% de la pauta indicada. Resultados: de 373 estudiantes a evaluar, 331 (89%) realizaron los estudios, se diagnosticó TB en 4 (1.2%) a partir de RxT y fueron excluidos. En los 327 restantes, edad promedio 15 ± 1,6 años, hubo 132 (40%) hombres. La PPD basal fue ≥ 10 mm en 20 (6.1%) casos, siendo más frecuente en el grupo de 16-19 años vs 12-15 años: 10.7% vs 3.1%; p = 0,004. No hubo diferencia significativa al considerar el sexo. Realizaron 135 (45%) la segunda PPD encontrándose 3 conversiones (2.2%). Se indicó QP primaria a todos y secundaria a 23, el cumplimiento fue 57% y 26% respectivamente, sin asociación significativa con edad o sexo. Se reportaron 4 eventos adversos (1.2%): polineuritis (n = 2), alergia cutánea e intolerancia digestiva. Con PPD ≥ 5mm dieron positivo 63 (19.3%), siendo más frecuente en varones: 26.5% vs 14.4%, p = 0.006; y en 16-19 vs 12-15 años: 28.2% vs 13.2%; p < 0.001. Con la segunda PPD se observaron 3 conversiones (2,2%). Conclusión: Encontramos un 6,1% de ITBL considerando PPD+ ≥ 10 mm y 19,3% con PPD ≥ 5 mm. El cumplimiento en la realización de la segunda PPD fue bajo, como también en completar el esquema de QP secundaria.

Introduction: The risk of tuberculosis infection is associated with close and prolonged contact. Daily contact, poorly ventilated classrooms or delay in diagnosing the index case in a school setting could contribute to contagion. Objective: To know the incidence of latent tuberculosis infection (LTBI) in school contacts comparing two cut-off points of the tuberculin test (PPD, Purified Protein Derivative): ≥ 10mm and ≥ 5mm. To determine the degree of compliance with the performance of control and chemoprophylaxis (ChP) studies. Materials and Methods: We carried out a retrospective analysis of bacillary TB school contacts in teenagers between 12 and 19 years old of the Programmatic Area of the Hospital Parmenio Piñero between February 2016 and December 2017. The evaluation included chest x-ray, lab tests and PPD. We analyzed 2 cut-off points of the PPD: ≥ 10mm and ≥ 5mm, and repeated the test after 3 months in cases with negative results. Primary ChP with isoniazid was indicated for all the contacts, and secondary ChP for those with basal PPD+ or conversion at the third month. We considered ChP compliance in cases where patients followed > 80% of the indicated regime. Results: 331 (89%) of 373 students to be evaluated participated in the study. TB was diagnosed in 4 students (1.2%) through chest x-ray, and they were excluded. Within the remaining 327, the mean age was 15 ± 1.6 years, and 132 (40%) were male. The basal PPD was ≥ 10mm in 20 cases (6.1%), being more frequent in the group of 16-19 years versus the 12-15 years: 10.7% vs. 3.1%; p = 0.004. There wasn't any significant difference regarding sex. 135 contacts (45%) did the second PPD, and 3 conversions were found (2.2%). Primary ChP was indicated for everyone, and secondary ChP for 23 contacts, with 75% and 26% compliance respectively, and without any significant association with age or sex. 4 adverse events were reported (1.2%): polineuritis (n=2), skin allergy and food intolerance. 63 contacts (19.3%) were positive PPD, with PPD ≥ 5mm, being more frequent in males: 26.5% vs. 14.4%, p = 0.006; and in 16-19 vs. 12-15 years: 28.2% vs. 13.2%; p < 0.001. With the second PPD we observed 3 conversions (2.2%). Conclusion: We found 6.1% of LTBI considering a PPD+ ≥ 10mm and 19.3% with PPD ≥ 5mm. Compliance with the second PPD and the secondary ChP scheme was low.

ABSTRACT Neutrophil extracellular traps (NETs) were described more than one decade ago, but recently, the interest in these structures has increased due to their involvement in cancer progression, cancer-related thrombosis, and development of metastasis. This protumoral role of NETs strengthens their potential as new prognostic markers of cancer.

ABSTRACT Purpose: The current first - line treatment for non - seminomatous germ cell tumor (NSGCT) consists of four cycles of cisplatin, etoposide, and bleomycin (BEP), which results in 5 - year overall survival < 60% in patients with poor - risk features. Autologous hematopoietic stem cell transplantation (auto - HSCT) as a method for overcoming high toxicity after high dose chemotherapy (HDC) has been explored in different solid tumors, but has remained standard practice only for NSGCT. Our objective was to describe outcomes of patients with poor - risk NSGCT who underwent first - line autologous HSCT in a tertiary center in Mexico. Patients and Methods: Twenty nine consecutive patients with NSGCT who received first - line, non - cryopreserved autologous HSCT at the National Institute of Medical Sciences and Nutrition Salvador Zubiran in Mexico City, Mexico, from November 1998 to June 2016, were retrospectively analyzed. Results: The median age at transplantation was 23 (15 - 39) years. Most patients (n = 18, 62%) had testicular primary tumor, and 23 had metastases (79%). Complete response after auto - HSCT was observed in 45%. Non - relapse mortality was 0. Five - year relapse / progression free and overall survival were 67% and 69%, respectively. Conclusions: This small single limited - resource institution study demonstrated that patients with poor - risk NSGCT are curable by first - line HDC plus autologous HSCT and that this procedure is feasible and affordable to perform using non - cryopreserved hematopoietic stem cells.

La terapia larval es utilizada desde los años 30 del siglo pasado para la remoción del tejido necrótico en el tratamiento de úlceras crónicas infectadas logrando con ello promover la formación de tejido granuloso para el crecimiento de piel sana; especialmente a partir de la aparición de la resistencia a los antibióticos se la reconoce como una alternativa eficaz. Se presentan los resultados de la aplicación de la terapia larval en 42 pacientes con úlceras de diferentes orígenes, utilizando, como una nueva alternativa en larviterapia, la especie Lucilia eximia. Se presenta documentación fotográfica de la aplicación de esta terapia en cuatro casos.


Larval therapy is used in the treatment of infected chronic wounds by allowing the removal ofnecrotic tissue, which induces the formation of granular tissue and the growth of healthy skin.Considering the increasing prevalence of antibiotic resistance, this type of therapy may be an effectivealternative in the management of infected chronic wounds. In this article we report the use ofmaggot therapy using the Lucilia eximia species in 42 patients with chronic skin wounds associatedto different pathologies including: venous and arterial ulcers, diabetic foot, sickle cell disease,vasculopathy, elephantiasis, Berger disease, pyoderma gangrenosum (PG), traumatic wounds, erysipelas, and hospital acquired infections. Four cases are depicted photographically.

RESUMO

Nos meados de 1.950, começaram a ser realizadas cirurgias de revascularizaçäo por Gross. Com o passar dos anos a cirurgia vascular vem sendo aprimorada, através de novas pesquisas em relaçäo a novos materiais que foram utilizados como enxerto, onde podemos citar as próteses sintéticas de PTEE e dacron. Aos poucos a fisioterapia vem ocupando um espaço dentro da cirurgia de BY PASS, mas de acordo com a pesquisa observou-se que näo däo ênfase à fisioterapia no pré-operatório, näo por falta de interesse, e sim pela falta de indicaçäo médica. Desta maneira o processo de recuperaçäo pós-cirúrgica torna-se mais lento, levando a acamamento prolongado, aumentando o índice de complicaçöes
